Potentially Inappropriate Prescriptions of Antibiotics in Patients with Acute Rhinosinusitis in Ambulatory Settings in South Korea (외래 급성 비부비동염 환자의 잠재적으로 부적절한 항생제 사용)

  • Background: Acute rhinosinusitis (ARS) is a common condition encountered in ambulatory practice and is one of the most common reasons for antibiotic prescriptions. This study aimed to evaluate the potentially inappropriate antibiotic prescribing for ARS in South Korea and identify influencing factors. Methods: We analyzed Health Insurance Review and Assessment Service-National Patient Samples data. We selected outpatients aged 20 to 64 with ARS, prescribed antibiotics between February and November 2020. Potentially inappropriate antibiotic prescribing was categorized as: 1) inappropriate antibiotic selection and 2) inappropriate antibiotic dosage or duration. Multiple logistic regression was conducted to estimate the impact of various factors on inappropriate antibiotic prescribing. Results: Of 1,210 patients, 80.83% received potentially inappropriate ARS antibiotic prescriptions. Inappropriate antibiotic selection accounted for 43.55%, and inappropriate antibiotic dosage and duration contributed to 37.28%. Otolaryngologists had higher odds ratio (OR) of potentially inappropriate antibiotic prescribing compared to internal medicine practitioners, while dentists had lower OR. Patients aged 20 to 29 years had a higher OR than other age groups, and those who visited primary care clinics had a higher OR than those who visited hospitals. Conclusion: Potentially inappropriate antibiotic prescribing for ARS is prevalent in South Korea. This study identified physician specialty, patient age group, and the level of healthcare facility as factors influencing potentially inappropriate antibiotic prescriptions. Addressing this issue through targeted interventions, such as improved guidelines adherence and patient education, is imperative to mitigate the risks associated with antibiotic misuse and antibiotic resistance.

Prevalence Rate and Associated Factors of Urinary Incontinence among Nursing Home Residents (노인요양시설 거주노인의 요실금 유병률과 관련요인)

  • Purpose: The aim of this study was to estimate the prevalence of urinary incontinence among nursing home residents and to identify the factors associated with this condition. Method: The data were collected from 618 subjects (146 males and 472 females; mean age $79.9{\pm}8.4$ yr; range 65-102 yr) of 30 nursing homes in Seoul, Gyeonggi-do and Gangwon-do in this cross-sectional study. The data were analyzed by chi-square test, t-test, and multiple logistic regression by using the SPSS/PC ver 12.0 program. Results: The prevalence of UI was 64.7% (64.6% in women; 65.1% in men). After adjustment for each of the variables considered in this study, six potential factors were strongly associated with UI: activities of daily living, comorbidity, age, cognition, specialty of the facility, and a bladder training program. Conclusion: Our finding suggests that it is necessary to develop a program for promotion of activities of daily living and to provide a bladder training program to prevent urinary incontinence among nursing home residents.

Utilization Characteristics and User Satisfaction by Type of Physical Therapy Service in Senior Centers

  • Purpose: This study was conducted to identify the characteristics of physical therapy service utilization and user satisfaction depending on the experience with therapeutic exercise or ultrasound intervention among elderly persons using senior centers. Methods: The subjects were adults aged over 60 years (total 215) recruited in 40 senior centers located in each province in South Korea. Subjects responded to questions concerning overall demographics factors, utilized characteristics of physical therapy service and 12 variety user satisfactions with effectiveness, facilities, and therapist using a survey instrument. The collected data were analyzed by Fishers' exact tests and t-tests using the SPSS 21.0 program to compare the results of elderly persons who had or had not experienced exercise therapy or ultrasound therapy. Results: The participants that experienced ultrasound therapy or therapeutic exercise reported significantly higher overall results pertaining to effectiveness of physical therapy and a clear explanation from physical therapist's satisfaction than those who were non-experienced. Satisfaction with pain relief was significantly higher among elderly who experienced therapeutic exercise than those who did not. Those who underwent ultrasound therapy showed significantly higher satisfaction with facilities and location than those who did not. Conclusion: The results of this study suggest that satisfaction among users differs by type of physical therapy. In the future, physiotherapy services provided in senior centers needs to be designed to improve the effectiveness of physical therapy, professionalism of physical therapists and comfort of facility.

The Type of Medical Service Desired by Those Communities Which at Present have None (일부(一部) 무의면 주민(住民)들이 원(願)하는 의료시설의 형태(形態)에 관(關)한 조사(調査))

  • To try to determine the type of medical service wanted by the rural population, in Chungnam Province, a survey amongst the populations of 6 counties was conducted; Within the 6 counties, 2 local communities, which had no access to local medical services, were surveyed. The 12 communities were actively involved in Sae-maul activities, and total number of households surveyed, was 822. The survey was conducted over a 1 month period, from July 16th, 1976, thru August 20th, and the followings are the results, summarised. 1. The largest number of respondents desired a combination of Public Health Center and Country Hospital, followed in order by Private Clinic and Modern Medical Facility. 2. The respondents, aged under 40 years, desired the Private Clinic type medical service, whilst those over 40 years of age, wanted the County Hospital, and as the numbers in this age bracket, were larger, so the ratio was much higher. 3. Sex, educational background, and occupation did not play any particular emphasis in the decisions. 4. Monthly income affected the responses to the survey. These in the lower-income bracket, wanted the County Hospital, and the ratio was high. These in the high income bracket desired the Modern Medical Service, accordingly. Those with an income of 50,000 won or less, amongst the low-income bracket, favored the Public Health Sub-center type of service. The ratio for this service was very high.

