Is productive welfare possible in Korea? (대한민국 과연 생산적 복지가 가능한가?)

  • Productive welfare is a form of welfare policy that helps welfare recipients move beyond being mere objects of support and develop into members who can actively contribute to society and the economy. This aims to improve individual self-reliance, including education, vocational training, and employment support services, ultimately reducing the economic burden on society as a whole and promoting economic growth. By examining whether productive welfare is possible in Korea, this study emphasizes the role and importance of productive welfare as a solution to social and economic problems. The Republic of Korea has experienced various social problems along with rapid economic growth. The entry into an aging society, increased youth unemployment, and widening social gaps have created complex and diverse social welfare needs. In this situation, productive welfare is attracting attention as a method that goes beyond simple financial support and provides a foundation for beneficiaries to become self-reliant. This study seeks to present a new horizon for social welfare policy by examining the possibility of implementing productive welfare in Korea and exploring ways to achieve it.

Comparative analysis of food intake according to the family type of elderly women in Seoul area (서울 일부지역 여자 노인들의 가구유형에 따른 영양소 섭취실태 및 식사의 질 평가)

  • Purpose: As the rate of senior citizens living alone increases in the current aging society, there is much concern regarding the health and nutritional intake of solitary senior citizens. Therefore, this study compared the nutritional intake of senior citizens according to their family type. Methods: In July and August of 2011, two senior citizen welfare centers in Seoul were visited to survey 267 elderly women. Excluding 54 subjects for which the data were incomplete, information from 213 subjects was analyzed. The subjects were divided into three family types, living alone (LA, n = 74), living with spouse (LS, n = 78), and living with children (LC, n = 61). Results: The mean age of the LA group was the highest, while the mean age of the LS group was the lowest (p < 0.001), and WHR of the LC group was the highest (p = 0.049). Income was the highest in the LS group (p < 0.001). Frequency of eating out was the lowest in the LA group (p = 0.031). By Duncan's multiple analysis, the amounts of energy intake, vegetable protein, fat, calcium, phosphorus, potassium, selenium, Vit D, Vit E, $Vit\;B_2$, niacin, $Vit\;B_6$, $Vit\;B_{12}$, and cholesterol were significantly higher in the LS group compared with the LA or LC group (p < 0.05). The intakes of calcium, Vit D, $Vit\;B_{12}$, and cholesterol were still significantly different among the three groups, even after adjustment for age and monthly income. The LA group ate less fruit and fish than the LS or LC group (p < 0.05). The LA group showed the lowest dietary diversity and the LS group showed the highest diversity (p = 0.014), however, the significance of dietary diversity score among the three groups disappeared after adjustment for age and monthly income. Conclusion: Elderly women living with spouse were receiving better nutrition than elderly women living alone or living with children. Therefore, solitary elderly women who do not live with their spouse or children should be offered greater opportunities to receive a balanced meal at a congregational kitchen or welfare center. To ensure their healthy diet, it is essential to provide continuous nutrition education with these groups in mind.

The Effects of Family Friendship on the Elderly's Consciousness: A Study on the Effects of Mediation on the Recognition of the Elderly and the Attitude to Dementia (청소년이 지각한 가족친밀감이 노인부양의식에 미치는 영향: 노인인식과 치매에 대한 태도의 매개효과 검증)

  • This study is to verify the mediated effects of attitudes toward old people and dementia in the influence of elderly couples in the aging society amid the rapidly changing family structure and functions due to the combination of individualization, marital status and divorce rate. In order to carry out such research purposes, data were collected from students of elementary, middle and high schools in Gwangju, through self-subscribed questionnaire. For statistical analysis, the SPSS 20.0 and AMOS 18.0 programs were used and frequency, percentages, technical statistics, correlation, factor analysis, structural model validation, and the Sobel-Test were performed. The results of this study are as follows. First, family intimacy, elderly awareness, and elderly care were the highest among elementary school students, followed by middle school and high school students (P.<.001). Also, in religion, the family intimacy of teenagers with religion was higher than those without religion (p.001). Second, family intimacy directly affected elderly people's attitudes toward dementia and elderly care, old people's attitudes toward dementia and attitudes toward dementia directly affected elderly care. Third, family intimacy (parent-child) was found to be 7.8% for older adults, 20.2% for family intimacy and attitudes toward dementia, and 34.1% for elderly care (p.<.001). Fourth, it has been verified that the absolute value of attitudes toward dementia and elderly people's awareness of elderly people and attitudes between family intimacy and elderly care has been higher than 1.96 and thus acts as a mediating role. These findings are intended to contribute to the welfare of senior citizens' education to improve the quality of life for senior citizens through the resolution of conflicts between generations, as well as the resolution of positive stimulus, by developing various programs such as family friendship, elderly awareness, culture with parents, and various experiences to improve attitudes toward dementia in early adolescence.

