Foramen of Morgagni hernia is the least common type of all congenital diaphragmatic hernias. These foraminal hernias result from a congenital defect in the development or attachment of the diaphragm to the sternum and costal arch. They occur most commonly on the right side, possibly because of pericardial reinforcement of the left. In our case, occurred on the right side and the contents of the hernial sac were omental fat and a part of transverse colon. We performed transthoracic approach for reduction and repair of foraminal hernia with ligation and interrupted mattress sutures of the margin of the defected diaphragm to the posterior part of the sternum and costal cartilage. The postoperative course was uneventful except posttraumatic delirium and discharged at 21st postoperative day.
Mussels are stubborn organism attached to solid substrate by byssus threads and caused operational problems in utility of power generating stations. Sole and combined usage of ultrasonic (28 kHz- and 42 kHz- frequencies) and hydrogen peroxide ($H_2O_2$) has studied for control of blue mussel larvae and adult stage in seawater condition. A theoretical wo rking model using disinfection (Chick and Watson type) approaches is presented based on helpful results of experiments. This study also demonstrate that the combined treatment (ultra-sonication with $H_2O_2$) is overall highly efficient than individual treatment would, but on the basis of exposure time, the ultra-sonication was the most efficient among them. Therefore the development of sole and combined technique might be effective practical mitigation strategy against mussel attachment for water handling facilities.
This research was intended to design an experimental girdle with thermal insulation function for adult women in their 20s. The design of the experimental girdle was based on the pattern of commercially available girdle. The final pattern of the experimental girdle was established according to the drawing equations determined based on the result of appearance evaluation. The equations were (waist circumference${\times}0.88$)/2 for waist circumference, (hip circumference${\times}0.77$)/2 for hip circumference, and (thigh circumference${\times}0.85$) for thigh circumference. In order to develop a heating device, the most effective fabric heater was adopted based on the experiments about the number of caron fibers, heater size and attachment site. Three heaters-one with a size of $14.5{\times}9.5$ cm, and the other two with the size of $8.0{\times}15.0$ cm-were attached to the areas corresponding to the lower abdomen and the hip, 5 cm below the waist. A heater was developed by connecting these heaters to a controller, 2 batteries (7.4 V 2000 mAh lithium polymer batteries) and a switch (for mode conversion between high/medium/low temperatures). The heater was integrated into the inside of the girdle, so that attachment and detachment were possible without the change of appearance. The tentative configuration plan was proposed for the development of a functional smart girdle with an excellent thermal insulation effect.
In order to evaluate the effects of the early exposure of e-PTFE membrane on the periodontal regeneration, 21 cases of 21 patients diagnosed as the chronic adult periodontitis were evaluated. All were class II furcation involvement cases. The control group was composed of 7 cases treated only by the flap operation. 14 cases were treated by the e-PTFE membrane as the experimental group, the membranes of 7 cases were exposed more than 1mm during healing period, which were named as the experimental group I, and the others, experimental group II. Clinical parameters such as probing pocket depth, clinical attachment level, bone probing depth, and gingival recession were recorded before the treatment and 6 months after the treatment. The results were as follows. 1. Significant probing depth reductions were observed for all groups(p<0.05), but no group shows significantly greater reductions than another. 2. Significant clinical attachment gains were observed for the experimental group II(p<0.05), no significant gains were observed in the other groups. 3. Significant bone probing depth reductions were observed for the experimental group II(p<0.05), no significant reductions were observed in the other groups. 4. All but the experimental group II exhibited a significant increase in gingival recession(p<0.05). The result suggested that is case of the e-PTFE membrane is exposed, the result is similar to that of flap operation without membrane. Therefore selecting the proper treatment case, intricate surgical procedure and infection control are essential for minimizing the chance of membrane exposure and finally for the good treatment results.
The aim of the present investigation was to see the effect of combined use of PDGF BB and IGF -1 on the guided tissue regeneration(GTR) using barrier membrane in the treatment of human furcation involvement. Twelve patients with initially diagnosed as having moderate to advanced adult periodontitis with mandibular class II buccal furcation defects have been wer selected. Initial scaling and root planing has been performed and baseline data consisting of probing depths and attachment levels have been recorded prior to surgical procedures. The GTR procedures using either barrier membrane(control : ePTFE) alone or together with the application of PDGF - BB and IGF -l(experimental : ePTFE+PDGF/IGF) have been done under the routine guidelines. During the surgery, the distance from CEJ either to the bottom of the bone defects(CEJ - BD) or to the bone crest(CEJ-BC) were measured. Horizontal distance to the deepest area in the furcal defects were measured from the reference line connection the most prominent bony walls of the two buccal roots. 6 months following the GTR therapy, all the measurements were made repeatedly. The probing attachment gain of the experimental and the control grous were 2.14mm and l.07mm, respectively with no statically significnant difference. Amont of vertical bone fill in the experimental and the control groups were 2.43mm and 2.29mm, rexpectively. Amonut of horizontal bone fill were 2.86mm in the experimental group and 2.17mm in the control group, respectively. However, there were no significant differences in the amount of bone fill(both vertical and horizontal)between the two groups.
