  • Purpose: A study of the family planning and Maternal-Child Health Services that disciplined itself in primary health care post form 1980 to 2009. Method: Investigation studies family planning in primary health care post and a change process of a Maternal-Child Health Services into case by case until 2009 from 1980. Results: Our country family planning business began at economic development dimensions in order to solve a poverty issue. This business goal were childbirth decrease of pregnancy possibility couple aged 19~49 and improve to mother and child health. For this goal, all kinds of health education included sex education and contraception education, contraception service, comprehensive maternal and child health service that management of front and back of childbirth etc. are provided. According to fail down a birthrate from 6.0(1962) to 1.25(2009), the nation reached to a dilemma called childbirth encouragement policy. Conclusions: Decrease of labor supply by low birthrate, decrease numerical an employed person by aging was brought a labor shortage and decrease of productivity of labor of industrial manpower. Deterioration phenomenon of financial income and expenditure by consumption and investment contraction caused decrease of slowdown of economic growth and potential growth rate, and a social cost burden is increased by deterioration financial old man support burden increase by this and pensions and health insurance, a sharp increase of social welfare cost etc. Now, in order to solve a low birth issue, the government establishes a whole nation forwarding system and establishes basic plan social low birth and advanced age, and to prepare for childbirth fault factors removal and advanced age society shall endeavor.

Effects of Socio-demographic Factors on the Decreasing Trend in the Sex Ratio at Birth in Korea, 1997-2017

  • Objectives: This study investigated the effects of socio-demographic factors on the decreasing trend in the sex ratio at birth from 1997 to 2017 in Korea. Methods: Data from 10 349 602 live births registered with Statistics Korea from 1997 to 2017 were analyzed. The secondary sex ratio (SSR), defined as the ratio of male to female live births, during the study period (1997-1999 [phase I], 2000-2002 [phase II], 2003-2005 [phase III], 2006-2008 [phase IV], 2009-2011 [phase V], 2012-2014 [phase VI], and 2015-2017 [phase VII]) was calculated according to selected socio-demographic factors, such as parental age, education, occupation, and birth order. Logistic regression models were used to estimate odds ratios (ORs) and 95% confidence intervals for a male birth after adjusting for potential confounders. Results: The SSR gradually decreased from 1.10 (in 1998-2000 and 2002) to 1.05 (in 2013-2016). While a decreasing trend in the SSR was not noted among first births, male-biased sex ratios were prominent among third and higher-order births, for which the highest SSR was 1.46 in 1998. Higher birth order was significantly associated with an excess of male births in phases I-VI (≥third vs. first, OR range, 1.03 to 1.35). Advanced maternal age was significantly associated with an excess of female births in phases II, III, and V (≥40 vs. 20-24 years, OR range, 0.92 to 0.95). Conclusions: This study demonstrated that advanced maternal age and reduction of the artificially-biased SSR among third and higher-order births may partially explain the decreasing trend in the SSR from 1997 to 2017 in Korea.

Parental Age-Related Risk of Retinoblastoma in Iranian Children

  • Background: Retinoblastoma is a rare malignant intraocular neoplasm. About 90% of cases feature a germline mutation in the RB1 gene and these will develop retinoblastoma during their early childhood. An association between mutations in germline cells and aging has been demonstrated. This suggests a higher incidence of childhood cancer including retinoblastoma among children of older parents. Materials and Methods: In the present study we aimed to determine the association of paternal and maternal age with an increased risk of retinoblastoma in a case-control study in Iranian population. The study was carried out on 240 persons who were born during 1984-2012 in Mahak and Mofid hospitals in Tehran, Iran. The statistical analysis included studying the mean age of parents and in order to know whether parental age of patients is different from parental age of control group, (t-test) compare averages test is used perfectly. By binary logistic regression, odds ratios (ORs) and 95% confidence intervals (CIs) were calculated. Results: The results of statistical analysis including the study of mean parental age by the use of (t-test) compare averages test showed a significant difference between parental ages of patients and controls. Logistic regression showed that coefficients were significant for maternal but not paternal age. Conclusions: Our findings indicate that advanced maternal age can increase the risk of retinoblastoma in offspring, but the paternal age has no significant effect.

출산 후 산모들의 주관적인 신체 평가에 대한 연령과 분만방식에 따른 분석 (A Study on the Self-related Postpartum Body Evaluation by Maternal Age and Delivery Method)

  • Objectives: The purpose of this study is to compare postpartum symptoms of 528 women by age groups and delivery method through health questionnaires of postpartum care center. Methods: From January 2018 to December 2018, we statistically compared the 528 women's postpartal symptoms who divided into 4 age groups using SPSS Statistics 21.0 program. Results: There were no differences in weight changes, gestational age and degree of subjective discomfort by age (p>.05). On the other hand, parity, fetal weight and delivery method were statistically associated with age (p<.05). Especially the rate of cesarean delivery was increased with age. Degree of feeling dyspepsia and edema was higher in cesarean section delivery group (p<.05). Also, as the weight increased by 1 kg during pregnancy, the degree of feeling postnatal edema increased by 0.204 (β=0.204, t=4.204, p<.05). Conclusion: This study showed that age itself will not affect postpartum symptoms alone. Instead it is necessary to consider post-cesarean delivery symptoms for advanced maternal age.

