The main objective of this study was to investigate the chemical characteristics of post-harvest biomass burning aerosols from field burning of barley straw in late spring and rice straw in late fall in rural area in Korea. 12-hr integrated intensive sampling of $PM_{10}$ and $PM_{2.5}$ biomass burning aerosols had been conducted continuously at Gwangju, Korea 4-15 June 2001 and 8 October-14 November 2002. The fine and coarse particles of biomass burning aerosols were collected for mass, ionic, elemental, and carbonaceous species analysis. Average fine and coarse mass concentrations of biomass burning aerosols were measured to be 129.6, 24.2 ${{\mu}gm}^{-3}$ in June 2001 and 47.1, 33.2 ${{\mu}gm}^{-3}$ in October to November 2002, respectively. Exceptionally high level of $PM_{2.5}$ concentration up to 157.8 ${{\mu}gm}^{-3}$ well above 24-hour standard was observed during the biomass burning event days under stagnant atmosphere condition. During biomass burning periods dominant ionic species were $Cl^{-}$, ${NO_3}^{-}$, ${SO_4}^{2-}$, and ${NH_4}^{+}$ in fine and coarse mode. In the fine mode $Cl^{-}$ and ${KCl}^{+}$ were unusually rich due to the high content of the semiarid vegetation. High OC values and OC/EC ratios were also measured during the biomass burning periods. Increased amount of fine aerosols with high enrichment, which were originated from biomass burning of post-harvest agricultural waste, resulted in extremely severe particulate air pollution and visibility degradation in the region. Particulate matters from open field burning of agricultural wastes cause great adverse impact on local air quality and regional climate.
Background: Rhabdomyosarcoma (RMS) is the most common soft tissue sarcoma in the pediatric age group. All patients with RMS regardless of their initial stage or group receive combination chemotherapy as 'standard therapy' consisting of vincristine, actinomycin-D and cyclophosphamide. Actinomycin-D was not readily available in Turkey at one time. Carboplatin was used instead in order to prevent delays in treatment. The aim of this report is to present the results of patients with rhabdomyosarcoma receiving carboplatin or actinomycin-D therapy. Materials and Methods: Twenty four patients with rhabdomyosarcoma treated between December 2000 and June 2011 were included in this retrospective study. The patients were treated according to International Rhabdomyosarcoma Study Group guidelines. Eleven patients were treated with actinomycin-D and 13 with carboplatin ($250mg/m^2/dose$ for 2 days). The two groups were then compared in terms of 2- and 5-year overall survival (OS) and hematological and non-hematological toxicities. Results: Age, sex, stage and the mean duration of follow-up were similar in both groups (p>0.05). Two- and five-year OS levels were 68.2% in the carboplatin group and 78.0% and 40.0%, respectively, in the actinomycin-D group. There was no statistical difference in the number of febrile episodes (p=0.86) and no other hematological and non-hematological adverse effects were recorded in both groups. Conclusions: The findings show that carboplatin can be used as an alternative drug in the primary treatment of rhabdomyosarcoma in the event that actinomycin-D is unavailable or not tolerated.
Objective : The purpose of this study was to analyze in detail the relationship between outcome and time course of effect in medically refractory primary cervical dystonia (CD) with phasic type that was treated by bilateral globus pallidus internus (Gpi) deep brain stimulation (DBS). Methods : Six patients underwent bilateral implantation of DBS into the Gpi under the guide of microelectrode recording and were followed for $18.7{\pm}11.1$ months. The mean duration of the CD was $5.8{\pm}3.4$ years. The mean age at time of surgery was $54.2{\pm}10.2$ years. Patients were evaluated with the Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS) and relief scale using patient self-reporting. Results : The TWSTRS total scores improved by 64.5%, 65.5%, 75.8%, and 76.0% at 3, 6, 12 months, and at the last available follow-up after surgery, respectively. Statistically significant improvements in the TWSTRS scores were observed 3 months after surgery (p=0.028) with gradual improvement up to 12 months after surgery, thereafter, the improvement was sustained. However, there was no statistically significant difference between the scores at 3 and 12 months. Subjective improvement reported averaged $81.7{\pm}6.8%$ at last follow-up. Mild dysarthria, the most frequent adverse event, occurred in 3 patients. Conclusions : Our results show that the bilateral Gpi-DBS can offer a significant therapeutic effect from 3 months postoperatively in patients with primary CD with phasic type, without significant side effects.
