The combination of the traditional and the modern medicine in North Korea (북한의 동.서의 결합)

  • According to the North Korean documents , North Korean government have emphasized the development of the Koryo medicine (traditional korean medicine) and encouraged the combination of the Koryo and the modern medicine. It is regarded that this attitude arose from the ruler's will toward independency in the medical field and at the same time the lack of modern medical resources . Combinations of the Koryo medicine and the modern medicine in the clinical textbooks are classified as follows: first, mixed medication of the Koryo and the modern medicine. Second, combination of the traditional acupuncture(or moxibustion) and the medication of modern medicines. Third, injection of modern medicines at acupoints. Forth, an intramuscular or an intravenous injection of the Koryo medicine. Fifth, anesthesia using Koryo medicine. Sixth, simple surgery at the acupoints. Lastly, combination of injection and the traditional therapy(eg. acupuncture, moxibustion and cupping) Despite of many achievements in the field of combined medical treatment, recent economic failure and severe famine for several years caused collapse of the health care delivery system in North Korea, and it is hard to find combined practices actually.

Clinical Trial Study about Preventing Winter Diseases through Summer Prophylactic Treatment Using Acupoint Sticking (삼복첩을 이용한 동병하치법에 관한 임상연구고찰)

  • Objectives The purpose of this study is to analyzing clinical trials of the current acupoint sticking treatment on Sambok* to prevent winter diseases through summer prophylactic treatment. Methods 33 Chinese clinical studies published in last fifteen years were selected for analysis. Results Acupoint sticking treatment on Sambok* is used in asthma, rhinitis, bronchitis. Commonly used herbs are Sinapis Semen(白芥子), Asari Herba Cum Radix(細辛), Euphorbiae Kansui Radix(甘遂), Corydalis Tuber(玄胡索), Moschi Moschus(麝香), Ephedrae Herba(麻黃), Cinnamomi Cortex Spissus(肉桂) and Commonly used acupoints are Feishu(肺兪), Kohwang(膏肓), Shinsu(腎兪), Pisu(脾兪). 33 studies showed that the effect of treatment in different age, symptom, acupoints, herbs, methods. However, it shows that acupoint sticking treatment on Sambok* is effective in various diseases, especially in respiratory disease. Conclusions This study shows that acupoint sticking treatment on Sambok* is effective in preventing many diseases. More clinical studies about acupoint sticking treatment and standardizations to evaluate effectiveness are needed to prove its efficacy clearly.

The Trends in Foreign Clinical Trials for Pediatric Asthma in Complementary and Alternative Medicine (소아 천식 치료의 국외 보완대체의학 임상 연구 동향)

  • Objectives : The purpose of this study is to analyze the trends in foreign clinical trials for pediatric asthma in complementary and alternative medicine (CAM). Methods : We reviewed 31 randomized controlled trials (RCT) for pediatric asthma treatment in CAM published from 2005 to 2015 in PubMed and analyzed their treatments, compounds of herbal medicines, acupoints, methods of assessment, and treatment results. Results : The treatments used in the studies were herbal medicines, acupuncture, acupoint application, breathing training, supplement, normocaloric diet, yoga, massage therapy, and art therapy. The frequently used herbs were Asari Herba Cum Radix (細辛). Ephedrae Herba (麻黃), Astragali Radix. The most common acupoints were Feishu (肺兪, BL13). The methods of assessment in pediatric asthma were symptom scale, questionnaire of Quality of Life, lung function test, laboratory studies, and medication usage. In 28 out of 31 studies, treatment group gets better than non-treated group. Conclusions : This study shows that CAM is effective in treating pediatric asthma. More clinical studies using objective outcome measures are needed to prove its efficacy clearly.

