Qualitative Analysis of Medical Usage Patterns of Medical Aid Patients (의료급여환자의 의료이용행태에 관한 질적 분석)

  • This was a qualitative study on medical aid patients to understand the cause and process of statistical difference of health service utilization between medical aid and health insurance patients. The main results were the following; 1) There was few overuse of health service in medical aid patients. The reason of heavy utilization was mainly due to the complicated disease. Some of them were considered to overuse physical therapy and oriental acupuncture. 2) In case of medical aid patients, medical cost was paid by their welfare benefit of government or by the support of family or neighbors. They usually could not adequately use the services of uninsured benefit or large hospitals due to the cost. Some patients just endured the pain. There was still discrimination for medical aid patients in some medical institutions. 3) The health officials and institutions did not provide sufficient information to medical aid patients about the policy of medical cost support. 4) Health policies, such as selective clinic system, medial aid case management, approval of extended care, were considered to contribute in preventing unnecessary use of health service. However, this might limit adequate use of medical aid service. In conclusion, there is little evidence of overuse of health service for medical aid patients, which is different from the previous studies. A new plan is necessary, because medical aid patients thought that the necessary health service was not accessible to them.

Evaluation on the Usefulness of Lung Tumor Stereotactic Radiosurgery Applying the CyberKnife $Synchrony^{TM}$ Respiratory Tracking System (사이버나이프 $Synchrony^{TM}$ 호흡 추적 장치를 이용한 폐종양 방사선수술의 유용성 평가)

  • This study evaluated the motion of tumors during the entire period of therapy and the accuracy of radiosurgery among forty eight lung tumor patients who were underwent radiosurgery using the CyberKnife Synchrony Respiratory Tracking System. The motion of lung tumor was measured by the coordinates of a gold acupuncture needle inserted into the tumor or the area around the tumor using the CyberKnife image guided system. Then the accuracy of radiosurgery was evaluated based on the error of correlation computed with the motion tracking system. The lung tumor motion is Cranio-Caudal direction by an average of $2.63{\pm}1.87\;mm$, moved left-right direction by $1.13{\pm}0.71\;mm$, and anterior-posterior direction by $1.74{\pm}1.16\;mm$. The degree of rotational movement was $1.66{\pm}1.66^{\circ}$ on X axis, $1.20{\pm}0.97^{\circ}$ on Y axis, and $1.18{\pm}0.73^{\circ}$ on Z axis. The vector of translation movement was measured to be $3.78{\pm}2.00\;mm$ on the average. The results show that directions of Cranio-Caudal(p < 0.001), anterior-posterior direction(p < 0.029), and three dimensional vector value(p < 0.002) showed statistical significance, because the lower side of tumor showed more intensive movement compared to the upper side of tumor. The radiosurgery was carried out by compensating the motion of tumor after accurate investigation of the correlation error with the average of $0.95{\pm}0.62\;mm$ during the lung tumor radiosurgery with the CyberKnife Synchrony Respiratory Tracking System.

The Assessment of Risk of Bias on Randomised Controlled Trials of Oriental Medicine in Korea (한의학 관련 무작위배정비교임상연구의 비뚤림 위험 평가)

  • Objectives: To examine the current status of clinical research in oriental medicine, and to assess 'risk of bias'(ROB) of randomized clinical trials(RCTs) in oriental medicine in Korea. Methods: Special committee for EBM, KOMS(Korean Oriental Medicine Society) reviewed 17 journals related to oriental medicine in Korea (from the first issue to May 2010), the Cochrane Central Register of Controlled Trials (The Cochrane Library Issue 2, 2010), and PubMed (1966 to May 2010). Then we selected eligible RCTs in terms of oriental medicine, and assessed 'risk of bias'. Results: We reviewed 12,653 articles from the 17 journals, and 41 articles from CENTRAL and PubMed. After non-clinical articles were excluded, 1,004 articles were left. Among them, the number of eligible studies in terms of oriental medicine was 306. In these eligible studies, 130 were RCTs. Of RCTs, 69 were on acupuncture, 25 on herbal medicine. The proportion of 'unclear' is high in the criteria of 'Allocation concealment', 'Blinding of participants and personnel', 'Blinding of outcome assessment' and 'Other bias'. On the other hand, 'low' has high in the criteria of 'Incomplete outcome data' and 'Selective reporting'. Conclusions: Risk of bias on oriental medicine is unclear in terms of 'allocation concealment' and 'blinding'. For high-quality research in oriental medicine, further research should be needed on randomization and blinding in the RCTs.

