Prediction of Maximal Oxygen Uptake Ages 18~34 Years (18~34 남성의 최대산소 섭취량 추정)

  • The purpose of this study is to predict VO2max with body index and submaximal metabolic responses. The subjects are consisted of 250 male aging from 18 to 34 and we separated them into two groups randomly; 179 for a sample, 71 for a cross-validation group. They went through maximal exercise testing with Bruce protocol, and we measured the metabolic responses in the end of the first(3 minute) and second stage(6 minute). To predict VO2max, we applied multiple regression analysis to the sample with stepwise method. Model 1's variables are weight, 6 minute HR and 6 minute VO2(R=0.64, SEE=4.74, CV=11.7%, p<.01), and the equation is VO2max(ml/kg/min)= 72.256-0.340(Weight)-0.220(6minHR)+0.013(6minVO2). Model 2's variables are weight, 6 minute HR, 6 minute VO2, and 6 minute VCO2(R=0.67, SEE=4.59, CV=11.3%, p<.01), and the equation is VO2max(ml/kg/min)= 68.699-0.277(Weight) -0.206(6minHR)+0.020(6minVO2)-0.009(6minVCO2). And the result did not show multicolinearity for both models. Model 2 demonstrated more correlation compared to Model 1. However, when we conducted cross-validation of those models with 71 men, measured VO2max and estimated VO2 Max had statistical significance with correlation (R=0.53, 0.56, P<.01). Although both models are functional with validity considering their simplicity and utility, Model 2 has more accuracy.

Humeral Head Decentralization of Preoperative Magnetic Resonance Images and the Treatment of Shoulder Dislocations in Large to Massive Rotator Cuff Tears in Elderly over 65 Years Old (65세 이상 고령의 회전근 개 대파열 및 광범위 파열에 동반된 견관절 탈구의 치료 및 술 전 자기공명영상의 상완골두 탈중심화)

  • Purpose: This study analyzed the features of humeral head decentralization in large to massive rotator cuff tears with a shoulder dislocation in the elderly. Moreover, shoulder instability and treatment were reviewed. Materials and Methods: From May 2005 to February 2017, Group A containing 45 cases (45 patients) over 65 years old accompanied by a large or massive rotator cuff tear with a shoulder dislocation and Group B containing 45 cases (45 patients) without a shoulder dislocation were enrolled. The mean ages in Groups A and B were 73.2 and 72.1 years old, and the mean follow-up periods were 30.7 and 31.3 months, respectively. Twenty-one cases (46.7%) in Group A underwent rotator cuff repair, and 8 cases (17.8%) underwent concomitant rotator cuff repair with Bankart repair. Sixteen cases (35.6%) underwent reverse total shoulder arthroplasty for cuff tear arthropathy. 45 cases (100%) in Group B underwent rotator cuff repair. The off-the center and head elevation were measured in the preoperative magnetic resonance imaging (MRI) of Groups A and B. The preoperative and postoperative visual analogue scale (VAS) score, American Shoulder and Elbow Surgeons (ASES) score, and University of California Los Angeles (UCLA) score in Groups A and B were compared. Results: In Groups A and B, the mean off-the centers were posterior 7.41 mm and posterior 2.02 mm (p=0.03), and the mean head elevations were superior 6.66 mm and superior 2.44 mm (p=0.02), respectively. The mean ASES scores of Groups A and B were 32.8 and 33.4 before surgery, and 77.1 (p=0.02) and 78.1 (p=0.02) after surgery (p=0.18), respectively. The mean UCLA scores of Groups A and B were 13.1 and 12.8 before surgery, and 28.9 (p=0.02) and 29.5 (p=0.01) after surgery (p=0.15), respectively. Conclusion: Patients over 65 years old with a shoulder dislocation in large to massive rotator cuff tears had higher off-the center and head elevation on the preoperative MRI than those without a shoulder dislocation. This measurement can help predict preceding shoulder instability. Early rotator cuff repairs should be performed and other treatments, such as Bankart repair and reverse total shoulder arthroplasty, should also be considered.

