Porcelain is the first ceramic material to be introduced into dentistry. Porcelain jacket crown was introduced by Dr. Charles H Land in 1886, which was an excellent aesthetic dental restoration but has not been widely used due to high firing shrinkage and low tensile strength. Then metal-ceramic system, which combines the esthetic properties of ceramics and the mechanical properties of metals, was introduced and nowadays it is still used in dental clinical field. However, the metal-ceramic system has shown some problems, such as increased lightness by reflection of light at opaque layer, shadow beneath the gingival line due to the block-out of light by metal coping, exposure of metal in margin part, bond failure between metal and porcelain, oxidation of metal coping during firing the porcelain, etc. Recently, along with the advance of fabrication methods of dental ceramics, the all-ceramic restorations with high esthetic and mechanical properties has increased and gradually replaced metal-ceramic restorations. Especially, CAD/CAM technology has opened a new era in fabricating the dental ceramic restorations. This overview will take a look at the past, present and future possibility of the dental ceramic materials.
A conventional approach for the treatment of long-span edentulous areas is the use of removable dentures. However, placing implants in these areas results in superior functional outcomes by increasing the stability, support, and resistance of the prostheses and improving the masticatory efficiency. Treatment modalities utilizing implants can be further classified into either removable or fixed-type prostheses. Several factors such as the amount of alveolar bone resorption, inter-arch relationship, patient preferences, and socioeconomic status should be considered when determining the appropriate treatment approach. Monolithic zirconia has been considered a suitable material for implant-supported fixed dental prosthesis, because of the drastic improvement in its mechanical properties. It exhibits fewer incidences of fracture and chipping of the prostheses, and has greater bulk of material than metal-ceramic crowns and zirconia-veneered ceramics. Moreover, highly translucent monolithic zirconia is also available in the market, and its application is gradually increasing for anterior tooth rehabilitation. The present report describes a patient who underwent full-mouth rehabilitation with fixed dental prostheses (eight upper and three lower implant placements). All teeth, except bilateral mandibular canines and left mandibular first and second premolars, were extracted after the diagnosis of generalized chronic moderate-to-advanced periodontitis of the remaining teeth. The patient reported satisfactory esthetic and functional outcomes during the one-year follow-up visit.
Silicon nitride ceramic has been verified as an excellent rolling bearing material because of its high strength and outstanding rolling fatigue life properties. However under some corrosive circumstances it showed drawbacks such as hardness reduction and severe wear caused by corrosion. In this work, the variations of the rolling wear and hardness of three kinds of ceramics were studied for the specimen aged 15 days in alkali water (90 $\pm$ 2$\^{C}$,25 wt% NaOH ). All of the specimens, ① Si$_3$N$_4$, ② 3Y-TZP and ③ 3Y-TZP alloyed with 5 wt% CeO$_2$, were sintered and post-HIPed, and then polished up to 0.02 $\mu$mRa of surface roughness. Rolling wear tests were conducted by MJ type rolling fatigue life tester under the initial theoretical maximum contact stress of 3.16 GPa and the spindle speed of 1,000 rpm. Spindle oil was used as a lubricant. The specimens were not worn before aging. For the specimen aged in alkali water, Si$_3$N$_4$ and 3Y-TZP were worn by rolling wear tests, and hardness was decreased. While aging the specimens, the phase was transformed from tetragonal to monoclinic in 3Y-TZP and the microstructure change occurred in Si$_2$N$_4$. 3Y-TZP specimens alloyed with 5 wt% CeO$_2$ were not worn after aging and no phase transformation occurred while aging.

In this work, the variations of the rolling wear and hardness of three kinds of ceramics were studied for the specimen aged 15 days in alkali water (90$\pm$2$^{\circ}C$, 25 wt% NaOH). All of the specimens, \circled1Si$_3$N$_4$, \circled23Y-TZP and \circled33Y-TZP alloyed with 5 wt% CeO$_2$, were sintered and post-Hipped, and then polished up to 0.02 ${\mu}{\textrm}{m}$Ra of surface roughness. Rolling wear tests were conducted by MJ type rolling fatigue life tester under the initial theoretical maximum contact stress or 3.76 Gra and the spindle speed of 1,000 rpm. Spindle oil was used as a lubricant. The specimens were not worn before aging. For the specimen aged in alkali water, Si$_3$N$_4$and 3Y-TZP were worn by rolling wear tests, and hardness was decreased. 20vol% Al2O3-dispersed 2Y-TZP ceramics was prepared by mixing of 2Y-TZP and Al2O3 powder with different particle sizes, and investigated the influence of Al2O3 particle size and sintering condition on the microstructure and fracture toughness. Sintering conditions of the Al2O3-dispersed 2Y-TZP specimens showed high density at sintering condition of 1350℃ and 1500℃ for 1∼5h, and homogeneous microstructure. The grain size of tetragonal zirconia and the fracture toughness increased with the size of dispersed Al2O3 particle. The highest fracture toughness (∼17.2MPa·m1/2) of all specimens was obtained in the specimens with dispersed Al2O3 particle size of 1.0㎛ and sintered at 1500℃ for 2h.