Effect of Tinnitus after Bleeding, Acupuncture, Moxa, and Laser treatment (부황, 뜸, 침 레이저요법 시술이 耳鳴에 미치는 영향)

  • Although tinnitus is a common disease, it's reason, pathogenesis and treatment are not examined clearly. As tinnitus is a subjective complaint, it is not easy to evaluate objectively and to compare with another. In this study, we intend to estimate the effect of a variety of oriental medical treatments to tinnitus. We inquired about 30cases of tinnitus patients visited the Kwang-ju Oriental Medical Hospital of Wonkwang University from Apr. 1998 to Sep. 1998. We examined the 30cases in the view of sex, age, occupation, duration, etiologic factors, past history, associating symptoms, quality of tinnitus, and recovery rate after treatments. The results were as follows: 1. In distribution of sex, the ratio of male was $63.3\%$(19cases)and\;female\;was\;36.6\%$(11cases). 2. In distribution of age, the ratio of the 2th decade was $6.6\%(2cases),\;the\;3th\;decade\;was\;20.0\%(6cases),\;the\;4th\;decade\;was\;16.6\%(5cases),\;the\;5th\;decade\;was\;23.3\%(7cases),\;the\;6th\;decade\;was\;30.0\%\;(9cases),and\;the\;7th\;decade\;was\;3.3\%(1case)$. 3. In distribution of occupation, the ratio of employee was $23.3\%$(7cases), house-keeper was $63.3\%(11cases),\;farmer\;was\;16.6\%(5cases),\;teacher\;was\;13.3\%$ (4cases), and student, merchant, soldier was each $3.3\%$(1case). 4. In distribution of duration, the ratio of under 1month was $6.6\%$ (2cases), 1 month -6 months was $20.0\%$(6cases), 6 months- 12months was $30.0\%$(9cases), 12months-36months was $23.3\%$(7cases), and over 36months was $20.0\%$(6cases). 5. In etiologic factor of tinnitus, the ratio of unknown reason was $40.0\%$ (12cases), overwork was $16.6\%$(5cases), emotional stress was $10.0\%$(3cases), noise was $6.6\%$(2cases), cases of laying overwork upon stress was $13.3\%$(4cases), head trauma was $3.3\%$(1case), gun report was $3.3\%$ (1case), and after sickness was $6.6\%$(2cases). 6. 9 cases had experienced such a disease as meniere's disease, tympanitis, labyrinthitis, trauma of tympanum, hypertension, etc. 7. Commonly associated symptoms were dizziness, deafness, uneasiness, headache, ear fullness, fatigue, insomnia, nausea, vomiting, and forgetfulness. 8. Most frequent quality of tinnitus were buzzing, whistling, humming etc. 9. In $76.6\%$(23cases) of tinnitus patients, it was improved, but in $23.3%$(7cases) of them, not improved. Total recovery rate was $42.2\%$.

Current status of allergic rhinitis patients in Korean Medicine hospitals - Exploratory study based on electronic medical records of 3 hospitals (국내 한방병원의 알레르기 비염 환자 현황에 대한 탐색적 연구 - 3개 대학한방병원의 전자의무기록 자료를 중심으로)

  • Objective : To study current status on characteristics of allergic rhinitis patients by using the electronic medical record (EMR) data and to explore the feasibility of clinical studies using the EMR data in three different Korean medical hospitals. Methods : We studied allergic rhinitis patients who visited the department of ophthalmology, otolaryngology and dermatology in the three different Korean hospitals from January 1, 2012 to December 31, 2012. We retrospectively collected medical history and characteristics of study subjects using data of EMR. Results : In hospital A, we were able to collect data of 18 years of age or older. In hospital C, we could only collect data after July 1, 2012. Therefore, each hospital's data had different settings in measuring them. Men and women were accounted for similar percentage, and teens were the highest in the age group. J30.4, unspecified allergic rhinitis, was the highest in diagnosis of the allergic rhinitis. Most of the patients have received acupuncture treatments. Moreover, 74.6 percent of the total patients were prescribed with Chinese medicine. Conclusions : Based on this exploratory study, further studies were needed on clinical studies using data of systematic EMR.

