  • For the adequate intraarticular exposure in medial talar dome lesions, medial malleolar osteotomy is necessary in some cases. Many operative techniques including transverse, oblique, inverted V-shape, crescentic and step-cut osteotomies of the medial malleolus have been described previously. But their techniques have several problems such as nonunion, rotation and limited access to lesions. So we introduce the new reverse chevron medial malleolar osteotomy which provides excellent access to lesions, good stability and a broad cancellous surface for rapid healing.

  • While many fertility preservation (FP) options now exist for reproductive-aged cancer patients, access to these services continues to be limited. A comprehensive FP program should be organized to serve oncofertility patients effectively. Also, much effort is needed from various individuals-patients, specialists from various fields, and consultants-to facilitate FP in a timely manner. Various challenges still exist in improving access to FP programs. To improve access to FP treatment, it is important to educate oncologists and patients via electronic tools and to actively navigate patients through the system. Reproductive endocrinology practices that receive oncofertility referrals must be equipped to provide a full range of options on short notice. A multidisciplinary team approach is required, involving physicians, nurses, mental health professionals, office staff, and laboratory personnel. The bottom line of FP patient care is to understand the true nature of each patient's specific situation and to develop a patient flow system that will help build a successful FP program. Expanding the patient flow system to all comprehensive cancer centers will ensure that all patients are provided with adequate information regarding their fertility, regardless of geography.

Developing Subject Headings for Children's Picture Books based on A to Zoo (어린이 그림책을 위한 주제명표 개발 연구: 『A to Zoo』를 바탕으로)

  • Subject headings support the effective access of children's picture books. However, it is difficult to select subject terms from titles or table of contents in children's picture books because of their relatively little textual information. Therefore, it is necessary to assign subject terms to each picture book. However, it is not adequate to use general subject headings because the types and levels of general subject headings are different from special subject headings for the children's materials. For this reason, this study aims to develop subject headings for children's picture books. The subject terms in A to Zoo were selected, and the selected terms were translated into Korean and modified for the Korean culture and language. Other reference books, such as Elementary Korean Dictionary, were also used to determine adequate terms for children. The resulting subject headings were assigned to the recommended picture books for children and used to search by subject, browse, and recommend books.

Design of high-speed preamble searcher adequate for RACH preamble structure in WCDMA reverse link receiver (RACH 프리앰블 구조에 적합한 WCDMA 역방향 링크 수신기용 고속 프리앰블 탐색기의 설계)

  • In this paper, we propose a high speed preamble searcher feasible for RACH(Random Access Channel) preamble structure in WCDMA reverse link receiver. Unlike IS-95, WCDMA system uses AISMA(Acquisition Indication Sense Multiple Access) process. Because of the time limit between RACH preamble transmission and AI(Acquisition Indicators), and the restriction on the number of RACH signatures assigned to RACH preamble, fast acquisition indication is required for efficient operation. The preamble searcher proposed in this paper is based on 2-antenna system and has adopted FHT algorithm that has the radix-2 16 point FFT structure. The acquisition speed using FHT is 64 times faster than the conventional method that correlates each signature. Based on their fast aquisition scheme, we improved the acquisition performance by calculating the correlation up to the 4096 chips of the total preamble length. The performance is analyzed by using Neyman-pearson method. The proposed algorithm has been applied for the implementation of WCDMA reverse link receiver modem successfully.

Development of Clustering-Based Multi-Channel MAC Protocol to Improve Efficiency of Network in VANET (차량 환경에서 통신 효율 향상을 위한 클러스터링 기반의 멀티채널 매체접속제어 프로토콜 개발)

  • In VANET(Vheicle Ad hoc Network), the researches based on the wireless LAN are conducting and the method based on 802.11a is being adapted in IEEE 802.11p WAVE(Wireless Access in Vehicular Environments). However, wireless LAN which uses a single channel in a competition can cause transmission delays because of a frequent collision and a use of CSMA/CA to avoid competition in VANET requiring a fast access. In this paper, we designed CMMP (Clustering based Multi-channel MAC protocol) adequate to VANET and then confirmed the appropriate channel conditions in a V2V communication on the basis of this protocol. The simulation results showed that a packet collision and a transmission delay by the use of an existing single channel based on the contention decreased more than 60% by CMMP.

