Factors Influencing Health Status in Patients with Acute Coronary Syndrome (급성관상동맥증후군 환자의 건강상태에 영향을 미치는 요인)

  • Purpose: This study was done to investigate the correlation of illness perception, coping strategy and health status in patients with acute coronary syndrome who had undergone a coronary angioplasty. Methods: A descriptive correlational design was used in this study. A total of 102 patients with acute coronary syndrome was recruited. Data were collected using structured questionnaires including scales for the illness perception, the coping strategy, and health status. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis. Results: Health status showed significant correlations with illness perception (r=-.44, p<.001) and avoidance coping (r=-.33, p=.001); illness perception, avoidance coping explained 21% of variance in health status (F=8.58, p<.001). The factors that influenced health status were illness perception (${\beta}=-.37$, p<.001), and avoidance coping (${\beta}=-.21$, p<.001). Conclusion: The findings of this study indicate that illness perception and avoidance coping were significant variables for health status in patients with acute coronary syndrome who had undergone a coronary angioplasty.

  • Acute coronary syndrome involves three types of coronary artery disease associated with sudden rupture of coronary artery plaque, and has a clinical presentation ranging from ST-segment elevation myocardial infarction (STEMI) to non-ST-segment elevation myocardial infarction (NSTEMI) or unstable angina. Cardiac CT can help quantify and characterize atherosclerotic plaques. According to a previous study, low-attenuation plaque, napkin ring sign, positive remodeling, spotty calcification, and increased perivascular fat attenuation are associated with plaque ruptures on cardiac CT. Therefore, coronary artery stenosis, as well as acute coronary artery syndrome, can be diagnosed using cardiac CT.

  • Objective Patients with acute coronary syndrome (ACS) are at an increased risk of suicide. It is well known that epigenetic mechanisms may explain the pathophysiology of suicidal behavior including suicidal ideation (SI), but no study has explored these mechanisms in ACS populations. Methods In total, 969 patients were initially recruited within 2 weeks of the acute coronary event and, 711 patients were successfully followed up 1 year after ACS. SI was evaluated using the relevant items on the Montgomery-${\AA}sberg$ Depression Rating Scale and covariates potentially affecting SI were estimated. Results Brain-derived neurotrophic factor (BDNF) hypermethylation was associated with SI in both the acute and chronic phases of ACS, although the association was not statistically significant in the acute phase after applying Bonferroni's correction. Conclusion These results suggested that BDNF hypermethylation may have played a role in an epigenetic predisposition for SI in ACS patients, particularly during the chronic phase.

Verification of Reliability and Validity of K-SART to Assess of Situational Awareness of Patients with Acute Coronary Syndrome (급성관상동맥증후군 환자의 상황인식 측정을 위한 K-SART의 신뢰도와 타당도 검증)

  • The purpose of this study was to test the reliability and validity of the SART(Situation Awareness Rating Technique) tool to measure the situational awareness of patients with acute coronary syndrome. The translated SART was tested on 118 hospitalized patients with acute coronary syndrome to verify the reliability and validity of the tool. The analysis was conducted with Cronbach's alpha, exploratory & confirmatory factor analysis. Study findings found that the tool's reliability and model fit index improved when the two problematic items, namely, Situation Awareness 1 (Instability of Situation) and Situation Awareness 7 (Spare Mental Capacity) were removed. According to the results, the revised K-SART is deemed to be an appropriate method for assessing the degree of situational awareness of patients with acute coronary syndrome in decision making about treatment-seeking.

  • The carbamates are a group of insecticides derived from carbamic acid, with a broad spectrum of uses as agricultural and household garden insecticides. Carbamate insecticides are reversible cholinesterase inhibitors. Their inhibitory action is mediated by reversible carbamylation of acetylcholine, as with the organophosphate insecticides. Carbamates are absorbed by the body through multiple routes, including inhalation, ingestion, and dermal absorption. Although poisoning can result from occupational exposure or accidental ingestion, in most cases there is suicidal intent. This is particularly true in developing countries, where the highest incidence of morbidity and mortality from this cause occurs. Cardiac complications often accompany poisoning by carbamate compounds, which may be serious and often fatal. The extent, frequency, and pathogenesis of cardiac toxicity from carbamate compounds has not been clearly defined. Possible mechanismsinclude sympathetic and parasymphatetic overactivity, hypoxemia, acidosis, electrolyte derangements, and a direct toxic effect of the compounds on the myocardium. Patients with carbamate poisoning should immediately be transferred to an intensive or coronary care unit where appropriate monitoring and resuscitative facilities are available. We here report a case of acute coronary syndrome resulting from acute carbamate ingestionthat resulted in a healthy discharge.

