Splenic Artery Bleeding into the Extraperitoneal Space Mimicking Mesenteric Injury: A Rare Case of Blunt Trauma

  • Splenic injury is a common result of blunt trauma, and bleeding occurs mainly inside the splenic capsule and may leak into the peritoneal space. Herein, we report a case where active bleeding occurred in the splenic artery and only leaked into the extraperitoneal space. This is the first case of this phenomenon in a trauma patient in the English-language literature. Bleeding passed through the peritoneum, leaked into the anterior pararenal space, and continued along the extraperitoneal space to the prevesical space of the pelvis. Therefore, on the initial computed tomography (CT) scan, the bleeding appeared to be in the left paracolic gutter, so we suspected mesenteric bleeding. However, after the CT series was fully reconstructed, we accurately read the scans and confirmed splenic injury with active bleeding. If there had been a suspicion of bowel or mesenteric injury, surgery would have been required, but fortunately surgery could be avoided in this case. The patient was successfully treated with angioembolization.

  • A Morel-Lavallée lesion is a closed degloving injury caused by traumatic separation of the skin and subcutaneous tissue from the underlying fascia. However, since physicians tend to focus on treating the bone fracture, hemodynamic instability accompanying a Morel-Lavallée lesion can sometimes be overlooked. We report the case of a hemodynamically unstable 73-year-old man who had a Morel-Lavallée lesion of the thigh, but no femur fracture. Angiography showed active bleeding from the muscular branch of the right deep femoral artery, which was then successfully embolized.

  • This is a rare case of a 73-year-old male patient who presented with hematochezia and was treated using transcatheter arterial embolization following upper gastrointestinal bleeding in the third portion of the duodenum. The cause of the bleeding was not found on gastrointestinal endoscopy and CT. On the third day of hospitalization, the hemoglobin level continued to decrease. A technetium-99m-labeled red blood cell scan revealed suspicious bleeding in the diverticulum of the third portion of the duodenum. Superior mesenteric artery angiography showed active bleeding from the posteroinferior pancreaticoduodenal artery, which was embolized with N-butyl cyanoacrylate. The patient was discharged on the seventh day after embolization without re-bleeding or complication. We report a rare case of a patient with active bleeding from a duodenal diverticulum that was difficult to diagnose using routine modalities.   • Objectives : This study was performed to evaluate the effects of thoracic vertebral region's Du mai(督脈) and Jia ji xue(夾脊穴) intervention on active range of motion(ROM) restricted shoulder pain. Methods : 1. Measurement : The unpleasantness of pain was measured by visual analogue scale(VAS), and ROM was measured by using Goniometer and scratch test. 2. Intervention : Use Acupuncture and bee venom injection in thoracic vertebral region's Du mai(督脈) and Jia ji xue(夾脊穴) that display main tender points. Results : The pain and ROM were improved after thoracic vertebral region intervention. Conclusion : Thoracic vertebral region intervention was effective to active ROM restricted shoulder pain, to reduce the pain and to improve ROM. And this study shows that active ROM restricted shoulder pain and thoracic vertebral region's tender points are connected to each other. Also, region that suggest main tender points previews region of thoracic vertebrae 5-8.

  • Uninterrupted power supply has become indispensable during the maintenance task of active electric power lines as a result of today's highly information-oriented society and increasing demand of electric utilities. The maintenance task has the risk of electric shock and the danger of falling from high place. Therefore it is necessary to realize an autonomous robot system using electro-hydraulic manipulator because hydraulic manipulators have the advantage of electric insulation. Meanwhile it is relatively difficult to realize autonomous assembly tasks particularly in the case of manipulating flexible objects such as electric lines. In this report, a discrete event control system is introduced for automatic assembly task of electric lines into sleeves as one of the typical task of active electric power lines. In the implementation of a discrete event control system, LVQNN (linear vector quantization neural network) is applied to the insertion task of electric lines to sleeves. In order to apply these proposed control system to the unknown environment, virtual learning data for LVQNN is generated by fuzzy inference. By the experimental results of two types of electric lines and sleeves, these proposed discrete event control and neural network learning algorithm are confirmed very effective to the insertion tasks of electric lines to sleeves as a typical task of active electric power maintenance tasks.

Brassinosteroid substances in immature Perilla frutescense seeds (들깨의 brassinosteroid 활성물질)

  • In order to explore the brassinosteroid-active component in Perilla frutescense, methanol extract of immature seeds was purified by sequences of solvent fractionation, silica gel adsorption chromatography, Sephadex LH-20 chromatography, charcoal adsorption chromatography and Bondesil chromatography. The activity of brassinosteroid was monitored by the rice inclination test and its presence could be confirmed in each purification step. The purified active components were seperated by silica gel adsorption chromatography. The seperated main and minor active brassinosteroid fractions were identified as castasterone and homobrassinolide, respectively, by HPLC. We acknowledge that our work is probably the first report of endogenous brassinosteroid in Perilla frutescense. The content of brassinosteroid in Perilla frutescense as converted into brassinolide was $0.5{\sim}0.8\;ng/g$ fresh weight.