The purpose of the present investigation was to compare the effectiveness of porous hydroxyapatite (PHA) and coralline based porous calcium carbonate(PCC) as implant materials in human periodontal osseous defects. 10 adult patients having periodontitis and 2 similar angular osseous defects ${\ge}$5mm as verified by radiographic analysis and clinical probing depth ${\ge}$4mm were selected. The measurements were recorded just before surgery and after 6 month. Clinical parameters used in this study included gingival recession, pocket depth, probing attachment level, Sulcus Bleeding Index, Plaque Index, tooth mobility and bone defect depth measurements. After initial therapy, patients were treated with mucoperiosteal flap surgery. The contralateral bony defects in each patient randomly assigned to either bone graft material, one with PHA(Interpore 200) and the other with coralline based calcium carbonate(Biocoral). After 6 month both groups showed statistically significant reduction of pocket depth, Sulcus Bleeding Index, Plaque Index and significant improvement in probing attachment level. No statistically significant differences were found between the groups. There were 3.0mm or 68% of bone repair with PHA and 3.1mm, 61% with PCC. These values were likewise not significantly different. The data and clinical impression strongly suggest that both PHA and PCC are alloplastic implants with clinically apparent acceptance by the soft and hard tissue and that they can be used as bone graft materials successfully.
The primary aim of this systematic review was to assess the evidence on periodontal disease progression after treatment in patients receiving supportive periodontal therapy (SPT) and to identify predictors of clinical attachment level (CAL) loss. A protocol was developed to answer the following focused question: In adult patients treated for periodontitis, what is the disease progression in terms of CAL loss after surgical or non-surgical treatment? Randomized controlled clinical trials, prospective cohort studies, and longitudinal observational human studies with a minimum of 5 years of follow-up after surgical or non-surgical treatment that reported CAL and probing depth changes were selected. Seventeen publications reporting data from 14 investigations were included. Data from 964 patients with a follow-up range of 5-15 years was evaluated. When the CAL at the latest follow-up was compared to the CAL after active periodontal therapy, 10 of the included studies reported an overall mean CAL loss of ${\leq}0.5mm$, 3 studies reported a mean CAL loss of 0.5-1 mm, and 4 studies reported a mean CAL loss of >1 mm. Based on 7 publications, the percentage of sites showing a CAL loss of ${\geq}2mm$ varied from 3% to 20%, and a high percentage of sites with CAL loss was associated with poor oral hygiene, smoking, and poor compliance with SPT. The outcomes after periodontal therapy remained stable over time. Disease progression occurred in a reduced number of sites and patients, mostly associated with poor oral hygiene, poor compliance with SPT, and smoking.
Co-residence is a type of intergenerational private transfers of resources: money, time and space. Adult daughters and their elderly parents decide to co-reside, depending on their utility levels before and after co-residence that mainly depend on the health status of the elderly. Therefore, co-residence implies positive net benefits to both parties in the sense that, when they co-reside, elderly parents share childcare and adult daughter provide elderly care. In other words, formal (paid) care can be substituted with informal (unpaid) one. Both marriage and giving births are considered as the major obstacles to labor market attachment of women who bear burdens of home production and childcare. Co-residence can be a solution for married women to avoid career interruption by sharing burdens with their elderly parents. However, most previous studies using the U.S. data on intergenerational private transfers focused on elderly care and have concluded that they reduce government expenditures associated with public subsidies to the elderly. This study focuses on adult daughters and it examines effects of co-residence on labor supply of married women in Korea, who face limited formal childcare programs in terms of both quantity and quality. It applies the Tobit model of married women's labor supply to the data from the Second Wave of the Korean Labor and Income Panel Survey( 1999), in order to investigate effects of co-residence and the work and health status of the co-residing elderly as well as their own health status. Four specifications of the empirical model are tested that each includes co-residence with elderly parents, their gender, or their work and health status. Estimation results show that co-residence, co-residence with female elderly, and co-residence with not-working female elderly have significant positive effects on labor supply of married women while poor health status of co-residing female elderly does not bring about any negative effects. However, co-residence with male elderly, regardless of their work and health status, has no significant effect The results indicate that co-residence is closely related to sharing of home production among female elderly and adult daughters who are married and, through intergenerational private transfers of resources in terms of time, it helps women avoid career interruption.
The purpose of this study was to classify latent profiles based on SNS addictive tendencies of married women and to examine the effects of age, adult attachment, extrinsic motivation of the fear of missing out and marriage satisfaction. A total of 300 married women who have been using SNS participated in this study. The result indicated that there were three latent profiles for SNS addictive tendencies of married women : (1) high-risk group (2) potential-risk group (3) low-risk group. The high-risk group had high scores in all sub-areas (obsession and withdrawal symptoms, over-communication and immersion, over-consumption). The high-risk group accounted for 24.51% of the total and the potential-risk group was 44.33%, which had the highest proportion of them. While attachment anxiety, attachment avoidance, extrinsic motivation of the fear of missing out, and marriage satisfaction were significant determinants of latent profiles, age was not. This study divided groups according to the characteristics of married women's SNS addiction tendencies and provided implications for counseling and education according to group characteristics, and discussed the need for various research through the development of standardized SNS addiction scale for married women.

Adolescence is a period of transition from child to adult and is a very important period in which to think about the direction to go in the future and establish one's ego. However, the level of life satisfaction experienced by Korean youths in the adolescent period has been shown to be the lowest among OECD countries. It is very critical to understand which factors are important in determining the level of life satisfaction and how they affect it. In this study, to analyze the factors which affect the life satisfaction of youths and their learning attitude - essential school activity prior to life satisfaction in the youth period, we firstly focused on the relationships with their parents, peers and teachers as potential factors affecting their learning attitude and life satisfaction. This study found that Parenting Style, Peer Attachment and Relationship with Teachers significantly influenced Life Satisfaction either directly or indirectly. We analyzed the structural relationships among these factors using the structure equation model, discussed the analysis results and concluded with further research directions.
