인공호흡기연관 폐렴의 경험적 항생제 선택 시 이전 호흡기검체의 유용성 (Role of Microbiologic Culture Results of Specimens Prior to Onset of Ventilator-Associated Pneumonia in the Patients Admitted to Intensive Care Unit)

  • Background: Patients with ventilator-associated pneumonia (VAP) in intensive care unit (ICU) have a high mortality rate. The routine surveillance cultures obtained previously or an ATS guideline for hospital-acquired pneumonia was used in selecting initial antimicrobials. The object of this study was to compare the respiratory samples before VAP and bronchoalveolar lavage (BAL) culture. Methods: 54 patients underwent fiberoptic bronchoscopy to obtain BAL samples. We reviewed microbiologic specimen results of prior respiratory specimens (pre-VAP) and BAL. Results: Among 51 patients with 54 VAP episodes, 52 microorganisms of pre-VAP and 56 BAL samples were isolated. Pre-VAP included 21.2% of MRSA, and 32.6% of multidrug resistant-Acinetobacter baumannii (MDR-AB). BAL samples comprised 25.0% of MRSA, 26.7% of MDR-AB, 14.3% of Stenotrophomonas maltophilia and 3.6% of Klebsiella pneumonia in order. In pre-VAP samples compared to BAL samples, only 35.2% were identical. In BAL samples compared to pre-VAP samples obtained in 5 days before the onset of VAP, only 43.6% were identical. However, among BAL samples compared to pre-VAP samples obtained after more than 5 days, 13.3% were identical (p=0.037). Conclusion: Based on these data, pre-VAP samples obtained prior to 5 day onset of VAP may help to predict the causative microorganisms and to select appropriate initial antimicrobials.

입원화상환자의 특성과 항생제 사용 현황 (An Evaluation of Antibiotic Use in the Hospitalized Burn Patients)

  • Infection is one of the main causes of death in severe burn cases. Treatment of burn patient includes fluid therapy, wound care, complication care and antibiotic therapy for infection. The objective of this study was to evaluate the characteristics of burn patients, the type of isolated microorganisms and their susceptibility, and the systemic antibiotics used. This is a retrospective study of 126 burn patients treated in the Hanil General Hospital from January to December 2001. Total 126 patients were assessed with 103 males and 23 females (4.5:1). The average age was $34.8\pm17.6$ years and extent of burn $(TBSA\;\%)\;was\;24.5\pm18.5\%$. The burn was caused by electric accident $(47.6\%),\;flame\;(29.4\%),\;scalding\;(21.4\%),\;and\;chemical\;accident\;(1.6\%)$. The overall mortality rate was $7.14\%$ (9/126) and all expired patients were males. The average age (n=9) was $48.8\pm15.6$ yrs and the extent of burn was $65.0\pm19.0\%$. The causes of death were due to flame burns $(13.5\%)$ and electric burns $(6.7\%)$. The culture sites of the isolated microorganisms were wound $(85.3\%),\;sputum\;(9.3\%),\;urine\;(2.7\%),\;blood\;(1.3\%)\;and\;catheter\;tip\;(1.3\%)$. Pseudomonas aeruginosa was the most commonly isolated organism $(35\%)$, followed by Staphylococcus aureus $(30.1\%)$, Acinetobacter baumannii $(21.4\%)$, and Enterococcus spp. $(3.9\%)$. The number of systemic antibiotics administered was 4.5. The classes of the antibiotics were cephalosporines $(38.5\%)$, aminoglycosides $(31\%)$, quinolones $(13.3\%)$, penicillins $(12.4\%)$, carbapenems $(2.4\%)$, glycopeptides $(1.9\%)$ and others $(0.6\%)$. In conclusion, most of burn patients had wound infection and Pseudomonas aeruginosa was the most commonly isolated organism. Cephalosporins were administered the most frequently among antibiotics.

A Literature Review of Infection with ESKAPE Pathogens in Oral and Maxillofacial Region

  • Odontogenic infection in the oral and maxillofacial regions caused by bacteria (mostly of oral origin) is one of the most common diseases encountered by dentists. Localized infection can easily be treated with incision and drainage followed by antibiotics. Emergence of multidrug resistant (MDR) bacteria called "Superbacteria" has become one of the serious problems in modern society, due to its small window of opportunity for treatment and high casualty. The acronym "ESKAPE", encompassing the common and serious MDR pathogens stand for Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa and Enterobacter spp. Literature search was performed in Medline, PubMed and Google Scholar ranging from 2012 to 2020. ESKAPE patient's infection period was longer than that of non-ESKAPE group, and the treatment method due to antibiotic resistance was also complicated. The purpose of this study is to investigate infection caused by ESKAPE pathogens in the oral and maxillofacial regions through literature review and to inform dental surgeons of the danger of ESKAPE pathogens and to suggest viable treatment options. Many studies worldwide reported infections associated with ESKAPE pathogens, but only limited number of studies targeted infection in oral and maxillofacial regions. Further research is required with more data on ESKAPE bacteria and their infection, especially in oral and maxillofacial regions.

