  • The purposes of this study were to examine bakery cafe use and to identify differences according to gender and age. The questionnaires developed for this study were distributed to 320 males and females aged 20 and over in Seoul. A total of 285 questionnaires were used for analysis(89.1%) and the statistical analyses were completed using SPSS Win (12.0) for descriptive analysis and $x^2-test$. Over 80% of customers used bakery cafes once a month or less, and female customers visited more often than males. The main reasons for using bakery cafe were to chat and then, to buy and eat bakery products. There was a significant difference by age. Average time consumed at a bakery cafe was usually under 1 hour and 30 minutes. Female and younger customers stayed longer than male and older customers, respectively. The majority of customers(85.6%) were accompanied by 2 or less guests and there was no significant difference between the groups. The expenses per person per visit were usually $3,000won{\sim}10,000won$, and the percentage of customers who spent at least 15,000 won was highest in those aged 41 and more. The groups with different gender and age also differed in their choices of products. Customers who are female or under 41 years old used coffees or beverages more often than other group customers. There was no significant difference in understanding on bakery cafe between groups, and 64.9% of respondent answered that they understood bakery cafe as a complex of bakery and cafe. Overall, for early settlement and development of the bakery cafe industry, it seems very important to establish marketing strategy that meets target customers' needs on the basis of understanding their patterns of bakery cafe use.

  • Background: Current trends in Korea population aging with advances in public health and clinical medicine foretell rises in the prevalence of not only chronic diseases but also patients with multimorbidity. One important aspect in analyzing multimorbidity is to define the list of chronic diseases included when calculating multimorbidity index. The objective of this study is to describing the effect of multimorbidity on healthcare cost in Korea using US Office of the Assistant Secretary of Health (OASH) list. Methods: We analyzed the Korea Health Panel Data representing non-institutionalized Korean adult populations aged 20 and more. We calculated multimorbidity index based on OASH list and estimated the prevalence and healthcare cost for each OASH chronic disease. Results: In 2011, 15.2 million (39.6%) Koreans aged 20 and more were living with chronic condition. The health care cost due to chronic diseases, accounted for 80.2% of the overall healthcare costs and the prevalence of chronic conditions, the prevalence of multimorbidity and healthcare cost increased with ages. In the analysis using OASH list, 40% of the adult population over the age of 20 and 66.7% of the population over the age of 65 was affected with multimorbidity. In most of diseases in OASH list, prevalence of mulitmorbidity was high and healthcare cost increased with multimorbidity. Conclusion: OASH chronic disease list that accounts for 72.4% of prevalence and 86.7% of healthcare cost of persons with chronic conditions in Korea. OASH chronic disease list would be a useful and representative indicator for studying multimorbidity.

  • This study aims at surveying how Koreans look upon and prepare for the age of longevity("age of 100 years") drawing near at hand, analyzing factors affecting such a perception and behavior. To this end, a telephone survey was launched for 1200 persons sampled to evenly comprise the young, middle-aged, and elderly groups. Our findings indicate that more Koreans looked upon the upcoming "age of 100 years" as a 'disaster' rather than a 'blessing.' The sense of combined uneasiness over the aged life being further elongated while they are unprepared for it, anxiety over maintaining health and so forth came into play. However, the better a person is prepared by way of health management, savings for retirement, preparation for leisure activities and employment for the later life, the more positively he or she looked upon the "age of 100 years". An analysis by group indicates that women rather than men, the elderly rather than the young, the less educated rather than the highly educated, the low-income bracket rather than the high-income bracket looked upon the "age of 100 years" more negatively. Under such circumstances, it is suggested that the nation's social system tailored to a life expectancy of 80 years should be readjusted, tailored to a 'longevity paradigm' and that to this end social systems should be reoriented toward the direction where 'healthy aging' and 'active aging' are fully supported.

