한방병원 외래에 내원한 COPD환자 대한 후향적 임상 분석 (A Retrospective Clinical Analysis of Chronic Obstructive Pulmonary Disease (COPD) Outpatients Who Presented To a Korean Medicine Hospital: Symptoms and Treatment Effects)

  • Objective: This study was designed to analyze the symptoms of chronic obstructive pulmonary disease (COPD) patients who attended a Korean medicine hospital and treatment effects through retrospective chart reviews.Methods: The medical records of 192 outpatients who had been diagnosed with COPD and visited the Allergy, Immune, and Respiratory System Department of Kyung Hee Korean Medicine Hospital from 1 February 2006 to 1 February, 2016 were retrospectively reviewed.Results: The study group consisted of 112 and 80 females. The median age of the patients was 59.80±15.46 y. Fifty of the patients had been diagnosed with chronic upper respiratory diseases, such as chronic rhinitis, nasopharyngitis, or sinusitis. The chief complaints were cough (n=136), sputum (n=124), and dyspnea (n=82). Other frequent symptoms were fatigue (n=11), hyperhidrosis (n=8), and a bad taste in the mouth (n=7). All the patients were prescribed Korean herbal medicine. In the study, 61 (31.77%) patients were treated with acupuncture, moxibustion, cupping therapy, or herbal steam therapy. Symptoms improved in 126 (65.63%) patients 141±272.82 d after the first treatment.Conclusions: Some of the COPD patients had chronic upper respiratory disease. The chief complains were cough, sputum, and dyspnea. Oher frequent symptoms related to body malfunction and pain. The symptoms improved in 126 (65.63%) patients 141.00±272.82 d post-treatment.

한방 비만클리닉에 내원한 일부 비만 여성의 삶의 질 (Quality of Life of Some Obese Patients Wanted to Receive Korean Traditional Medicine)

  • Obesity is chronic disease which influenced on health severly. The causes of obesity have been known as life change, lack of excercise, genetic factor, mental and social economic factors. Especially the obesity of women increased the risk of the diseases such as DM, osteoarthritis, cardiovascular disease, breast cancer and infertility. The limitations of the widely used negative definition of health as the absence of disease and WHO's 1946 definition of health as total social, psychological and physical well-being have long been recognized (WHO 1958). The Quality of Life (QoL) includes functional ability, the degree and quality of social and community interaction, psychological well-being as somatic sensation and life satisfaction. I investigated to compare the differences between obese women (n=63), non-obese women (n=37) in clinic and general women (n=43, control) on baseline characteristics and WHO QoL-BREF. The purpose of this study is to assist the diagnosis and treatment of obesity. WHO QoL-BREF is self administered type which consisted of 26 questions. The prospective question is calculated with 5 scores by Likert's method. The results are as follows : The means of physical, psychological, social, overall and total scores of QoL were significant among BMI group (P<0.05). The score of control group (BMI < 25) was higher than other groups significantly (P<0.05). In multiple regression analysis, the variable of high school/below middle school was significant in environmental and overall domain of QoL scores (P<0.05). The variable of college/below middle school was significant in environmental, overall domain and total score of QoL scores (P<0.05). The variable of above university/below middle school was significant in physical health, environmental, overall domain and total score in QoL scores (P<0.05). The variable of Health perception (moderate/bad) was positively significant in physical health, environmental, overall domain and total score of QoL scores (P<0.05). The variable of Health perception (good/bed) was positively significant in physical health, environmental, social, overall domain and total score of QoL scores (P<0.05). The variable as BMI non-=obese women/control was negatively significant in social domain of QoL scores (P<0.05). Above the results, It suggests that the variable as BMI did't affect on the QoL in patients and control, but the variables as education and health perception affected on the QoL scores. Further study is required to conduct QoL differences between before and after treatment of obese patients.

