Diabetic Atherosclerosis and Glycation of LDL(Low Density Lipoprotein)

  • Diabetes carries an increased risk of atherosclerotic disease that is not fully explained by known car-diovascular risk factors. There is accumulating evidence that advanced glycation of structural proteins, and oxidation and glycation of circulating lipoproteins, are implicated in the pathogenesis of diabetic ather-osclerosis. Reactions involving glycation and oxidation of proteins and lipids are believed to contribute to atherogenesis. Glycation, the nonenzymatic binding of glucose to protein molecules, can increase the ather-ogenic potential of certain plasma constituents, including low density lipoptotein(LDL). Glycation of LDL is significant increased in diabetic patients compared with normal subjects, even in the presence of good glycemic control. Metabolic abnormalities associated with glycation of LDL include diminished recognition of LDL by the classic LDL receptor; increased covalent binding of LDL in vessel walls ; enhanced uptake of LDL by the macrophages, thus stimulating foam cell formation ; increased platelet aggregation; formation of LDL-immune complexes ; and generation of oxygen free radicals, resulting on oxidative damage to both the lipid and protein components of LDL and to any nearby macromolecules. Oxidized lipoproteins are characterzied by cytotoxicity, potent stimulation of foam cell formation by macrophages, and procoagulant effects. Combined glycation and oxidation, "glycoxidation" occurs when oxidative reactions affect the initial products of glycation, and results in irreversible structural alterations of proteins. Glycoxidation is of greatest significance in long lived proteins such as collagen. In these proteins, glycoxidation products, believed to be atherogenic, accumulate with advancing age : in diabetes, their rate of accumulate is accelerated. Inhibition of glycation, oxidation and glycoxidation may form the basis of future antiaterogenic strategies in both diabetic and nondiabetic individuals.dividuals.

Diet and Metabolic Disease Risk Related to Sarcopenia in Pre- and Postmenopausal Adult Women: Based on the Korea National Health and Nutrition Examination Survey (2009-2011) (폐경 전·후 성인 여성에서 근감소증과 관련된 식생활 요인 및 대사성 질환 위험도: 국민건강영양조사(2009-2011) 자료를 활용하여)

  • Diet is important for muscle health and offers a protective effects against the loss of skeletal muscle mass and physical functions with advancing age. We analyzed the relationship between diet, metabolic disease risk, and sarcopenia in Korean female adults using the 2009-2011 Korea National Health and Nutrition Examination Survey (KNHANES). A total of 2038 adult women aged 50-64 years were classified into four groups based on the diagnosis of menopause and sarcopenia. The groups were the non-menopause and non-sarcopenia group (NMNS, n=249), the non-menopause and sarcopenia group (NMS, n=14), the menopause and non-sarcopenia group (MNS, n=1,653), and the menopause and sarcopenia group (MS, n=122). The socio-demographics, anthropometrics, blood profile, and dietary data of the subjects were collected. Those who were in both the sarcopenia groups were more obese (p<0.001), had greater waist circumferences (p<0.001), higher body mass index (p<0.001), and higher obesity rates (p<0.001) after adjustment for covariants. Both the sarcopenia groups also had higher plasma levels of total cholesterol (p<0.001), HbA1c (p=0.001), and vitamin D (p=0.020) than both the non-sarcopenia groups. Both the sarcopenia groups demonstrated a decreased intake of calcium (p=0.05), potassium (p=0.008), and niacin (p=0.008) than both the non-sarcopenia ones. Among the four groups, the NMS group showed the highest levels of total cholesterol, obesity, and lowest intake of micronutrients such as calcium, niacin, and potassium. Thus, muscle mass maintenance through weight control and adequate nutrient intake appears to demonstrate a potential association with preventing sarcopenia in Korean middle-aged women.