  • As the older adult period $({\geq}65y)$ is increasing, it is needed to investigate the trend of aging-dependent anthropomeric index and nutrient intake, and establish the more specific dietary guide for the different stages of aging period. To find the difference in nutrient intake among the Koreans aged 50-64, 65-74 and 75 years and older, and also any characteristics of dwellers in longevity area, we recruited 1,083 subjects (385 male and 698 female) aged 50-95 years (mean age, 71.3 yrs) living in Kugoksoondam area (Kurye, Goksung, Soonchang and Damyang counties), known as a longevity-belt region in Jeonlaprovince, Korea. We measured some anthropometric index and collected 2 day-dietary record. Nutrient intakes were analyzed by using DW24 program. The mean height and weight of subjects aged 75 years and older were lower than Korean national reference. BMI and obesity $({\geq}25kg/m^2)$ prevalence were significantly decreasing with aging. Underweight $(BMI<18.5kg/m^2)$ prevalence was also increasing with aging, especially in males, and it was slightly higher than national average, but similar to that in some other rural area. Obesity prevalence of male subjects was lower compared to national prevalence, but abdominal obesity prevalence $(waist{\geq}80cm)$ was very high in females (about 89%). In both genders, the average proportional contribution of carbohydrate, protein and fat to energy intake was not different between 65-74 years and 75 years and older. On overall, nutrient intake and quality of diet of females were inferior to those of males so that many of females aged 75 years and older assumed to be at risk of malnourished status. Fiber, folate and vitamin E intakes were substantially higher compared to those in 2007 KNHNES and other some studies in rural area. While almost nutrient %EAR was significantly decreased with aging in females, there was no significant difference in %EAR for protein, vitamin A, $B_1$, $B_6$, $B_{12}$, niacin, Ca and Zn between 65-74 years and 75 years and older in males. Vitamin $B_2$ for male aged 50-64 years, vitamin $B_2$ and Ca for male aged 65 years and over and female aged 50-74 years, and vitamin $B_2$, vitamin C, Ca and folate intake for female aged 75 years and older were assessed to be at risk to undernutrition based on the prevalence of intake below EAR. MAR of 13 nutrients and the number of nutrients consuming below EAR were significantly decreasing with aging in both genders, however, the number of nutrients of INQ < 1 and the average mini-nutritional assessment score were not significantly different between 65-74 years and 75 years and older. Taken together, decreasing tendency of nutrients intake and the quality of diet with aging was more evident in females than in males, and it is unique that our subjects consumed substantially higher fiber, folate and vitamin E compared to not only urban but also some other rural areas.

  • Growth of suture line and anastomosis is required for long-term success after the tracheal and bronchial surgery in infant and pediatric patient. We used various suture materials in these cases, but the results were differ. To select the adequate suture material in tracheal surgery, we tried next. Tracheal anastomoses were performed in 150 Sprague Dawley rats, aged 4 to 8 [mean 5.8] weeks and weight 62 to 106[mean 83.6] gram, to compare polydioxanone[PDS] 7-0, polyglactin 910[Vicryl]7-0, and polypropylene [prolene] 8-0 suture materials. In 150 rats, only 29[20%] were lived over 300 days, and the weight was 250 to 320[mean 289.5]gram. Cross sectional area of the anastomoses and two or three tracheal rings below anastomosis site were measured under microscope, and calculated and compared as Hsieh`s equition. Cross-sectional area, anastomosis site/normal site 100, were 89.4 $\pm$ 5.34% in PDS group[n=9], 75.7 $\pm$ 6.06% in Vicrylgroup [n = 10], and 80.8$\pm$ 4.06% in Prolene group[n = 10]. Histopathologic studies were done for all autopsies or put in death around 300 days postoperatively. PDS absorblion was not seen 16 weeks after suture but disappeared over 24 weeks slide. Vicryl absorbtion was noted postoperative 8 to 16 weeks, with marked tissue reaction. Prolene showed least tissue reaction, but the suture material was persisted with regional fibrotic capsule.Causes of death were respiratory failure in 76 cases, tracheal rupture in 22 cases, hemorrhage, biting, starvation and etc. in 23 cases. With the brief review of literatures, we report the results.
