BACKGROUND/OBJECTIVES: This study examined the relationship between famine exposure in early life and the risk of type 2 diabetes in adulthood during the 1959-1961 Chinese Famine. SUBJECTS/METHODS: A total of 3,418 individuals aged 35-74 years free of diabetes from two studies in 2006 and 2009 were followed up prospectively in 2009 and 2012, respectively. Famine exposure was classified as unexposed (individuals born in 1962-1978), fetal exposed (individuals born in 1959-1961), child exposed (individuals born in 1949-1958), and adolescent/adult exposed (born in 1931-1948). A logistic regression model was used to assess the relationship between famine exposure and diabetes after adjustment for potential covariates. RESULTS: During a three-year follow-up, the age-adjusted incidence rates of type 2 diabetes were 5.7%, 14.5%, 12.7%, and 17.8% in unexposed, fetal-exposed, child-exposed, and adolescent/adult-exposed groups, respectively (P < 0.01). Relative to the unexposed group, the relative risks (95% confidence interval) for diabetes were 2.15 (1.29-3.60), 1.53 (0.93-2.51), and 1.65 (0.75-3.63) in the fetal-exposed, child-exposed, and adolescent/adult-exposed groups, after controlling for potential covariates. The interactions between famine exposure and obesity, education level, and family history of diabetes were not observed, except for the urbanization type. Individuals living in rural areas with fetal and childhood famine exposure were at a higher risk of type 2 diabetes, with relative risks of 8.79 (1.82-42.54) and 2.33 (1.17-4.65), respectively. CONCLUSIONS: These findings indicate that famine exposure in early life is an independent predictor of type 2 diabetes, particularly in women. Early identification and intervention may help prevent diabetes in later life.

Vagal nerve stimulation (VNS) has been proposed as a possible way to improve the control of refractory epilepsy. We report the effects following VNS treatment in patients with refractory epilepsy. Seventeen patients with a mean age of 12.8 years, ranging from 5 to 29 years, underwent the implantation of vagal nerve stimulation (Cyberonics, Houston, TX). We reviewed the clinical findings before and after VNS in seizure frequency, number of antiepileptic drugs (AED), and quality of life (QOL). All of the patients had intractable seizures, eleven of the patients had additional medical complications, three had hippocampus atrophy, one had encephalomalacia, five had encephalitis, one had pachygyria, and one had schizencephaly. Thirteen patients had symptomatic partial epilepsies, three patients had Lennox-Gastaut syndrome and one had cryptogenic partial epilepsy. The mean follow up duration was 35 months. The mean reduction of seizure frequency compared with baseline before VNS was 26.1% after 3 months (p<0.005), 41.9% after 6 months (p<0.001), 46.9% after 1 year (p<0.001), and 53% at the latest follow-up (p<0.001). Twelve patients showed an improvement of QOL such as mood, language, alertness, expression, and motor function. The most common side effects were transient hoarseness or voice change or cough, which was detected in six patients (35%) and wound infection in one patient (5%). This study has shown a good anti-seizure effect of VNS, decrease in seizure frequency and improvements in QOL. We concluded that VNS is a beneficial therapy in refractory epilepsy with a non-resectable epileptic focus. Further studies should be focused on the prediction of unresponsiveness and the adjustment of VNS parameters for maximum efficacy in patients with various medical histories.

The purpose of this study was to examine the relationship between major satisfaction and university life adaptation according to the amount of physical activity for university students. This study implement survey to the 201 university students. Descriptive statistics, reliability analysis, one-way ANOVA and pearson's correlation analysis were conducted for data analyses. The results are as follows. First, university students' major satisfaction with the amount of physical activity is statistically significantly higher than that of low-intensity physical activity groups (p<.05). Second, university life adaptation according to the amount of physical activity was found to be statistically significantly higher in high-intensity physical activity groups than in medium-intensity physical activity groups and low-intensity physical activity groups (p<.05). Lastly, according to the relationship between physical activity, major satisfaction, and university life adaptation of university students, male students have a static correlation between physical activity, major satisfaction (general satisfaction), and university life adaptation (personal emotion adaptation) and female students have a static correlation between physical activity and university life adaptation(social adaptation) (p<.05). This study confirmed that the amount of physical activity of university students is related to their major satisfaction and adaptation to university life. Based on this conclusion, it is expected that university students will be able to manage their university life through physical activities.
Purpose: The purpose of this predictive study was to identify factors affecting health related quality of life (HRQoL) in patients with rheumatoid arthritis (RA). Methods: The participants in this study were 131 patients with RA who were recruited from the outpatient clinic of a university hospital in Seoul. Disease activity in rheumatoid arthritis was evaluated by calculating the Disease Activity Score 28. Disability in activities of daily living (ADL) was assessed with the Korean Health Assessment Questionnaire, and depression with The Center for Epidemiologic Studies Depression Scale. HRQoL was evaluated using The Short Form 36 Health Survey. Data were analyzed using descriptive statistics, correlation, and hierarchical multiple regression. Results: Pain, disability in ADL, disease activity, and depression correlated negatively with physical and mental dimensions of HRQoL. But hierachical multiple regression analysis revealed that disability in ADL and depression were the only variables negatively influencing physical and mental QoL after adjustment for influences of sociodemographic variables. Conclusion: Results of this study suggest that disability in ADL and depression, rather than disease activity and pain have profound effects on HRQoL in patients with RA. Further studies are needed to assess the predictive ability of disease activity and pain on HRQoL in this population.