  • Background Various surgical management methods have been proposed for ischial sore reconstruction, yet it has the highest recurrence rate of all pressure ulcer types. A novel approach combining the advantages of a perforator-based fasciocutaneous flap and a muscle flap is expected to resolve the disadvantages of previously introduced surgical methods. Methods Fifteen patients with ischial pressure ulcers with chronic osteomyelitis or bursitis, who underwent reconstructive procedures with an inferior gluteal artery perforator (IGAP) fasciocutaneous flap and a split inferior gluteus maximus muscle flap from January 2011 to June 2016, were analyzed retrospectively. The split muscle flap was rotated to obliterate the deep ischial defect, managing the osteomyelitis or bursitis, and the IGAP fasciocutaneous flap was rotated or advanced to cover the superficial layer. The patients' age, sex, presence of bursitis or osteomyelitis, surgical details, complications, follow-up period, and ischial sore recurrence were reviewed. Results All ischial pressure ulcers were successfully reconstructed without any flap loss. The mean duration of follow-up was 12.9 months (range, 3-35 months). Of 15 patients, one had a recurrent ulcer 10 months postoperatively, which was repaired by re-advancing the previously elevated fasciocutaneous flap. Conclusions The dual-flap procedure with an IGAP fasciocutaneous flap and split inferior gluteus maximus muscle flap for ischial pressure ulcer reconstruction is a useful method that combines the useful characteristics of perforator and muscle flaps, providing thick dual padding with sufficient vascularization while minimizing donor morbidity and vascular pedicle injury.

  • In this study, We confirmed the three major policy directions presented in "Educational Policy Direction and Core Tasks in the Age of Artificial Intelligence" announced by the government in 2020, and analyzed how the direction and key tasks are reflected in the policy from keywords selected from government policy data related to artificial intelligence education published between '20 and '22. It was extracted and analyzed how the direction and key tasks are reflected in the policy. As a result of text mining and the topic analysis, the direction of education set was analyzed and various types of activities for nurturing talents in the field of artificial intelligence were confirmed. Ultimately, the government's policy direction is to apply the '25 revised curriculum in earnest, while advancing and activating the AI education policy and allowing it to settle naturally in the field. It could be predicted that related policies and tasks would appear more and more.

Gingival crevicular fluid CSF-1 and IL-34 levels in patients with stage III grade C periodontitis and uncontrolled type 2 diabetes mellitus

  • Purpose: Periodontal diseases are inflammatory conditions that alter the host's response to microbial pathogens. Type 2 diabetes mellitus (T2DM) is a complex disease that affects the incidence and severity of periodontal diseases. This study investigated the gingival crevicular fluid (GCF) levels of colony-stimulating factor-1 (CSF-1) and interleukin-34 (IL-34) in patients with stage III grade C periodontitis (SIII-GC-P) and stage III grade C periodontitis with uncontrolled type 2 diabetes (SIII-GC-PD). Methods: In total, 72 individuals, including 24 periodontally healthy (PH), 24 SIII-GC-P, and 24 SIII-GC-PD patients, were recruited for this study. Periodontitis patients (stage III) had interdental attachment loss (AL) of 5 mm or more, probing depth (PD) of 6 mm or more, radiographic bone loss advancing to the middle or apical part of the root, and tooth loss (<5) due to periodontal disease. Radiographic bone loss in the teeth was also evaluated; grade C periodontitis was defined as a ratio of the percentage of root bone loss to age greater than 1.0. The plaque index (PI), gingival index (GI), presence of bleeding on probing (BOP), PD, and clinical AL were used for clinical periodontal assessments. GCF samples were obtained and analyzed using an enzyme-linked immunosorbent assay. Results: All clinical parameters-PD, AL, GI, BOP, and PI-were significantly higher in the SIII-GC-PD group than in the PH and SIII-GC-P groups for both the full mouth and each sampling site (P<0.05). The total IL-34 and CSF-1 levels were significantly higher in the SIII-GC-PD group than in the PH and SIII-GC-P groups (P<0.05), and there were significant differences between the periodontitis groups (P<0.05). Conclusions: These findings suggest that IL-34 and CSF-1 expression increases in patients with SIII-GC-PD. CSF-1 was associated with the inflammatory status of periodontal tissues and T2DM, while IL-34 was associated only with T2DM.