This study evaluated the effects of various cleaning methods on the shear bond strength of zirconia ceramics after saliva contamination. Eighty zirconia disk specimens were divided into 8 groups. All groups were treated with one coat of MDP primer. All specimens (except the negative control) were contaminated with human saliva on the zirconia surface. The positive control went through the bonding procedure immediately after contamination without any cleaning procedure. With the exception of control groups, the remaining six groups were rinsed with water and either applied with MDP recoating (WATER+MDP) or without MDP recoating (WATER). While some were cleaned with a Ivoclean with MDP recoating (IVOCLEAN+MDP) or not applied with MDP recoating(IVOCLEAN), others were cleaned with a 1% NaOCl solution with MDP recoating (NaOCl+MDP) or without MDP recoating (NaOCl). The shear bond strength of all specimens were measured after being stored in distilled water at $37^{\circ}C$ for 24 hours. The data was analyzed statistically by an analysis of ANOVA, Tukey's post hoc test and Student's t-test was used to compare the shear bond strength according to the re-coating of MDP after the cleaning procedure. The positive control group showed the lowest shear bond strength value, and the WATER group and NaOCl group showed no significant difference when compared to the positive control group. The IVOCLEAN group showed significantly higher shear bond strength when compared to Water group and NaOCl group but not with the group of negative control. After rinsing with water or the NaOCl solution, the comparison of the single coating of MDP and re-coating of MDP showed different shear bond strengths but there was no significant difference to the negative control. After rinsing with Ivoclean, there was no significant difference to the negative control regardless of the recoating of MDP. In conclusion, the shear bond strength was affected by the cleansing procedure and Ivoclean was found to be effective regardless of the re-coating of MDP. When water or the NaOCl solution is used to remove surface contaminants, the re-coating of MDP provides a positive effect on cementation.

Zirconia is well known in industrial applications for its mechanical characteristics. DLC (diamond-like carbon) have high elastic modulus, high electric resistivity, high dielectric constant, high wear resistance, low friction coefficient, bio compatibility, chemically inert and thermally stable. Because of all these physical and chemical properties these types of coatings have become key procedure for thin coating. Friction coefficient of DLC films is already evaluated and the current work is a further advancement by calculating the fracture toughness and wear resistance of these coatings. In the present study DLC thin film coatings are developed on $ZrO_2$ alloy surface using Plasma Enhanced Chemical Vapor Deposition (PECVD) method. Vicker hardness test is employed and it was concluded that, DLC coatings increase the Vickers hardness of ceramics.
Purpose: The aim of this in vitro study is to investigate load bearing capacity of esthetic abutments according to the type of material and wall thickness. Materials and methods: 70 specimens equally divided into seven groups according to their abutment wall thicknesses. The abutments prepared with titanium 0.5 mm wall thickness were used as a control group (Ti-0.5), whereas zirconia abutments and resin nano ceramic abutments with wall thickness 0.5 mm, 0.8 mm and 1.0 mm were prepared as test groups (Zir-0.5, Zir-0.8, Zir-1.0 and RNC-0.5, RNC-0.8, RNC-1.0). All specimens were tested in a universal testing machine to evaluate their resistance to fracture and all of them underwent thermo-cycling before loading test. Mean fracture values of the groups were measured and statistical analyses were made using two-way ANOVA. Results: Zir-1.0 showed the highest mean strength ($2,476.3{\pm}342.0N$) and Zir-0.8 ($1,518{\pm}347.9N$), Ti-0.5 ($1,041.8{\pm}237.2N$), Zir-0.5 ($631.4{\pm}149.0N$) were followed. The strengths of RNC groups were significantly lower compared to other two materials (RNC-1.0 $427.5{\pm}72.1$, RNC-0.8 $297.9{\pm}41.2$) and the strengths of all the test groups decreased as the thickness decreases (P < .01). RNC-0.5 ($127.4{\pm}35.3N$) abutments were weaker than all other groups (P < .05). Conclusion: All tested zirconia abutments have the potential to withstand the physiologic occlusal forces in anterior and posterior regions. In resin nano ceramic abutments, wall thickness more than 0.8 mm showed the possibility of withstanding the occlusal forces in anterior region.