A Study for Usage Patterns and Recognition toward Korean Medicine in Korea High School Students (고등학생의 한의약 의료 이용 형태 및 인식에 관한 연구)

  • Objectives : This study is to research on usage patterns, recognition of system to Korean medicine among high school students who are the potential future medical consumer. Methods : Based on the questionnaire used by KIHASA(Korea Institute for Health and Social Affairs) in 2011 and KHIDI(Korea Health Industry Development Institute) in 2014, modifications were made to make questionnaire more suitable for this research. This self-administered questionnaire was conducted among 1,025 highschool students of 3 high schools in Ulsan city on september, 2015. Except 19 cases which considered statistically insignificant or with unreliable responses, 1,007 cases were analyzed by p-paired test and ANOVA test through the SPSS 22.0 for Win program. Results : Most of the high school students(66.5%) do not have an experience on Korean medical care. The number of students' first experience on Korean medical care is the largest during their elementary school (34.9%), which means the treatment was mostly for restorative reasons like growth care. The later the time of their first experience on Korean medical care, the higher ratio was shown on their recent utilization of Korean medical care. Korean medicine was rarely covered in most cases of public health education at school, and this public health education and usage pattern on Korean medicine have positive correlation. Compared 'Group who have used Korean medical care in 1 years except recent 3 months' to the 'Group who have used Korean medical care in 3 recent months', the former showed higher recognition in Korean medical care and insurance system. Conclusions : The proportion of Korean medicine is as low as 4.5% to whole medical market, and most of the consumers are senior. In addition, the aging phenomenon of Korean medical consumer have been continued. This specialized study on usage patterns and recognition among youth can be the basic research data which can be used for promotion, education and establishment of health policy and health care system.

A Study on the Job Analysis of Assistive Product Specialist (복지용구 전문가 직무분석에 관한 연구)

  • As Korea is now an aging society, the demand and necessity of assistive products for senior citizens are increasing. On the other hand, their satisfaction with the assistive-product payment service is pretty low, so that the necessity of assistive-product specialists is newly rising. Therefore, it is important to establish a frame of assistive product specialist and clarify the duties. This study aimed to define and analyze the job of assistive-product specialists. This study was carried out from June 1, 2015 to February 10, 2016. After the literature consideration and analysis of the current status, a job analysis(plan) of assistive product specialists was drawn after the job of assistive product specialists was defined, and the duties, tasks, and task elements were analyzed by experts through an advisory conference. A Delphi method was conducted with five professional panels to verify the validity. For the job contents of assistive product specialists, 5 duties, 12 tasks, and 23 task elements were identified. The significance of this study was to establish the system and standard for assistive device specialists defining and analyzing the job of assistive product specialist. Through this study, it would be possible to secure the professionalism of an assistive-product specialist and qualitative improvement of an assistive-product payment service.

A Study on the Prediction Model of the Elderly Depression

  • Purpose: In modern society, many urban problems are occurring, such as aging, hollowing out old city centers and polarization within cities. In this study, we intend to apply big data and machine learning methodologies to predict depression symptoms in the elderly population early on, thus contributing to solving the problem of elderly depression. Research design, data and methodology: Machine learning techniques used random forest and analyzed the correlation between CES-D10 and other variables, which are widely used worldwide, to estimate important variables. Dependent variables were set up as two variables that distinguish normal/depression from moderate/severe depression, and a total of 106 independent variables were included, including subjective health conditions, cognitive abilities, and daily life quality surveys, as well as the objective characteristics of the elderly as well as the subjective health, health, employment, household background, income, consumption, assets, subjective expectations, and quality of life surveys. Results: Studies have shown that satisfaction with residential areas and quality of life and cognitive ability scores have important effects in classifying elderly depression, satisfaction with living quality and economic conditions, and number of outpatient care in living areas and clinics have been important variables. In addition, the results of a random forest performance evaluation, the accuracy of classification model that classify whether elderly depression or not was 86.3%, the sensitivity 79.5%, and the specificity 93.3%. And the accuracy of classification model the degree of elderly depression was 86.1%, sensitivity 93.9% and specificity 74.7%. Conclusions: In this study, the important variables of the estimated predictive model were identified using the random forest technique and the study was conducted with a focus on the predictive performance itself. Although there are limitations in research, such as the lack of clear criteria for the classification of depression levels and the failure to reflect variables other than KLoSA data, it is expected that if additional variables are secured in the future and high-performance predictive models are estimated and utilized through various machine learning techniques, it will be able to consider ways to improve the quality of life of senior citizens through early detection of depression and thus help them make public policy decisions.