The Middle Age Women's Image Evaluation of Tailored Jacket according to the Body Type and the Degree of Satisfaction with the Body - Focused on Silhouette - (중년여성의 체형과 신체 만족도에 따른 테일러드 재킷의 이미지 평가 - 실루엣을 중심으로 -)

  • The purpose of this study is to analyze the image evaluation of tailored jacket according to the body type and the degree of satisfaction with the body of middle-aged women. The subjects used for this study were three hundred and twenty three middle-aged women ranging from 35 to 50 years old. We measured their bodies and asked them report to the questionnaire. The results are as follows. There was a significant difference in the image evaluation of tailored jacket according to the body type of middle-aged women. The women of the type I evaluated that X silhouette was the most fascinating, elegant, active, and tender, and the women of the type II, A silhouette. The women of the type III evaluated that H silhouette was the most fascinating, elegant, active, and tender, and the women of the type IV, H silhouette. There was a meaningful difference in the image evaluation of tailored jacket according to the degree of satisfaction with their bodies. As satisfied with the girth of the body, weight, and figure they evaluated X silhouette was more attractive, graceful, active, and soft.

  • In this paper, we introduce the 3D modeling of the coronal magnetic field in the solar active region by extrapolating from the 2D observational data numerically. First, we introduce a nonlinear force-free field (NLFFF) extrapolation code based on the MHD-like relaxation method implementing the cleaning a numerical error for Div B proposed by Dedner et al. 2002 and the multi-grid method. We are able to reconstruct the ideal force-free field, which was introduced by Low & Lou (1990), in high accuracy and achieve the faster speed in the high-resolution calculation (512^3 grids). Next we applied our NLFFF extrapolation to the solar active region NOAA 10930. First of all, we compare the 3D NLFFF with the flare ribbons of Ca II images observed by the Solar Optical Telescope (SOT) aboard on the Hinode. As a result, it was found that the location of the two foot-points of the magnetic field lines well correspond to the flare ribbon. The result indicates that the NLFFF well capture the 3D structure of magnetic field in the flaring region. We further report the stability of the magnetic field by estimating the twist value of the field line and finally suggest the flare onset mechanism.

A Study on the Relationship among Perceived Stress, Vulnerability Variables, and Somatization Tendency in University Entrants (대학교 신입생들에서 지각된 스트레스 및 취약성 변인과 신체화 경향의 관계에 관한 연구)

  • For the purpose of examining the relationship between perceived stress, vulnerability variables, and somatization tendency, the self-report questionnaires of perceived stress, styles of stress coping(passive and active copings), self-perception, gender, and somatization tendency were administered to university entrants(n=2,024). The results were as follows: 1) Perceived stress, styles of stress coping(passive and active copings), self-perception, and gender accounted for 15.56% of the total variance in somatization tendency. As a result of comparing the relative contributions of all predictor variables to somatization tendency, the highest was perceived stress, and the next in order were passive coping style, self-perception, and gender, whereas direct effect of active coping style was not significant. 2) The two-way and three-way interaction effects of perceived stress X vulnerability variables were not significant. 3) The two-way and three-way interaction effects of gender X psychosocial variables were not significant. To conclude, perceived stress and vulnerability variables independently contribute to somatization tendency in university entrants, and furthermore it is suggested that vulnerability variables as well as perceived stress must be considered to account for somatization tendency.

Purification, Characterization and Application of a Cold Active Lipase from Marine Bacillus cereus HSS

  • Lipases (triacylglycerol acylhydrolases [EC 3.1.1.3]) are water-soluble enzymes. They catalyze the hydrolysis of fats and oils. A cold-active lipase from marine Bacillus cereus HSS, isolated from the Mediterranean Sea, Alexandria, Egypt, was purified and characterized. The total purification depending on lipase activity was 438.9 fold purification recording 632 U/mg protein. The molecular weight of the purified lipase was estimated to be 65 kDa using sodium dodecyl sulfate (SDS)-polyacrylamide gel electrophoresis. The optimum substrate concentration, enzyme concentration, pH, and temperature were 1.5 mM, 100 µl, pH 6 and 10℃, respectively. The lipase was tolerant to NaCl concentrations ranging from 1.5 to 4.5%. The lipase was affected by the tested metal ions, and its activity was inhibited by 16% in the presence of 0.05 M SDS. The application of the cold-active lipase for the removal of an oil stain from a white cotton cloth showed that it is a promising biological agent for the treatment of oily wastes and other related applications. To the best of our knowledge, this is the first report of the purification and characterization of a lipase from marine B. cereus HSS isolated from the Mediterranean Sea.