Prevalence of Multi-Antibiotic Resistant Bacteria Isolated from Children with Urinary Tract Infection from Baghdad, Iraq

  • Urinary tract infections (UTIs) are one of the most common infections in different age groups, including children. Bacteria are the main etiological agents of UTIs. The aim of the present study was to isolate, identify, and determine the antibiotic susceptibility of bacteria isolated from children with UTIs from Baghdad, Iraq. Three hundred and two urine samples were collected from children aged 6 months to 12 years. The samples were cultured on blood agar and MacConkey agar. The selected colonies were subjected to biochemical tests and antibiotic susceptibility analysis using the Vitek® 2 Compact automated microbial identification system. In this sample, 299 bacteria were identified, of which, 267 were gram-negative bacteria, and 32 were gram-positive bacteria. Escherichia coli (56%) was the most commonly isolated gram-negative bacteria, followed by Pseudomonas aeruginosa (14%), Enterobacter spp. (10.48%), Klebsiella pneumoniae (9.36%), Proteus spp. (7.8%), Acinetobacter baumannii (1.5%), and Morganella morganii (0.37%). Enterococcus faecalis (62.5%) was the most commonly detected gram-positive bacteria, followed by Staphylococcus aureus (37.5%). E. coli and P. aeruginosa were the most antibiotic-resistant bacteria. Among the tested antibiotics, meropenem showed 100% sensitivity, followed by imipenem (97.4%), amikacin (91.8%), and tobramycin (83.5%). In contrast, the high frequencies of resistance were observed with cefixime (93.2%), cefotaxime (78.7%), and ceftriaxone/cefotaxime (71.2%). In conclusion, carbapenems and aminoglycosides are highly recommended for the empirical treatment of UTIs, while, Quinolones, penicillins, and cephalosporins are not suggested. Frequent antibiotics susceptibility testing are warranted to determine the resistance pattern of UTI bacteria.

Development of a Magnetic Bead-Based Method for Specific Detection of Enterococcus faecalis Using C-Terminal Domain of ECP3 Phage Endolysin

  • Bacteriophage endolysins are peptidoglycan hydrolases composed of cell binding domain (CBD) and an enzymatically active domain. A phage endolysin CBD can be used for detecting bacteria owing to its high specificity and sensitivity toward the bacterial cell wall. We aimed to develop a method for detection of Enterococcus faecalis using an endolysin CBD. The gene encoding the CBD of ECP3 phage endolysin was cloned into the Escherichia coli expression vector pET21a. A recombinant protein with a C-terminal 6-His-tag (CBD) was expressed and purified using a His-trap column. CBD was adsorbed onto epoxy magnetic beads (eMBs). The bacterial species specificity and sensitivity of bacterial binding to CBD-eMB complexes were determined using the bacterial colony counting from the magnetic separations after the binding reaction between bacteria and CBD-eMB complexes. E. faecalis could bind to CBD-eMB complexes, but other bacteria (such as Enterococcus faecium, Staphylococcus aureus, Escherichia coli, Acinetobacter baumannii, Streptococcus mutans, and Porphyromonas gingivalis) could not. E. faecalis cells were fixed onto CBD-eMB complexes within 1 h, and >78% of viable E. faecalis cells were recovered. The E. faecalis recovery ratio was not affected by the other bacterial species. The detection limit of the CBD-eMB complex for E. faecalis was >17 CFU/ml. We developed a simple method for the specific detection of E. faecalis using bacteriophage endolysin CBD and MBs. This is the first study to determine that the C-terminal region of ECP3 phage endolysin is a highly specific binding site for E. faecalis among other bacterial species.

