방사선조사가 구루병 백서의 발치창 치유에 미치는 영향 (Effect of irradiation on wound healing after tooth extraction in the rachitic rats)

  • Purpose: To observe the histopathological changes following irradiation on the wound healing after tooth extraction in the rachitic rats. Materials and Methods: In order to carry out this study, the rats were divided into four groups: Group 1 (normal diet/non-irradiation group), Group 2 (normal diet/irradiation group), Group 3 (rachitogenic diet/non-irradiation group), and Group 4 (rachitogenic diet/irradiation group). Rachitic changes were induced with rachitogenic diet No. 2 (high calcium, low phosphorus, and Vitamin D deficient diet) for 5 weeks. After the extraction of both maxillary first molars of the rats in Group 2 and 4, the head and neck of the rats were irradiated with single absorbed dose of 10 Gy. The rats were sacrificed at the 1st, 5th, 10th, and 15th day after tooth extraction. The specimens including the extraction wound were sectioned, stained with the hematoxylin-eosin and Masson's trichrome method and examined under the light microscope. Results: In the Group 2, the amount of newly formed bone trabeculae on the periphery of extraction socket and osteoblastic activity were reduced. In the Group 3, epithelial fusion was not revealed on the 5th day after toothe extraction and growth rate of osteoid formation was reduced. In the Group 4, necrotized tissue at the outer surface of extraction socket and destructive changes on the alveolar bones were noted on the 10th day. Epithelial fusion was not revealed and large amounts of osteoclast were noted on alveolar bone on the 15th day. Conclusion: The healing process of wound after tooth extraction was retarded by irradiation and especially in the rachitic rats.

Effects of fresh mineralized dentin and cementum on socket healing: a preliminary study in dogs

  • Objectives: Dentin is composed of many minerals and growth factors. Based on this composition, we studied its effect as a possible regenerative material for alveolar healing. Materials and Methods: This study was conducted using four 2.5-year-old mongrel dogs (male; weight, 25 to 30 kg). The third mandibular premolars were carefully mobilized with a dental elevator and then removed using forceps. The crown portions of the extracted teeth were removed with cutters, and the root portions of the remaining teeth were collectively trimmed as closely as possible to 350 to $500{\mu}m$. Dentin and cementum (DC) chips harvested from the extracted teeth were soaked in blood and packed into the fresh sockets (autograft). Biopsies were performed at the ends of day 14 and day 56 following implantation. Data were expressed as $mean{\pm}standard$ deviation and compared with t-test results. Results: The ratio of $S_A$(bone) to total area of each probe was determined and was $170{\pm}16{\mu}m^2$ for the control group and $71{\pm}14{\mu}m^2$ for the DC group, a significant difference (P <0.05). Conclusion: DC particulate grafts offered no improvement in bone regeneration in alveolar extraction sockets.

편평세포암종과 병발한 치성점액종 (Simultaneous occurrence of an Odontogenic Myxoma and a Squamous Cell Carcinoma of the Mandible)

  • Squamous cell carcinoma is the most common type of oral cancer and odontogenic myxoma is relatively uncommon benign tumor of mesenchymal origin. There are, to our knowledge, no prior reports of simultaneously occurring squamous cell carcinoma and odontogenic myxoma of the jaw bones. In this case, at first, the plain films and computed tomograms revealed a large expansile multilocular radiolucent lesion on left mandible and marked expansion of cortical plate. In addition this radiograms revealed also infiltrative bony destruction of anterior and medial border of ascending ramus of left mandible and alveolar bone of left maxilla, floating teeth on left lower molar area and metastatic enlargement of left submandibular, jugular digastric and spinal accessory lymphnodes. Magnetic resonance imaging of this patient revealed infiltrative growth of tumor on alveolar bone of left maxilla, left retromolar fat pad. left masseter and left medial pterygoid muscle. Intraoral presurgical biopsy presented typical features of squamous cell carcinoma. After chemotherapy with radiation therapy during 6 months. this central lesion was diagnosed as odontogenic myxoma by the postsurgical biopsy. After 3 months, this patient presented multiple metastatic signs at lumbar spines, rib and liver. Consequently, our case is simultaneous occurrence of squamous cell carcinoma and odontogenic myxoma.

