Canine juvenile cellulitis의 진단과 치료 증례 (Two Cases of Diagnosis and Treatment in Canine juvenile cellulitis)

  • Canine Juvenile cellulitis was diagnosed in 2 puppies hospitalized in Veterinary Medical Teaching Hospital of Seoul National University. Characterized dermatological problems were presents of scale, crust, purulent exudation and alopecia limited on their face. On cytologic examinations of direct impression smears for the lesions, numerous neutrophils and macrophagies were observed. No bacteria and fungus were noted. Treatments with administration of predinisolone and antibiotics orally combined with topical shampoo treatment had good results for the two puppies.

  • In a harbor seal in captivity from the Yellow sea of Korea, alopecia began on flippers and tail, and spread to ventral part of abdomen and dorsal part of body in order. Multiples coalesced, and well-circumscribed lesions such as patches and maculas appeared on foreflipper, hind flipper, perianus, ventral part of tail, lips, and some parts of trunk were appeared in. Phialophora richardsiae, Gemella morbillorum, and coagulate-negative etaphyococci were isolated from Besions and hairs. After treatment with internal and external ketoconazole, povidone iodine, cephalexinl Pink Skin~l and multivitamins hair grew up and lesions were disappeared completely 8 months after occurrence.

흉터성 눈썹소실환자에 실시한 단일모 이식술 (Single Hair Transplantation for Cicatrical Eyebrow Alopecia)

  • Purpose: Single hair transplantation for eyebrow loss has been considered as a good method of achieving natural appearance of eyebrow. To date, however, no study has reported the survival rate following single hair transplantation for cicatricial eyebrow alopecia. Methods: We performed single hair transplantation using Choi hair transplanter for 49 cases of scarring eyebrow loss from March 2003 to March 2006. Of these cases, 33 could be under a follow-up postoperatively for more than seven months. These 33 cases comprised 20 men and 13 women. Mean follow-up period was 12 months (7-36 months). In 17 patients(51.5%), 100 to 200 hairs were unilaterally transplanted. Results: Following initial hair transplantation, the surgical outcome was excellent in 11 cases(33.3%), good in 12 cases(36.4%), fair or poor in the remaining ten cases(30.3%). All procedures were done under intravenous sedation and local or regional block anesthesia. It took about one and a half hours for us to transplant 200 hairs. The shape of eyebrow was aesthetically acceptable with a good direction and an ideal inclination of hairs. No patients developed notable complications, while 30 percent had a low survival rate. Conclusion: Single hair transplantation using Choi hair transplanter is a safe, effective surgical method for scarring eyebrow loss.

In Vivo Hair Growth Promotion Effects of Ultra-High Molecular Weight Poly-γ-Glutamic Acid from Bacillus subtilis (Chungkookjang)

  • We investigated the effect of ultra-high molecular weight poly-γ-glutamic acid (UHMW γ-PGA) on hair loss in vitro and in vivo. 5-Alpha reductase is an enzyme that metabolizes the male hormone testosterone into dihydrotestosterone. By performing an in vitro experiment to analyze the inhibitory effects of UHMW γ-PGA on 5-alpha reductase activity, we determined that UHMW γ-PGA did in fact inhibit 5-alpha reductase activity, indicating the use of UHMW γ-PGA as a potential 5-alpha reductase inhibitor in the treatment of men with androgenetic alopecia. To evaluate the promotion of hair growth in vivo, we topically applied UHMW γ-PGA and minoxidil on the shaved dorsal skin of telogenic C57BL/6 mice for 4 weeks. At 4 weeks, the groups treated with UHMW γ-PGA showed hair growth on more than 50% of the shaved skin, whereas the control group showed less hair growth. To investigate the progression of hair follicles in the hair cycle, hematoxylin and eosin staining was performed. Histological observations revealed that the appearance of hair follicles was earlier in the UHMW γ-PGA-treated group than in the control group. The number of hair follicles on the relative area of shaved skin in the UHMW γ-PGA-treated group was higher than that observed on the shaved skin in the control group. These results indicate that UHMW γ-PGA can promote hair growth by effectively inducing the anagen phase in telogenic C57BL/6 mice.

