Gender Differences of Adolescent Suicidality: Focused on the General Strain Theory (일반긴장이론에 근거한 청소년의 자살성 남녀 비교 : 서대문구 중학생을 중심으로)

  • The purpose of this study is to examine relationship between social strains such as status strains and relational strains and adolescent suicidality by gender. As method, a self-report survey was completed by students (n = 819) from 3 middle schools in Seodaemun area, Seoul, Korea. Logistic regression analyses were conducted to identify factors associated with adolescent suicidality, t-test analyses were used to verify gender difference. Results show that male adolescents are more likely to experience abuse from their father, and school violence related to relational strains than female. Differences were found in strains for males and females contributing to suicidality; male are responsive to economic status, a dimension of status strain, whereas female are reactive to non-physical abuse from father, a type of relational strains. Non-physical school violence was appeared to be a significant factor influencing suicidality for both genders. Based on these findings, research draws implications for social work interventions. First, different approaches by gender are needed to prevent adolescent suicide in consideration of the tendency that men are status-oriented and women are relationship-oriented. Second, it is suggested to hire full-time school social worker to provide consistent social service for students. Third, intensive effort is necessary to reduce non-physical school violence.

The Effect of Academic Engagement on Self-esteem in Adolescents: The Mediating Effect of Learning (학업열의가 자아존중감에 미치는 영향: 학습시간의 매개효과)

  • This study attempted to find out whether learning time has a mediating effect according to the gender, region, and grade of middle school students in the relationship between academic engagement and self-esteem. To this end, a survey of 1,045 middle school students in Gyeongsangnam-do was conducted on academic engagement, learning time, and self-esteem. Difference verification was conducted to determine the difference in academic engagement, learning time, and self-esteem according to the general characteristics of the study subjects, correlation analysis was conducted to determine the correlation between major variables, and regression analysis was conducted to verify the mediating effect of learning time. As a result of the analysis, first, there was no difference in the academic engagement of middle school students by group. In the learning time, middle school students in the city area were significantly higher than those in the township area, male students had higher self-esteem than female students, and students in the city area had significantly higher self-esteem as the grade went up. Second, as a result of correlation analysis, learning time, academic engagement, and self-esteem showed a positive correlation. Third, in the entire group not divided by group, both the direct path through which academic engagement reaches self-esteem and the partial mediating model from learning time to self-esteem showed significant effects. In the analysis by gender, only female students excluding male students showed a partial mediating effect, and the analysis results by region showed a partial mediating effect only on students in the city. The analysis results by grade showed a partial mediating effect only for second-year middle school students. In order to improve the self-esteem of middle school students, education and counseling should be conducted in consideration of not only individual differences by gender and grade, but also the region in which they live.

Comparison of Nutritional Knowledge, Dietary Habits, and Practice Level of Eating Behavior Guidelines Between Part-Time Working and Non-Working Adolescents Attending a Vocational High School in Yongin, Gyeonggi-Do (용인지역 일부 실업계 고등학생의 아르바이트 여부에 따른 영양지식, 식습관, 식생활 지침 실천도 비교)

  • The purpose of this study was to investigate nutritional knowledge, practice of eating behavior guidelines, and problematic dietary habits in working high school students in comparison with non-working high school students. A survey questionnaire was formulated to obtain information on demographic variables, body size, part-time jobs, nutritional knowledge, practice of eating behavior guidelines, and dietary habits. The developed questionnaire was given out to 515 students attending a vocational high school in Yongin, Gyeonggi-Do. Those who had been working 5 hours or more per day for at least a month at the time of survey administration were defined as working students for this study. Proportions of working students were similar between male and female students (i.e. 49.5% for male, 50.5% for female). No significant difference was found in scores of nutritional knowledge between working ($8.43{\pm}2.29$) and non-working students ($8.60{\pm}2.19$). However, some dietary habits were found to be dependent on working status. While approximately 43% of non-working students reported skipping a meal, about 57% of working students did so (p<0.01). The proportion of students with any drinking experience was significantly higher among working students (92%) compared to non-working students (80%) (p<0.001). Working students were found to drink alcoholic beverages more often than non-working students (p<0.001). The mean score of practice of eating behavior guidelines was lower in working students than non-working students. The magnitude of this difference was modest ($51.33{\pm}0.63$ in working students, $53.40{\pm}0.51$ in non-working students), but reached statistical significance (p<0.01). Based on the findings from this study, it is suggested that specific behavior-oriented messages to improve certain problematic dietary habits need to be directed toward working high school students.