State of problem : The use of zirconium oxide all-ceramic material provides several advantages, including a high flexural strength(>1000MPa) and desirable optical properties, such as shading adaptation to the basic shades and a reduction in the layer thickness. Along with the strength of the materials, the cementation technique is also important to the clinical success of a restoration. Nevertheless, little information is available on the effect of different surface treatments on the bonding of zirconium high-crystalline ceramics and resin luting agents. Purpose : The aim of this study was to test the effects of surface treatments of zirconium on shear bond strengths between bovine teeth and a zirconia ceramic and evaluate differences among cements Material and methods : 54 sound bovine teeth extracted within a 1 months, were used. They were frozen in distilled water. These were rinsed by tap water to confirm that no granulation tissues have left. These were kept refrigerated at $4^{\circ}C$ until tested. Each tooth was placed horizontally at a plastic cylinder (diameter 20mm), and embedded in epoxy resin. Teeth were sectioned with diamond burs to expose dentin and grinded with #600 silicon carbide paper. To make sure there was no enamel left, each was observed under an optical microscope. 54 prefabricated zirconium oxide ceramic copings(Lava, 3M ESPE, USA) were assigned into 3 groups ; control, airborne-abraded with $110{\mu}m$$Al_2O_3$ and scratched with diamond burs at 4 directions. They were cemented with a seating force of 10 ㎏ per tooth, using resin luting cement(Panavia $F^{(R)}$), resin cement(Superbond $C&B^{(R)}$), and resin modified GI cement(Rely X $Luting^{(R)}$). Those were thermocycled at $5^{\circ}C$ and $55^{\circ}C$ for 5000 cycles with a 30 second dwell time, and then shear bond strength was determined in a universal test machine(Model 4200, Instron Co., Canton, USA). The crosshead speed was 1 mm/min. The result was analyzed with one-way analysis of variance(ANOVA) and the Tukey test at a significance level of P<0.05. Results : Superbond $C&B^{(R)}$ at scratching with diamond burs showed the highest shear bond strength than others (p<.05). For Panavia $F^{(R)}$, groups of scratching and sandblasting showed significantly higher shear bond strength than control group(p<.05). For Rely X $Luting^{(R)}$, only between scratching & control group, significantly different shear bond strength was observed(p<.05). Conclusion : Within the limitation of this study, Superbond $C&B^{(R)}$ showed clinically acceptable shear bond between bovine teeth & zirconia ceramics regardless of surface treatments. For the surface treatment, scratching increased shear bond strength. Increase of shear bond strength by sandblasting with $110{\mu}m$$Al_2O_3$ was not statistically different.
Statement of problem: The interest in all-ceramic restorations has increased as more techniques have become available. With the introduction of machinable dental ceramics and CAD/CAM systems there is a need to evaluate the quality levels of these new fabrication techniques. Purpose: This study is to evaluate the crown fidelity(absolute marginal discrepancy and internal gap) of various zirconia-based all-ceramic crowns fabricated with different CAD/CAM(computer-assisted design/computer-assisted manufacturing) systems and conventional cast metal-ceramic crowns. Material and methods: A resin tooth of lower right second premolar was prepared. After an impression was taken, one metal master die was made. Then 40 impressions of metal master dies were taken for working dies. 10 crowns per each system were fabricated using 40 working dies. Metal-ceramic crowns were cast by using the conventional method, and Procera, Lava, and Cerec inLab crowns were fabricated with their own CAD/CAM manufactruing procedures. The vertical marginal discrepancies and internal gaps of each crown groups were measured on a metal master die without a luting agent. The results were statistically analyzed using the one-way ANOVA and Tukey's HSD test. Results: 1. Vertical marginal discrepancies were $50.6{\pm}13.9{\mu}m$ for metal-ceramic crowns, $62.3{\pm}15.7{\mu}m$ for Procera crowns, $45.3{\pm}7.9{\mu}m$ for Lava crowns, and $71.2{\pm}2.0{\mu}m$ for Cerec inLab crowns. 2. The Internal gaps were $52.6{\pm}10.1{\mu}m$ for metal-ceramic crowns, $161.7{\pm}18.5{\mu}m$ for Procera crowns, $63.0{\pm}10.2{\mu}m$ for Lava crowns, and $73.7{\pm}10.7{\mu}m$ for Cerec inLab crowns. Conclusion: 1. The vertical marginal discrepancies of, 4 crown groups were all within the clinically acceptable range($120{\mu}m$). 2. The internal gaps of LAVA, Cerec inlab, and metal-ceramic crowns were within clinically acceptable range except Procera crown($140{\mu}m$).
