치근활택술과 Tetracycline fiber적용의 임상적 효과 (The Clinical Effect of Tetracycline Fiber used in conjuction with Root Planing)

  • In this study, 21 patients diagnosed as adult periodontitis were divided into 4 groups. One quadrant with an average of 6mm deep pocket depth was chosen from each individual - Group A inserted tetra-cycline fiber after removing supragingival calculus while group RP had calculus removal and root planning alone. Group RP+A received combination of these treatments while group C received none. Plaque index, bleeding on probing, pocket depth, attachment level, and distribution of subgingival plaque were compared and evaluated among these groups at periods of first visit, 4th week and 8th week. The results were as follows ; 1. Plaque index and bleeding on probing improved after treatment and no significant difference was found between the groups. 2. When comparing the change in pocket depth between the groups, the use of tetracycline fiber showed significant reducton in pocket depth comparable to root planing. Combined therapy of tetracycline fiber and root planing showed synergistic effect in pocket depth reduction. 3. When comparing the change in attachment level between the groups, the use of tetracycline fiber showed significant increase in clinical attachment level comparable to root planing, but no synergistic effect was found in the combined therapy. 4. When comparing the change in the motile bacteria ratio between the groups, group RP and group RP+A showed significant decrease compared with control group. 5. There were no severe adverse effects from using tetracycline fiber, except for a few patient who experienced mild discomfort. In summary, the use of local adminstration of tetracycline fiber in adjunction to mechenical treatment can be effective for adult periodontitis.

치면세균막내의 Fusobacterium nucleatum과 Actinobacillus actinomycetemcomitans의 동정을 위한 세균배양법 및 Multiplex PCR법의 비교 (Comparison between Bacterial Culture Method and Multiplex PCR for Identification of Fusobacterium nucleatum and Actinobacillus actinomycetemcomitans from the Dental Plaques)

  • 본 연구는 성인성 치주염 환자의 치은연하 치면세균막을 총 60개 치아에서 채취하여 F. nucleatum과 A. actinomycetemcomitans의 동정을 위해 세균배양법, single PCR법 및 mutliplex PCR법을 실시하였고, 세균 동정법간의 비교를 통해 다음과 같은 결과를 얻었다. 1. F. nucleatum과 A. actinomycetemcomitans의 동정을 위해 세균배양법, single PCR 및 multiplex PCR을 실시한 결과 F. nucleatum은 각각 12개(20.0%), 45개(75.0%), 43개(71.7%) 치아에서 양성반응을 보였지만, A. actinomycetemcomitans는 각각 0개(0.0%), 4개(6.7%), 1개(1.7%) 치아에서 양성반응이 나타났다. 2. F. nucleatum은 세균배양법에 비해 single PCR법 및 multiplex PCR법에서 높은 검출 빈도를 보여 좀 더 효율적인 세균 동정법으로 생각되었지만, 통계적으로는 유의한 차이가 없었다. 3. A. actinomycetemcomitans는 세균배양법에서 전혀 검출되지 않아 통계적으로 검정할 수 없었고, 세균 동정법간의 비교도 어려웠다. 4. F. nucleatum과 A. actinomycetemcomitans의 동정을 위한 single PCR법과 multiplex PCR법 간의 비교에서 두 세균 모두 검출 빈도에 있어서는 큰 차이를 보이지 않았지만, 통계적으로는 유의한 차이를 보였다.

치주낭 탐침깊이와 치은연하치태내 Trypsin-like activity와의 관계 (RELATIONSHIP BETWEEN PROBING POCKET DEPTH AND TRYPSIN-LIKE ACTIVITY IN SUBGINGIVAL PLAQUE)

  • In order to determine the relationship between probing pocket depth and trypsin-like activity in subgingival plaque, probing pocket depth and loss of attachment were measured by Michigan-O probe on mandibular incisors of 30 patients with adult periodontitis. And the trypsin-like activity of Treponema denticola, Porphyromonas gingivalis, and Bacteroides forsythus was evaluated by the hydrolysis of N-Benzoyl-DL-Arginine-2-Naphthyla-mide (BANA) using PerioScan reagent cards(Oral-B Laboratories, Redwood City, CA). The obtained data were statistically analyzed by Microstat program. The results were as follows. 1. The number of teeth showing negative trypsin-like activity was more in shallow periodontal pocket groups, but the number of teeth showing positive trypsin-like activity was more in deep periodontal pocket groups. 2. There was a significant positive correlation between probing pocket depth and trypsin-like activity in subgingival plaque(y=0.413X - 0.955, r = 0.7024, p<0.001). 3. There was no consistent relationship between loss of attachment and trypsin-like activity in subgingival plaque(p>0.01).

