A Case Study on the Alleviation of Symptom of a Myasthenia Gravis Patient after Korean Medicine Treatment (중증근무력증이 한의학적 치료로 개선된 임상 1례)

  • Objective: The purpose of this study was to report on the alleviation of symptoms of a myasthenia gravis (MG) patient after Korean medicine treatment. Methods: A 39 year-old male patient who suffered from MG was examined. The patient was treated with herbal medicine, acupuncture, and pharmacopuncture. The Myasthenia Gravis Composite (MGC) scale and Myasthenia Gravis Activities of Daily Living (MG-ADL) profile were used to assess the change of MG symptoms after Korean medicine treatment. Results: The MGC and MG-ADL scores indicated significant improvement after 14 days of treatment. In addition, the degrees of fatigue, dyspepsia, vomiting, sweating, and dizziness were decreased. Conclusion: This study may suggest that Korean medicine treatment could be effective in treating the symptoms of MG.

Influencing Factors that Affect the Psychological Well-being in Family Caregivers of Stroke Patients (뇌졸중 환자를 돌보는 가족의 심리적 안녕감에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to investigate the factors that affect the psychological well-being in family caregivers of stroke patients. Method: The General Health Perception, short form 36, Health Survey Questionaire was used to measure health perception. The Caregiving Mastery Scale was used to assess the mastery, while the Psychological General Well-Being Index was used to examine the level of well-being. Result: Subjective health, caregiving mastery, patient's ADL and caregiving duration influenced on caregiver's psychological well-being. Subjective health had effect on psychological well-being both directly and indirectly. Caregiving duration and patient's ADL had indirect effect on psychological well-being through caregiving mastery. Conclusion: It is need to develop a health program for the caregivers of stroke patient's and to provide nursing intervention to improve the caregiver's ability, thereby improving the well-being of the family caregivers.

The Relationship among Life Habits, Health Status and Sleep Time of the Elderly (노인의 생활습관 및 건강상태와 수면시간의 관계)

  • This study was a descriptive study to investigate factors affecting sleep time in the Korean elders. A secondary data analysis was conducted using the data of "The Fifth Korea National Health and Nutrition Examination Survey, 2011". The survey included 3,487 people aged over 65 years and 1,163 samples were used for this study. Data were analyzed with the ${\chi}^2$-test and logistic regression using the SPSS/WIN 18.0 program. Analysis showed that sleep time was related with the number of chronic disease and limit on the activities of daily living(ADL). These findings suggest that nursing researchers need to consider chronic disease, and ADL as important factors affecting sleep time of Korean elders. Therefore nursing intervention focusing on the issues in needed to develope.

A Study on the Relationship between Hypertension and Depression among the Elderly in Urban Areas (일 도시지역 노인의 고혈압과 우울 간의 관계)

  • Purpose: This study was designed to identify the correlation among perceived health status, health behavior, activities of daily living and depression among the elderly with hypertension. Methods: The subjects consisted of 238 person aged over 65 with hypertension who live in house located in BukGu Daegu city. The Data was collected through interview with questionnaire from July, to August 10, 2010. Results: There was a significant relation between perceived health status & ADL (r=.328, p=.000) and between depression & ADL (r=-.293, p=.000). There was s significant relation between depression & perceived health status (r=-.199, p=.002). The general characteristics which significantly affected depression was sex (t=2.49, p=.013), age (F=5.882, p=.001) education (F=6.550, p=.000), occupation (t=-6.990, p=.000). Conclusion: The findings of this study provide useful information for constructing an intervention program and care for the elderly with hypertension.

A Study on the Level of ADL in Community Dwelling Elderly Registered in a Public Health Center (일개 시 보건소에 등록된 재가노인의 일상생활 동작수행 정도에 대한 조사연구)

  • Purpose: The purpose of this study was to assess the level of activities of daily living (ADL) in a group of community dwelling elderly. Methods: The subjects were 100 low-income elderly registered in a public health center. The data were analyzed using the SPSS/PC program. Results: The results were as follows: 1. The basic ADLs of the subjects were scored at 7 levels, and the mean score was 6.24 out of 7 points. The subjects did not necessarily require help from others, but might have needed more times or instrumental assists to perform ADLs. The lowest ADL score was reported on walking up to the stairs (5.05), for which the subjects did not need physical assistance, but needed a certain degree of supervision. The highest score was reported on self eating (6.74), followed by bowel management (6.60). 2. The mean IADL score of the subjects was 1.77 out of 4 points. indicating that the subjects were very independent in performing IADLs, without help from others. 3. In relation to general characteristics, the level of ADLs of the subjects was significantly different by age (F=6.65, P=.000), main activities (F=6.36, P=.001), perceived health status (P=4.66, P=.012), educational background (F=4.64, P=.03), marital status(F=4.62, P=.005), and major household income (F=4.15, P=.002). 4. The subjects' level of IADLs in relation to their general characteristics was significantly different by perceived health status (F=8.08, P=.001), major activities (F=7.33, P=.000), age (F=6.18, P=.000), family context (F=3.53, P=.018), religion (F=3.10, P=.019), marital status(F=2.62, P=.055), and major household income (F=2.31, P=.049).