Changes in the Profiles of Serum LH, Testosterone, Estrogen and IFG-I during Sexual Development in Male Korean Native Chickens (한국재래 수탉에서 부화 후 혈청내 LH, Testosterone, Estrogen과 IGF-I 농도의 변화)

  • Changes in serum estradiol, insulin-like growth factor-1, leuteinizing and testosterone levels, and leuteinizing hormone-stimulated testosterone production per testis in vitro from hatching to adulthood were studied in Korean native chickens of 1, 2, 4, 6, 8, 10, 12, 14, 16, 18, 21, 24, 28, 32, 44, 52 and 64 weeks (n=13 chickens per group) of age. The changes in the profiles of the levels in the incubation medium of luteinizing hormone-stimulated (100 ng/mL) testosterone secretion per testis in vitro, and the serum LH, testosterone, estradiol, and insulin-like growth factor-I were determined by radioimmunoassay. Serum estradiol levels were not significantly different at week 4 compared to that of 8, 12, 16, 21, 32, :md 44. Significant decreases were observed at weeks 52 and 64. Serum leuteinizing hormone concentrations were not significantly different from 1 week to 12 weeks, increased gradually up to 32 weeks of age, and declined significantly thereafter; the highest value was at 32 weeks, and the lowest value was detected at 2 weeks of age. Serum insulin-like growth factor-I concentrations increased significantly from 1 week to 16 weeks, remained low and unchanged with advancing age. Serum testosterone concentrations were not significantly different at week 1 compared weeks 2, 4, 6, and 8. Significant increases were observed from 10 weeks to 32 weeks of age. Values at weeks 24, 28 and 32 and at weeks 32, 44, 52, and 64 were not significantly different. The highest value was at weeks 28 and the lowest value was detected at weeks 1 week. LH-stimulated testosterone production per testis in vitro increased gradually with age from 1 to 32 weeks and decreased significantly from 44 weeks to 64 weeks of age.

Studies on the Post-hatching Development of Sertoli and Leydig Cells in the Testis of Korean Native Chickens (한국재래 닭 고환 지지세포와 간질세포의 부화 후 발달에 관한 연구)

  • Morphometric changes in testicular Sertoli and Leydig cells from hatching to adulthood were studied using Korean native chickens of 1, 2, 4, 6, 8, 10, 12, 14, 16, 18, 21, 24, 28, 32, 44, 52 and 64 weeks (n=13 chickens per group) of age. The objective of this study was to understand the developmental phase of the Sertoli and Leydig cells with age. Testis of chickens was fixed by whole body perfusion using a fixative containing $2.5\%$ glutaraldehyde in cacodylate buffer, processed and embedded in Epon-araldite. Using 1 Um sections stained with methylene blue-azure II, qualitative and quantitative (stereological) morphological studies were performed. The average volume of a testis of 1 week old Korean native chickens was determined as $0.148\;cm^3$ and the parameter increased linearly from 1 week to 21 weeks days $(28.86\;cm^3)$, and did not change from 21 weeks to 64 weeks. The volume density of the seminiferous tubules increased with age from $32.6\%$ at week 1 to $92.89\%$ at week 64. The volume density of the interstitium represents $67.4\%$ of the testicular parenchyma at week 1. This proportion progressively diminished during development to reach a value of $7.11\%$ at week 64. The volume density of the Leydig cells decreased almost linearly from 1 week $(4.9\%)$ to 14 weeks $(1.7\%)$ and remained unchanged thereafter. In contrast, the Sertoli cells occupied a volume density of $3.4\%$ at week 1, increased progressively up to 18 weeks of age $(10.79\%)$ and remained unchanged thereafter. The absolute volume of the Leydig and Sertoli cells per testis increased significantly from week 1 to week 21 but did not change significantly from week 24 to week 64. The number of Leydig cells per testis increased almost linearly from 1 week to 21 weeks, remained high and unchanged with advancing age. The number of Sertoli cells per testis increased gradually with age from 1 week to 14 weeks and remained unchanged thereafter.