The sexual dimorphism of Mauremys reevesii (남생이의 성적 이형성)

  • In this study, a total of 21 parts of the head, carapace, and plastron of 21 tortoises were examined to investigate the characteristics of male and female sexual dimorphism of the Reeve's turtle (Mauremys reevesii), a natural monument in Korea. The ratio values of a total of 19 areas were derived from Head Length (HL) and Carapace Length (CL). As a result of comparing the ratio values, it was found that male tortoises had larger head length, head width, and head height than females compared to the size of the carapace. Since females grow eggs inside the body through internal fertilization after mating, female carapaces are judged to be larger than males overall. As a result of statistical analysis of the ratio values of the measured values, Plastron Length (PL), Bridge Length (BL), Gular Length (GL), Interhumeral Length (IH), Interanal Length (IAN), Femoral-Anal seam (FA) and Head Height (HH) showed statistically significant differences. Statistically significant in the part of the plastron, it is judged that the tortoise acts directly on each deck of the plastron as it grows. In addition, the head size of male tortoises was smaller than that of females, but it was found to be larger in proportion. It is considered that the tortoise has a rather large head compared to the body due to the male's habit of shaking his head and courtship behavior before mating. This study, we were able to identify the characteristics of sexual dimorphism in Reeve's turtle. However, since most of them were rescued, it was not possible to accurately determine habitat and age. In the future, if additional studies are conducted on individuals with clear habitats and ages, it is expected that the characteristics of the physical differences that develop as they grow can be identified.

A Study on the Production Performance of Korean Native Chickens for Samgye Chicken Production (삼계용 토종닭의 생산능력 고찰)

  • This study aimed to develop a high-productivity breed of Korean native Samgye chicken. We evaluated the production performance of six Korean native chicken combinations (KNC-SCYC, SCYD, SDYC, SDYD, SYYC, SYYD) and GSP-Hanhyup Korean native chickens, with Baeksemi chickens used as a control group. The performance test was conducted from hatching to 7 weeks of age on 756 chickens, and we measured survival rate, body weight, shank length, feed utility, and carcass yield. The overall survival rate was nearly 100% for all strains. However, body weight showed significant differences between strains at all ages (P<0.01), with Baeksemi weighing 863.8±76.9 g, GSP-Hanhyup weighing 804.7±72.5 g, and KNC-combinations weighing 543.0±61.8 g at 5 weeks of age. The duration needed to reach 850 g was estimated to be 34.5 days for Baeksemi, 37.5 days for GSP-Hanhyup, and 45.8-48.8 days for KNC-combinations. Carcass yield percentage was highest for KNC-SYYD combination at 63.3%, followed by Baeksemi at 60.4%, and GSP-Hanhyup at 56.1%. Shank length at 850 g body weight was 7.6 cm for KNC-SYYD combination, 7.8 cm for Baeksemi, and 8.0 cm for GSP-Hanhyup. The feed conversion ratio at 850 g body weight was 1.81 for Baeksemi, 2.17 for GSP-Hanhyup, and 2.27 for KNC-SCYC combination. Our results suggest that the KNC-SYYD combination and GSP-Hanhyup breed have the potential to be used in Samgye production due to their moderate growth performance, higher carcass yield, and shorter shank length, despite their lower growth productivity and feed efficiency when compared to Baeksemi.

A Inquiry of Zhang Bo-duan's Writings (장백단(張伯端)의 저술고(著述考))

  • Zhang Bo-duan compiled about internal alchemy in Taoism. Although he lived in the mundane world, he wished to seek theory on neidan of Taoism(internal alchemy). After finding enlightenment, he elucidated that the enlightenment was a state of rising above world not needed to leave the world. After ages, he was admired as the founder of Taoism in Southern school and his Oh Jin Peon which contents internal alchemy was considered seriously to have more than 30 people who annotated with it until Ch'ing Empire. At his age of 80, he met the real person who gave him theory on neidan of Taoism(internal alchemy), its preface tells that he organized its main point, and then wrote Oh Jin Peon with it in 1075. Generally Zhang Bo-duan was known to leave three books as Oh Jin Peon, Guem Dan Sa Baek Ja, and Cheung Hwa Bi Mun, most of critics have been studying on the basis of them. However, it is not correct whether all of them is his writings and there is not exact analysis but simple belief about it. I think accuracy and details are indispensible in philosophical approach. The study not having verification about primary data is no more than a visionary projet which soon collapses. So the purpose of this study is adding the detail analysis on it and making its exact basis of philosophical approach. Zhang Bo-duan over his age of 80, became enlightened, in his old age handed down his student the secret as a record and theory on neidan of Taoism(internal alchemy). And not in his living but after his dying his status was soared. Because of his high status in internal alchemy Taoism, it seems that there are more interest in it and some published books which just leave his name. In this study, I accept Oh Jin Peon as a his real writing among unsure his writings and criticize systematically and classify its characteristics. And I demonstrate that Guem Dan Sa Baek Ja, Cheung Hwa Bi Mun couldn't be his real writings, these could be forgeries by posterity, with proposing some basis of the argument.