Textbook Analysis of Middle School-Home Economics and Survey on Consumption Status and Nutritional Knowledge of Milk and Dairy Products of Middle School Students in Gongju City, Chungnam Province (중학교 가정교과서의 우유 교육 내용 분석과 중학생의 우유·유제품 섭취 실태 및 영양지식 조사 - 충남 공주시 중학생을 중심으로 -)

  • This study was performed to analyze education contents related to milk in current home economics textbooks of middle school, and to investigate intake status, consumption behaviors, perception and nutritional knowledge of milk and dairy products among 364 middle school students in Gongju for improvement of milk education in home economics subjects and milk intake of adolescents. As a result, education contents of milk and dairy products in home economics textbooks currently applied in middle school were major nutrients, consumption method for balanced diet, and selection and storage of milk and dairy products, thus it tended not to match current food trend. Only 30.5% of subjects met 2 cups of milk a day, the recommended level. The main reason for drinking milk was to 'be taller' and 'to quench thirst' and there was a difference by gender(p<0.01). The rate of not participating in school milk program was 23.1% of total and its satisfaction was moderate. The most popular dairy products by subjects were ice cream, followed by yogurt and cheese, and the choice of milk was focused on 'taste' or 'expiration date'. The rate of knowing certification mark of K-MILK was low at 28.8%, and most subjects knew as 'domestic milk use'. In home economics class, experience-based learning such as cow ranch experience was the most preferred instruction method for milk followed by laboratory practice and lecture, and there was a difference by gender(p<0.001). Perception degree of milk and dairy products was moderate and male subjects were more positively perceived than female subjects(p<0.01). Nutritional knowledge level of milk and dairy products was moderate and female subjects were higher than male subjects(p<0.01). Therefore, education contents of milk and dairy products of home economics textbooks of middle school should be centered on real life in accordance with food trend, and applied student participation-based instruction methods such as experience-based learning. In addition, it is necessary to enhance taste and merchandise of milk and to provide them with preferred milk and dairy products in school milk program for improvement of milk intake of adolescents.

Sleep Patterns of Middle and High School Students in Seoul (서울 거주 중.고등학교 학생의 학년별 수면 양상)

  • Objectives: It is been reported that sleep patterns change significantly during the adolescent years. In Korea, the sleep patterns of adolescents have not been investigated systematically. The authors performed preliminary study about sleep patterns of middle and high school students in Seoul before nationwide study. Methods: The authors selected one of middle and high schools located in typical middle class area in Seoul. The study was performed during the periods when examination or vacation did not affect the sleep cycle. Fifty to one hundred male and female students were selected from one school year each. One of investigators explained the purposes and methods of this study to the students and gave additional explanation to the teachers in charge. We designed a questionnaire for subjects to describe the nap time, bedtime, sleep latency, total sleep time, rise time and sleep efficiency, and the students recorded sleep logs for seven days including weekend. Eight hundred three students answered the questionnaires. Among them, we selected the subjects who recorded sleep logs at least three days and filled up the questionnaires completely. Finally, seven hundred seventy one subjects were included in this study. Weekend was defined as the time from Saturday morning to Sunday morning, and Sunday sleep patterns were treated separately. Results: Across the six school years, school-night bedtimes were delayed one hundred twenty four minutes and school-ight rise times were advanced by thirty five minutes. School-night total sleep times were decreased by one hundred fifty eight minutes, and weekend bedtimes were delayed by one hundred twenty five minutes. The students tended to oversleep on weekends, and the mean of weekend oversleep was ninety six minutes. But, bedtime in weekends was not delayed. Daytime sleep, sleep latency and sleep efficiency were not changed greatly across six school years. Total sleep time of female students was decreased significantly between third middle school year and first high school year, but time of male students was decreased between first and second high school year. Conclusion: It is supposed that sleep deficiency of middle and high school students in Seoul was due to delayed bedtimes and advanced rise times across the school years.