Clinical Analysis of Iatrogenic Pneumothorax (의인성 기흉에 대한 임상 고찰)

  • Background: This study was designed to determine etiologic factors for iatrogenic pneumothorax in an era of in-creased use of invasive procedures and to evaluate its impact on morbidity. Material and Method: Subjects were 112 patients (65 men and 47 women ranging in age from 20 to 90 years) who were diagnosed with an iatrogenic pneumothorax between January 2005 and December 2008. We reviewed medical records retrospectively. Result: The leading causes of iatrogenic pneumothorax were percutaneous needle aspiration (50), central venous catheterization (29), acupuncture (14), thoracentesis (8) and positive pressure ventilation (7). The majority of the patients (60 of 114) were treated with chest tubes. The mean duration of hospital treatment was 5.8 $({\pm}4.0)$ days. Hospitalization was prolonged in 24 patients (21.1%). No patient died from iatrogenic pneumothorax. Conclusion: In our study, the most common cause of Iatrogenic pneumothorax was percutaneous needle aspiration. The mortality and morbidity from iatrogenic pneumothorax is not significant. The recognition of pneumothorax, depends on careful examination after completion of an invasive procedure, and should be followed by prompt and definitive therapy.

Effect of Electroacupuncture at SP-6 with Different Durations on Minimum Alveolar Concentration and the Cardiovascular System under Isoflurane Anesthesia in Dogs (개에서 Isoflurane 마취시 SP-6 혈위의 전침자극시간이 최소폐포농도 및 심맥관계에 미치는 영향)

  • The effects of electroacupuncture (EA) at SP-6 with different durations on the minimum alveolar concentration (MAC) and on the cardiovascular system were evaluated in dogs under isoflurane anesthesia. Eight healthy male beagles were randomly assigned to four study groups (n = 5/group) with washout period of 7 days for recovery and anesthetic withdrawal between experiments. Four study groups were control, nonacupoint electrical stimulation (NA), EA for 30 minutes (SP-6) and continuous EA for 70 or 90 minutes (SP-6C). For the nonacupoint electrical stimulation group, needles were inserted into the nonacupoint at the muscle bellies of left triceps brachii and right quadriceps femoris. MAC and cardiovascular parameters were determined after EA at SP-6 acupoint and at nonacupoint. Thirty minutes of EA and continuous EA until re-determination of MAC at SP-6 acupoint lowered the MAC of isoflurane by 21.3$\pm$8.0% and 16.1$\pm$4.6%, respectively (p<0.05). The decrements in MAC values were not significantly different between two EA groups. However, electrical stimulation of nonacupoint did not induce a significant change in MAC. In SP-6 and SP-6C groups, significant changes in cardiovascular parameters were not observed. These results indicate that EA at SP-6 have an advantage in isoflurane anesthesia in terms of reducing the requirement for anesthetics and minimizing cardiovascular side effects. EA for 30 minutes at maximum might be the sufficient time to produce acupuncture analgesia.

Treatment of Canine Tracheal Collapse by Injection-Acupuncture and Herbal Medicine (기관지허탈 견에서 약침과 한약제를 이용한 치료)

  • A 15 years old castrated male Pug dog was referred with chief complaint of cough, described as a goose honk. He was diagnosed into of tracheal collapse by clinical signs and radiography. He was received by injection- AP with butorphanol (0.15 mg/kg, SID) at BL13, LU01, LU05, LU06, LU07, LU09 and CV22 for 10 days, and he was also received by injection-AP with butorphanol (0.40 mg/kg, SID) at BL13, LU01 and CV22 for 9 days. The patient was given with Sochungryong-Tang (0.5 g/head, TID) for 7 days, and he was additionally given with Sojagangki-Tang (1 ml/kg, TID) for 12 days. Cough was not detected at all, and tracheal diameter was more increased than that of session 1 on radiograph at session 19. Cough was not detected at all and tracheal diameter at follow-up study of three month later was more dilated than that of session 19. In conclusion, the present patient was a case with canine tracheal collapse which showed favorable therapeutic response by injection-AP with butorphanol combined by administration of herbal medicine.

