  • There have been many reports about surgical treatments of flexible flatfoot in children and acquired adult flatfoot deformity due to posterior tibial tendon dysfunction common in the 5th and 6th decades. However there has been a controversy for surgical treatment guideline for painful idiopathic flexible flatfoot deformities in young adults. Therefore, we present a 27-year-old female with severe painful idiopathic flexible flatfoot who was treated with medial sliding calcaneal osteotomy and subtalar arthroereisis using $Kalix^{(R)}$ (Newdeal SA, Vienne, France) endoprosthesis and had good clinical outcome with high patient satisfaction at 10 months follow-up postoperatively.

  • Aspiration of foreign body, a clinical problem seen more commonly in the pediatric age group, is infrequently seen in the adult population. Although rigid bronchoscopy has been the mainstay of treatment, recently, easier manipulation, greater range of visulization, flexibility, topical anesthesia has made flexible bronchoscopy of choice for dealing with aspirated foreign body in adult. Operation increase morbidity and mortality, delay foreigh body removal. A 41-year old male was admitted to this hospital due to aspiration of pushpin. He showed high opaque density protruding in the orifice of right lower lobar bronchus. We report a case of foreign body removal by flexible bronchoscopy with brief review of the literature.

The Efficacy of Surgical Treatment with Flexible Alligator Forceps in Dogs with Heartworm Infection (심장사상충 감염견에서 Flexible Alligator Forceps을 이용한 외과적 치료)

  • This study was performed to assess the efficacy of surgical treatment with flexible alligator forceps (FAF) on heartworm infected dogs. Twenty dogs (10 males and 10 females) with heartworm infection, age $5.3\pm3.0\;(mean{\pm}S.D.)$ years and weight $9.3\pm6.5(mean{\pm}S.D.)kg$ were treated with FAF. Adult heartworms were removed by fluoroguided technique with FAF under inhalation anesthesia with isoflurane. For evaluation of the efficacy, removal rate and surgical time were measured, and removal sites were found. Antigen ELISA kit test and autopsy were performed to examine remained adult heartworms. Removal rate of total heartworms was $91.4\%$ including 15 complete retrieval cases and surgical time was $30.0\pm7.6(mean{\pm}S.E.)$ minutes. Heartworms were removed from the right ventricle, main pulmonary artery, and right and left pulmonary artery. Surgical treatment with FAF is an effective method with high removal rate, short surgical time, and various removal sites and it could be considered for the removal of adult heartworms in a dog.

  • To identify health behavior of Peptic ulcer patients among all employees in Korea and examine relevant factors to present basic data for improving Korean adult employees' health conditions and quality of living, 3,515 employees aged 20 to 69 were sampled from the group that responded to the question concerning the presence of Peptic ulcer in the fourth Korea National Health and Nutrition Examination Survey for the third year (2009). After logistic regression analysis was carried out to see the effects of working conditions, harmful factors, shift work, stress perception, and experience of depression on the presence of Peptic ulcer, it was found that unsafe or unpleasant working conditions led to higher risk of getting Peptic ulcer, that flexible workers were more likely to get Peptic ulcer, and that depression or stress perception exerted almost no effect on Peptic ulcer. With the increase in the number of flexible workers, increasingly more interest is taken in the effects of shift work on health; occupational Peptic ulcer tends to become chronic as compared with the condition in the general population and reappear even after recovery in many cases; therefore, it is necessary to take special management measures by estimating the number of those at a risk of getting Peptic ulcer and by finding out its risk factors.

  • Objective: The aim of this study is to analyze the effect of the strength of the ankle support on the walking characteristics and ankle joints when men with flexible flat feet walk. Method: 13 adult male subjects (age: 23.9 ± 2.4 yrs, height: 173.0 ± 5.0 cm, weight: 76.9 ± 13.2 kg, Navicular Drop Test (NDT): 10.2 ± 0.8 mm) participated in this study. Each participant had to walk with the 3 conditions, barefoot, soft arch support and hard arch support, along a walkway while their kinematics was recorded at 100 Hz. Results: Based on the results of this study, it is considered that men with flexible flat feet should use hard arch support rather than bare feet to induce normal arch shape, relieve foot damage caused by excessive ankle joint abnormalities and improve stability. Conclusion: Our results for men with flat flexibility, there was a significant difference in the value of step length when walking was performed using two arch supports with different strengths. The angle of ankle dorsiflexion was significantly increased, and the ankle eversion angle was significantly decreased.

Successful Transition from Pediatric to Adult Care in Inflammatory Bowel Disease: What is the Key?