수궐음 심포경근의 해부학적 고찰 (Study on the Anatomical Pericardium Meridian Muscle in Human)

  • Objectives : This study was carried to identify the component of the Pericardium Meridian Muscle in human. Methods : The regional muscle group was divided into outer, middle, and inner layer. The inner part of body surface were opened widely to demonstrate muscles, nerve, blood vessels and to expose the inner structure of the Pericardium Meridian Muscle in the order of layers. Results We obtained the results as follows; He Perfcardium Meridian Muscle composed of the muscles, nerves and blood vessels. In human anatomy, it is present the difference between terms (that is, nerves or blood vessels which control the muscle of the Pericardium Meridian Muscle and those which pass near by the Pericardium Meridian Muscle). The inner composition of the Pericardium Meridian Muscle in human is as follows ; 1) Muscle P-1 : pectoralis major and minor muscles, intercostalis muscle(m.) P-2 : space between biceps brachialis m. heads. P-3 : tendon of biceps brachialis and brachialis m. P-4 : space between flexor carpi radialis m. and palmaris longus m. tendon(tend.), flexor digitorum superficialis m., flexor digitorum profundus m. P-5 : space between flexor carpi radialis m. tend. and palmaris longus m. tend., flexor digitorum superficialis m., flexor digitorum profundus m. tend. 음파진동을 이용한 토끼의 림프관내 프리모관 분리와 수득률 향상 특성 연구 (Isolation and Yield Enhancement of Primo Vessels Inside of Rabbit Lymph Vessels by Using Sound Wave Vibration)

  • Objectives : The fact that Primo vessel as a new circulatory system exists in the lymphatic vessels of the rabbits which were treated with sound wave vibration therapy is confirmed with the anatomy of rabbit. Isolated Primo vessel is investigated, particularly focused on morphological features. Methods : Before the anatomy, a rabbit in a cage box was laid on the sound wave vibrating apparatus and then is applied by various frequencies and intensities for 30 min with music that a rabbit likes. Results : Isolation and observation of Primo vessel was easier when the lymphatic circular system was applied at a certain frequency and intensity of 7 Hz and 50. The probability of observation for Primo vessel enhanced to 90%. Conclusions : The sound wave therapy by a vibration apparatus is considered necessary as major process to facilitate the reproducible isolation and observation of Primo vessel.

조선후기 의서 『낙산당신집의방금낭지보(樂山堂新集醫方錦囊至寶)』 수재(收載) 약성가(藥性歌)에 대한 연구 (A Study on Rhymes of Herbal Medicine in Yosandangsinjipeuibangkeumnangjibo during Late Joseon Period)

  • The medical book Yosandangsinjipeuibangkeumnangjibo (『樂山堂新集醫方錦囊至寶』, below as Yosandang, 『樂山堂』) is housed in the Handok museum of Medicine and Pharmacy. At the end of the Yosandang, written in manuscript, contains a rhymes of herbal medicine (藥性歌). The rhymes of herbal medicine is a record of song form about herbal medicine. Song-forms were widely used because they were easy to sing and memorize. In particular, there are many records of song forms in Korean medical books, which have been used in various fields such as herbal medicines, acupuncture points, and diagnosis. Although Yosandang is not a widely known medical book, it is meaningful in that it shows a cross section of late Joseon Korean medicine. So, this paper considers rhymes of herbal medicine in Yosandang. Yosandang is a medical book of the late Joseon period written by doctor Byun Gwangwon in 1806. This book consists of 6 books and 14 volumes, 13 of which are rhymes of herbal medicine. The rhymes of herbal medicine in Yosandang is 7 words-2 phrases form for the first time in Korea. This is almost 80 years earlier than the same form of Bangyakhabpyeon (『方藥合編』). The first part of rhymes of herbal medicine in Yosandang is considered to refer to the rhymes of herbal medicine in Jejungsinpyeon (『濟衆新編』), and the last part seems to be based on the contents of Donguibogam (『東醫寶鑑』). In other words, rhymes of herbal medicine in Yosandang could be considered as the result of trying to contain herbal medicine knowledge as a new 7 words-2 phrases form based on the Jejungsinpyeon (『濟衆新編』) and Donguibogam (『東醫寶鑑』). Unlike the previous rhymes of herbal medicine made during the compilation of medical books led by the late Joseon government, the rhymes of herbal medicine in Yosandang is a new type of rhymes created based on individual efforts in the early 19th century. It has a medicine historical significance in that it can show some aspects of Korean medicine in the late Joseon period.