골결손과 치주질환 유도 후 성견 절치의 실험적 함입이동시 치주조직의 반응 (PERIODONTAL RESPONSE FOLLOWING APPLICATION OF INTRUSIVE FORCES TO THE INCISORS WITH BONE LOSS AND PERIODONTAL DISEASE IN ADULT DOGS)

  • 치주질환에 이환된 성인환자에서 상악전치의 치간공간과 아울러 정출이 발생된 경우 기능 및 심미의 회복을 위해서는 함입이동이 필요하다. 본 연구는 골하치주낭 즉, 수직적 골결손이 있는 절치의 함입이동시 치주조직 반응을 살펴보기 위하여 시행되었다. 상악 좌우 측절치에 골결손과 함께 치주질환을 유발시킨 성견 4마리를 실험대상으로 하여 양측 모두에 치주수술을 시행하고 결손부 최하방 치근면에 notch를 형성 한 2주후 우측 제2측절치는 4주간 함입이동 후 4주간 보정을 시행한 실험측으로, 좌측 제2측절치는 구강위생관리만 시행한 대조측으로 사용한 바 조직소견을 통하여 다음과 같은 결론을 얻었다. 1. 함입이동없이 치주수술만 시행한 대조측의 조직소견은 치주낭 깊이의 감소를 보였으나 notch 부위부터 치주낭 기저부까지 긴 접합상피 상태를 보였으며 신생백악질이 형성된 일부에서만 결합조직 부착의 양상이 보였다. 2. 함입이동을 시행한 실험측은 대조측에 비하여 상피부착이 줄어든 반면 보다 넓은 신생 결합조직 부착 소견을 나타내었다. 3. 대조측과는 달리 실험측 notch근처의 결합조직내에는 다수의 세포가 관찰되었으며, 신생 백악질 생성 역시 대조측보다 많이 관찰되었다. 이상의 결과는 치주질환으로 인하여 정출된 치아에서도 철저한 염증조절과 구강위생이 유지되고 적절한 크기의 힘에 의하여 순수한 함입이동이 적용된다면 바람직한 치료결과가 가능함을 시사하였다.

Real-time PCR quantification of 9 periodontal pathogens in saliva samples from periodontally healthy Korean young adults

  • Purpose: Few studies have examined periodontal pathogens from saliva samples in periodontally healthy young adults. The purposes of this study were to determine the prevalence of periodontopathic bacteria and to quantify periodontal pathogens in saliva samples using real-time polymerase chain reaction (PCR) assays in periodontally healthy Korean young adults under 35 years of age. Methods: Nine major periodontal pathogens were analyzed by real-time PCR in saliva from 94 periodontally healthy young adults. Quantification of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Prevotella intermedia, Fusobacterium nucleatum, Campylobacter rectus, Peptostreptococcus anaerobius, and Eikenella corrodens was performed by DNA copy number measurement. Results: F. nucleatum and E. corrodens were detected in all subjects; the numbers of positive samples were 87 (92.6%), 91 (96.8%), and 90 (95.7%) for P. gingivalis, P. anaerobius, and C. rectus, respectively. Other pathogens were also detected in periodontally healthy subjects. Analysis of DNA copy numbers revealed that the most abundant periodontal pathogen was F. nucleatum, which was significantly more prevalent than all other bacteria (P<0.001), followed by P. anaerobius, P. gingivalis, E. corrodens, C. rectus, and T. denticola. There was no significant difference in the prevalence of each bacterium between men and women. The DNA copy number of total bacteria was significantly higher in men than in women. Conclusions: Major periodontal pathogens were prevalent in the saliva of periodontally healthy Korean young adults. Therefore, we suggest that the development of periodontal disease should not be overlooked in periodontally healthy young people, as it can arise due to periodontal pathogen imbalance and host susceptibility.