Effects of paternal age on human embryo development in in vitro fertilization with preimplantation genetic screening

  • Objective: As paternal age increases, the quality of sperm decreases due to increased DNA fragmentation and aneuploidy. Higher levels of structural chromosomal aberrations in the gametes ultimately decrease both the morphologic quality of embryos and the pregnancy rate. In this study, we investigated whether paternal age affected the euploidy rate. Methods: This study was performed using the medical records of patients who underwent in vitro fertilization (IVF) procedures with preimplantation genetic screening (PGS) from January 2016 to August 2017 at a single center. Based on their morphological grade, embryos were categorized as good- or poor-quality blastocysts. The effects of paternal age were elucidated by adjusting for maternal age. Results: Among the 571 total blastocysts, 219 euploid blastocysts were analyzed by PGS (38.4%). When the study population was divided into four groups according to both maternal and paternal age, significant differences were only noted between groups that differed by maternal age (group 1 vs. 3, p= 0.031; group 2 vs. 4, p= 0.027). Further analysis revealed no significant differences in the euploidy rate among the groups according to the morphological grade of the embryos. Conclusion: Paternal age did not have a significant impact on euploidy rates when PGS was performed. An additional study with a larger sample size is needed to clarify the effects of advanced paternal age on IVF outcomes.

Preferred strategy for euploid single embryo transfer in advanced maternal age: Fresh versus frozen

  • Objective: The purpose of this study was to compare fresh and frozen-thawed euploid blastocyst transfer protocols following preimplantation genetic screening (PGS) in cases of advanced maternal age. Methods: A total of 330 patients were examined retrospectively. PGS was performed on the embryos of 146 patients for whom fresh transfers were chosen. In contrast, frozen-thawed euploid single embryo transfer (ET) was selected after PGS for 184 patients, and their embryos were vitrified. The percentage of euploid embryos and rates of implantation, pregnancy, and pregnancy continuity, as well as clinical and biochemical abortion rates, were compared. Results: The numbers of retrieved oocytes, metaphase II oocytes, and fertilized ova were greater in the frozen-thawed group. The percentages of euploid embryos were comparable between the fresh and frozen-thawed groups (32% vs. 34.8%, respectively). The rates of implantation (46.6%vs. 62.5%), pregnancy (50% vs. 66.8%), ongoing pregnancy (38.4% vs. 53.8%), and live birth percentage (37.0% vs. 53.8%) were significantly higher in the frozen-thawed group. However, no significant differences were found in the clinical and biochemical abortion rates. Conclusion: The use of frozen-thawed single euploid ET is associated with increased implantation and pregnancy rates compared to fresh single euploid ET with PGS.

Endocrine problems in children with Prader-Willi syndrome: special review on associated genetic aspects and early growth hormone treatment

  • Prader-Willi syndrome (PWS) is a complex multisystem genetic disorder characterized by hypothalamic-pituitary dysfunction. The main clinical features include neonatal hypotonia, distinctive facial features, overall developmental delay, and poor growth in infancy, followed by overeating with severe obesity, short stature, and hypogonadism later in development. This paper reviews recent updates regarding the genetic aspects of this disorder. Three mechanisms (paternal deletion, maternal disomy, and deficient imprinting) are recognized. Maternal disomy can arise because of 4 possible mechanisms: trisomy rescue (TR), gamete complementation (GC), monosomy rescue (MR), and postfertilization mitotic nondisjunction (Mit). Recently, TR/GC caused by nondisjunction at maternal meiosis 1 has been identified increasingly, as a result of advanced maternal childbearing age in Korea. We verified that the d3 allele increases the responsiveness of the growth hormone (GH) receptor to endogenous GH. This paper also provides an overview of endocrine dysfunctions in children with PWS, including GH deficiency, obesity, sexual development, hypothyroidism, and adrenal insufficiency, as well as the effects of GH treatment. GH treatment coupled with a strictly controlled diet during early childhood may help to reduce obesity, improve neurodevelopment, and increase muscle mass. A more active approach to correct these hormone deficiencies would benefit patients with PWS.

