우리나라 노인 방문 구강건강관리 서비스의 현황과 향후과제 (Current status and future tasks of visiting oral health care services for elders)

  • Objectives: This study confirms the current status of visiting oral health-care services for the elderly to draw policy implications for revitalization of the visiting oral health care services in the future. Methods: First, a survey was conducted on health centers about the current status of the elderly visiting oral health-care service and how to revitalize it. Next, the number of oral hygiene services provided to the elderly was checked in the long-term care insurance system. Results: Oral health education (100%) was the most common practice in visiting oral health-care service for the elderly, and the most difficult thing in providing services was the lack of dental hygienists (38.9%). The status of oral health-care services in the long-term care insurance system for elderly revealed that the total number of service claims has been confirmed to be zero since the introduction of the system. Conclusions: Despite the existence of a system that provides elderly visiting oral healthcare services, to revitalize it, the law must be amended to secure a dental hygienist as the main agent of the activity and to further take responsibility for autonomous authority and performance.

Reliability and Validity of the Korean Translation of the Pediatric Evaluation of Disability Inventory in School-Aged Children With Cerebral Palsy

  • The purposes of this study were to examine the reliability and validity of Korean translation of Pediatric Evaluation of Disability Inventory (PEDI-K) in school-aged children with cerebral palsy (CP). The PEDI-K, Functional Independence Measure for Children (WeeFIM) and Gross Motor Function Classification System (GMIFCS) were completed in 104 school-aged children with CP by therapists. The internal consistency of the PEDI-K was calculated by Cronbach's alpha (${\alpha}$) for assessing reliability. Concurrent validity was evaluated by correlation with the subsets of WeeFIM. Discriminant validity was assessed by comparing GMFCS levels with tests of the PEDI-K. The results showed that internal consistency was good (Cronbach's ${\alpha}$ ranged from .97~.98). Concurrent validity was demonstrated. The correlation with WeeFIM was high in the Functional Skills (self-care, r=.74~.94; mobility, r=.59~.91; social function, r=.65~.93) and in the Caregiver Assistance (self-care, r=.75~.94; mobility, r=.63~.90; social function, r=.78~.96). Discriminant validity was demonstrated on significant decreases in domain scores with increasing GMFCS levels. Reliability and validity have been demonstrated on the PEDI-K. This study extends usage of PEDI-K in clinical activities and research.

농촌건강장수마을 사업의 성과와 과제 (A Study on the Achievements and Issues of the Senior Well-being Villages Project)

  • In order to improve the quality of life and support the successful living of the elderly people in rural community, the Senior Well-being Villages Project has been carried out by Rural Development Administration since 2005 in Korea. This timely project, however, produces good results with 582 villages so far, we must endeavor to upgrade it more effectively. This study is to find complementary way of improving the project, and the results of the study are summarized as follows. First of all, the sustainable and practical strategies of the health care for the rural elderly must be adopted. The elderly people's mutual care system can be a good option for improving the existing project. Secondly, the cooperative heath care system must combine with this project. In this regards, health cooperative federation system can be also a challenging option. The beneficially activity of rural elderly people, which is a component of this project, must be linked with rural social enterprise or community venture. The local experts or capable men of various fields including family, the aged problem, social welfare, and regional development, must be appointed, and the support system enables them to do their jobs actively in the Senior Well-being Villages Project as well as other community works.

중년여성의 건강추구행위의 장애요인 (Barriers to Health-Seeking Behavior in Midlife Women)

  • Health behavior constitutes the single most important factor in an individual's health maintenance program. Pender's health promotion model emphasizes the positive aspects of health-seeking behavior hut omits some negative ones. Although Pender's work does include the concept of barriers, the main focus is upon health habits rather than upon the interaction between the consumer and the health care system. Therefore, since individuals actually do face many barriers in their health-seeking behavior, the present study deals with negative concepts-the barriers to health and healthy behavior. For this reason the expression health-seeking behavior was chosen over health promotion. In conclusion, the results show that barriers to health-seeking behavior are causal factors that could explain and predict the health-seeking behavior of middle life women. Midlife women shows that they have barriers to health-seeking behavior especially in inconvenience, cost, healthcenter site-related problem, relationship, fear. These findings suggest the need to develop a nursing strategy to improve the empowerment of self-determination in middle-aged women. Consequently, a goal of nursing care for middle-aged women should be to help them pursue health care with a greater degree of self-sufficiency.

생애주기별 당뇨질환자의 구강건강영향요인 (Factors influencing oral health according to life cycle characteristics of patients with diabetes)

  • Objectives: This study aimed to identify the factors influencing the relationship between diabetes and oral health in Korean adults. Methods: We analyzed 5,319 adults who were included in the 9th Korea National Health and Nutrition Examination Survey. The data were analyzed for demographic characteristics, daily health care, and oral health care according to diabetic conditions using a complex sample analysis. Multiple logistic regression analysis was performed to analyze the factors influencing the oral health of patients with diabetes. Results: The factors influencing the oral health of patients with diabetes varied according to life cycle. In late middle-aged adults, statistically significant differences were observed in sex (p<0.001), educational level (p=0.030), economic activity (p=0.018), aerobic exercise (p=0.034), smoking (p=0.004), periodontal therapy (p=0.011), and prosthesis production/repair (p=0.025). In younger elderly individuals, statistically significant differences were found in terms of whether they lived together (p=0.027) and educational level (p=0.032). Conversely, no statistically significant differences were observed in the older elderly group. Conclusions: The results of this study showed that the level of oral health of patients with diabetes is already determined in middle and old age; therefore, a system should be prepared to ensure that health care can be systematically performed in late middle-aged adults.