The Effect of Footbaths on Sleep and Fatigue in Older Korean Adults (족욕요법이 한국노인의 수면과 피로에 미치는 효과)

  • Purpose: To determine the type and degree of effect that a hot footbath has on sleep quality and fatigue level in older Korean adults. Methods: A non-equivalent control group quasi-experimental design was used. Fifty participants from a long-term care facility in Kwangju, South Korea, were randomly selected and assigned to two groups: experimental group (27 participants) and control group (23 participants). The participants in the experimental group received hot footbaths in a temperature-controlled water tub of $42^{\circ}C$. They soaked their feet up to 20cm above the ankles for 30 minutes before going to bed for 3 nights. Actigraphy was used to measure their sleep patterns. The data were analyzed using the SAS program. Results: After the intervention, the total sleep satisfaction rate of the participants increased, while sleep latency and fatigue decreased significantly in the experimental group compared to the control group. Conclusion: The study results suggest that hot footbaths are beneficial for older Korean adults in enhancing sleep quality as well as reducing fatigue. Therefore, hot footbaths are recommended as a nursing intervention to improve sleep quality and to reduce fatigue in older Korean adults.

A Study on the Environmental Evaluation of the Elderly's Day Care Center (노인 주야간보호시설 환경 평가에 관한 연구)

  • Korean medical welfare facilities for the elderly provide the conveniences to care geriatric illness such as dementia and stroke. The medical welfare facility categorizes elderly care facilities, nursing homes, and geriatrics hospitals based on the Korean welfare of the aged act. The government makes an effort to secure feed rate and finances of medical welfare facilities. However, the qualitative improvement of facilities is inadequate. The purpose of this study is to analyze the conditions and problems of existing facilities using an environmental evaluation and to discuss the improvement direction related to the daycare centers for the elderly. The method of this study is literature review and field survey. Firstly, we analyzed the previous studies to develop the tool, evaluating the environment of day care centers for the old. Secondly, the items of environmental evaluation are deduced. Thirdly, we select the facilities to conduct field survey and analyze the results of field survey. Lastly, We discussed the problems and improvement directions through the results. It is judged that this study is an useful as the basic guideline to strengthen the minimum legal standards of day care center for the elderly due to the suggestion of insufficient environmental evaluation items.

Changes of Addition by Accommodative Training on Early Presbyopia (초기 노안의 조절훈련에 의한 가입도 변화)

  • In order to examine whether the accommodative trainings with push-up or flipper methods induce the decrement of near addition on early presbyopia aged in 40s having less than 1.00 D addition, daily home vision training was performed for 12 weeks. We evaluated accommodative amplitude, accommodative facility, relative accommodation, and presbyopic addition at every one week. Two accommodative trainings significantly decreased the presbyopic addition as a degree from 0.125 D to 0.375 D, and push-up training was more effective than that of flipper training. Both push-up and flipper trainings are an useful methods to decrease the near addition of early presbyopia as an improvement of the accommodative amplitude.

The Effect of Kindergarten's Factors on Athletic Movement Ability in $4{\sim}5$ Years Old Children (유치원 특성 변인이 $4{\sim}5$세 유아의 운동능력에 미치는 영향)

  • The purpose of this study was to investigate the effect of kindergarten's factors on athletic movement ability in early childhood. The factors included teacher's perception of athletic movement, length of time for physical activity, and number of athletic facilities in the kindergarten. The subjects consisted of 551 children aged from four to five years in 8 private kindergartens. The instruments used for this study were the obstacle course of Project Spectrum for measuring athletic movement ability, and the questionnaire to investigate the teacher's perception, length of time for physical activity, and number of athletic facilities in the kindergarten. The data from all the tests were analyzed by correlation and regression analyses. The results of this study exhibited that the length of time for physical activity was the most important factor among three factors, and the teachers' perception of athletic movement and the number of athletic facilities in the kindergarten were also deciding factors for the children's athletic movement ability.

A Study on the Activation of Leisure Activities for Middle-aged (중년층의 여가활동 활성화 방안에 관한 연구)

  • The purpose of this study is to grasp actual status and needs of leisure of middle age. The questionnaire survey is used, the subjects are middle age from 40$\~$50's living in seoul. The questionnaire consists of leisure conscious, leisure actual condition, leisure satisfaction and need. Frequency, percent, average, $x^2_test$ and ANOVA are used for data analysis. The results of this study are as following; First, leisure is necessary to elevate quality of life in terms of spiritual, psychological stability and physical health. Second, the subjects are not satisfied with current leisure for unsufficiency of leisure time, cost, facility and program. Third, present leisure behavior characteristics are restrictive and passive. In the near future, however, they showed desire to engage in fivers forms of leisure including active and participant leisure. Fourth, even though current main areas for leisure activity are in their house, they are more likely to change from in the house to around and near there house.

Development of Benchmark Index of LoS for Asset Management of Water Treatment Facilities (정수시설 자산관리 LoS분석 벤치마크지수 개발)

  • Since aged water treatment facilities could threaten the sustainable water supply, asset management system has been adopted for their systematic management. Level of Service(LoS) is one of critical components of asset management and could be quantified through benchmark index(BMI). Water supplier could estimate consumer's satisfaction and their performance through BMI to improve the LoS. We developed BMI for water treatment facilities from customer's satisfaction survey. BMI, represented with the Total Service Score(TSS), was assessed with water quality, water pressure, taste and odor, water rate, and service quality with weighing factors. BMI could, further, be used to assist the analysis of the life cycle cost to increase the unit of LoS.