Effect of IgY on Periodontitis and Halitosis Induced by Fusobacterium nucleatum

  • Fusobacterium nucleatum is a morbific agent in periodontitis and halitosis. Egg yolk antibody (IgY) was obtained from egg yolks from chickens stimulated with F. nucleatum. This study was to assess the effectiveness of IgY on periodontitis and halitosis caused by F. nucleatum in vitro and in vivo. The growth of F. nucleatum was inhibited (p <0.05) by different concentrations of IgY in vitro and the results of a Halimeter show volatile sulfur compounds (VSCs) were reduced to $904{\pm}57ppb$ at a concentration 40 mg/ml of IgY. The changes of fatty acids of F. nucleatum were determined using GC-MS. The scores for odor index of rat saliva were decreased. The major constituent of volatile organic compounds (VOCs) including short-chain acids decreased 46.2% in 10 mg/ml IgY, ammonia decreased 70% in 40 mg/ml IgY, while aldehydes and olefine ketones were almost unchanged. The ELISA assay revealed that IL-6 and TNF-${\alpha}$ were decreased after 4 weeks' IgY treatment. Morphometric (X-ray) and histological analyses (HE) showed that IgY reduced alveolar bone loss and collagen fibers became orderly in rat models. As a result, IgY may have the potential to treat periodontitis and halitosis.

저한성 외배엽 이형성증을 가진 소아 환자에서 의치 유지력 향상을 위한 보철 치료 전략: 두 가지 증례 보고 (Prosthetic Treatment Strategies for Improving Denture Retention in Pediatric Patients with Hypohidrotic Ectodermal Dysplasia: a Report of Two Cases)

  • 이 논문은 저한성 외배엽 이형성증(Hypohidrotic Ectodermal Dysplasia, HED)으로 인한 부분무치증을 가진 소아 환자에게 의치의 유지력 증가 방법에 관한 두 가지 증례를 보고한다. 두 환자 모두 다수의 치아 결손 및 원추형의 견치와 치조골 위축, 골격적 III급 경향이 나타났다. 첫 번째 증례에서는, 변형된 형태의 Conical-Crown Retained Denture (CCRD)를 이용해 견치를 피개하였다. 이 방법은 치아를 삭제하거나 코핑 없이 진행되었다. 두 번째 증례에서는, 심한 치조제 흡수와 함께 하악 좌측 견치가 원심으로 경사지어 맹출한 상황에서, 하이브리드 세라믹 크라운을 사용하여 견치를 수복한 후, 흡착성 원리를 이용한 클라스프 유지 가철식 의치로 변경하였다. 두 환자는 현재 보철물 유지 관리 및 성장양상을 평가하기 위해 주기적으로 치료를 받고 있다. 이 증례는 HED로 인하여 의치의 유지력이 부족한 소아 환자에게 적용된 창의적인 보철 치료 방법을 제시하였다.

한국인 7-17세 아동의 두개저, 상악, 하악의 성장에 관한 준종단적 연구 (A Semilongitudinal Study on Cranial Base, Maxillary and Mandibular Growth of Korean Children Aging 7 to 17 Years Old)