코퍼스 분석방법을 이용한 『동의보감(東醫寶鑑)』 영역본의 어휘 분석 (An Analysis on the Vocabulary in the English-Translation Version of Donguibogam Using the Corpus-based Analysis)

  • Objectives : A quantitative analysis on the vocabulary in the English translation version of Donguibogam. Methods : This study quantitatively analyzed the English-translated texts of Donguibogam with the Corpus-based analysis, and compared the quantitative results analyzing the texts of original Donguibogam. Results : As the results from conducting the corpus analysis on the English-translation version of Donguibogam, it was found that the number of total words (Token) was about 1,207,376, and the all types of used words were about 20.495 and the TTR (Type/Token Rate) was 1.69. The accumulation rate reaching to the high-ranking 1000 words was 83.54%, and the accumulation rate reaching to the high-ranking 2000 words was 90.82%. As the words having the high-ranking frequency, the function words like 'the, and of, is' mainly appeared, and for the content words, the words like 'randix, qi, rhizoma and water' were appeared in multi frequencies. As the results from comparing them with the corpus analysis results of original version of Donguibogam, it was found that the TTR was higher in the English translation version than that of original version. The compositions of function words and contents words having high-ranking frequencies were similar between the English translation version and the original version of Donguibogam. The both versions were also similar in that their statements in the parts of 'Remedies' and 'Acupuncture' showed higher composition rate of contents words than the rate of function words. Conclusions : The vocabulary in the English translation version of Donguibogam showed that this book was a book keeping the complete form of sentence and an Korean medical book at the same time. Meanwhile, the English translation version of Donguibogam had some problems like the unification of vocabulary due to several translators, and the incomplete delivery of word's meanings from the Chinese character-culture area to the English-culture area, and these problems are considered as the matters to be considered in a work translating Korean old medical books in English.

"유문사친(儒門事親)"의 '한법(汗法)'에 관한 연구 (A Study of Diaphoretic Therapy[汗法] in "Yumunsachin(儒門事親)")

  • Jangjahwa(張子和) was influenced by "Hwangjenaegyeong(黃帝內經)" and Yuhagan(劉河間)'s theory, and other classics. Also, his clinical experience was helpful to theorize his thought. Thus, he improved medical theory by combining previous medical theory and his own experience. The essence of his thought is the importance of pathogenic Gi[邪氣] as the cause of disease and is Sambeop(三法) of Hantoha(汗吐下) as the methodology for removing pathogenic Gi[邪氣] away. He regarded pathogenic Gi as the cause of disease, and eliminated pathogenic Gi for the remedy. Namely, Sambeop(三法) of Hantoha(汗吐下) was selected as the best efficient method for driving pathogenic Gi away. Sambeop of Jangjahwa(張子和) have different meaning from previous one. Traditionally, Diaphoretic Therapy[汗法] was regarded as therapy for exogenous disease[外感病], and its effect was regarded as Balhanhaepyo(發汗解表). Emetic therapy[吐法] was throwing up Dameumsuksik(痰飮宿食) of stomach and above diaphragm. Purgation therapy[下法] means Tongbyeon(通便), Hajeok(下積), Sasil(瀉實), Chuksu(逐水) were regarded as therapy for Yangmyeongsiljeung(陽明實證) of Sanghan(傷寒). He submitted a new extensive concept of Sambeop adding traditional one, and expanded the application range of Sambeop. All methods, can cause circulation of Gihyeol(氣血) by opening the 'Hyeonbu(玄府)', like Moxibution therapy[灸薰], Steaming[蒸], Washing[洗],Heat therapy[慰], Cauterization[烙], Acupuncture therapy[鍼刺], Stone needling, Physical and breathing exercise[導引], Massage[按摩] were regarded as Diaphoretic Therapy[汗法]. Especially, he thought that Diaphoretic Therapy and venesection[瀉血] have same medical implication. If we examine the process of pushing out pathogenic Gi[邪氣] by means of Sambeop(三法), we can find the intermediation, that is circulation of Gihyeol(氣血). Its meaning is implied in the word of 'opening Hyeonbu(玄府)'. He thought that the circulation of Gihyeol(氣血) is the key to control health. Gihyeol(氣血) was circulated well under the physiological balance, but it was not circulated well under the invasion of pathogenic Gi[邪氣]. In other words, pathogenic Gi is the immediate cause of bad circulation of Gihyeol(氣血) and disease. Naturally, the doctor must remove pathogenic Gi that cause bad circulation for healing by means of Sambeop(三法). In my opinion, because the ultimate goal of Jangjahwa(張子和) was circulation of Gihyeol(氣血) by removing pathogenic Gi[邪氣], the concept of Sarnbeop(三法) could be expanded.