Effect of High Calcium Diet on the Zinc and Copper Balance in Korean Female Adolescents (청소년기 여자에서 고칼슘 섭취가 아연과 구리 평형에 미치는 영향)

  • Intakes and excretions of zinc and copper were determined for 8 female adolescents (aged 16.4$\pm$0.5 y; body mass index 20.4$\pm$1.3kg/$m^2$; body fat 33.3$\pm$2.5%; bone mineral density of lumbar spine in L2-L4; 0.96$\pm$0.08g/$\textrm{cm}^2$) when they consumed diets basal and high in calcium for 6 days each. All subjects consumed a basal Ca diet containing 800 mg, Korean RDA level of the subjects, and a high Ca diet containing 1200mg, RDA plus 2 SDs of calcium intake. The diets provided 58% of energy intake as carbohydrate, 25% as fat, and 17% as protein. Food, urine and fecal samples were collected during the last 3 days of each feeding period and were assayed. Mean daily intakes on the basal and high calcium diets, respectively, were 6.57 and 6.37 mg for zinc and 910 and 812 $\mu\textrm{g}$for copper. Fecal excretion of copper and zinc in relation to intake was significantly greater on the high calcium than on the basal calcium diet. Hence, apparent absorption rate was significantly lowered from 98.7% on the basal calcium diet to 97.9% on the high calcium diet for zinc from 66.3% to 56.4% for copper, respectively. Urinary loss of copper was not detectable but that of zinc was 0.38mg on the basal diet and 0.47mg on the high calcium diet. Copper retention was 899$\pm$105$\mu\textrm{g}$/day on the basal calcium diet and 792$\pm$20.8$\mu\textrm{g}$/day on the high calcium diet, and zinc retention was 3.95$\pm$0.91mg/day and 3.11$\pm$0.89mg/day. Thus, copper and zinc retention was significantly decreased on the high calcium diet (p<0.05). Summarizing the results, apparent absorption and retention of zinc and copper were significantly decreased by calcium supplementation. Therefore, it is suggested that interactions among minerals should be considered in determining RDA.

The timing of sexual maturation and secular trends of menarchial age in Korean adolescents (한국 청소년의 성성숙 시기 및 장기간의 초경연령 추세분석)

  • Purpose : The purpose of this study was to evaluate the timing of sexual maturation and secular trends of menarchial age in Korean adolescents. Methods : Nine hundred eighty two school students and 119 university female students were included. Tanner's pubertal assessment method was applied using a self-assessment sheet with pictures for their developmental stages. A medical checkup database from 1,156,022 women was also used to analyze the age of menarche from the year 1900 to 1980. Results : Onset of puberty was at $11.3{\pm}1.3$ years(y) in girls and $12.1{\pm}1.5$ y in boys and total pubertal duration was 3.6 y in girls and 3.3 y in boys. Height at the onset of puberty was $146.1{\pm}7.9cm$ in girls and $152.7{\pm}9.8cm$ in boys. Weight at the onset of puberty was $39.3{\pm}6.9kg$ in girls and $47.7{\pm}14.4kg$ in boys. The mean age of menarche was $12.0{\pm}1.0y$ and mean age at nocturnal emission was $12.3{\pm}1.8y$. By age ten, 3.0 percent of girls experienced menarche, which increased to 41.1 percent by age 12, and 98.0 percent by age over 15. By SMR 2 breast stage, 18.2 percent of girls experienced menarche, which increased to 66.1 percent and 91.7 percent by SMR stages 3 and 4, respectively. By SMR 2 pubic hair stage, 52.0 percent and by SMR 3, 93.8 percent of girls experienced menarche. While the menarchial age of women born around 1980 was $12.4{\pm}1.1y$, the menarchial age of women born around 1990 was $12.0{\pm}1.1y$. Age at menarche has been advanced approximately by 2 years between 1900 and 1980 birth cohorts. Conclusion : There was a downward secular trend in pubertal timing. Age at menarche has been advanced by 2 years during the last 80 years. Further large scale, prospective studies, including inspection by specialists, are needed to provide reference data for pubertal timing in Korean adolescents.

Yearly trend of milk intake in Korean children and adolescents and their nutritional status by the milk intake level using 2007-2015 Korea National Health and Nutrition Examination Survey (아동과 청소년의 연도별 우유 섭취량 변화와 우유 섭취량에 따른 영양상태 평가: 2007-2015 국민건강영양조사를 바탕으로)