ESKAPE Pathogens in Oral and Maxillofacial Infections

  • Purpose: Most cases of oral and maxillofacial infections are usually easily treated by proper diagnosis, elimination of causative factors, and antibiotic therapy. However, the emergence and the increase of multidrug-resistant bacteria make treatment challenging. "ESKAPE" pathogens are the most common opportunistic organisms in nosocomial infections and have resistant to commonly used antibiotics. There are many medical reviews of ESKAPE pathogens, but few in dentistry. This study focuses on oral and maxillofacial infection especially with ESKAPE pathogens. The purpose of this study is to prepare feasible data about tracing and treatment of infection related to pathogens that may be beneficial to clinicians. Methods: A total of 154 patients with oral and maxillofacial infections were reviewed by analyzing retrospectively hospitalized data in the Department of Oral and Maxillofacial surgery, Chosun University Hospital, Korea, past 5 years from January 2014 to December 2018. Based on the medical records and microbiological tests, the results were divided into two groups: infections with ESKAPE pathogens and other bacteria. Results: A total of 22 species were isolated from 154 patients. The proportion of ESKAPE pathogens among all bacterial isolates collected from infected patients was 39.6%. Causative factors, especially in post-operative infection, showed a statistically significant correlation to ESKAPE infections (29 cases). And average of treatment period in ESKAPE group was longer than non-ESKAPE groups. Overall, Klebsiella pneumoniae (60.7%) was the most frequently isolated ESKAPE pathogen. And high antibiotic resistance rates had been detected in the ESKAPE during the five-year period. Conclusions: Infections with ESKAPE pathogens are now a problem that can no longer be overlooked in Dentistry. Based on results of this study, ESKAPE pathogens were highly associated with post-operative or opportunistic infections. Clinicians should be careful about these antibiotic resistant pathogens and use appropriate antibiotics to patients while having dental treatments.

A Moonlighting Protein Secreted by a Nasal Microbiome Fortifies the Innate Host Defense Against Bacterial and Viral Infections

  • Evidence suggests that the human respiratory tract, as with the gastrointestinal tract, has evolved to its current state in association with commensal microbes. However, little is known about how the airway microbiome affects the development of airway immune system. Here, we uncover a previously unidentified mode of interaction between host airway immunity and a unique strain (AIT01) of Staphylococcus epidermidis, a predominant species of the nasal microbiome. Intranasal administration of AIT01 increased the population of neutrophils and monocytes in mouse lungs. The recruitment of these immune cells resulted in the protection of the murine host against infection by Pseudomonas aeruginosa, a pathogenic bacterium. Interestingly, an AIT01-secreted protein identified as GAPDH, a well-known bacterial moonlighting protein, mediated this protective effect. Intranasal delivery of the purified GAPDH conferred significant resistance against other Gram-negative pathogens (Klebsiella pneumoniae and Acinetobacter baumannii) and influenza A virus. Our findings demonstrate the potential of a native nasal microbe and its secretory protein to enhance innate immune defense against airway infections. These results offer a promising preventive measure, particularly relevant in the context of global pandemics.

국소화 농흉의 외과적 치료 - 폐쇄식 늑골절제 배농술 - (Surgical Treatment of Loculated Empyema - Closed Rib Resectional Drainage)

  • 연구배경 : 다발성으로 국소화된 농흉은 치료를 더욱 어렵게 만들며 특히 흉막천자와 흉관배농술에 반응하지 않는 경우는 더욱 그렇다. 재료 및 방법 : 저자들은 1991년 12월에서 1997년 12월까지 18례(남 14례, 여 4례)의 국소화된 섬유소화농성기 농흉(16례) 및 초기의 만성기 농흉(2례)환자들에 있어서 폐쇄식 늑골절제 배농술(closed rib resectional drainage)을 실시하였다. 결과 : 수술은 증상이 지속되면서 흉관을 통한 부적절한 배농(10례)이나 흉막천자가 실패(8례)한 국소화된 농흉을 가진 환자에 대하여 시행하였다. 농흉의 선행질환은 13례(72.3%)에서 폐렴, 3례(16.7%)에서 응고혈흉, 그리고 담낭절제술과 결핵성 흉막염이 각각 1례씩(5.5%)이었다. 원인균은 8례(42.1%)에서 배양이 되었는데 methicillin-resistant staphylococcus aureus가 3례로 가장 많았으며, pseudomonas aeruginosa가 2례, 그리고 enterococcus aerogens, α-hemolytic streptococcus, acinetobacter baumannii가 각각 1례씩 있었다. 농흉의 크기는 매우 다양하였는데 흉부단층촬영에서 1∼4개(평균 1.78±1.00)의 국소화 소견을 보였다. 수술시간은 55∼140분(평균 102.8±30.8)으로 짧았으며 술 후 모든 환자에서 발열이 소실되었고 전신 상태의 호전을 이루었다. 술후 흉관 거치 기간은 3∼42일(평균 11.4±11.5), 재원기간은 6∼36일(평균 12.9±8.1)이었다. 합병증은 지연된 배농이 2례에서 있었고 수술사망은 없었다. 술 후 재발한 례는 없었으며 3∼6개월에 촬영한 흉부사진 소견상 14례(77.8%)에서는 정상 소견을, 그리고 4례(22.2%)에서는 경미한 흉막비후의 소견을 보여 전 례에서 만족스러운 결과를 나타내었다. 결론 : 폐쇄식 늑골절제 배농술은 수술이 간편하고 합병증이 적으며 효과는 매우 양호하여 국소화된 섬유소화농성기 및 초기의 만성기 농흉환자에게 시행할 수 있는 적절한 수술이라 하겠다.