Research Trends of the Traditional Korean Medicine Treatment for Atopic Dermatitis -Based on the Journal of Pediatrics of Korean Medicine- (아토피 피부염의 한의학적 치료에 대한 연구 동향 -대한한방소아과학회지를 중심으로-)

  • Objectives The goal of this review was to investigate clinical, experimental and literature studies of Korean medicine on atopic dermatitis (AD) of Korean children in Korean medicine literature, seeking for the better research methods for more effective treatments. Methods Electronic investigations were practiced on AKOP (The Association of Korean Oriental Pediatrics, http://www.akop.or.kr) to collect theses which were published by J Pediatr Korean Med. The key word 'Atopy' was used for searching to ensure that every related thesis was collected. The publication date was limited from 1996 to 2016. The selected literatures were assessed mainly according to distributions of study type, publication year, scale, treatment and its efficacy. Results 55 papers were selected from 59 studies. In these collected 55 related theses, there were 29 experimental studies (52.73%), 18 clinical studies (32.73%), 8 review studies (14.55%). In the 29 experimental studies, there were 20 studies (68.97%) using NC/Nga mouse as subjects. The mite antigen was used to trigger AD by 8 studies (27.59%). The studies have been utilizing DNCB and DNFB instead of mite antigen since 2011. All the experimental studies showed that the Korean medicine was effective in AD. In total 18 clinical studies, 3 studies (16.67%) were case reports, only 1 thesis (5.56%) was conducted by case control study and the rest were case series studies. 7 studies (38.89%) of 18 studies diagnosed AD according to Hanifin and Rajka's diagnostic criteria (1980), only 1 study (5.56%) according to the Korean standard and the rest 10 studies (55.56%) didn't mention diagnostic criteria. 12 clinical studies (66.67%) showed efficacy in treating AD. There were 10 studies (55.56%) only employing internal treatments and 1 thesis (5.56%) didn't mention the exact prescription. Naesowhajungtang-kamibang, Saenghyeoryunbue-um were used 3 times (16.67%) as the internal medicine respectively. 5 theses (27.78%) were combined with acupuncture therapy. Hegu (LI4), Sanyinjiao (SP6) were used in 4 theses, and Taichong (LR3), Quchi (LI11), Zusanli (ST36) in 3 theses. The number of acupoints chosen from The Spleen meridian of Foot-Taiyin was the largest. There were 8 review theses. 2 were about the foreign oriental medicine, 3 were about the external medicine or external treatment methods, and 3 other studies were about severity scoring systems, the methodological study in the latest clinical study, overview for pattern and results of herbal medicine-derived AD clinical researches. Conclusions The experimental studies and clinical studies showed the effectiveness of Korean medicine treatments. However, this study still needs improving by conducting more comparative studies and using better research methods, in order to find more effective treatments to improve clinical efficacy.

  • The subjects of this clinical study are 17 patients with ALS(Amyotrophic Lateral Sclerosis), who were diagnosed in other or our hospital from January 1987 to Nomember 1997. The age of onset, clinical signs, type of ALS, methods of treatment and outcomes, etc. were studied and analyzed. The data are summarized as follows ; 1) The mean age was $52.4{\pm}11.5$ years old, and the ratio of male to female was 2.4:1. There is increasing frequency with rising age. 2) Pseudopolyneuritic type(67%) is the most in male patients, bulbar type(60%) is the most in female patients, and totaly Pseudopolyneuritic type(54%) is the most. 3) In the period of hospitalization after onset, 1 year(11 cases, 64.8%) is the most frequent, and 2 year(2 cases, 11.7%), 1 month(2 cases, 11.7%), 4 years(1 case, 5.9%) and 6 months(1 case, 5.9%) in orders. 4) In the duration of admission, 10 days(8 cases, 47.2%) is the most frequent, and 30 days(4 cases 23.5%), 2 months(2 cases, 11.7%), 20 days(2 cases, 11.7%) and 3 months(1 case, 5.9%) in orders. 5) In the signs of patients, muscle weakness(17 cases, 100%), bulbar signs(14 cases, 82.4%), increased reflex of deep tendon(10 case, 58.8%), muscle atrophy (7 cases, 41.2%) and fasciculations(7 cases, 41.2%) were in orders. 6) In the methods of treatment, herb-medication(17 cases, 100%), acupuncture therapy(16 cases, 94.1%), physical therapy(7 cases, 41.2%), moxibustion therapy(5 cases, 29.4%), cupping therapy(4 case, 23.5%) and moxa-pack(1 case 5.9%) were administered in orders. 7) As to the outcome of treatment, 8(47.1%) were no changed, 4(23.5%) improved, 4(23.5%) aggravated, 1(5.9%) expired in orders.