A study on the change of complication incidence rate according to introduction of quality evaluation by the DRG payment -focussing on patients with lens surgery (질병군 포괄수가 적정성 평가 도입에 따른 합병증 발생률 변화에 대한 연구 -수정체 수술 환자를 대상으로)

  • The purpose of this study is to investigate incidence rates of complications in response to the introduction of quality evaluation of the DRG(diagnosis related group) payments, focusing on an increasing number of patients with lens surgery as the population aging increases.Fourthly, there were three dependent variables ('vitreous prolapse', 'IOP elevations', and 'other complications') in this study, and therefore multivariate logistic regression was performed. The result of the analysis indicates that as the number of hospitalized days increased, vitreous prolapse decreased to 0.27 times(95% CI 0.08~1.00) and IOP elevation decreased by 0.14 times(95% CI 0.03~1.59), compared to other complications, and this was statistically significant. From the above results, this study is meaningful in that it has compared the evaluation results of the appropriateness of DRG payment and the medical quality for lens surgery complications patients, in response to the introduction of quality evaluation by DRG payment.

  • Our country has become an aging society with unparalleled speed, compared to other advanced countries. Many of the elderly are physically, economically, and psychologically dependent; this dependence is a factor that increases the cost burden not only on the children and relatives of the elderly, but also at the social level. Under the prevailing environment of the family-oriented informal support system, the burden of support for elderly parents who rely on limited resources is highly likely to lead to abuse. Until now, dealing with the abuse of senior citizens has been mostly a one-off approach through media rather than academics. Therefore, this study attempts to take an objective approach to the elder abuse problem (which has so far been dealt with in the media as a form of domestic violence) in order to grasp the specificity and factors of elder abuse and to present the Korean situation with regard to elder abuse. In addition, this study seeks to find ways to prevent elder abuse in formal and informal ways.

  • Today, according to population decrease and aging of the old due to the decline of birthrate in our society in $21^{st}$ century, there is a stiff increase of the aged group. Complying with the request of epic needs, as there was a full scale amendment of law of the aged welfare in 1997, the centers appeared differently based on the regional dimension, the difference of the number of persons to be accommodated, and the social and economic characteristics. Coping with these problems occurring, along with the necessity of welfare facilities for the leisure of the aged, the government prepared and carried out a number of plans for the service of the aged and securing of facilities. This practice of the government, however, centered on the crease of service in quantity other than the quality, resulting in a situation of lacking the welfare facilities for the aged meeting the requirement of the aged in their aspect of consumer. On the contrary, building the welfare centers in Japan started in 1960 with a rapid increase of the number of centers in 1970s, and there were 2,214 welfare centers in operation in Japan in 1995, keeping a high level of service in quality and quantity. Thus, this study aims to present basic materials necessary for the criteria of space designing plans for the aged welfare centers in Korea through case analysis of the facility criterion, area constitution and space constitution by the floor that play an important role in regional welfare facilities for the aged in Japan.

Effect of Website Quality on Silver Portal Adoption: Focused on Perception of Potential Users Who Are Over 40 Years Old (웹품질요인이 실버포탈 수용에 미치는 영향에 관한 연구: 40세 이상의 잠재 실버포탈 이용자의 인식도를 중심으로)

  • Senior Internet users have been tremendously increasing for the past few years in line with increase in the aging populations, and silver portals began to provide some sorts of services to them in Korea. Thus, this paper aimed at identifying Web quality factors of silver portal for its successful adoption, and examining the moderating effects of gender on the relations between Web quality factors and intention to use. Information quality, system quality, service quality and entertainment quality were identified as Web quality factors of silver portal based on the intensive review on prior researches. A total of surveys 525 from the potential users of silver portal, who are over 40 years old, were analyzed to verify our research model. The findings show that information quality, system quality, and entertainment quality are related to intention to use whereas service quality has no relationship with intention to use. It is found that gender moderates the relationship between entertainment quality and intention to use.