Safety and Efficacy of Gemcitabine Based Chemotherapy in Elderly Patients with Advanced Pancreatic Cancer - Safety of Gemcitabine in Elderly (진행성 췌장암을 가진 노인환자에서 Gemcitabine 항암화학요법의 안정성과 효과 - 노인에서 Gemcitabine의 안정성 -)

  • Background/Aims: Gemcitabine is regarded as a reference regimen for advanced pancreatic cancer and shows relatively safe toxicity profiles compared with other cytotoxic agents. However, many oncologists are appeared to be still reluctant to treat elderly pancreatic cancer patients with cytotoxic chemotherapy because of predicted low response rate and potential adverse events. Methods: All patients who were received gemcitabine based chemotherapy between 2007 and 2010 were identified and clinical, laboratory, radiographic data were retrospectively reviewed. Patients were divided into two groups based on their ages: less than 65, and equal or more than 65 years old. Gemcitabine, at a dose of 1,000 mg per square meter of body surface area, was administered by intravenously over 30 minutes weekly for 3 weeks followed by 1 week rest, alone or along with other chemotherapeutic agents including cisplatin, capecitabine and erlotinib. Results: A total of 61 patients were identified and all patients were not eligible to receive operation because of advanced stage at diagnosis. Twenty three patients (37.7%) were equal or more than 65 year of age. Mean age was 56 years old and 71 years old in each group. Laboratory data including CA 19-9 were not significantly different. More gemcitabine monotherapy was delivered (56.5% vs. 26.3%, p=0.029) and less second or third line therapy was adminis- tered (17.4% vs. 50.0%, p=0.014) in elderly group. Cholangitis occurred and stent placement were performed similarly in both groups. Conclusion: Gemcitabine based chemotherapy can be administered safely to elderly pancreatic cancer patients and comparable response rate and progression free survival can be expected as young patients.

The Objective and Perceived Level of Economy and Its Relationship with Mother's Mental Health, Parenting Behaviors, and Problem Behaviors in Preschoolers (실제 경제수준과 지각된 경제수준 간 차이에 따른 어머니의 정신건강, 양육행동 및 유아의 문제행동 비교)

  • This study investigated the objective and perceived level of economy and its relationship with mother's mental health, parenting behaviors, and problem behaviors in preschoolers. Data drawn from mothers and teachers of 238 preschoolers, ages 3-5, were used. Using the objective and perceived level of economy, subjects were classified into four groups. Individuals categorized as high by objective and perceived level of economy, low by objective and perceived level of economy, high by objective and low by perceived level of economy, and low by objective and high by perceived level of economy were categorized as in-agreement/high group, in-agreement/low group, over-estimator group, and under-estimator group, respectively. To investigate the difference between groups, one-way ANOVAs and post-hoc analyses were used. The results showed that in-agreement/low group and under-estimator group showed greater depression and anxiety than in-agreement/high group and over-estimator group. In addition, in-agreement/high group and over-estimator group showed greater affective parenting behaviors and lower aggressive parenting behaviors than in-agreement/low group and under-estimator group. In terms of internalizing problem behavior by mother, in-agreement/high group and over-estimator group were lower than under-estimator group. In terms of externalizing problem behavior by mother, in-agreement/high group was lower than under-estimator group. However, in terms of internalizing and externalizing problem behavior by teacher, there was no difference between groups. In externalizing problem behavior by teacher, in-agreement/low group was lower than over-estimator group.

Psychological, relational and financial resources: How do they influence happiness among Koreans (심리적, 관계적, 경제적 자원: 한국인의 행복에 어떠한 영향을 미치는가?)

