THE CLINICAL STUDY ON SHORT TERM PROGNOSIS OF IN-CERAM ALUMINA BRIDGE RESTORATION (In-Ceram Alumina Bridge Restoration의 단기예후에 관한 임상적연구)

  • All ceramic restorations except In-Ceram Alumina system gave a good esthetics and an exellent marginal fidelity. The flexural strength of them had about 150MPa, so the indication is only single crown. By using In-ceram Alumina System(450Mpa), it is thought to be possible to construct bridge for its high flexural strength. But the prognosis is unclear, The purposes of this study are to clear short term prognosis of In-Ceram bridge restorations, to elucidate its clinical significance. Among 22 In-Ceram Bridge restored in our department, 11 In-Ceram bridges with follow up were used. The period of placement is from 1 to 18 months. The results were as follows : 1. Among follow up 11 bridges, 2 bridges were fractured. One is 4 unit in maxillary lateral incisors, the other is 3 unit bridge in maxillary canine and premolar. Including 11 bridge without follow up, failure rate is very low(2/22). 2. The fracture sites are connector areas between abutment and pontic. To maintain In-Ceram bridge for long term period, it is needed to remove the nonphysiologic occlusal force and to have sufficient thickness of alumina core. For estabilishing clinical use of In-Ceram bridges, it is thought to need clinical research during long term period.

Influence of final crystallization process on precision of fit of monolithic CAD/CAM-generated restorations by lithium disilicate: A comparative study (리튬 디실리케이트 최종 결정화 과정이 CAD/CAM으로 제조된 수복물의 적합도에 미치는 영향)

  • Purpose: To quantify the effect of the crystallization process on lithium disilicate ceramic crowns that are fabricated using a computer-aided design/computer-aided manufacturing(CAD/CAM) system, and to determine whether they are clinically acceptable by comparing values before and after the crystallization process. Methods: The maxillary first molar was selected as the abutment for the experiments. Ten working models were prepared. Marginal and internal gap of 4 groups of lithium disilicate crowns(n=10) fabricated with conventional method. Comparison was performed using the silicone replica technique and 3D superimposition analysis. The marginal and internal gaps of the restoration were measured before and after the crystallization process of this prosthesis. The average value of each part(the average of values measured before and after the crystallization) was statistically analyzed using paired t-test(α=0.05). Results: The results from the second phase of this research, which compared the average value of the gap between the marginal and internal fits of the lithium disilicate single crown before and after the crystallization process, indicated that the marginal gap was larger and the internal gap was smaller after the crystallization process, and this difference was statistically significant (P<0.05) in all the parts evaluated. Conclusion: While the shrinkage that occurs during crystallization does affect the marginal and internal fit of the prosthesis, it cannot be concluded to be a major effect because the resultant distortion was within the clinically acceptable range.

  • The implant prosthesis can be divided into the screw retained prosthesis and cement retained prosthesis. Each type has advantages as well as disadvantages which is unfavorable to maintain the implants. To overcome these drawbacks, T-screw system was developed. T-screw system which utilizes a lingual direction of the screw to retain the implant prosthesis, has advantages of retrievability of the prosthesis, passive fit, and possibility to form esthetic and functional occlusal surface. The prior prosthesis which utilized horizontal screws had difficulty in fabrication especially in the case of multiple units, and also limited use with all-ceramic prosthesis. In this case, fabricating the implant prosthesis by using the T-screw system showed superior results in easy maintenance, esthetics, and also functions. In addition, we are to report the method of using the T-screw system in implant prosthesis, such as multiple units of implant prosthesis and all ceramic prosthesis.

