A study on the current status and development of the new health technology assessment of Korean medicine field (한의 신의료기술 평가 활성화 방안 제언)

  • Objectives: The purpose of this study is to examine the current status of Korean medicine health technology assessment and explore realistic plans to activate it. Methods: We investigated all the applications for new health technology assessment related to Korean medicine from 2007 to 2016. The several expert meetings were held to draw out the barriers and improvement strategies of the new health technology assessment of Korean medicine field. Results: There were 31 cases in total except for duplications or reapplies falling into 3 main types. First, 19 of them were to try to enter a medical market and be covered by National Health Insurance. Eight cases were to apply western medicine technology as new health technology in Korean medicine area. The rest was 4 cases, which were totally not appropriate for the purpose of new health technology assessment system. According to the expert opinion, the obstacles of activation in new health technology assessment of Korean medicine were application of unstandardized technology, lack of understanding and experience, lack of clinical trial supporting system for Korean medicine, lack of committee members within the nHTA(new Health Technology Assessment) review board, ambiguous definition of medical practice and sharp conflict between western medicine and Korean medicine. Conclusions: Several suggestions were derived. First of all, to activate Korean medicine in the nHTA system, the existing system should be used sufficiently, and multifaceted efforts are needed to upgrade the system, if necessary. Also, self-help efforts, Korean medicine clinical trial supporting system and increasing R&D investment, establishing extra-committee for Korean medicine in nHTA could be needed. Finally, long-term strategy for improving collaboration between Korean medicine and western medicine should be considered.

Determination of trace boron in steels by prompt gamma-ray activation analysis (즉발감마선방사화분석법에 의한 철강시료 중의 붕소 측정)

  • A trace amount of boron in steel significantly influences its mechanical and physical properties. A prompt gamma ray activation analysis (PGAA) method is used to measure boron in low alloy steel samples of KRISS 101-01-C21~C26. NIST SRMs of 362, 364, 1761 and 1767 serve as the control standards to validate the measurement method. The measured values of the NIST SRMs are consistent with their certified values within the expected uncertainties, except for that of NIST SRM 362. Experimental uncertainties are evaluated according to the guidelines given by the International Organization for Standardization (ISO). The expanded uncertainties are calculated with a coverage factor of 2, at approximately 95% confidence level. The calculated relative expanded uncertainties of boron mass fractions are between 3% and 7% at the mg/kg level. The results are compared with the results measured by the solvent extraction-inductively coupled optical emission spectrometry (ICP/OES) method.

  • The purpose of this study is to design of prototype for developing webpage of fisheries and marine education especially in the field of Busan area. For this, we extract the guideline for development from literature review, need and task analysis, and advices of subject domain experts. Followings were the results of the research: (a) We make the prototype for main menu and screen based on institute, time, subject, and location. (b) We make the prototype for program approach guide, which is consisted of location based menu constructing prototype guide, activation prototype guide, and selection prototype guide. (c) We make the prototype for functional guide, which is consisted of information section, participation section and other sections.

  • The purpose of this study was to develop the strategy on activation of village by forest healing. Researcher conducted the questionnaire survey for forest healing village development to classify the mountain's characteristics by the three factors(linkage resource, program, facilities). In result, village characteristics were divided into two types: single element outstanding type(resource), complex element outstanding type(resource+program, resource+facilities). The development of forest healing village have to focused on the forest healing service recipient and mountain village characteristics. In conclusion, relationship between forest healing recipient and mountain characteristics was as follows: single type (resource) - public; complex type (resource+program) - chronic disease, social vulnerable people; complex type (resource+facilities) - severe disease. The detailed guideline for forest healing village needs to be established according to the mountain characteristics.

Pore Filling Theory of Liquid Phase Sintering and Microstrcture Evolution (액상소결의 기공채움 이론과 미세구조 발달)

  • Based on the pore filling theory, the microstructure evolution during liquid-phase sintering has been analyzed in terms of interrelationship between average grain size and relative density. For constant liquid volume fraction, the microsturucture trajectories reduced to a single curve in a grain size(x)-density(y) map, regardless of grain growth constant. The slope of curves in the map was inversely proportional to average pore size, while it increased fapidly with liquid volume fraction. Increase in pore volume fraction retarded the densification considerably, but showed marginal effect on the slope. The activation energy of densification was predicted to be the same as that of grain growth as long as the liquid volume fraction is constant for any temperature range studied. The present analyses on microstricture evolution may demonstrate the usefulness of pore filling theory and provide a guideline for process optimization of liquid-phase sintering.