  • The incidence of pediatric-onset inflammatory bowel disease (IBD) is on the rise, accounting for up to 25% of IBD cases. Pediatric IBD often has extensive bowel involvement with aggressive and rapidly progressing behavior compared to adult IBD. Because IBD has a high morbidity rate and can have a lifelong impact, successful transition from pediatric to adult care is important to maintain the continuity of care. Furthermore, successful transition facilitates appropriate development and psychosocial well-being among patients, as well as comprehensive and harmonious healthcare delivery amongst stakeholders. However, there are various obstacles related to patients, family, providers, and organizations that interfere with successful transition. Successful transition requires a flexible and tailored plan that is made according to the patient's developmental abilities and situation. This plan should be established through periodic interviews with the patient and family and through close collaboration with other care providers. Through a stepwise approach to the transition process, patients' knowledge and self-management skills can be improved. After preparation for the transition is completed and the obstacles are overcome, patients can be gradually moved to adult care. Finally, successful transition can increase patients' adherence to therapy, maintain the appropriate health status, improve patients' self-management, and promote self-reliance among patients.

Plantar Pressure Distribution During Level Walking, and Stair Ascent and Descent in Asymptomatic Flexible Flatfoot

  • The first purpose was to identify the plantar pressure distributions (peak pressure, pressure integral time, and contact area) during level walking, and stair ascent and descent in asymptomatic flexible flatfoot (AFF). The second purpose was to investigate whether peak pressure data during level walking could be used to predict peak pressure during stair walking by identifying correlations between the peak pressures of level walking and stair walking. Twenty young adult subjects (8 males and 12 females, age $21.0{\pm}1.7$ years) with AFF were recruited. A distance greater than 10 mm in a navicular drop test was defined as flexible flatfoot. Each subject performed at least 10 steps during level walking, and stair ascent and descent. The plantar pressure distribution was measured in nine foot regions using a pressure measurement system. A two-way repeated analysis of variance was conducted to examine the differences in the three dependent variables with two within-subject factors (activity type and foot region). Linear regression analysis was conducted to predict peak pressure during stair walking using the peak pressure in the metatarsal regions during level walking. Significant interaction effects were observed between activity type and foot region for peak pressure (F=9.508, p<.001), pressure time integral (F=5.912, p=.003), and contact area (F=15.510, p<.001). The regression equations predicting peak pressure during stair walking accounted for variance in the range of 25.7% and 65.8%. The findings indicate that plantar pressures in AFF were influenced by both activity type and foot region. Furthermore the findings suggest that peak pressure data during level walking could be used to predict the peak pressure data during stair walking. These data collected for AFF can be useful for evaluating gait patterns and for predicting pressure data of flexible flatfoot subjects who have difficulty performing activities such as stair walking. Further studies should investigate plantar pressure distribution during various functional activities in symptomatic flexible flatfoot, and consider other predictors for regression analysis.

Adult Children's Perception of Types of Relationships with Elderly Parents (성인자녀가 지각하는 노부모와의 관계유형에 관한 연구)

  • This study explores the types of relationships between 410 adult children and their elderly parents based of exchange theory and the Konstanz model. In terms of the exchange of emotional, instrumental, and economic support based on exchange theory, the types of relationships identified included "support offers," "reciprocity," and "support benefits." In terms of conflict, intimacy, support offers, support benefits, and support obligations for adult children based on the Konstanz model, the type of relationships with the father included "an intimate exchange," "conflictual distance," "a conflictual sense of duty," and "a flexible exchange." The type of relationships with the mother included "an intimate exchange," "conflictual distance," "separate distance," and "a conflictual offer." There were no distinct characteristics of relationship types based on exchange theory. However, there were differences in characteristics of relation types based on the Konstanz model by gender. These results have important implications with respect to the Western model.

Calcaneal Lengthening Osteotomy for the Symptomatic Flexible Flatfoot in Adults (성인의 유연성 편평족에 대한 종골 연장술)

  • Purpose: Calcaneal lengthening osteotomy is one option for the treatment of symptomatic flexible flatfoot in adults. The aim of the study was to evaluate the short term clinical and radiologic results and analyze its complications. Materials and Methods: Twelve feet who had undergone calcaneal lengthening osteotomy without flexor digitorum longus transfer between December 2009 and July 2011 were included. The mean age was 40.6 years (23~75 years). The mean followup was 17.3 months (13~25 months). Clinical outcome were assessed using American Orthopadics Foot ans Ankle Society (AOFAS) score and visual analogue scale (VAS) for pain. Four radiologic parameters were measured from weightbearing radiographs to evaluate the difference between preoperatively and postoperatively measures. To analyze the complications, calcaneocuboid joint subluxation and degenerative change were measured postoperatively. Results: The mean AOFAS score improved from 55.3 points preoperatively to 82.2 points at lastest follow-up (p=0.000). The mean VAS improved from 6.3 points preoperatively to 3.2 points postoperatively (p=0.002).All radiologic parameters, the mean talonavicular coverage angle on AP view, the mean talo-1st metatarsal angle on AP and lateral view and the mean calcaneal pitch angle, significantly improved after calcaneal lengthening osteotomy. Nine feets (75%) were shown the degenerative change in the calcaneocuboid joint at latest follow-up radiographs. Conclusion: Calcaneal lengthening osteotomy for the symptomatic flexible flatfoot in adults produced significant improvement in clinical and radiologic parameters, but calcaneocuboid joint osteoarthritis occurred postoperatively remained a major problem.