Scaling and Root Planing with Concomitant Subgingival Curettage

  • Non-surgical therapy is still an important technique in periodontal treatment. In this study, scaling and root planing(SRP) with or without concomitant subgingival curettage were compared clinically and microbiologically. 14 moderate adult periodontitis patients were included in this study. After 2 weeks from screening visit, with split mouth design, one quadrant was treated by SRP, and the opposite side was treated by SRP with subgingival curettage. Clinical measurement and microbiological analysis was taken at baseline, 1 month, 3 month post-treatment. Clinical parameters used in this study was probing depth, gingival recession, gingival index, bleeding on probing, plaque index, tooth mobility(Periotest Value). Microbiological analysis consisted of determination of the percentages of 4 bacterial groups according to morphologic type with phase-contrast microscope and measuring Black-pigmented Bacteroides after anaerobic culture. 1. There were significant changes in probing depth and gingival recession at 1 month(P<0.05), and these changes remained through 3 month. However, no significant differences were observed between two groups(P<0.05). 2. There were also significant reductions in gingival index and bleeding on probing at 1 month(P<0.05),and these reduced levels were maintained through 3 month with no significant differences between two groups(P<0.05). 3. In both groups, motile bacteria decreased significantly at 1 months(P<0.05), but increased nearly to baseline level at 3 month. 4. The percentages of Black-pigmented Bacteroides, in both groups, decreased significantly at 1 month(P<0.05), and in the subgingival curettage group, significant more reductions were observed than in the root planing group(P<0.05). At 3 month, significant reduction was found in subgingival curettage group only(P<0.05). According to these results, we surmised that concomitant subgingival curettage and root planing give some advantageous effect on bacterial recolonization.

GTR치료시 e-PTFE 차단막의 노출에 따른 하악이개부의 치유효과 (Evaluation of the Effects of e-PTFE Membrane Exposure on the GTR in the Mandubular Furcation Involvement)

  • In order to evaluate the effects of the early exposure of e-PTFE membrane on the periodontal regeneration, 21 cases of 21 patients diagnosed as the chronic adult periodontitis were evaluated. All were class II furcation involvement cases. The control group was composed of 7 cases treated only by the flap operation. 14 cases were treated by the e-PTFE membrane as the experimental group, the membranes of 7 cases were exposed more than 1mm during healing period, which were named as the experimental group I, and the others, experimental group II. Clinical parameters such as probing pocket depth, clinical attachment level, bone probing depth, and gingival recession were recorded before the treatment and 6 months after the treatment. The results were as follows. 1. Significant probing depth reductions were observed for all groups(p<0.05), but no group shows significantly greater reductions than another. 2. Significant clinical attachment gains were observed for the experimental group II(p<0.05), no significant gains were observed in the other groups. 3. Significant bone probing depth reductions were observed for the experimental group II(p<0.05), no significant reductions were observed in the other groups. 4. All but the experimental group II exhibited a significant increase in gingival recession(p<0.05). The result suggested that is case of the e-PTFE membrane is exposed, the result is similar to that of flap operation without membrane. Therefore selecting the proper treatment case, intricate surgical procedure and infection control are essential for minimizing the chance of membrane exposure and finally for the good treatment results.

  • After periodontal surgery, the potential healing responses were occurred by interaction among junctional epithelium, gingival connective tissue, alveolar bone and periodontal ligament. The only cell that created periodontal regeneration was derived from periodontal ligament. The aim of the study was to evaluate the regenerative effects of the collagen membrane($collacote^{\circ}C$) and autogenous connective tissure graft with periosteum. Experimental periodontitis were created in furcation area of 4 adult dogs with bone removal and gutta percha packing. After 6 weeks later, the gutta percha was removed and experiment was performed divided by 3 groups. 1) Flap operation(control group). 2) Flap operation with collage membrane(Experimental group I). 3) Flap operation with autogenous connective tissue graft with periosteum (Experimental group II). After dogs were sacrificed after two and three weeks, specimens were prepared and stained with hematoxylin-eosin and masson-trichrome stain for light microscopic study. The results were as follows : 1. In all gruoups, connective tissue compartments were increased from two to three weeks especially in experimental group I. 2. Collagen membrane and connective tissue were increased collagen deposits of periodontal ligament. Therefore collagen fiber attached to tooth surface was seen. 3. In al experimental groups, newly forming alveolar bone was seen. 4. Collagen membrane and connective tissue were which prevented proliferation of epithelium, aided connective tissue new attachment and influenced periodontal regeneration.