Influence of Environmental Conditions on c-Jun N-terminal Kinase Mediated Apoptosis of HL60 Cells by Anti-Cancer Drugs

  • Activation of JNK has long been associated with the apoptotic response induced by various anti-cancer drugs including doxorubicin, vinblastine, and etoposide. In this study, we examined and compared patterns of apoptosis and JNK activation according to three different anti-cancer drugs (daunorubicin, vinblastine, and etoposide) and two different sources of HL60 cells (Jackson Laboratory and ATCC). HL60 cells from Jackson Laboratory (HL60/RPMI) were maintained in RPMI 1640 containing 5% fetal bovine serum and those from ATCC (HL60/IMDM) in IMDM containing 20% fetal bovine serum as to each manufacture's guideline. In general, HL60/RPMI cells were more sensitive to anti-cancer drugs compared to HL60/IMDM cells, demonstrated by the XTT and flow cytometric analyses. Apoptotic pathways after treatment with anti-cancer drugs seemed to be different between HL60/RPMI (daunorubicin and etoposide, caspase 3 dependent, but caspase 8 or 9 independent; vinblastine, caspase 3 independent) and HL60/IMDM (caspase 3 and caspase 9 dependent). The expression of apoptotic protein, BID, was consistent with caspase 3 activation. Immunoblotting of phospho-JNK and JNK kinase assay showed JNK activation by all three anti-cancer drugs in HL60/RPMI, while JNK activation was observed only in vinblastine-treated cells in HL60/IMDM. Our study results suggest that in vitro environmental conditions have a significant influence on JNK mediated apoptosis of HL60 cells by anti-cancer drugs and in vitro culture conditions are important factors in JNK or possibly other MAPK related studies.

A Study on Activation of Oriental Medicine in Public Health Sector : The role of Oriental Public Health Physicians (한방 공공의료의 활성화에 관한 연구 - 공중보건 한의사의 활동을 중심으로 -)

  • From 1998, Oriental Medical Physicians(OMP) is distributed in Public Health Sector. but long term plan for Oriental Medicine in Public Health is not existed. So, this study is designed for the activation of OMP Subjects in this study were comprised 3 groups of oriental medicine related persons, the group of which are Students of 11 Oriental Medical Schools, Oriental Medical Physicians in Public Health Sector, Specialist Croup of Oriental medicine Policy(total 1,458 persons). Data were collected from July 1st to November 30, 1999. Direct interview with key persons, systematic interview by using of interview protocol, e-mail and facsimile have been conducted. The results of survey were coded by Excel 5.0, and analysed with SAS 6.12 statistical package. Inter-group difference determined by T-test, and descriptive statistics have been examined. Major findings can be epitomized as follows. 1) OMP disposition to multifarious organizations and institutes such as Public Health Centers, Public Health Sub-centers, Public Hospitals, Private Hospitals in Remote-Vulnerable Area, Community Social Welfare Centers, Institutes for Heath Policy Research, etc, will promise an effective use of Oriental Medical Physician. 2) Average number of patients treated by OMP was 22.8, average budget for oriental medical department, in which OMP were affiliated, was 39.6 million Won per year. Direct cost per every patient visit was 7,210.9 Won, which is considered expensive for public health service. Therefore, development and transformation for Oriental Medical Service in Public Health Sector is desirable in economic and political aspects. 3) It is recommended that ${\ulcorner}Advisory\;Committee{\lrcorner},{\ulcorner}Planning\;Commission\;for\; Public\;Health\;in\;Oriental\;Medicine{\lrcorner}$ should be established for the activation and for the support of Oriental Medical Physician in Public Health Sector, 4) Most effective programmes for oriental public health doctor are health service programmes for the elderly, Home health visit, chronic degenerative disease control services(p<0.001). 5) Standard guideline for facilities and equipments of Oriental Medicine Department in Public Health Center is needed for optimal supply of resources and activation of public health activity.

Macrophage Activation Syndrome Triggered by Herpes Viral Infection as the Presenting Manifestation of Juvenile Systemic Lupus Erythematosus (헤르페스 바이러스 감염으로 발생한 대식 세포 활성 증후군을 첫 증상으로 한 소아 전신 홍반 루푸스)

  • Macrophage activation syndrome (MAS) is a rare complication in systemic lupus erythematosus (SLE) that can be triggered by infections. Due to the fact that MAS may mimic clinical features of underlying rheumatic disease, or be confused with an infectious complication, its detection can prove challenging. This is particularly true when there is an unknown/undiagnosed disease; and could turn into an even greater challenge if MAS and SLE are combined with a viral infection. A-14-year-old female came to the hospital with an ongoing fever for 2 weeks and a painful facial skin rash. Hepatomegaly, pancytopenia, increased aspartate aminotransferase, elevated serum ferritin and lactate dehydrogenase were reported. No hemophagocytic infiltration of bone marrow was reported. The patient was suspected for hemophagocytic lymphohistiocytosis. Her skin rashes were eczema herpeticum, which is usually associated with immune compromised conditions. With the history of oral ulcers and malar rash, positive ANA and low C3, C4 and the evidence of hemolytic anemia, she was diagnosed as SLE. According to the diagnostic guideline for MAS in SLE, she was diagnosed MAS as well, activated by acute HSV infection. After administering steroids and antiviral agent, the fever and skin rash disappeared, and the abnormal laboratory findings normalized. Therefore, we are reporting a rare case of MAS triggered by acute HSV infection as the first manifestation of SLE.