Purpose: This research was conducted to explore the experiences of graduate student women with children fulfilling multiple roles and to generate a substantive theory. Methods: This study uses the grounded theory method based on Strauss and Corbin(1988)'s method. Interviews were conducted with 11 graduate student women who had children aged three of older. Results: The core category of this research is 'Role balancing for self-generativity'. Based on the core category, three types of married women graduate students' experiences were found. They are 'concentrating on graduate-work' 'postponing graduate-work' and 'adjustment between two roles' Conclusions: Through understanding the married women's multiple roles experience process, there can be a basis for making a women's health system and social support system for the married women graduate student. Also, this research is to generate a substantive theory which can helps to change social perspectives on the quality of life for the women who will contribute in Korean society as professionals.
Purpose: The main purpose of this study was to explore the adaptation experience of adolescent cancer survivors during and after cancer treatment, and their perceived social support networks. Methods: This study was a qualitative descriptive study using the in-depth interview. Eight adolescent cancer survivors who were diagnosed with cancer between 11 to 18 years old participated in the study. Results: The adaptation experiences of adolescent cancer survivors over time were identified within five categories for during their treatment such as "being catapulted from one's life," "standing at the center of discomfort," "falling behind the line," "accepting the change," "being developed", and another five categories for after the treatment including "being shackled," "encountering the forgotten reality," "overcoming and emerging from the reality," "growing into adulthood," "entering into a new orbit." Participants reported the various members of their social support network and their roles during and after the treatment as well. Conclusion: While adolescent cancer survivors adjusted to their changing situations after the cancer diagnosis, their internalized adaptation, as well as perceived social support from their diverse surrounding network, played significant roles. These findings will become a valuable asset for developing age-appropriate nursing interventions to promote psychosocial adjustment of adolescents with cancer.
This study was conducted to investigate the effects of environmental sound rice farming method on the productivity of rice industry which may result in impacts on the staple food securities. Recently fanners have been concerned about adopting new rice cultivation method such as organic and low input farming system in which fertilizer and pesticide can be substantially reduced so as to alleviate the burden of agro-ecosystem. However, It has been argued about whether or not there are negative impacts on the self sufficiency rate of food, income of farm household and technological adaptability. Therefore this study examined the productivity trend of environmental rice farming system and predicted the long term rice self-sufficiency rate when environmental rice farming system are adopted by assuming various scenarios. It was estimated that rice self-sufficiency rate can be decreased up to 52.2% by 2010. Based on the analysis of results, policy recommendations for environmentally sound rice farming were suggested as follows: 1) gradual adopting and transferring of environmental rice cultivation method, 2) increasing profitability of rice growing fanners 3) developing the farm level technology specific to Korean farming condition, 4) institutionalization of direct payments for encouraging environmental rice fanning.
Purpose: The purpose of this study was to identify the patterns of activities of daily living (ADL) functional status and to examine the relationships between sense of coherence(SOC), depression, and uncertainty in gynecologic cancer patients who were receiving chemotherapy. Method: A prospective, longitudinal design with repeated measures was utilized. Women reported depression, SOC, and uncertainty at the beginning of the first cycle of chemotherapy, and they recorded ADL functional status everyday for two consecutive treatment cycles. the The instruments used were the Karnofsky Performance Status Index, Orientation to Life Questionnaire, Beck Depression Inventory, and Mishel Uncertainty in Illness Scale-Adult Form. Result: Data from 42 women showed that the ADL functional status during the second cycle was better than that of the first cycle with significant improvement each week. However, it did not completely recover to the baseline level even three weeks after the treatment ended in both cycles. SOC was correlated with depression(r=-.64, p<.001) and uncertainty(r=-.62, p<.001). Uncertainty was related to depression (r=.66, p<.001) and to functional status during the second cycle(r=-.45, p<.05), while the scores of the functional status during the two cycles were not related. Conclusion: Changing patterns and level of functional status during the treatment phase would be useful information for cancer patients to prepare coping strategies for positive health outcomes.
This study was conducted to determine the change in freezing tolerance of Chinese cabbage (Brassica campestris L. ssp. penkinensis). Four-week old seedlings grown in a greenhouse at $25^{\circ}C$$\pm$0.5 were transferred to 5, 10 or 15$^{\circ}C$ condition for acclimation for one day, and then transferred to a lethal temperature of -3$^{\circ}C$ condition. Optimal acclimation temperatures were 5$^{\circ}C$ and 1$0^{\circ}C$. Freezing tolerance of leaf tissues was investigated during a week-long cold stress. Development of freezing tolerance was shown by survival ratio, and amounts of total RNA, soluble protein and soluble sugar. LT$_{50}$ value increased from -3 to -4$^{\circ}C$ after 5 days acclimation at 5 or 1$0^{\circ}C$, and this was considered to be the first indication of enhanced freezing tolerance. Plants, cold acclimated for five days, reached to a survival ratio of 60%. This increase in freezing tolerance was found to be associated with the increased levels of total RNA, soluble sugar and soluble protein. These metabolic changes imply the association of adjustment of growth and cell metabolism with low temperatures at the beginning of cold acclimation in chinese cabbage.e.
