부산지역에서 발생한 Yersinia Pseudotuberculosis 감염에 의한 급성 신부전 2례 (Two Cases of Yersinia Pseudotuberculosis Infection with Acute Renal Failure in Pusan Province)

  • Yersinia pseudotuberculosis, a gram-negative coccobacillus, is a member of the enterobacteriaceae family, and the illness has fever, abdominal pain, diarrhea, mesenteric lymphadenitis, terminal ileitis, arthropathy, erythema nodosum. Specially in children, it is known as an important cause of acute renal failure due to acute tubulointerstitial nephritis. It has been recognized in a worldwide distribution. For the first time, the cases of acute renal failure caused by this organism were reported in Japan in 1980's. In Korea, the first case accompanying with ARF was reported in 1988. Since that, many cases have been reported in Seoul area, but no case in other places. So we report 2 cases of Y.P. infection accompanying with ARF in Pusan area, for the first time in other place than Seoul area.

  • 저자들은 핍뇨를 주소로 내원한 12세 남아에서 임상 소견 및 검사 소견 상 전신적 강직성 경련으로 유발뇐 횡문근융해증에 의해 초래된 급성신부전으로 진단하고 충분한 수액공급 소변의 알칼리화, 이뇨제 투여와 함께 항경련제의 사용으로 경련을 조절함으로써 급성신부전이 호전된 1례를 경험하였기에 보고하는 바이다.

대분청음(大分淸飮)이 Cisplatin으로 유발(誘發) 된 흰쥐의 급성신부전(急性腎不全)에 미치는 영향(影響) (Nephroprotective Effects of Daebuncheong-eum Aqueous Extracts on Cisplatin-induced Acute Renal Failure in Rats)

  • Purpose : This study was to observe the nephroprotective effects of the traditional prescription, Daebuncheong-eum (DBCE). DBCE has generally been used for treating various renal diseases, including renal failure. Methods : Three different dosages of DBCE extract were orally administered once a day for 28 days. At the 23rd day after DBCE extract treatment, cisplatin was also treated. Then, 5 days after cisplatin treatment, all rats (6 groups of 8 rats each) were sacrificed. Changes on the body weight, kidney weight, serum BUN and creatinine levels were observed, along with changes to the kidney MDA and GSH contents. The results were compared with captopril 100mg/kg, from which the effects on cisplatin-induced acute renal failures have already been confirmed. Results : Cisplatin induced ARF are induced by oxidative stress and related lipid peroxidation in the present study. However, these ARFs and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment of all three different dosages of DBCE extracts. Conclusion : This study suggests that DBCE extracts show favorable effects on cisplatin-induced rat ARF.

  • Objectives : The object of this study was to observe the nephroprotective effects of Sojongchobisunki-tang (SCST), which has traditionally been used as Korean medicine for treating various renal diseases, on cisplatin-induced rat acute renal failure. Methods : Three different dosages of SCST were orally administered once a day for 23 days before cisplatin treatment (5 mg/kg, single intraperitoneally administered) and 5 days after cisplatin treatment (once a day for 28 days). 6 groups, of 8 rats per group were used in the present study after 7 days of acclimatization. Changes of the body weight, kidney weight, serum BUN and creatinine levels were observed, as well as changes of the kidney MDA and GSH contents. The results were compared with captopril 100 mg/kg of which the effects on cisplatin-induced acute renal failures are already confirmed. Results : Acute renal failure induced by cisplatin were induced by oxidative stress and related lipid peroxidation in the present study. However, these acute renal failures and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment at all three different dosages of SCST extracts. Methods : Three different dosages of OPS extracts were administered once a day for 28 days (5 mg/kg, single intraperitoneally administered). Changes of body weight, kidney weight, serum BUN and creatinine levels were observed, as well as changes to the kidney MDA and GSH contents. The results were compared with captopril 100 mg/kg, of which the effects on cisplatin-induced acute renal failures were already confirmed. Results : Cisplatin-induced ARF were induced by oxidative stress and related lipid peroxidation in the present study. However, these ARF and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment of OPS extracts except in the 100 mg/kg treated group. The effects of OPS extracts at 500 mg/kg were similar to those of 100 mg/kg of captopril. Conclusion : This study suggests that OPS extracts 500 mg/kg showed favorable effects on the cisplatin-induced rat's ARF as similar to 100 mg/kg of captopril.

삼음교(三陰交)(SP6), 방광유(膀胱兪)(BL28)의 침자 및 애구자극이 흰쥐의 실험적 급성신부전에 미치는 영향 (Effect of Acupuncture and Moxibustion at SP6(Sanyinjiao) and BL28(Pangguangshu) on Experimental Acute renal failure in rats)

  • Objective : The aim of this experiment is to investigate the effect of acupuncture and moxibustion at SP6(Sanyinjiao) and BL28(Pangguanshu) on uranyl acetate-induced acute renal failure in rats. Methods : To induce experimental acute renal failure, uranyl acetate was intraperitoneally injected to rats by 5mg/kg. 3 days later acupuncture and moxibustion treatment was done at SP6(Sanyinjiao) and BL28(Pangguanshu) bilaterally of the rats. And then 4 days later, we measured rats' body weight, kidney weight, serum BUN, creatinine and serum eletrolyte levels(Na, K and Cl). Results : In the moxibustion group, hypertrophy index of kidney and serum BUN level were decreased marginally significantly, and serum creatinine, K levels were decreased significantly. But in the acupuncture group, there was no significance at this experiment. Conclusions : SP6(Sanyinjiao) and BL28(Pangguanshu) moxibustion can decrease serum BUN, creatinine, and serum eletrolyte levels on uranyl acetate-induced acute renal failure in rats. And the study will be done continuously about non-effect in the acupuncture group at this experiment.

