Analysis of New Patient's Willingness to Pay Additional Costs for Securing Satisfactory Consultation Time (외래 초진환자의 만족진료시간과 관련 추가 비용 지불 의사)

  • Background: We evaluated new patient's satisfactory consultation time (SCT) and their willingness to pay additional costs (WPAC) for their SCT. Methods: We surveyed medical service satisfaction, SCT, WPAC for their SCT, and payable amount to 612 new patients of single general hospital and measured their real consultation time (RCT). To compare WPAC and payable amount, we divided RCT into 4 groups (${\leq}3$ minutes, 3-5 minutes, 5-10 minutes, and > 10 minutes), and SCT into 3 groups (${\leq}5$ minutes, 5-10 minutes, and > 10 minutes). On the basis of WPAC, we estimated new patient's SCT. Results: RCT was 6.2 minutes, SCT was 8.9 minutes, and medical service satisfaction score was 4.3 (out of 5). The number of patients having WPAC (payable group) was 381 (62.3%) and the amount was 5,853 Korean won. Their RCT and SCT were longer than non-payable group (6.4 minutes vs. 5.7 minutes, 9.3 minutes vs. 8.2 minutes). From multiple logistic regression analysis, WPAC of RCT 5-10 minutes was higher than that RCT ${\leq}3$ minutes (odds ratio= 1.78). Payable amount was highest in RCT > 10 minutes (6,950 Korea won) and SCT > 10 minutes (7,458 Korean won). Intuitively we suggest 10 minutes as SCT, based on payable group's SCT (9.3 minutes) and cut-off time differentiating payable group with non-payable group (10 minutes). Conclusion: We found that new patient had WPAC for their SCT and the longer the SCT, the greater the amount. From this, we hope that current simplified new patient consultation fee calculating system should be modified combining the consultation time factor.

Power Generation Cost Comparison of Nuclear and Coal Power Plants in Year 2001 under Future Korean Environmental Regulations -Sensitivity and Uncertainty Analysis- (미래의 한국의 환경규제여건에 따른 2001년도의 원자력과 석탄화력 발전단가비교 -민감도와 불확실도 분석-)

  • To analyze the impact of air pollution control on electricity generation cost, a computer program was developed. POGEN calculates levelized discounted power generation cost including additional air pollution control cost for coal power plant. Pollution subprogram calculates total capital and variable costs using governing equations for flue gas control. The costs are used as additional input for levelized discounted power generation cost subprogram. Pollution output for Rue Gas Desulphurization direct cost was verified using published cost data of well experienced industrialized countries. The power generation costs for the year 2001 were estimated by POGEN for three different regulatory scenarios imposed on coal power plant, and by levelized discounted power generation cost subprogram for nuclear power. Because of uncertainty expected in input variables for future plants, sensitivity and uncertainty analysis were made to check the importance and uncertainty propagation of the input variables using Latin Hypercube Sampling and Multiple Least Square method. Most sensitive parameter for levelized discounted power generation cost is discount rate for both nuclear and coal. The control cost for flue gas alone reaches additional 9-11 mills/kWh with standard deviation less than 1.3 mills/kWh. This cost will be nearly 20% of power generation cost and 40% of one GW capacity coal power plant investment cost. With 90% confidence, the generation cost of nuclear power plant will be 32.6-51.9 mills/kWh, and for the coal power plant it will be 45.5-50.5 mills/kWh. Nuclear is favorable with 95% confidence under stringent future regulatory requirement in Korea.

Clinical usefulness of teleradiology in general dental practice

  • Purpose: This study was performed to investigate the clinical usefulness of teleradiology in general dental practice. Materials and Methods: Two hundred and seventy five cases were submitted for inquiry to the case presentation board of the website of The Korean Academy of Oral and Maxillofacial Radiology for a 5 year periods. The diagnosis results of those cases were analyzed according to the disease classification, the correlation with the patient's chief complaint, the necessity of additional examinations or treatments, the image modalities, and the number of dentists inquiring. Results: Differential diagnoses of normal anatomic structures were the most frequently submitted cases, covering 15.6% of all cases. Among 275 cases, 164 cases required no additional treatments or examinations. Panoramic radiographs were the most frequently submitted images, accounting for 248 inquiries. The 275 cases were submitted by 96 dentists. Fifty-two dentists wrote one inquiry, and 44 inquired 2 or more times. The average inquiry number of the latter group was 5.0 cases. Conclusion: A teleradiology system in general dental practice could be helpful in the differential diagnosis of common lesions and reduce unnecessary costs.