Studies on Electroacupuncture Anesthesia of Experimental Animal (실험동물의 전침마취에 관한 연구)

  • To establish the electroacupuncture anesthesia for experimental animal, 5 adult rabbits (Newzealand-white species) and 6 rats (Spraque-dawley) were examined. The acupoints used for electroacupuncture anesthesia were Tian-ping and Bai-hui. After perpendicular insertion of needle to the acupoints, positive electrode was connected at Tian-ping and negative electrode was connected at Bai-hui using veterinary electroacupuncture anesthesia apparatus, respectively. Electric conditions were 2~3 V and 30 Hz in rabbits, and 1.5~2 V and 30 Hz in rats, respectively. To examine the effect of electroacupuncture anesthesia, laparotomy (2 heads), castration (2 heads) and ovariohysterectomy (1 head) were applied in rabbits, and laparotomy (5 heads) and castration (1 head) were peformed in rats, respectively. The induction time of electroacupuncture anesthesia was very rapid and approximately 1 minute, and pain of body surface including the extremities was not detected in rabbits and rats. As for the reactions with electroacupuncture anesthesia, consciousness was vivid, blepharoreaction was not observed and the tail was up at early stage but was down afterwards in rabbits. The same findings except cotinuous lifting of the tail were observed in rats. The pain was not observed during incision and suture, bleeding was comparatively small volume and the class of anesthesia effect was excellent.

Study on the Application of DITI in Bell's Palsy Patients with Acupoints (Bell's Palsy 환자에서 두면부 경혈의 적외선 체열촬영의 활용)

  • Objectives : The aims of this study are to find out proper methods to assess Bell's palsy with DITI and to validate the correlation between DITI of acupoints and the H-B scale. Methods : 36 Bell's palsy patients and 10 healthy people were measured. We checked thermal differences between abnormal and normal acupoint sites measured by using DITI and the H-B scale. Results : There was no significant thermal difference between abnormal and normal sites. The H-B scale was significantly correlated with thermal differences between $ST_6$ and $TE_{17}$. Conclusions : DITI was useful for evaluating the severity of Bell's palsy.

The Studies on Treatment of Thoracic Disease Using Lasers and Acupuncture in Dogs (소동물에서 Lasers 및 침술을 이용한 흉부질환 치료에 관한 연구)

  • This study was performed to investigate the effect of acupuncture in dogs with experimentally induced thoracic disease. Twenty four dogs and 16 cats for the heart study, were divided into four groups(each of 6 in dogs, each of 4 in cats): laserpuncture, electroacupuncture, acupuncture and control group. Heart disease was induced by partial pericardiectomy, and then treatment was done once daily for 4 days. The acupoints used were Xin-shu and Nei-guan. The effect of acupuncture was evaluated by clinical symptom, blood chemical values, electrocardiogram(ECG) and histopathological findings. In dogs, acupuncture group revealed relatively fast recovery and laserpuncture group revealed secondly fast recovery after treatment. In catsl lasewuncture group revealed relatively fast recover and acnpuncture group reveaBed secondly fast recovery after treatment Sixteen dogs for the lung study, were divided into flour groups(each of 4) : laserpuncture, electroacupunchuf, acupuncture and control group. Lung disease was induced by partial lung lobectomy, and then treatment was done once daily for 4 days. The acupoints used were Fei-shu and Shen- zhu. The effect of acupuncture was evaluated by clinical symptoms blood chemical values and histopathological findings. Acupuncture and laserpuncture group revealed relatively fast recovery compared with other groups.

Studies on canine Electroacupuncture Anesthesia 2. Investigation on the Effect of Dorsal Acupoints (개의 전침마취에 관한 연구 2. 배부혈위의 효과에 대한 검토)

  • To elucidate the effect of dorsal acupoints for canine electroacupuncture anesthesia, mongrel dogs(6 heads: 2.6-10.5kg, 3 males and 3 females) were used. After needling perpendicularly to Tian-Ping(GV-5) and Bai-Hui(GV-20), the electrodes were connected with positive to Tian-Ping and negative to Bi-hui, respectively using with veterinary electroacupuncture apparatus(TEC AM-3000). The electric conditions were 2-4V and 30 Hz. The pain of body surface was checked by hemostatic forceps. The effect of electroacupuncture andesthesia was appreciated by application of laparotomy(2 heads), ear trimming and tail amputation(1 head), castration(2 heads) and enterotomy(1 head), respectively. Induction time, clinical findings and operation findings were investigated under the electroacupuncture anesthesia. The induction time of electroacupuncture anesthesia was very rapid, approximately 1 minute and the pain of body surface including the extremities was not detected. As for clinical findings during electroacupuncture anesthesia, consciousness was evident, pupil reflex was weak and blepharoreaction was existed. As for operation findings, pain was not detected during incision and suture, and bleeding was relatively small volume. The grade of electroacupuncture anesthesia was excellent in all cases applied with ear trimming and tail amputation, laparatomy, castration and enterotomy. Considering above all findings, it was thought that electroacupuncture stimulation at Tian-Ping and Bai-hui examined in the present study induced excellent electroacupuncture anesthesia for dogs.