Effect of surface treatments and universal adhesive application on the microshear bond strength of CAD/CAM materials

  • PURPOSE. The aim of this study was to evaluate the microshear bond strength (µSBS) of four computer-aided design/computer-aided manufacturing (CAD/CAM) blocks repaired with composite resin using three different surface treatment protocols. MATERIALS AND METHODS. Four different CAD/CAM blocks were used in this study: (1) flexible hybrid ceramic (FHC), (2) resin nanoceramic (RNC), (c) polymer infiltrated ceramic network (PICN) and (4) feldspar ceramic (FC). All groups were further divided into four subgroups according to surface treatment: control, hydrofluoric acid etching (HF), air-borne particle abrasion with aluminum oxide (AlO), and tribochemical silica coating (TSC). After surface treatments, silane was applied to half of the specimens. Then, a silane-containing universal adhesive was applied, and specimens were repaired with a composite, Next, µSBS test was performed. Additional specimens were examined with a contact profilometer and scanning electron microscopy. The data were analyzed with ANOVA and Tukey tests. RESULTS. The findings revealed that silane application yielded higher µSBS values (P<.05). All surface treatments were showed a significant increase in µSBS values compared to the control (P<.05). For FHC and RNC, the most influential treatments were AlO and TSC (P<.05). CONCLUSION. Surface treatment is mandatory when the silane is not preferred, but the best bond strength values were obtained with the combination of surface treatment and silane application. HF provides improved bond strength when the ceramic content of material increases, whereas AlO and TSC gives improved bond strength when the composite content of material increases.

Evaluation of the marginal and internal fit of lithium disilicate crowns fabricated by different scanning methods (다른 스캐닝 방법으로 제작된 리튬 디실리케이트 단관의 변연 및 내면 적합도 평가)

  • Purpose: This study was to compare adaptations of lithium disilicate CAD/CAM crowns fabricated with different scanning systems. Methods: This study selected the mandibular right first molar as an abutment for experiments and produced 10 working models. Lithium disilicate crowns appropriate for each abutment were produced by using a CEREC$^{(R)}$ CAD/CAM system. The independent t-test was then used to compare and analyze the data obtained from the two groups(${\alpha}$=0.05). Results: Mean(SD) adaptation were $86.93(22.82){\mu}m$ for the InS group, $88.42(26.77){\mu}m$ for the ExS group. They were no statistically significant differences between groups for adaptation(p>0.05). Conclusion: Within the limitations of this study, the results suggest that the accuracy of all investigated optical scanner were satisfactory for clinical use. Further assessment and improvement of the lithium disilicate ceramic for the fabrication of FPDs is evidently still required.

Esthetic improvement in patient with deep bite using porcelain laminate veneers and a full veneer crown (과개교합 환자에서 도재 라미네이트 비니어와 전부도재관을 이용한 심미수복 증례)

  • This case is a porcelain laminate veneer (PLV) and all-ceramic crown treatment case on a female patient in her thirties who visited for esthetic improvement. The patient's chief complaint was protrusive and uneven anterior teeth, and she wanted minimum tooth reduction. The patient was in skeletal class II deep bite. Considering this, check bite method was used for mounting on articulator during diagnostic wax-up. Tooth reduction was done using mock-up base on diagnostic wax-up, and the amount of deletion was checked by index. For the accordance of shade, PLVs were restored in prior to all ceramic crown. After using all-ceramic crown, satisfactory esthetic improvement was obtained.

  • As patients' esthetic expectations increase, there is an increase in demand for cosmetic restorations of anterior and posterior teeth that resemble the natural tooth morphology and color. An example of high-strength all-ceramic systems is zirconia with CAD/CAM application. This case report describes a full-mouth rehabilitation using zirconia all-ceramic crowns supported by upper and lower implants and by a minimal increase in the occlusal vertical dimension in a patient with severe abrasion due to loss of posterior teeth.

Clinical convergence angle and rounding radius on tooth preparation for zirconia all-ceramic crown (지르코니아 전부도재관 지대치 삭제시 축면 경사각과 만곡 반경에 대한 조사)