  • Nuclear myocardial perfusion imaging is very effective in the evaluation of patients with suspicious acute coronary syndrome (ACS), for adequate diagnosis and treatment. There have been many clinical evidences to support the efficacy and cost-effectiveness. In addition, many authoritative guidelines support the utility of myocardial perfusion imaging in ACS with an appropriate diagnostic protocol. However, with the development of other cardiac imaging modalities, the choice of modality for the diagnosis of suspicious ACS now depends on the availability of each modality in each institute. Newly developed imaging technologies, especially including molecular imaging, are expected to have great potential not only for diagnosis but also for primary, secondary, and tertiary prevention of ACS.

Predictors of Intention to Quit Smoking in Patients with Acute Coronary Syndrome (급성관상동맥증후군 환자의 금연의도에 영향을 미치는 요인)

  • Objectives: This study is a descriptive research to investigate the predictors of intention to quit smoking in patients with acute coronary syndrome(ACS). Methods: A total of 192 ACS patients hospitalized for an angiogram during symptom management were conveniently recruited from a university hospital cardiovascular care unit. Data were collected from January to December in 2018 and were analyzed using binominal logistic regression. Results: The predictors of intention to quit smoking in patients with ACS were drinking(odds ratio[OR]=0.315, p=.006), experience of smoking cessation education(OR=0.325, p=.007), depression(OR=0.739, p<.001), and smoking-related self-efficacy(OR=1.091 p=.006). Conclusion: The findings suggest that the alleviation of depression and enhancement of smoking-related self-efficacy can prevent recurrence and enhance the treatment of ACS.

  • Despite medical and invasive advancements, cardiovascular mortality attributable to ischemic heart disease, including acute coronary syndrome (ACS), persists at a substantial level. Clinical guidelines have been issued by the European Society of Cardiology (ESC) and American College of Cardiology/American Heart Association to support clinicians in making appropriate decisions and improving the prognosis of ACS patients. ESC recently introduced a new ACS guideline in August 2023. This review discusses the recent modification in the diagnosis and management of early-phase ACS in the 2023 ESC guideline.

  • Purpose: The purpose of this study was to explore the differences of health locus of control and treatment compliance according to general characteristics and severity in acute coronary syndrome(ACS) patients after percutaneous coronary intervention. Methods: This descriptive study was conducted with a convenient sample of 103 ACS patients. The dependent variables were measured by the scales for the Multidimensional Health Locus of Control and the Treatment Compliance. The collected data were analyzed by the Fisher's exact test, Chi-square and t-tests, and ANOVA using the SPSS program. Results: Pre-interventional severity was significantly different between men and women. In terms of internal health locus of control, there was a significant difference according to gender, educational status, economic status, and severity. The level of medication compliance was the lowest among the sub-scales of treatment compliance. Conclusion: These findings suggest that clinical nurses should evaluate the general characteristics and severity of the patients with ACS for providing tailored nursing interventions.

Triage level and treatment time according to mode of arrival to emergency department in patients with acute coronary syndrome (급성 관상동맥 증후군 환자의 응급실 내원 수단에 따른 중증도 분류와 치료시간 비교)

  • Purpose: The purpose of this study was to determine whether, in patients with acute coronary syndrome (ACS), according to the mode of arrival affect the emergency medical process. Methods: The participants of this study were 118 adult patients (46 patients admitted by emergency medical services (EMS), 48 walk-in patients, and 24 transferred from other hospitals) admitted to the emergency departments at one regional-level medical center who underwent coronary angiography between January 1, 2016 and December 31, 2017. To compare treatment courses, the median values of the following variables were compared among groups: symptom to door time; door to triage time; and door to ECG time. All data were analyzed using SPSS program. Results: Based on the initial assessment at triage, there was a significantly greater proportion of Korean Triage and Acuity Stage (KTAS) Level 1 or 2 among patients admitted by EMS than among walk-in patients. All three analyzed variables were lower in patients admitted by EMS than in the other two groups. Conclusion: Our results show that ACS patients who accessed EMS reached the emergency center faster after symptom onset, received initial triage assessment at earlier stages, and underwent sooner important examinations (i.e., the 12-lead ECG).