A Improving Case Report of Idiopathic Trigeminal Neuralgia by Body-form Correction (특발성 삼차신경통의 체형교정에 따른 호전 사례보고)

  • Objectives : To understand spatial cause hypothesis of idiopathic trigeminal neuralgia, the body truncus area was divided into intracranial, thoracic and pelvic cavity and was illustrated, the subjective pain degrees of idiopathic trigeminal neuralgia were compared with the illustraion of three cavity. Methods : The frontal view of pictures of idiopathic trigeminal neuralgia subject truncus area were illustrated into three circles which was treated with conservative methods like the pelvic correction, cervical correction, FCST, posture training without drugs and acupuncture. The spatial analysis of three circle vertical centerlines were compared with the pain degrees. Results : The vertical centerlines of three circles were agreed with the body gravity centerline depending on the treatment progresses. namely, as the parts of truncus were matched to the body gravity centerline, the degrees of pain were decreased. Conclusions : The vascular pressure on the trigeminal nerve which was causing the idiopathic trigeminal neuralgia might be induced by the spatial misalignment of truncus area because the spatial misalignment of it can press to move vascular into trigeminal nerve partially. Further study will be progressed.

Measure of the Associations of Accupoints and Pathologies Documented in the Classical Acupuncture Literature (고의서에 나타난 경혈과 병증의 연관성 측정 및 시각화 - 침구자생경 분석 예를 중심으로 -)

  • Objectives : This study aims to analyze the co-occurrence of pathological symptoms and corresponding acupoints as documented by the comprehensive acupuncture and moxibustion records in the classical texts of Far East traditional medicine as an aid to a more efficient understanding of the tacit treatment principles of ancient physicians. Methods : The Classic of Nourishing Life with Acupuncture and Moxibustion(Zhenjiu Zisheng Jing; hereinafter ZZJ) was selected as the primary reference book for the analysis. The pathology-acupoint co-occurrence analysis was performed by applying 4 values of vector space measures(weighted Euclidean distance, Euclidean distance, $Cram\acute{e}r^{\prime}s$ V and Canberra distance), which measure the distance between the observed and expected co-occurrence counts, and 3 values of probabilistic measures(association strength, Fisher's exact test and Jaccard similarity), which measure the probability of observed co-occurrences. Results : The treatment records contained in ZZJ were preprocessed, which yielded 4162 pathology-acupoint sets. Co-occurrence was performed applying 7 different analysis variables, followed by a prediction simulation. The prediction simulation results revealed the Weighted Euclidean distance had the highest prediction rate with 24.32%, followed by Canberra distance(23.14%) and association strength(21.29%). Conclusions : The weighted Euclidean distance among the vector space measures and the association strength among the probabilistic measures were verified to be the most efficient analysis methods in analyzing the correlation between acupoints and pathologies found in the classical medical texts.

The Review of the Blood-Letting Therapy in the Zhenjiudacheng (침구대성에 나타난 자락요법에 대한 고찰)

  • Objectives : This study was designed to establish the basis for expanding the indications for Blood-letting therapy by reviewing the contents recorded in the Zhenjiudacheng. Methods : Articles associated with the Blood-letting therapy were categorized according to the factors such as the characteristics of the treatment site, the used instruments, the caution, the side effects, the diseases, the acupoints, etc. Results : 1. The characteristics of treatment site were the red, purple, purplish red, purplish black color and the localized collection of fibrous exudate. 2. There were several instruments used for Blood-letting therapy; Samreongchim, Sochim, Loguan, Stone needle, Jukyeop, etc. 3. Several cautions and side effects of the Blood-letting therapy were recorded in the Zhenjiudacheng. 4. The Blood-letting therapy could be performed for a broad spectrum of symptoms such as headaches, backaches, schizophrenia, hyperemia, etc. 5. Not only acupoints but also extra points were used for blood-Letting therapy. 6. In addition to the usual Blood-letting region, Blood-letting therapy was performed directly on the pathologic site and pathologic reaction site. Conclusions : In the Zhenjiudacheng, Blood-letting therapy was described in a concrete and substantive manner in order to enable practical clinical application.