Transfer Energy Capacitive and Resistive (TECAR) Therapy와 한방치료를 병행한 경부 근 긴장성 사경 환자 증례 보고 1예 (Clinical Study on 1 Case of Cervical Dystonia Treated by Korean Medicine and Transfer Energy Capacitive and Resistive (TECAR) Therapy)

  • Cervical dystonia (CD) is a disorder characterized by sustained or intermittent, involuntary muscle contractions which cause twisting, repetitive movements and abnormal postures. In this case report, a CD patients was treated with transfer energy capacitive and resistive (TECAR) therapy and conventional Korean medicine, which consists of acupuncture, Chuna manual medicine, pharmacopuncture and herbal medicine. For outcome measures, this study evaluated Cobb's angle, EuroQol five dimension scale (EQ-5D), numerical rating scale (NRS), neck disabillity index (NDI) and cervical range of motion. As a result, improvements were found in Cobb's angle ($18.65^{\circ}$ to $15.90^{\circ}$), EQ-5D score (0.808 to 0.862), NRS (5 to 3), NDI (16 to 8) and angle of cervical lateral bending ($15^{\circ}$ to $25^{\circ}$). In conclusion, this study shows that combined therapy of conventional Korean medicine treatment and TECAR therapy can be effective for CD patients.

저신장 및 성장과 관련된 질적연구에 대한 체계적 문헌 고찰 (Systematic Review of Short Stature and Growth Related Qualitative Researches)

  • Objectives The aim of this study is systematic evaluation of the quality of qualitative studies related to short stature. It will be utilized as fundamental data for future qualitative research about Korean Medicine Pediatrics treatment for short stature. Methods We searched qualitative studies related to short stature in the Pubmed. Two researchers independently assessed the quality of the qualitative researches by using Consolidated criteria for reporting qualitative research (COREQ) and Critical Appraisal Skills Program (CASP). We also qualitatively synthesized theme of each included studies. We presented integrated theme and categories with in vivo coding about qualitative research about short stature. Results A total of 47 papers were screened and 7 papers were finally selected. In the quality assessment using COREQ, detalied information about the researchers, methods of approach, reasons for non-participation, presence of non-participants, participant's review of the transcript, and participant's review of the findings were insufficient. In the quality evaluation using CASP, there were lack of justifications for the research methods, description method of research, and bias in process of data analysis. The findings were synthesized into three themes: fixed idea about height, discomfort caused by small stature and personal coping strategy with stress caused by short stature. Conclusions In further qualitative research, it is necessary to report the research according to COREQ and CASP checklist. It is also necessary to understand the stress and coping strategies to short stature of patients, We need to develop a social management strategy for short stature.

일개 한방병원에 내원한 유방암 환자의 특성 및 치료 분석 (Analysis of the Characteristics and Treatment of Breast Cancer Patients in a Korean Medicine Hospital)

  • Objectives: The purpose of this study is to provide basic information on Korean medical research and treatment through analysis of breast cancer patients, who visited ${\bigcirc}{\bigcirc}$ university Korean medicine hospital. Methods: To analyze characteristics of breast cancer patients who visited korean medicine hospital, we searched medical records from January 1, 2016 to May 13, 2019, and 86 breast cancer patients were analyzed. Results: 1. The general characteristics of the subjects are as follows. The average age was $47.65{\pm}9.62years$ and 40's was the most (46.51%). The average height was $159.78{\pm}4.91cm$ and the average weight was $57.29{\pm}9.34kg$. The average body mass index (BMI) of the patients was $22.40{\pm}3.50kg/m^2$. 2. The stage distribution record indicated stage0 (9.72%), stageI (31.94%), stageII (34.72%), stageIII (20.83%) and stageIV (2.78%). 14 patients with unknown stage were excluded. 3. Before coming to the korean medicine hospital, of the 86 patients, 72 (83.72%) patients received surgical therapy, 55 (63.95%) patients received chemotherapy and 44 (51.16%) patients received radiotherapy. 11 (12.79%) patients chose oriental medicine as their primary treatment option. 4. The mean duration from diagnosis to hospital visit was $13.87{\pm}15.53months$. Among the total 86 patients, 46 (53.49%) patients visited for symptom relief during the follow-up period. 5. The most common symptom was general weakness in 29 (33.72%) patients, followed by hot flash, myalgia, insomnia, digestion disorder, numbness, edema, arthralgia, operation site pain, cold sensation and mastalgia. 6. Among 86 patients, 55 (63.95%) patients received acupuncture and moxibustion together. The most prescribed herbal medicine to subjects was Sibjeondaebo-tang-gamibang and Cheonhye-dan (25%). Conclusions: These results could be helpful to provide basic data on the Korean medical approach of breast cancer patients.