  • 한국인 7세-17세 아동의 두개저, 상악, 하악의 사춘기 성장 양상을 파악해 보고자 남자 251개, 여자 286개의 측모두부계측방사선 사진을 이용, 10개의 계측점과 16개의 계측항목을 설정하고 계측항목에 대한 계측치와 각 연령별 년간 누년차를 산출한 다음 이들간의 통계적 유의성을 검증하였으며 각 계측항목을 두개저, 상악, 하악의 세군으로 분류하고 또 남녀별로 분류하여 비교함으로써 다음과 같은 결론을 얻었다. 1. 한국인 성장기 아동에 있어서의 성장은 남자 아동에 비해 여자 아동이 빠른 시기에 일어났다. 2. 두개저, 상악골, 하악골 모두 사춘기 성장을 나타내었으나 두개저의 성장은 안면골 성장에 비해 상대적으로 작았다. 3. 두개저 성장에 있어서 전두개저 길이의 증가에 비해 중두개저 및 후두개저의 길이 증가가 현저했으며 사춘기 성장증가(circumpuberal growth spurt) 양상도 보다 명확했다.4. 상악골의 전하방 이동은 상악골 자체의 길이 성장과 상악골 주위 봉합부의 성장에 따른 변위가 종합된 결과로 두개저에 대한 상악골의 상대적 위치를 나타내는 Ar-ANS와 Ar-Pr이 상악골 자체의 길이 성장을 나타내는 ANS-PNS보다 많은 성장량을 보였다. 5. 하악골은 수평 성장량에 비해 수직 성장이 약간 크게 나타났으나 유의할 수준은 아니다. 6. 상악 치조골과 하악 치조골은 유치 탈락후 영구치 맹출시기에 최대 성장률을 보인후 감소하는 경향을 보이지만 하악 치조골의 수직성장에서는 사춘기 성장증가가 관찰되었다.

실험적구루병에서 백서 치아조직 변화에 관한 현미경학적 연구 (A MICROSCOPIC STUDY OF THE EFFECTS OF EXPERIMENTAL RICKETS ON THE DENTAL STRUCTURE OF RATS)

  • Rickets is not the deposite of minerals in the skeletal tissue and the retardation of skeletal growth in growing in growing animals. This study was undertaken to investigate the histologic effects of experimental rickets on the dental structure of the albino rats, and to show the relationship between the histological effects and the pulpal disease which induced premature loss of the primary teeth. This study was based on material obtained from 40 white rats that were placed on a rachitogenic diet for a period 1 to 56 days after weaning (at 24 days). In addition, a study was made of 25 litter mates, 24 to 80 days, that were fed a normal diet. The following results were obtained: 1. Enamel formation and calcification showed no significant changes and no hypoplasia. 2. Dentin formation and calcification was retarded and disturbed. In the experimental group, predentin/calcified dentin was remarkablly increased. 3. Newly formed dentin showed interglobular texture (less homogenous calcification) and the predentin was significantly wider and thicker, and there was an irregular wave in the basal portion of the rat's incisors. 4. In cementum, Matrix formed at almost a normal rate but calcification was defective. So cementoid tissue was increasesd. 5. The formation of the alveolar bone was at almost a normal rate but calcification was retarded. The trabecular bone was filled with osteoid tissue and thicker than in normal groups.

  • The prevalence of congenital teeth missing is about 2-10% of population and it is more common in permanent dentition than primary dentition. In the patients with congenital missing teeth symptoms such as conical tooth, prolonged retention of primary tooth, tooth attrition. and deficient alveolar bone growth can be accompanied. In this paper the results of clinical observation of the congenital teeth missing brothers and their familial history are presented.

  • The patient, 19 years old male, visited with complaints of trismus and consults for construction of denture. 4 years ago, he received /sup 60/Coteletherapy of left laryngeal area for treatment of throat tumor. Clinical apperance showed rampant dental caries, multiple root rests, xerostomia, disturbance of mandibular growth and asymmetry of face. Roentgenographic examination disclosed deep cervical caries, destruction of alveolar crest, punched out bone destruction of mandible and maxilla.

  • Clinicians often meet the growing patient who is not treated with fixed prosthesis until alveolar bone growth is completed. When both esthetic and function needs are important, and if proper donor tooth is present, the application of autotransplantation may be better treatment option than space maintainer. Although prognosis of autotransplantation is not always favorable, it depends on the treatment design which is based on the ability of case selection and consideration. This report will present the clinical cases of autotransplantation in growing patient, and discuss about several clinical aspects of autotransplantation.