  • Taiwan has experienced rapid economic growth during the past two decades. As a result, the demand for health care in Taiwan has increased rapidly. To meet the rising demand, Taiwan implemented a National Health Insurance (NHI) program on March 1, 1995. This program now covers more than 96 percent of Taiwan's citizens. Implementation of the NHI in 1995 represents fulfillment of a primary social and health policy goals of Taiwan. The goals of the NHI program is to eliminate financial barriers of health care for the citizens, to improve the quality of care. 해외 사례 고찰을 통한 지자체 노인요양시설 인증 체계 및 조사원 고도화 방안 (A Study on the Advancement of Accreditation Systems and Surveyors' Expertise for Long-term Care Facilities: Focusing on Overseas Cases)

  • Purpose: This study aimed to suggest strategies for advancing local-government-based accreditation systems and surveyor training in long-term care facilities in Korea. Methods: A comprehensive review of the literature including research papers and official reports issued by governments from the United States, Australia, and Japan was conducted to explore domestic and international policies related to long-term care facility certification and accreditation systems. Results: The USA has two types of care quality assurance systems including mandatory certification (5-star rating system) by the Centers for Medicare and Medicaid Services and voluntary accreditation by the Joint Commission on Accreditation of Healthcare Organizations. Australia operates a government-based mandatory accreditation system for all long-term care facilities through the Australian Aged Care Quality Agency. Japan, particularly the Tokyo district, operates a third-party evaluation system that involves the voluntary participation of long-term care facilities. Conclusion: This study provides several strategies to enhance accreditation processes and surveyors'expertise. For instance, motivating facilities to voluntarily participate in accreditation is necessary by 1) providing sufficient and continuous consultations and feedback about how to improve care quality, 2) differentiating accreditation domains and indicators from the national health insurance certification system, and 3) actively utilizing accreditation results and providing incentives.

The Institutionalization Process of the Visiting Rehabilitation System in Japan and the Challenges Faced in the Process

  • Purpose: In Japan, the long-term care insurance and health insurance acts have stipulated the visiting rehabilitation system to provide support at the national level. The prior case of Japan would provide guidelines for seeking a suitable policy direction in South Korea. This study aims to examine the historical transition process of the visiting rehabilitation system in Japan, and the issues that emerged in the process of the institutionalization of this system. Methods: To examine the historical transition process of the system, the regulations announced by the government and their reports were reviewed. The relevant issues were qualitatively analyzed based on the opinions of scholars, therapists, and interested organizations that were reported in published papers on the topic. Result: The visiting rehabilitation system has been implemented in the following chronological order: The Health and Medical Service Act for the aged (1982-2006), the Health Insurance Act (1988-), and the Long-term Care Insurance Act (2000-). Currently, visiting rehabilitation is provided through hospitals, clinics, visiting nursing stations, etc. The following issues came to the fore in the process of the institutionalization of the system: (1) the complexity of the system, (2) the necessity of changing perceptions into a life model approach, (3) insufficient service provision by therapists, (4) the lack of human resources and an education system, (5) the lack of awareness of care managers and doctors about visiting rehabilitation, and (6) the necessity of quality enhancement through a team approach. Conclusion: It is deemed worthwhile to refer to the visiting rehabilitation system in Japan and the issues that emerged in the process of institutionalizing the system while seeking a policy direction for a similar system in South Korea.

  • Article 10 of the Constitution All citizens have dignity and values as humans and have the right to pursue happiness. There is a need for support measures such as a means to realize the respect of dignity and values as humans, and how to efficiently maintain policies on welfare for the elderly classified as the socially disadvantaged. It was considered necessary to develop an elderly-friendly city for economic, physical, and social life. Dasan Jeong Yak-yong's respect for adults was to practice the ideology of Confucianism, so this was the most important thing in the past Joseon Dynasty. In particular, it was considered important to think of adults first in practicing filial piety.This study focuses on the long-term care insurance system for the elderly, one of the welfare policy measures for the elderly. The significance of implementing the long-term care insurance system for the elderly is to benefit not only the elderly, but also all generations, including the middle-aged and children who were in charge of long-term care. To this end, the government should properly manage the factors of the social insurance financial crisis caused by the low birth rate and aging population.In addition, concerns about health insurance are high, so it is necessary to secure an appropriate level of government budget for health insurance financial management and minimize unnecessary non-benefit. In addition, it is necessary to induce appropriate medical use through connection with construction medical insurance.

Predictors of the Utilization of Oral Health Services by Children of Low-income Families in the United States: Beliefs, Cost, or Provider?

  • Purpose. This study examined the predictive factors enabling access to children's oral health care at the level of financial barriers, beliefs, and the provider. Methods. In-depth interviews were conducted with 320 immigrant mothers of low-income families regarding their use of oral health services for children aged four to eight years old. Access to oral health care was measured with frequency of planned dental visits, continuity of care, and age at first visit to dentist. Results. The mother took her child to the dentist at a younger age if she received referrals to a dentist from pediatrician. Regular dental visits were significantly related to household income, provider availability on week-ends, and insurance coverage. The extended clinic hours in the evenings, and the belief in the importance of the child's regular dentist visits increased the likelihood of continuing care. The mothers perceiving a cost burden for the child's dental care were also less likely to return to the dentist. Conclusion. The available care delivery system, coordinated medical care, and health beliefs were among important predictors of the health service use. The study findings suggest need for culturally competent dental health interventions to enhance access to oral health care among particularly vulnerable populations such as low-income children in Korean communities.