Determinants of Adoption and Use of Open Access Publishing by Academic Staff in Nigeria Universities

  • The emergence of the World Wide Web (WWW) has changed the scholarly publishing system. Today, Open Access (OA) scholarly publishing offers free access and wide dissemination for research findings anytime and anywhere as an additional value for scholarly content. Despite the potentials of OA publishing, its adoption and use is still low.This paper determines factors influencing the adoption and use of OA publishing by academic staff in universities in Nigeria using an adapted Unified Theory of Acceptance and Use Technology (UTAUT). The study applies methodological triangulation by combining quantitative and qualitative approaches. The empirical data were collected from 317 academic staff in universities in southwest Nigeria. The questionnaire forms were analyzed using the Statistical Package for Social Sciences (IBM-SPSS) 19.0 version to generate descriptive statistics, Analysis of Variance, and Hierarchical Multiple Regression. Thematically, analysis also applies to the interviews. The results show that awareness, attitude, performance expectancy, Internet self-efficacy, and facilitating conditions significantly influenced the adoption and use of OA publishing. The study also reveals that due to lack of OA policy, most of the respondents did not know about OA. The findings further establish that all predictors and moderating variables jointly contributed 64.4% total variance towards the adoption and use of OA publishing. One of the implications of this study is that there is a need for adequate facilities to support adoption and use of OA publishing. The findings inform the proposed framework for improving and evaluating the adoption and use of OA publishing. The findings also have theoretical, societal, and methodological significance to all stakeholders.

A MODIFIED ENDAURAL AND NECK APPROACH TO THE PAROTID GLAND TUMOR (이하선 종양에 대한 변형된 이내 및 경부접근법)

  • Generally we use the preauricular incision to access and remove the parotid gland tumor. But the preauricular approach has some complications such as damage of facial nerve and sensory nerve, Frey's syndrome, and postoperative scar. Especially, the postoperative scar can often cause an unesthetic result and mental stress in young patients. Therefore, if we avoid preauricular incision to be performed outside of tragus, the postoperative scar would be hardly remarkable, and patients would be satisfied cosmetically. We performed surgical excision using a modified endaural and neck approach in a 21-year-old female with a pleomorphic adenoma and 15-year-old male with a neurofibroma occured in the parotid gland. A new, modified endaural and neck approach is a combined method of the modified endaural incision by Starck et al and Gutierrez's neck extension. We obtained an adequate access and the cosmetically acceptable postsurgical scar. The postoperative scars were hidden in the external ear and the hairline. Moreover, except the neck dissection can this approach be applied to the surgery of temporomandibular joint as well as the parotid gland tumor.

  • The paper deals with the performance of contention-based medium access control (MAC) protocols for underwater sensor networks. We extensively analyze the number of received-packets and the end-to-end delay of ALOHA, CSMA, CSMA-RTS-CTS and CSMA-RTS-CTS-ACK protocols using a Qualnet underwater network simulator which accommodates diverse underwater acoustic channel environments. Using simulation results, we support an engineering table to determine an adequate contention-based MAC protocol for underwater sensor networks.

  • Greater use of mobile phone devices seems inevitable because the health industry and cancer care are facing challenges such as resource constraints, rising care costs, the need for immediate access to healthcare data of types such as audio video texts for early detection and treatment of patients and increasing remote aids in telemedicine. Physicians, in order to study the causes of cancer, detect cancer earlier, act in prevention measures, determine the effectiveness of treatment and specify the reasons for the treatment ineffectiveness, need to access accurate, comprehensive and timely cancer data. Mobile devices provide opportunities and can play an important role in consulting, diagnosis, treatment, and quick access to health information. There easy carriage make them perfect tools for healthcare providers in cancer care management. Key factors in cancer care management systems through a mobile phone health approach must be considered such as human resources, confidentiality and privacy, legal and ethical issues, appropriate ICT and provider infrastructure and costs in general aspects and interoperability, human relationships, types of mobile devices and telecommunication related points in specific aspects. The successful implementation of mobile-based systems in cancer care management will constantly face many challenges. Hence, in applying mobile cancer care, involvement of users and considering their needs in all phases of project, providing adequate bandwidth, preparation of standard tools that provide maximum mobility and flexibility for users, decreasing obstacles to interrupt network communications, and using suitable communication protocols are essential. It is obvious that identifying and reducing barriers and strengthening the positive points will have a significant role in appropriate planning and promoting the achievements of mobile cancer care systems. The aim of this article is to explain key points which should be considered in designing appropriate mobile health systems in cancer care as an approach for improving cancer care management.

Wi-Fi-Based Low-Complexity Floor Number Determination Method Based for Multistory Buildings (다층 건물을 위한 Wi-Fi 기반의 저 복잡도 층간 위치 측위 기법)

  • Floor number determination has attracted considerable attention because many applications require accurate floor number information for providing better localization services in multistory buildings. This paper describes a Wi-Fi-based, low-complexity floor number determination method for multistory buildings. In our scheme, with the exception of floor ID and received signal strength of each WAP, detailed information on the wireless access point (WAP) coordinates is not needed. The multi-wall-floor model is used for the simulation and analysis. The simulation results show that the floor determination accuracy is nearly 100 given an adequate deployment density of WAPs on each floor. The results also show that the proposed method provides a good estimate of floor number even when only a few WAPs are implemented on each floor. Specifically, the proposed method can work under extreme conditions, i.e., where there are no WAPs on a floor.
