Adrenal insufficiency is caused by adrenal failure or impairment of the hypothalamic-pituitary-adrenal axis. The main symptoms of adrenal insufficiency are chronic fatigue, nausea, vomiting, anorexia, and weight loss. We report a case of adrenal insufficiency in a 38-year-old female. The patient complained of headache, dizziness, anorexia, and general weakness. We treated her with Iggisaengjin-tang and acupuncture. After treatment, the patient's symptoms were improved and serum cortisol levels rose to a normal range without the aid of steroid supplementation therapy. This case suggests that Korean medicine can be effective in the treatment of adrenal insufficiency, but more clinical reports are needed.
The ultrastructural changes of adrenal chromaffin cells and ganglion cells in the adrenal gland of vacor-induced diabetic Mongolian gerbils were studied by electron microscopy. After one month of vacor-induced diabetes, some chromaffin cells were filled with dense bodies and large cytosomes with formy contents. Most of degenerating axon terminals were observed on chromaffin cells. A few macrophages were found among chromaffin cells at one month after induction. Several of these macrophages were filled with numerous phagosomes. After one month of vacor-induced diabetes, the ganglion cells showed increase in numbers of dense bodies and degenerating dendrites compared with the normal ganglion cells. Both electron dense and lucent types of degenerating axon terminals were found in interstitial space of the ganglion cells. Degenerating unmyelinated and myelinted axons contained dense and lamellar bodies. The satellite cells and macrophages with engulfed degenerated axon terminals were observed. After three months of vacor-induced diabetes, the unmyelinated and myelinated axons showed degenerative changes, whereas no structural changes could be demonstrated in adrenal ganglion and chromaffin cells. The satellite cells and macrophages containing partially digested debris were still commonly observed in the interstitial space of adrenal medulla. These results suggest that the degenerative changes occur in the adrenal ganglion cells as well as adrenal chromaffin cells of vacor-induced diabetic Mongolian gerbils.
To determine the regulatory mechanism of phenylethanolamine N-methyltransferase (PNMT) in the adrenal gland and in central nervous system, we observed the change of enzyme activity and mRNA level of PNMT in the adrenal gland, the brain stem, and hypothalamus of rats, which were injected with two neuroleptic agents(reserpine and haloperidol ). Reserpine depleting catecholamines in presynaptic vesicle increased PNMT activities in the adrenal gland and the brain stem to 150% of the control in time-dependent manner, but not in the hypothalamus. Haloperidol blocking dopamine receptor decreased PNMT activities in the adrenal gland and the hypothalamus, but not in the brain stem. Thus, the results indicate that catecholamines inhibit synthesis of epinephrine in the brain stem and the adrenal gland, and that dopamine stimulates synthesis of epinephrine in the hypothalamus and the adrenal gland. In addition, since the change of mRNA levels were nearly in accordance with the change of activities, the transcriptional regulation of PNMT is considered the mechanism of the regulation of epinephrine neuron.
The nature of the muscarinic receptors in bovine adrenal medulla was investigated in this study. [$^3$H]Quinuclidinyl benzilate(QNB) specifically bound to a single class of muscarinic receptor with a $K_{D}$ value of about 70 pM in bovine adrenal medullary, cardiac ventricular and ileal homogenates. Pirenzepine inhibition curves of [$^3$H]QNB binding to cardiac ventricular and ileal homogenates were steep, indicating the presence of a single class of binding site for pirenzepine with a Ki value of 990 nM and 508 nM, respectively. However, pirenzepine/[$^3$H]QNB competition binding curves in adrenal medulla suggested the presence of two binding sites (Hill coefficient=0.59) with a high( $M_1$) and a low( $M_2$) affinity. Respective Ki values for pirenfepine were 16 nM and 633 nM, with 44% of total sites having a high affinity( $M_1$). Gallamine, which is selective to cardiac $M_2$-receptor, inhibited [$^3$H]QNB binding to adrenal medullary, cardiac ventricular and ileal homogenates with Ki values of 12 $\mu$M, 6 $\mu$M and 13 $\mu$M, respectively. Thus, the binding affinities of pirenzepine and gallamine for $M_2$-receptor in adrenal medulla were similar to those in ileum, which contains the $M_3$-receptor. These results indicate that the $M_1$- and $M_3$- muscarinic receptor subtypes coexist in the bovine adrenal medulla.a.