  • Purpose: This study examined the yearly trend of milk consumption and the nutritional status of subjects aged 6-18 years using the 2007-2015 Korea National Health and Nutrition Examination Survey data. Methods: Milk and dairy products were classified into plain milk, flavored milk, and dairy products (ice cream, milkshakes, cheese, and yogurt, etc.). This study compared the milk and dairy products intakes, some nutrients intakes and percent of dietary reference intakes for Koreans in the milk intake and non-milk intake groups. Results: Plain milk intake decreased with year (male, p = 0.0199; female, p < 0.0001; elementary school, p = 0.0013; high school, p = 0.0061), whereas flavored milk and dairy products intake in these subjects increased with year. In all subjects, 49.9% of subjects did not drink milk at all. The intakes of energy, protein, fat, calcium, phosphorus, and riboflavin in the milk intake group were significantly higher than those in the non-milk intake group, even after adjusting for covariates (p < 0.05). The odds ratio of the prevalence of nutritional deficiency in the non-milk intake group was 3.2 times higher than that of the milk intake group (p < 0.001), even after adjusting for covariates. The odds ratio for the prevalence of excess intake of the energy/fat was not significant with milk intake. Conclusion: The prevalence of milk intake decreased every year in the subjects. Calcium deficiency and nutritional deficiency were very high in the non-milk intake group. Efforts should be made to improve the calcium status in children and adolescents by strengthening nutrition education about the importance of milk intake.

RELATIONSHIP BETWEEN ADOLESCENT INTERNET ADDICTION AND DEPRESSION, IMPULSIVITY, AND OBSESSIVE-COMPULSIVITY (청소년 인터넷 중독과 우울, 충동성, 강박성과의 연관성)

  • Objective : The aims of this study were to explore the relations of internet addiction to depression, impulsivity, and obsessive-compulsivity in adolescents. Methods : 1410 high school students (male=611, female=799 : mean age=$16.2{\pm}0.7$) were included in this study. The questionnaire consisted of items on demographic characteristics and internet use pattern. We assessed the level of internet addiction using Young's internet addiction scale (IAS) Barratt impulsiveness scale (BIS) and Maudsley obsessive compulsive inventory (MOCI) were also self-rated. In this study, We defined upper 25% of IAS as 'addiction group' and lower 25% as 'non-addiction group'. Results : The results were as follows ; 1) Male students had significantly higher mean scores on Internet addiction scale than females and there was significantly higher rate of male students in addiction group. 2) The addiction group spent more time for internet use, especially for games than non-addiction group. 3) The addiction group showed significantly higher total scores on BDI, BIS, and MOCI than non-addiction group. 4) Significant associations have been found between the level of internet addiction and depression, impulsivity, and obsessive-compulsivity, respectively. Conclusion : Depression, impulsivity, and obsessive-compulsivity could be significant factors predicting internet addiction. Especially, direct effect of impulsivity could be the most significant to explain internet addiction. Adolescents with high impulsivity may be vulnerable to internet addiction.

Korean Children and Adolescents with Crohn's Disease Are More Likely to Present with Perianal Fistulizing Disease at Diagnosis Compared to Their European Counterparts

  • Purpose: We aimed to investigate the disease phenotype of Korean pediatric Crohn's disease (CD) patients at diagnosis according to the Paris classification by comparison with patients from the European multicenter 5-years recruitment of children with newly developed IBD (EUROKIDS registry). Methods: Korean children and adolescents who had been newly diagnosed with CD at the age of <18 years during 2013-2016 were included in this multicenter retrospective study. Disease phenotype at diagnosis was classified according to the Paris classification, and compared with the published data from the EUROKIDS study. Results: A total of 255 patients were included. The median diagnosis age was 14.7 years (range, 0.8-17.9 years). No significant difference was observed in male-to-female ratio with EUROKIDS (1.9:1 vs. 1.45:1, p=0.062). The proportion of children aged <10 years was significantly lower in Koreans (7.1% vs. 19.6%, p<0.001). Colonic disease was less prominent (10.0% vs. 27.3%, p<0.001), while upper GI involvement was more prominent in Korean children (59.3% vs. 46.2%, p<0.001). The proportion with perianal fistulizing disease at diagnosis was significantly higher in Korean patients (44.8% vs. 8.2%, p<0.001). A separate analysis of Korean patients revealed that perianal fistulizing disease at diagnosis was positively associated with male sex and body mass index z-score (odds ratio [OR]=2.12, 95% confidence interval [CI]=1.20-3.76, p=0.010; and OR=1.29, 95% CI=1.05-1.58, p=0.015, respectively). Conclusion: Approximately half of pediatric CD patients in Korea present with perianal fistulas and/or abscesses at diagnosis, which is a distinct feature of CD in Korean children and adolescents compared to their European counterparts. An underlying genetic difference between ethnicities may play a role in this expression of different phenotypes in pediatric CD.