Improved Shamir's CRT-RSA Algorithm: Revisit with the Modulus Chaining Method

  • RSA signature algorithms using the Chinese remainder theorem (CRT-RSA) are approximately four-times faster than straightforward implementations of an RSA cryptosystem. However, the CRT-RSA is known to be vulnerable to fault attacks; even one execution of the algorithm is sufficient to reveal the secret keys. Over the past few years, several countermeasures against CRT-RSA fault attacks have tended to involve additional exponentiations or inversions, and in most cases, they are also vulnerable to new variants of fault attacks. In this paper, we review how Shamir's countermeasure can be broken by fault attacks and improve the countermeasure to prevent future fault attacks, with the added benefit of low additional costs. In our experiment, we use the side-channel analysis resistance framework system, a fault injection testing and verification system, which enables us to inject a fault into the right position, even to within $1{\mu}s$. We also explain how to find the exact timing of the target operation using an Atmega128 software board.

  • Forging machinery traditionally gives an impression of heavy, greasy, noisy and approximate equipment. Recent demands from markets, especially automobile, have made machine designers to reconsider their concept. It is also noted that more advanced technologies in other industries, such as electronics and IT, have become accessible. Naturally these technologies were first applied in highly sophisticated fields for additional performance or features with extra expense. Drastic cost reduction in last decade changes this picture. They provide not only performance and functions but also a chance in reducing costs. Powerful servomotors, accurate sensors and fast processor satisfy performance, precision and machine control requirement. Control by wires replaces traditional mechanical linkage. Additional features such as factory automation will come almost free.

  • The validation technique is classified with two methods whether to demand of additional experimental points. The method which requires additional experimental points such as RSME is actually impossible in engineering field. Therefore, the method which only use experimented points such as the cross validation technique is only available. But the cross validation not only requires considerable computational costs for generating metamodel each iterations, but also cannot measure quantitatively the fidelity of metamodel. In this research we propose a new validation technique for representative metamodels using an variance of metamodel and confidence interval information. The proposed validation technique computes confidence intervals using a variance information from the metamodel. This technique will have influence on choosing the accurate metamodel, constructing ensemble of each metamodels and advancing effectively sequential sampling technique.

  • This paper examines the Spaghetti bowl effect that different tariffs and rules of origin in multiple FTAs have resulted in increasing the significantly additional burden for business when it comes to apply for the use of FTA preference. The wide spread of FTAs in the several years, from 2003 to 2010, has been the most important trade policy development in economically important Korea. Korea presently has 5 FTAs in effect, and made 3 additional agreements which will be expected to take effect in next year. With the study result and expecting a growing number of FTAs in Korea in a next decade, the international trading firms will face rise of transaction costs for enterprises, particularly small- and medium-sized enterprises(SMEs) to cope with multiple tariffs and rule of origins in FTAs. To help mitigate negative effects and facilitate a more SEMs to use the FTA preference, providing new computer programs system, increasing the awareness of FTA provisions, improving business participation in FTA consultations, and SME support in light of education, and financial support are needed.

  • In recent automotive industries, fine blanking technology widely used in production of components with relatively thick gauges in brake systems, seat recliner, door locks, and auto transmission systems. Due to its advantages to obtain almost final product quality using fine blanking forming process without additional finish machining processes, consequently saving the production costs. In this paper we intended to develope the small sized module gear toothed dual seat recliner sector gear(0.5mm module) for car seats using fine blanking process which needed semi piercing with computer simulation and a lot of try and errors to achieve required accuracy and geometric quality. However through the some corrections of tool geometries with tryout test, we could get successful results.

  • This paper considers the problem of determining the optimal process mean and screening specification limits of a surrogate variable associated with product quality under two-stage screening procedure. In two-stage screening, the surrogate variable is inspected first to decide whether an item should be accepted, rejected or additional observations should be taken. If additional observations are required, the performance variable of interest is then observed to classify the undecided items. Assuming that the performance variable and the surrogate variable are jointly normally distributed, the optimal process mean and the screening limits are obtained by maximizing the expected profit which includes selling price, production, reprocessing, inspection and penalty costs. A numerical example is presented and numerical studies are performed to compare the proposed two-stage screening procedure with single-stage screening procedures.

  • Recently, we use a static synchronous compensator(STATCOM) with cascaded H-bride topologies, because it is easy to increase capacity and to reduce total harmonic distortion(THD). When we use equipment for reactive power compensation, dc-link voltage unbalances occur from several reasons although loads are balanced. In the past, switching pattern change of single phase inverter and reference voltage magnitude change of inverter equipped with power sensor have been used for dc-link voltage balance. But previous methods are more complicated and expensive because of additional component costs. Therefore, this paper explains reasons of dc-link voltage unbalance and proposes solution. This solution is complex method that is composed of reference voltage magnitude change of inverter without additional hardware and shifted phase angle of inverter reference voltages change. It proves possibility through 1000[KVA] system simulation.