  • 3연령 잡종편이 소양감, 탈모의 증상으로 내원하였다. 신체검사에서 전신적으로 발적, 구진, 탈모, 비듬 그리고 가피의 병변이 관찰되었다. 전반적인 피부검사결과 개선충, Staphylococcus spp과 Malassezia spp의 혼합 감염으로 진단되었다. Selamectin 6 mg/kg을 2주간격으로 2회 경부피부에 적용하였다. 그리고 한 달간 구균과 Malassezia 치료를 위해 amoxicillin trihydrate/clavulanate pattassium과 ketoconazole 10 mg/kg을 경구투여하고 5% chlorhexidine 샴푸를 주2회 실시하였다. 첫 치료 후 7,14,21 그리고 28일에 피부를 재검사 하였을 때 개선충은 관찰되지 않았으며 피부 증상은 현저하게 개선되었다. 피모는 거의 정상으로 회복되었으며 Staphylococcus와 Malassezia spp는 거의 관찰되지 않았다. 그러므로 이 치료절차에 따른 selamectin 적용은 개선충이 감염된 개에서 매우 효과적이라고 사료된다.

Forehead reconstruction with a custom-made three-dimensional titanium implant in a Parry-Romberg syndrome patient

  • Parry-Romberg syndrome is a rare neurocutaneous syndrome characterized by progressive shrinkage and degeneration of the tissues usually on only one side of the face. It is usually difficult to restore the facial contour due to skin tightness. In this case report, we report a forehead reconstruction with custom-made three-dimensional (3D) titanium implant of a Parry-Romberg syndrome patient who was treated with multiple fat grafts but had limited effect. A 36-year-old man presented with hemifacial atrophy. The disease progressed from 5 to 16 years old. The patient had alopecia on frontal scalp and received a surgery using tissue expander. The alopecia lesion was covered by expanded scalp flap done 22 years ago. Also, he was treated with fat grafts on depressed forehead 17 years ago. However, it did not work sufficiently, and there was noted depressed forehead. We planned to make 3D titanium implant to cover the depressed area (from the superior orbital rim to the vertex). During the operation, we confirmed that the custom-made 3D implant accurately fit for the depressed area without any dead spaces. Previously depressed forehead and glabella were elevated, and the forehead contour was improved cosmetically. A custom-made 3D titanium implant is widely used for skull reconstruction and bring good results. In our case, the depressed forehead of a Parry-Romberg syndrome patient was improved by a 3D titanium implant.

Induction of transforming growth factor-beta 1 by androgen is mediated by reactive oxygen species in hair follicle dermal papilla cells

  • The progression of androgenetic alopecia is closely related to androgen-inducible transforming growth factor (TGF)-${\beta}1$ secretion by hair follicle dermal papilla cells (DPCs) in bald scalp. Physiological levels of androgen exposure were reported to increase reactive oxygen species (ROS) generation. In this study, rat vibrissae dermal papilla cells (DP-6) transfected with androgen receptor showed increased ROS production following androgen treatment. We confirmed that TGF-${\beta}1$ secretion is increased by androgen treatment in DP-6, whereas androgen-inducible TGF-${\beta}1$ was significantly suppressed by the ROSscavenger, N-acetyl cysteine. Therefore, we suggest that induction of TGF-${\beta}1$ by androgen is mediated by ROS in hair follicle DPCs.