소아에서 횡문근융해증을 동반한 급성신부전의 임상양상 (Clinical characteristics of acute renal failure of rhabdomyolysis in children)

  • 목 적 : 급성 신부전은 횡문근 융해증의 중요 합병증 중의 하나이다. 저자들은 횡문근 융해증으로 진단된 환아를 대상으로 원인, 임상 경과, 검사실 소견을 분석하고 급성 신부전 및 사망의 위험 인자를 밝히고자 한다. 방 법 : 1997년 1월부터 2005년 5월까지 경상 대학교 병원 소아과에 횡문근 융해증으로 입원한 60명의 환아들의 임상 기록지와 병리 기록지를 후향적으로 조사하였다. 횡문근 융해증 진단 기준은 creatine phosphokinase (이하 CK)치가 1,000 IU/L이상이고 CK isoenzyme 검사에서 MM형이 95%이상이거나 혈중 미오글로빈이 300 mg/dL 이상이거나 소변 미오글로빈이 양성이면서 급성 심근 경색증이나 뇌졸중의 증거가 없는 경우로 정의하였다. 결 과 : 혈중 입원 시 CK 및 최고 CK는 급성 신부전의 예측 인자가 아니었고 입원 시 혈중 크레아티닌, 요산, pH 및 칼륨, 입원 당시의 수축기 혈압, 의식 상태와 급성 신부전은 유의한 상관 관계가 있었다. 입원 당시 혈중 CK, 요산, pH와 최고 혈중 크레아티닌 등의 검사실 소견과 입원 당시의 의식 상태 및 수축기 혈압과 사망과는 상관 관계를 보이지 않았으며 최고 CK는 사망과 상관 관계가 있었다. 결 론 : 소아에서 횡문근융해증과 동반된 급성 신부전의 사망률은 매우 높게 나타났으므로 급성 신부전으로 진행 여부를 예측 할 수 있는 인자를 조기에 파악하여 적극적인 치료를 해야 할 것이다. 특히 검사실 소견뿐 아니라 입원시 혈압이나 의식 상태, 유발 요인 등의 임상 상태가 예후를 예측하는 중요 인자임을 명시해야 할 것이다.

  • PNH는 혈관내 용혈, 야간 혈색소뇨, 혈전증, 골수부전을 특징으로 하는 후천적 질환이며 소아에서는 매우 드물게 발생한다. 급성 신부전은 PNH의 합병증으로 알려져 있지만 우리나라 소아에서의 보고는 아직 없었다. 저자들은 소아에서 PNH로 인한 급성 신부전을 경험하였기에 이를 보고하며 용혈을 동반한 소아의 급성 신부전의 감별 진단에서 PNH가 포함되어야 한다고 생각한다.

Protective effects of a mineral aqueous solution on toxicity in mouse liver and kidney

  • We demonstrated that a mineral aqueous solution (MAS) administered to mice functionally and histologically protected against cisplatin-induced acute renal failure (ARF) and $CCl_4$-induced acute liver failure (ALF). In ARF model, 0.4 and 0.2% MAS decreased mortality and the serum concentrations of blood urea nitrogen (BUN) and creatine in mice. Additionally, 0.4 and 0.2% MAS reduced contraction of distal convoluted tubules and suppressed expression of the proinflammatory cytokines interlukein-6 (IL-6) and tumor necrosis factor (TNF-${\alpha}$) in the kidney. In ALF model, 0.4 and 0.2% MAS decreased serum concentrations of alanine aminotransferase and aspartate aminotransferase in mice. Additionally, 0.4 and 0.2% MAS reduced necrotic areas and suppressed expression of IL-6 and TNF-${\alpha}$ in the liver. These results indicate that a MAS might have protective effects against ARF and ALF.

Methods : At first, three different dosages of MJMTS extract were orally administered once a day for 28 days. On the 23rd day after MJMTS extract treatment, cisplatin was treated. Then, 5 days after cisplatin treatment, all the rats (6 groups of 8 rats each) were checked in the present study. The changes to the body weight, kidney weight, serum BUN and creatinine levels were observed with changes on the kidney MDA and GSH contents. The results were compared with captopril 100mg/kg in which the effects on cisplatin-induced acute renal failures were already confirmed. Results : It showed dramatical decrease on the body weight, increase of serum BUN and creatinine levels were detected in cisplatin control as compared with intact control. In addition marked increase of kidney MDA contests and decrease of kidney GSH contents were also detected in cisplatin control as compared with intact control. it means that cisplatin induced ARF are induced by oxiidative stress and related lipid peroxidation in the present study. However, these ARFs and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment of all three different dosages of MJMTS extracts. Conclusion : This study suggests that MJMTS extracts have favorable effects on the cisplatin-induced rat ARF.