Purpose: Several accelerated partial breast radiation (APBR) techniques have been investigated in patients with early-stage breast cancer (BC); however, the optimal treatment delivery techniques remain unclear. We evaluated the feasibility and toxicity of APBR delivered using intensity-modulated radiation therapy (IMRT) in elderly patients with stage I BC, using a novel fractionation schedule. Materials and Methods: Forty-two patients aged ${\geq}65$ years, with stage I BC who underwent breast conserving surgery were enrolled in a phase I/II study evaluating APBR using IMRT. Forty eligible patients received 40 Gy in 4 Gy daily fractions. Patients were assessed for treatment related toxicities, and cosmesis, before APBR, during, and after completion of the treatment. Results: The median age was 73 years, median tumor size 0.8 cm and the median follow-up was 54 months. The 5-year locoregional control was 97.5% and overall survival 90%. Erythema and skin pigmentation was the most common acute adverse event, reported by 27 patients (69%). Twenty-six patients (65%) reported mild pain, rated 1-4/10. This improved at last follow-up to only 2 (15%). Overall the patient and physician reported worst late toxicities were lower than the baseline and at last follow-up, patients and physicians rated cosmesis as excellent/good in 93% and 86 %, respectively. Conclusion: In this prospective trial, we observed an excellent rate of tumor control with daily APBR. The acceptable toxicity profile and cosmetic results of this study support the use of IMRT planned APBR with daily schedule in elderly patients with early stage BC.
Leukopenia is a commonly encountered adverse event experienced by cancer patients. It can cause serious infections and delays in chemotherapy. We describe a case of leukopenia induced by colon cancer chemotherapy. After acupuncture treatment, the patient showed improvement in the leukopenia and completed the scheduled chemotherapy without delay or additional administration of granulocyte colony-stimulating factor (G-CSF). Therefore, acupuncture may have a substantial benefit for treatment of leukopenia in this patient.
Fibrosis of skeletal muscle following acupuncture is an iatrogenic disorder. The present case illustrates a patient with a unilateral fibrotic formation on a thumb muscle after acupuncture injection therapy with red sage. The patient in the present case was a counter-terrorism police officer with right-handedness; he noted a palpable nodule three months after injection therapy at his left first dorsal interosseous in which the acupuncture point LI4 (He Gu) is located. He also found a reduction in the strength of his left pinch grip that noticeably affected his left handgun marksmanship. However, being ambidextrous in single-hand pistol shooting is an essential requirement for counter-terrorism police officers. Based on the patient's medical history and claims, no underlying disease or trauma was found to be associated with his current complaint. During physical examination, a fibrotic formation in his left first dorsal interosseous muscle was visualized by using diagnostic ultrasound; also, as confirmed with dynamometry, the strength of his left pinch grip was significantly lower than that of the right counterpart. Because acupuncture injection therapy has three components, antiseptic practices, the mechanical action of syringe insertion, and the pharmacological effect of the sterile herb extract, any one of the components may have contributed to the present adverse event. The first dorsal interosseous muscle is small in dimension and rather vascular; thus, it is not an ideal site for intramuscular injection. When a clinician needs to treat a patient by performing acupuncture at the LI4 acupoint and injecting a herbal extract simultaneously, the clinician should only mechanically stimulate the LI4 acupoint while injecting the herbal medicine into the LI14 (Bi Noe) acupoint on the same meridian, the LI14 acupoint being located in the distal portion of the deltoid muscle and being fairly close to the universally agreed upon site on the upper arm for safe administration of an injection.
Gender and intelligence of children, parenting practices and socioeconomic status of parents were studied to investigate whether they functioned as protective factors for resilient children. Total of 556 children of 12 years of age were contacted for the study. Teachers were asked to rate the children's interpersonal competence. Children reported their experience of stressful life events on a checklist. Based on the competence scores and the experience of stressful life events, 107 children were classified into adaptive, resilient, and maladaptive group. IQ scores were assessed using an intelligence test for the 107 children. Mothers reported their parenting practices and their socioeconomic status through a questionnaire. Descriptive statistics, chi-squares, t-tests, and one-way ANOVAs were performed to analyze the data. There was no difference in their experience of stressful life events between boys and girls. Girls were rated as being more competent by their teachers. There were more girls in the adaptive and the resilient group and more boys in the maladaptive group. Children in adaptive and the resilient group had higher intelligence scores than the maladaptive group. Mothers of resilient children replied as being warm and acceptive and more refusing and controlling than mothers of the adaptive children Group differences in socioeconomic status of the family were not found. Thus, being a girl and having higher IQ scores were the protective factors for the resilient children.