부챗말 Glucopyranosyldiacylglycerol의 탈모방지 효능 (The Effect of Glucopyranosyldiacylglycerol from Padina arborescens on the Prevention of Hair-Loss)

  • This study was conducted to evaluate the effects of Padina arborescens and 1-O-myristoyl-2-O-oleoyl-3-O-(α-D-glucopyranosyl)-glycerol(MOGG), its active component, on the prevention of hair loss. The P. arborescens extract and MOGG inhibited the activity of 5α-reductase, which converts testosterone to dihydrotestosterone(DHT), a main cause of androgenetic alopecia. When immortalized rat vibrissa dermal papilla cells were treated with MOGG, the proliferation of dermal papilla cells significantly increased. In addition, we found that the P. arborescens extract and MOGG could open the KATP channel, which may contribute to increase hair growth. Furthermore, MOGG promoted PGE2 production in HaCaT cells. The results suggest that MOGG from the P. arborescens extract has the potential to treat alopecia via 5α-reductase inhibition, the proliferation of dermal papilla, the opening of the KATP channel and/or increase of PGE2 production.

여성 암환우들의 탈모용 가발 착용 시 불편에 따른 개선방안 연구 (A Study on Improvement Strategies According to the Inconvenience Due to Female Cancer Wearing Alopecia Wigs)

  • 본 연구는 암환우들의 탈모용 가발 착용 시 불편 사항에 따른 개선 방안을 분석한 결과로, 60대 전후의 여성 암환우를 대상으로 조사 분석을 실시하였다. 탈모를 인지하게 된 시기는 항암제 또는 방사선 치료 1회 직후가 가장 높았고, 두번째로 2회 후부터가 23.9% 순으로 나타났으며, 암환우들의 가발 착용에 따른 불편 수준으로는 대체로 편리한 편이지만 두피트러블, 밀착성, 조화성, 등에 있어 불편을 겪는 것으로 나타났다. 또한 암환우들의 일반적 특성에 따른 가발 착용 시의 불편 요소 분석 결과 직업에 따라 편리성에 차이가 있는 것으로 나타났고 다른 요인에서는 큰 차이를 보이지 않았다. 대졸이나 고졸 혹은 대학원 졸업인 경우 현재 재학 중인 학생에 비해 높은 편리성을 보이며 졸업생과 재학생 간의 연령차에 의한 차이일 것으로 추측된다. 이러한 불편요소를 개선하기 위해 가발의 기술 개발과 연구가 끊임없이 필요할 것으로 판단된다.

Effect of Charge Carrier Lipid on Skin Penetration, Retention, and Hair Growth of Topically Applied Finasteride-Containing Liposomes

  • The aim of this study was to investigate the effect of charge carrier lipid on the skin penetration, retention, and hair growth of topically applied finasteride-containing liposomes. Finasteride-containing liposomes were prepared by traditional thin film hydration method using Phospholipon$^{(R)}$ 85 G and cholesterol with or without charge carrier lipid (1,2 dimyristoyl-sn-glycero-3-phosphate or 1,2-dioleoyl-trimethylammonium-propane for anionic and cationic charge, respectively). Freshly prepared finasteride-containing liposome suspension was applied on the hairless mouse skin, and skin penetration and retention were measured using Keshary-Chien diffusion cell. Non-liposomal formulation (ethanol 10% solution containing 0.5 mg/ml of FNS) was also used as a control. The amount of finasteride in the diffusion cell and mouse skin was measured by HPLC. The hair growth was evaluated using depilated male C57BL/6N mice. Mean particle size of all finasteride-containing liposomes was less than a micron, and polydispersity index revealed size homogeneity. Skin penetration and retention studies showed that significantly less amount of finasteride was penetrated when applied as anionic liposome while more amount of the drug was retained. Specifically, in liposome prepared with 10% anionic charge carrier lipid, penetration was 12.99 ${\mu}g/cm^2$ while retention was 79.23 ${\mu}g/cm^2$ after 24 h of application. In hair growth study, finasteride-containing anionic liposomes showed moderate efficacy, but the efficacy was not found when applied as cationic liposomes. In conclusion, topical application of finasteride using anionic liposome formulation appears to be useful option for the treatment of androgenetic alopecia to avoid systemic side effects of the drug.