Adrenal hemorrhages caused by blunt abdominal trauma have been frequently reported, and most of the lesions are unilateral. In contrast, bilateral hemorrhage of the adrenal glands after trauma rarely occurs in subjects with predisposing conditions such as coagulopathy, thromboembolism, and sepsis. Furthermore, bilateral hemorrhage of the adrenal glands is potentially fatal by inducing acute adrenal insufficiency. Here,a case of a 40-year-old man who developed traumatic bilateral adrenal hemorrhage after a car accident, without any predisposing condition, is reported. The spontaneous shrinkage of the bilateral lesions revealed in the follow-up abdominal computed tomography (CT) scansupported the aforementioned diagnosis. Fortunately, the patient had no clinical or biochemical evidence suggesting acute adrenal insufficiency. To these authors' knowledge, this is the first South Korean report of traumatic bilateral adrenal hemorrhage in a subject with no predisposing factors.

Testicular adrenal rest tumors are a well-known complication in male patients with congenital adrenal hyperplasia. Corticosteroid suppressive therapy usually results in the regression of these tumors. We describe a patient with 21-hydroxylase deficiency who developed bilateral testicular masses. Despite steroid suppressive therapy, the tumors did not regress and hormonal control was poor. Consequently, bilateral partial orchiectomies were performed.

The morphological and immunohistochemical characteristics of the adrenal gland of the lion(Panthera leo) have been studied. In the light microscopic level, the adrenal gland was surrounded by thick capsules arid weak development of the zona glomerulosa was detected. Some medullary tissues were directly connected with capsules, however the adrenal gland in the lion 'demonstrated somewhat pseudolobulated appearance. Weakly or strongly stained chromogranin-immunoreactive cells were detected in medullary regions of the adrenal gland of the lion by immunohistochemistry, and weakly stained chromogranin-immunoreactive cells were detected more frequent than strongly stained ones.
To investigate the effects of repeated immobilization-stress challenge on the the hypothalamic-pituitary-adrenal axis, the genomic transcriptome in the adrenal cortex of immobilization-stressed mouse was analyzed by using a cDNA microarray. Mice were subjected to immobilization stress for 2 h per day for 5 consecutive d. With a 4.0-fold cutoff of arbitrary criteria, the expression levels of 168 out of 41,174 genes were significantly modulated in the adrenal cortex by stress when comparing the control and experimental groups. These genes were related to apoptosis, cell cycle, immune response, inflammatory responses, and signal transduction, and thus may be used as potential targets for the development of therapeutics for chronic stress or depression. Six significant genes among these were selected for real time polymerase chain reaction analysis to confirm the change of their expression levels. The gene for phospho 1 was also further investigated because its expression showed the greatest fold-change.
It has been demonstrated the presence of leptin receptors (Ob-Ra) on epinephrine-secreting chromaffin cells in rat adrenal medulla, suggesting that leptin may directly affect the adrenal medulla (Cao et al., 1997). Leptin is found to stimulate catecholamine (CA) synthesis in cultured bovine adrenal medullary cells (Utsumomiya et al., 2001; Shibuya et al., 2002)and cultured porcine adrenal medullary cells (Takekoshi et al., 2001). Thus, the present study was designed to examine the effect of leptin on CA release from the isolated perfused rat adrenal gland, and to establish its mechanism of action. (omitted)
Adrenal rest tumors are a rare extra-adrenal complication of congenital adrenal hyperplasia (CAH) in women although they are more commonly found in the testes of male patients with CAH. An ovarian adrenal rest tumor (OART) may coexist with CAH or imitate its symptoms without CAH. In this case report, we present the case of a woman with OART without CAH, whose main complaint was infertility and who had a baby after successful surgical treatment.