인체 치간부위 치조골 결손에 사용된 합성골의 효과에 관한 연구 (THE EFFECTS OF POROUS HYDROXYAPATITE AND NATURAL CORAL ON HUMAN PERIODONTAL DEFECTS)

  • Various alloplastic materials have been used on the periodontally diseased ossous defects. Hydroxyapatite, which is used the most common alloplastic material is a non-resorbable form of calcium phosphate and natural coral which is a biodegradable by carbonic anhydrase in osteoclast was introduced recently. The purpose of the present study was to evaluate the clinical effects of porous hydoxyapatite and natural coral on the human periodontal defects. Four males and three females who had adult periodontitis were selected for this study. The teeth that had similar bone loss radiographically and periodontal pocket deeper than 5mm were selected. Gingival recession, pocket depth, plaque index(Silness & Loe), sulcus bleeding index and tooth mobility (measured by Periotest$^{(r)}$) were examined before graft. Before insertion of alloplastic materials, the depth from CEJ to bone crest and from CEJ to base of the osseous defect was recorded. Porous particulate hydroxyapatite(Interpore 200$^{(r)}$, A group) was place on the defect and natural coral(Biocoral$^{(r)}$, B group) was placed on the defect of the opposing tooth. Six months post-surgically the same parameters were recorded by reentry procedures. A and B group showed 0.6mm of mean recession. Mean reduction of pocket depth were 5mm for A group and 4.9mm of B group. Reduced SBI and tooth mobility were recorded. Osseous defect fills of the original defects were 2.9mm for A and 3mm for B group. Percentage defect fills were 71% for A and 59% for B group. The difference of defect fill between pre- and post-insertion was statstically significant(p<0.05). But the difference between the two groups was not significant statistically(p<0.05). The clinical impression at 6 month re-entry and the numerical date indicate that natural coral as well as porous particulate hydoxyapatite has a definite potential as an alloplastic implant in the treatment of periodontal osseous defects.

Platelet - derived growth factor-BB와 Insulin Iike gowth factor-1이 e- PTFE를 이용한 치근 이개부의 조직유도재생에 미치는 영향 (The Effect Of Platelet - Derived Growth Factor And Insulin - Like Growth Factor On The Guided Tissue Regeneration In The Treatment Of Human Furcation Involvement)

  • The aim of the present investigation was to see the effect of combined use of PDGF BB and IGF -1 on the guided tissue regeneration(GTR) using barrier membrane in the treatment of human furcation involvement. Twelve patients with initially diagnosed as having moderate to advanced adult periodontitis with mandibular class II buccal furcation defects have been wer selected. Initial scaling and root planing has been performed and baseline data consisting of probing depths and attachment levels have been recorded prior to surgical procedures. The GTR procedures using either barrier membrane(control : ePTFE) alone or together with the application of PDGF - BB and IGF -l(experimental : ePTFE+PDGF/IGF) have been done under the routine guidelines. During the surgery, the distance from CEJ either to the bottom of the bone defects(CEJ - BD) or to the bone crest(CEJ-BC) were measured. Horizontal distance to the deepest area in the furcal defects were measured from the reference line connection the most prominent bony walls of the two buccal roots. 6 months following the GTR therapy, all the measurements were made repeatedly. The probing attachment gain of the experimental and the control grous were 2.14mm and l.07mm, respectively with no statically significnant difference. Amont of vertical bone fill in the experimental and the control groups were 2.43mm and 2.29mm, rexpectively. Amonut of horizontal bone fill were 2.86mm in the experimental group and 2.17mm in the control group, respectively. However, there were no significant differences in the amount of bone fill(both vertical and horizontal)between the two groups.