  • Can money buy happiness? If not, what are the factors that influence happiness? What makes people happy? What are the factors that influence happiness among Korean adults? How can we better explain factors that influence happiness? These are the research questions that led to the analyses of psychological, relational, financial resources that influence happiness. To examine differences in socio-economic status, adults and elderly from three different districts that vary wealth and income were interviewed in their own home. A total of 313 respondents (male=133, female=180) between the ages of 20 through 80 completed a questionnaire that contained background information, emotional support scale and happiness scale developed by the present researcher and resiliency of efficacy developed by Bandura(1995). The results are as follows. First, monthly income influence happiness to some extent, but when the variable was excluded from the path analysis, the goodness-of-fit did not change significantly. Although those who have more money can be happier, those without much money can also be happy. These results indicate that financial resource has limited influence on happiness. In addition, monthly income did influence self-efficacy of respondents indicating that those with more money were not necessarily more confident about themselves. Second, an important factor influencing happiness is the relational resource. Emotional support was the most powerful predictor of happiness, four to five times more important than monthly income. Third, self-efficacy influenced happiness. Those respondents with higher resiliency of efficacy had higher happiness scores and the influence was two times greater than monthly income. Moreover, self-efficacy played a mediating role between emotional support and happiness. Fourth, those respondents with higher occupational achievement reported higher happiness score and the influence was two times greater than monthly income. Fifth, success of children influenced happiness score and the influence was 1.5 times greater than monthly income. Sixth, education did not directly influence happiness, but had an indirect influence through self-efficacy and occupational achievement. Seventh, age was not related to self-efficacy and happiness.

A Study on the Support Method for Activate Youth Start-ups in University for the Creation of a Start-up Ecosystem: Focused on the Case of Seoul City (지역 청년창업생태계 조성을 위한 대학의 지원방안 탐색: 서울시 사례를 중심으로)

  • The purpose of this study was to analyze the perception and demand of local youth and to find ways to support universities in order to create an youth start-up ecosystem. To this end, 509 young people living in Seoul were analyzed to recognize and demand young people in the region for youth start-ups, and to support universities. The findings are as follows. First, as a result of analyzing young people's perception of youth start-ups in the region, the "Youth Start-up Program" was analyzed the highest in terms of the demand for regional programs by university. In addition, there was a high perception that the image of youth startups in the region was "challenging" and "good for changing times." Second, after analyzing the demand for support for youth start-ups in the region, it appeared in the order of mentoring, start-up education, and creation of start-up spaces. And it showed different needs for different ages. Third, the results were derived from analysis of the demand for university support for the creation of a regional youth start-up ecosystem, the criteria for selecting local youth start-up support organizations, and the period of participation in local youth start-up support. Based on the results of the above research, the implications and suggestions of university support for the creation of a community of youth start-up ecosystem are as follows. First of all, it is necessary to develop and operate sustainable symbiosis mentoring programs focusing on university's infrastructure and regional symbiosis. Second, it is necessary to develop and utilize step-by-step systematic microlearning content based on the needs analysis of prospective youth start-ups. Third, it is necessary to form an open youth start-up base space for local residents in universities and link it with the start-up process inside and outside universities. The results of this study are expected to be used as basic data for establishing policies for supporting youth start-ups and establishing and operating strategies for supporting youth start-ups at universities.

The Effects of the Revised Elderly Fixed Outpatient Copayment on the Health Utilization of the Elderly (노인외래정액제 개선이 고령층의 의료이용에 미친 영향)

  • Background: In January 2018, revised elderly fixed outpatient copayment for the elderly were implemented. When people ages 65 years and older receive outpatient treatment at clinic-level medical institutions (clinic, dental clinic, Korean medicine clinic), with medical expenses exceeding 15,000 won but not exceeding 25,000 won, their copayment rates have decreased differentially from 30%. This study aimed to examine the changes of health utilization of elderly after revised elderly fixed outpatient copayment. Methods: We used Korea health panel data from 2016 to 2018. The time period is divided into before and after the revised elderly fixed outpatient copayment. We conducted Poisson segmented regression to estimate the changes in outpatient utilization and inpatient utilization and conducted segmented regression to estimate the changes in medical expenses. Results: Immediately after the revised policy, the number of clinic and Korean medicine outpatient visits of medical expenses under 15,000 won decreased. But the number of clinic outpatient visits in the range of 15,000 to 20,000 won and Korean medicine clinic in the range of 20,000 to 25,000 won increased. Copayment in outpatient temporarily decreased. The inpatient admission rates and total medical expenses temporarily decreased but increased again. Conclusion: We confirmed the temporary increase in outpatient utilization in the medical expense segment with reduced copayment rates. And a temporary decrease in medical expenses followed by an increase again. To reduce the burden of medical expense among elderly in the long run, efforts to establish chronic disease management policies aimed at preventing disease occurrence and deterioration in advance need to continue.