Factors Influencing Quality of Nursing Service among Clinical Nurses: Focused on Resilience and Nursing Organizational Culture (상급종합병원 간호사의 간호서비스 질에 영향을 미치는 요인: 회복탄력성과 간호조직문화를 중심으로)

  • Purpose: The purpose of this study was to examine nursing organizational culture and resilience and their effects on quality of nursing service. Methods: A cross-sectional study was conducted. A convenience sampling method was used to collect data from 199 participants who worked in a tertiary hospital in G city. Demographic and work related variables, quality of nursing service, resilience, and nursing organizational culture were measured using validated self-report questionnaires. Results: All of the participants were women and the majority were staff nurses and single. A statistically significant difference in quality of nursing service was found for age, marital status, educational level, clinical career, position and perceived health status. Age, educational level, clinical career, position, resilience, innovation-oriented culture, relation-oriented culture, and hierarchy-oriented culture were significant predictors of quality of nursing service, explaining 47% of total variance. Among the predictors, resilience was the strongest predictor, followed by innovation-oriented culture, and hierarchy-oriented culture. Conclusion: Findings indicate that quality of nursing service can be improved by raising individual nurse's resilience and advancing nursing organizational culture. Considering the identified factors, researchers and administrators need to develop and provide clinical nurses with a variety of programs to improve the quality of their nursing service.

Clinical Significance of Abdominal Fat Distribution in Korean Male Children and Adolescents (한국 소아 청소년 남아에서 복부 지방 분포 측정의 임상적 의의)

  • Purpose: Visceral adipose tissue may be strongly linked to increased metabolic risks in adults. However, because little is known regarding the effect of visceral adipose tissue in children and adolescents, we performed this study to determine the association between abdominal fat distribution and metabolic risk factors in this population. Methods: One hundred one children and adolescents (78 males and 23 females; mean age, 10.8${\pm}$2.4 years) were enrolled. The anthropometric data and metabolic risk factors were evaluated. Theabdominal fat distribution was assessed according to the CT measurement. Age-adjusted, partial correlations were performed among the visceral adipose fat area (VFA), subcutaneous adiposefat area (SFA), metabolic risk factors, and anthropometrics. Results: The SFA increased more rapidly than the VFA with advancing years in both genders. In males, the VFA and SFA were positively correlated with anthropometrics. The VFA was correlated with low HDL-cholesterol and the SFA was correlated with diastolic blood pressure (DBP). However, there was no statistical significance between the VFA, SFA, anthropometrics, and other metabolic risk factors. The VFA and SFA were strongly linked to a number of metabolic risk factors, such as other anthropometrics. Conclusion: This study investigated how a low HDL-C was correlated with VFA and how a high DBP was associated with SFA in Korean male children and adolescents. Our results suggest that the correlation between the VFA, SFA, and metabolic risk factors was relatively weak compared to that reported in previous adult studies.

An Epidemiologic Study of Low Back Pain of Women Working at a General Hospital (종합병원 근무 여성의 요통에 대한 요인 분석)

  • Background : The aim of this study was to confirm the risk factors for low back pain and injury to improve the prevention and treatment of lower back pain. Materials and Methods : An epidemiologic study of low back pain and injury was performed with questionnaires distributed to 471 women working at Yeungnam university hospital. The differences in low back pain and injuries among various hospital departments were analyzed by a one-way analysis of variance (ANOVA), and the relevant factors included in the questionnaire were determined by a factor analysis. Results : The frequency of low back pain in women in the department of diet and in the maintenance department was higher than among the other departments. The frequency of low back pain was mainly related to the frequency of psychosomatic symptoms. In addition, the low back pain was partially related to the frequency of psychosomatic symptoms and partially related to the frequency of pushing during the workday. The degree of disability from low back pain was increased by lifting and hard physical work and was related to the frequency of psychosomatic symptoms and the degree of work dissatisfaction. The frequency of low back injury was increased by increased standing time during work and hard physical work. The frequency of low back injury was related to advancing age and in part to psychosomatic symptoms. Conclusion : Lower back pain and its associated complications are related to psychosomatic factors and type of work. Lower back injury is related to physiological factors such as age. For the prevention and treatment of lower back pain, a multidisciplinary approach is required.