  • Purpose: This article attempted to examine how teeth for restoration is made in a clinical practice and utilize it as future educational material of teeth formation and basic data for additional research. Materials and methods: This experiment investigated the models sent to milling center for production of zirconia crowns. After scanned with Lava CAD/CAM System (3M ESPE, Seefeld, Germany), they are measured on 'ImageJ (version 1.32j, National Institutes of Health, USA)' program and compared and analyzed. Convergence angle from mesio-distal surfaces and bucco-lingual surfaces of each teeth are measured. Also, bucco-lingual diameter of the region lowered as much as 0.4 mm from incisal edge in anterior teeth except canines.(This measure is defined as the Peak 0.4) The analysis of data between each group was conducted by Windows SPSS statistic program, and was proved significant on 95% confidence level by independent t-test, one-way ANOVA and multiple analysis (Sheff${\'{e}}$ test). Results: The mean value of convergence angle was $18.67^{\circ}$ It is ranked as molar ($26.70^{\circ}$) > premolar ($16.87^{\circ}$) > anterior teeth ($14.81^{\circ}$) in the order of mesio-distal convergence angle; anterior teeth ($22.32^{\circ}$) > molar ($20.93^{\circ}$) > premolar ($15.41^{\circ}$) in the order of bucco-lingual convergence angle. The mean value of Peak 0.4 was 1.18 mm. Conclusion: Convergence angle of abutment of zirconia all ceramic crown has difference depending on the location in the arch. Due to the nature of production of zirconia all ceramic crown, convergence angle of abutment and line angle finishing degree can have an effect on internal suitability of restoration.

THE CONSTRUCTION METHOD OF IN-CERAM SPINELL CROWN AND ITS CLINICAL APPLICATION (In-Ceram Spinell Crown의 제작법과 임상적 응용)

  • By the concerns of esthetic restoration were increased recently. many all ceramic crowns were developed. But they except In-Ceram Alumina were used only single crown. In-Ceram Alumina, developed by Dr. Sadon, was revealed to have high flexural Strength(450MPa). So it could be used not only anterior bridges but also posterior bridges. But In-Ceram Alumina was seen to be opaque, a little green color in transillumination light by high content of alumina oxide(85%). So new all ceramics with high strength and high translucence were needed. Spinell($MgAl_2O_3$) have a high melting point, high flexural strength, low heat conductivity, high light conductivity. In-Ceram Spinell offers glasslike light transmission by using the spinell cores instead of the alumina cores. And they have a high translucency like to natural tooth, an excellent margin integrity and a high strength(350MPa). The purposes of this study are 1) to know about the construction method of In-Ceram Spinell System, 2) to investigate the its clinical possibiliy through patients and literature reviews.

Effects of Surface Treatments of The Zirconium-Based Ceramic on the Bond Strength of Resin Cement (지르코니움 세라믹에서 표면 처리 방법이 레진 시멘트의 접착력에 미치는 영향)

  • Recently, the need for esthetic results has increased the interest for all-ceramic crown prosthesis. Furthermore, the development of zirconium core via CAD/CAM system has allowed the all ceramic restorations to be applied to almost all fixed prosthesis situations. But, the increased strength has been reported to increase in proportion with the bond strength of cement, and recently, the tribochemical system which increases the bond strength through, silica coating and silanization has been introduced. The purpose of this study was to compare the $Rocatec^{TM}$ system and $CoJet^{TM}$ system with the traditional acid etching and silanization method of the irconium based ceramic. The surface character was observed via SEM(X2000), and the bond strength with the resin cement were measured. 50 In-Ceram Zirconia (Adens, Korea) discs were fabricated and embedded in resin, group 1 was treated with glass-bead blasting and cleaning, group 2 was treated with 20% HF for 10 minutes and silanized, group 3 was treated with the $Rocatec^{TM}$ system, and group 4 was treated with the $CoJet^{TM}$ system. Each group was comprised of 10 specimens. The specimens were cemented to a $3mm{\times}5mm$ resin block with Super-Bond C&B. The shear bond strength was measured with the $Instron^{(R)}$ 8871 at a crosshead speed of 0.5mm/min. The results were as follows. 1. According to SEM results, there were little difference between group 1 & group 2, but in group 3 and 4, silica coating was detected and there was increase in surface roughness. 2. The shear bond strength decreased in the order of group 3(46.28MPa), group 4(42.04MPa), group 2(31.56MPa), and group 1(27.46MPa). 3. There was significant differnce between group 1&2 and group 3&4(p<0.05). From the results above, it can be considered that the conventional method of acid etching and silane treatment cannot increase the bond strength with resin cements, and that by applying the tribochemical system of $Rocatec^{TM}$ system and $CoJet^{TM}$ system, we can achieve a stronger all ceramic restoration. Further studies on surface treatments to increase the bond strength are thought to be needed.