한방치료 후 임신에 성공한 반복 임신손실 환자 68례의 산과적 결과 및 출산 성공 영향 인자의 분석 (Obstetric Outcomes in 68 Pregnant Patients with Recurrent Pregnancy Loss on Oreintal Treatment and Analysis of Factors Affecting the Success of Birth)

  • Purpose: To estimate the effect of oriental treatment on recurrent pregnancy loss, a retrospective analysis was done. Methods: Sixty-eight pregnant women at the $\bigcirc\bigcirc$ oriental clinic, Korea, from January 2005 to May 2009 diagnosed as recurrent pregnancy loss were included in this study. The sixty-eight patients received oriental treatment such as acupuncture, moxibution, herbal acupuncture and herbal prescriptions, divided into two groups: Group A- live birth(N=58) and Group B- abortion(N=10). The maternal age, parity, menstrual history, gynecological history and period of treatment were compared. To find out factors affecting the success of birth, we performed binary logistic regression analysis(SPSS ver. 14.0 for windows). Results: The live birth rate was 85.3%. The maternal age, parity, menstrual history, gynecological history and period of treatment were not different between two groups. Logistic regression analysis showed that the significant factors predicting the occurrence of miscarriage were advanced maternal age(${\geq}35$)(P=0.005, Odds Ratio[OR]=3.809, 95% Confidence Interval[95%CI]: 1.514-9.585) and suffering from gynecological problems(P=0.044, OR=4.048, 95%CI: 1.037-15.801). Conclusions: The results suggest that oriental treatment has effectiveness on recurrent pregnancy loss. Further study will be needed.

An Analysis of the proteomics approach to the glycated peptides of human milk

  • Many studies have shown that advanced glycation end-products (AGEs) and glycation adducts are significantly linked to aging and disease. Particularly, the level of glycation in human milk is important because the AGE intake is closely related to AGE levels in infants. In this study, we used human milk samples obtained from four primiparae and four multiparae. We isolated proteins using acetone and trichloroacetic acid (TCA) precipitation. A total of 67 glycated proteins and 122 glycated peptides was quantified; among them, 19 glycated peptides were differentially expressed. We confirmed that the degree of glycation differed according to fertility. The study provides a foundation for using proteomics to evaluate the mother's milk quality and link between maternal health and human milk quality.

여성의 첫 출산 연령이 저체중출생아의 출생률에 미치는 영향 (Effect of women's first childbearing age on the risk of low birth weight)

  • 목 적 : 사회경제적 환경의 변화로 첫 아이를 출산하는 어머니의 연령이 늦어지고 있다. 많은 연구자들이 첫 아이를 출산하는 어머니의 연령이 늦어지는 것과 저체중출생아의 출생율과의 관계에 대해 연구하였으나 아직 일치된 연구결과를 얻지 못하고 있다. 이에 따라 본 연구자들은 산모의 연령이 저체중출생아의 출생률에 미치는 영향을 알아보고자 본 연구를 시행하였다. 방 법 : 2001년부터 2003년까지 통계청의 인구동태자료 중 출생기록에 등록된 1,545,338명 중 단태아면서 첫 번째 아이인 736,167명을 대상으로 저체중출생아와 극소저체중출생아의 출생률에 대해 어머니의 나이, 직업, 학력, 결혼유무, 거주지, 결혼 기간, 아버지의 직업과의 관계를 다중 로지스틱 회귀분석을 이용하여 분석하였다. 결 과 : 산모의 연령에 따라 저체중출생아가 출생할 위험률은 30-34세, 35-40세, 40세 이상의 초산부는 25세에서 29세의 초산부에 비해 각각 1.3배, 2.1배, 2.4배 높게 나타났으며 어머니의 직업, 학력, 결혼유무, 거주지, 결혼 기간, 아버지의 직업 등을 감안하여도 1.3배, 1.9배, 2.0배로 모두 유의하게 높게 나타났다. 산모의 연령에 따라 극소체중출생아가 출생할 위험률은 30-34세, 40-44세, 45세 이상의 초산부는 25세에서 29세의 초산부에 비해 각각 1.8배, 3.3배, 3.7배 높게 나타났으며 어머니의 직업, 학력, 결혼유무, 거주지, 결혼 기간, 아버지의 직업 등을 보정하여도 1.6배, 2.7배, 2.6배로 모두 유의하게 높게 나타났다. 각각의 산모의 연령과 교육 정도, 결혼유무에 따른 저체중출생아의 출생률을 비교하였을 때 30대의 고등학교 졸업 이상이고 기혼인 산모에서 저체중출생아의 출생률(3.7%)은 20대의 고등학교 졸업이상이고 기혼인 산모보다 더 낮았다. 결 론 : 본 연구에서 산모가 나이가 많아질수록 저체중출생아와 극소저체중출생아가 출생할 위험률이 높으며 어머니의 직업, 학력, 결혼유무, 거주지, 결혼 기간, 아버지의 직업 등을 고려하여도 높다. 하지만 나이가 많은 산모라도 사회경제적 환경이 개선된 다면 저체중출생아의 출생률을 줄일 수 있고 사료된다.