Related factors of oral health-related quality of life in the severely disabled people (중증 장애인의 구강건강 관련 삶의 질 관련요인)

  • Objectives: The purpose of the study is to investigate the related factors of oral health-related quality of life in the severely disabled people. Methods: The subjects were 205 severe disabled people in Busan and Gyeongnam. Data were collected by direct interview with the severe disabled persons from June to August, 2011. The study instruments included oral health impact profile(OHIP)-14 and Korean instrumental activities of daily living(K-IADL). Data were analyzed by SAS version 9.2. The questionnaire consisted of eight questions of the general characteristics of the subjects, five questions of oral health related problem, seven questions of K-ADL, ten questions of K-IADL, fourteen questions of oral health related quality of life. Results: The severely disabled's IADL was $19.9{\pm}7.8$ and the oral health-related quality of life was $17.5{\pm}10.5$. In multiple regression analysis, oral health-related quality of life was closely related to multiple disorders and IADL. Conclusions: The oral health-related quality of life was poor in the severely disabled people. It is necessary to provide the severely disabled people with self-supporting tools that help ADL and IADL.

Prediction Model of Quality of Life in Elderly Based on ICF Model (ICF 모델에 근거한 노인의 삶의 질 예측 모형)

  • Purpose: The purpose of this study was to identify from the International Classification of Functioning model, factors influencing quality of life in elderly persons and to describe the concrete pathway of influence and the power of each variable. Methods: The sample included 334 elders who lived in 5 districts of D Metropolitan City. A structured questionnaire was used and the collected data were analyzed for fitness, using the AMOS 18.0 program. Results: This model was concise and extensive in predicting the quality of life of elders. The research verified the factors influencing quality of life for elders as direct factors such as activity of daily living (ADL) (${\beta}$=.13, t=2.47), leisure activity (${\beta}$=.55, t=5.04), social disengagement (${\beta}$=-.25, t=-2.25), and depression (${\beta}$=-.62, t=-10.86). Indirect factors including economic status (${\gamma}$=.17, p=.009), type of residence (${\gamma}$=.19, p=.004), ADL (${\gamma}$=.12, p=.027) were important factors in predicting quality of life for elders. These variables explained 75.6% of variance in the prediction model. Conclusion: The findings indicate a need for the nursing scientific community to develop intervention programs considering these variables to improve the quality of life for elders.

Effects of a Muscle Strengthening Exercise Program on Muscle Strength, Activities of Daily Living, Health Perception, and Depression in Post-stroke Elders (근력강화운동 프로그램이 만성 뇌졸중 노인의 근력, 일상생활동작, 건강지각 및 우울에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects of an exercise program on muscle strength, Activities of Daily Living (ADL), health perception, and depression among post-stroke elders. Methods: Nonequivalent control group design with pretest and posttest was used. A total of 23 post-stroke elders who met inclusion criteria were recruited. Muscle strength in upper and lower extremities, ADL, health perception, and self-rated symptoms of depression were measured. The exercise program consisted of deep breathing, range of motion, and muscle strengthening exercises using Thera-band, 30~40 min/day, three days a week, for 12 weeks. Results: Muscle strength of right shoulder (U=11.50, p=.001), right leg (U=13.50, p=.002), and health perception (U=24.00, p=.010) in the experimental group was significantly higher compared to the control group at 12 weeks of post-intervention. Depression scores were significantly lower in the experimental group compared to the control group at 6 weeks (U=28.00, p=.021), at 12 weeks (U=22.00, p=.006) of an exercise program. Conclusion: By applying this program, post-stroke elders showed increases in muscle strength and a decrease in depression as well as improvement of health perception. Further study is needed to verify the effects of the exercise program on cardiovascular physiologic variables through long-term follow-up.

A Relationship of Care Time with Functional Status and Patients Characteristics among Patients in Long-term Care Hospitals (장기요양환자에서 환자 특징 및 기능상태와 환자돌봄 시간과의 관련성)

  • Objectives : The aim of this study was to investigate the functional status variables related to the care time of health professionals for patients in long-term care facilities. Methods : The functional stati of 1001 patients in 8 long-term care hospitals were examined by the Resident Assessment Instrument for Long-term Care Facility Version 2.0. The care time of health professionals for patients was calculated using data from a self-reported task survey by nurses, auxiliary nurses, private aides, doctors, physiotherapists and social workers. Results : The average care time per diem was 240.6 minutes. The care time by doctors, nurses and private aides were 11.0, 71.0 and 139.5 minutes, respectively. The lower the function of activities of daily living (ADL) and the greater the symptoms of extensive services, special care and clinical complexity, the more care time was served. On the contrary, the greater the symptoms of nursing rehabilitation, depression, cognitive disorder, behavior problem and psychiatry/mood disorder, the less care time was served. Age and gender were not significantly related to the care time. Conclusions : Developing a case mix classification system for elderly long term care patients may be helpful for both of patients and health care providers. The ADL, extensive services, special care and clinical complexity of variables should be considered in the development of a case mix system for the long term care of patients in Korea.

Automatic ADL Classification Using 3 Axial Accelerometers and RFID Sensor (3차원 가속 센서 및 RFID 센서를 이용한 ADL 자동 분류)

  • We propose a new method for recognizing the activities of daily living(ADL) based on the state-dependent motion analysis using 3-axial accelerometers and a glove type RFID reader. Two accelerometers are used for the classification of 5 body states based on the decision tree. Classification of the instrumental activities is performed based on the hand interaction with an object ID using an accelerometer and a RFID reader. Object-dependent hand movements are classified into 5 categories in advance and final decision combines the body state and the instrumental activities. Experiment shows that the suggested hierarchical motion analysis provides accuracy rate of over 90% for all 20 ADLs.