Objectives: This study aims to assess the subfertility support project of Korean medicine in Gyeonggi-do. Methods: A total of 231 women ($36.84{\pm}3.79yrs$) out of 361 applicants completed the treatment in this study. The copies of the medical records and study-related documents were provided by the association of Korean Medicine in Gyeonggi-do, in which the personal information was discarded. Descriptive analysis and inferential statistics (e.g., Mann-Whitney U test, Paired t-test, Wilcoxon signed rank test, & logistic regression) were applied to examine the statistical differences between groups and before/after treatments utilizing SPSS 23. All Type I errors (${\alpha}$) for the statistical significance were set at .05 Results: After the treatment, 11.3% of participants became pregnant spontaneously. There was no significant change in blood tests before and after the treatment and the observation period. Also there was no adverse event during the project. After the project, the satisfaction survey was conducted, and 87.5% of participants were satisfied with the project. Conclusions: The findings of this study proved the efficacy of Korean medical treatment for infertile women.
Defense-in-depth is a fundamental safety principle for the design and operation of nuclear power plants. Despite its general appeal, defense-in-depth is not without its drawbacks, which include its potential for concealing the occurrence of hazardous states in a system, and more generally rendering the latter more opaque for its operators and managers, thus resulting in safety blind spots. This in turn translates into a shrinking of the time window available for operators to identify an unfolding hazardous condition or situation and intervene to abate it. To prevent this drawback from materializing, we propose in this work a novel safety principle termed "observability-in-depth". We characterize it as the set of provisions technical, operational, and organizational designed to enable the monitoring and identification of emerging hazardous conditions and accident pathogens in real-time and over different time-scales. Observability-in-depth also requires the monitoring of conditions of all safety barriers that implement defense-in-depth; and in so doing it supports sensemaking of identified hazardous conditions, and the understanding of potential accident sequences that might follow (how they can propagate). Observability-in-depth is thus an information-centric principle, and its importance in accident prevention is in the value of the information it provides and actions or safety interventions it spurs. We examine several "event reports" from the U.S. Nuclear Regulatory Commission database, which illustrate specific instances of violation of the observability-in-depth safety principle and the consequences that followed (e.g., unmonitored releases and loss of containments). We also revisit the Three Mile Island accident in light of the proposed principle, and identify causes and consequences of the lack of observability-in-depth related to this accident sequence. We illustrate both the benefits of adopting the observability-in-depth safety principle and the adverse consequences when this principle is violated or not implemented. This work constitutes a first step in the development of the observability-in-depth safety principle, and we hope this effort invites other researchers and safety professionals to further explore and develop this principle and its implementation.

Objectives: Considering high prevalence of myopia in Asian countries, social cost paid for myopia may gradually increase in Korea. However, studies for developing myopia treatment are deficient. The study was to evaluate whether the periocular acupressure with a medical massager is effective for suppressing myopia progression. Methods: This study was an investigator-sponsored, prospective, open-labeled, and superiority pre and post single-armed study. 14 myopia children aged 7-12 years wore a medical massager for 15 minutes to stimulate periocular acupoints. For 24 weeks, the participants used the device twice a day. Based on prior studies, the refraction change of naturally proceeded myopia was set as -0.38 D. The axial length change of naturally proceeded myopia was set as 0.228 mm. To assess the safety, we performed vital sign check, physical examination, visual acuity test, slit lamp examination, IOP measurement, and fundus examination. Results: The refraction and axial length of the participants increased. For 24 weeks, the change in refraction was -0.38 D at the right eye and -0.40 D at the left. The change in axial length was 0.21 mm at the right and 0.22 mm at the left. In the safety assessment, any adverse event did not occur. Conclusions: There was no significant difference between the control group and the intervention group. The effect of growth may have more contributed to our result than that of acupressure. Based on our result, a full-scale study will not be conducted.