촬영 테이블과 IP Cassette의 세균 오염도 측정 및 소독에 관한 연구 (A Study on the Measurement of the Pollution Level of Bacteria and Disinfection of Table and IP Cassette)

  • 목적 : 검사실별 IP카세트와 테이블의 미생물 수와 세균학적 오염도를 알코올과 소독제가 포함된 티슈를 사용 전 후로 나누어 동정한 후 통계적으로 검정을 실시하여 영상의학과내 병원감염관리의 예방과 올바른 소독 지침을 위한 기초자료를 제공하고자 연구하였다. 대상 및 방법 : 본 연구는 대전광역시 소재의 1개 대학병원 영상의학과의 일반촬영실을 대상으로 하였다. 기간은 2007년 4월 5일부터 4월 12일까지 이루어졌으며, 검사실별 IP카세트와 테이블의 미생물 수와 세균학적 오염도를 알코올과 소독제가 포함된 소독용 티슈를 사용 전, 후로 나누어 실시하였으며, 정확한 수집을 위하여 병원내 감염관리 전문 간호사와 함께 시료를 채취하였다. 통계처리는 SPSS V13.0을 사용하였으며, 전 후 비교를 위해 T-검정을 실시하였으며 사후검증을 실시하였다. 결과 : 24개의 대상 중 19개(79.2%)에서 세균이 검출 되었으며 5개(20.8%)에서는 세균이 검출되지 않았다. 분리된 세균의 종류는 7종의 세균이 검출 되었는데 그 중 Methicillin Resistant coagulase-negative Staphylo-cocci(MRCNS)가 15(62.5%)곳으로 가장 많이 검출되었으며, 그 다음으로 Methicillin Resistant Staphylococcus Aureus(MRSA)가 6(16.7%)곳으로 많았으며, Enterococcus Faecium(EFM)이 5(20.8%), Acinetobacter baumannii(ABA) 2(8.3%), Bacillus sp, Coagulase-negative Staphylococci(CNS), Enterococcus sp(ENT)가 각각 1(4.2%)곳에서 검출되었다. 또한 ABA를 제외한 모든 것이 그람양성구균(Gram positive bacilli)이 30(97%) 례에서 검출 되었으며, 1(3%)례에서만 그람음성균(Gram positive bacilli)이 검출되었다. Group별 70% Alchol 사용 전 후 균의 종류 및 군락 수에서는 70% Alchol로 소독을 한 후 세균을 동정한 결과 70% Alchol 소독군에서는 IP Cassette와 Table 그룹 모두에서 100% 세균이 사멸되었다. Group별 Tissue Cleaner 사용 전 후 균의 종류 및 군락 수에서는 Tissue Cleaner 그룹에서의 균의 사멸률은 10그룹(71.2%)에서만 세균이 완전히 사멸되었으며, 4 그룹에서는 많은 감소를 보였으나 여전히 세균이 검출되었다. 전체 균의 사멸률은 91.5%로 나타났는데 다른 균들은 모두 사멸된 반면 MRCNS 균은 사멸률이 가장 낮았(83.6%)으며, 다음으로 MRSA(95%)가 낮았다. 결론 : 70% Alchol과 Tissue Cleaner의 사용전 후 검출되는 세균의 평균을 비교한 결과 모두에서 유의수준5%에서 통계적으로 유의한 것으로 나타났다.

카바페넴분해효소 생성 장내세균 검출을 위한 Multiplex PCR의 개발 및 평가 (Development and Evaluation of Multiplex PCR for the Detection of Carbapenemase-Producing Enterobacteriaceae)

  • 배경: 다양한 임상검체에서 카바페넴분해효소 생성 장내세균(carbapenemase-producing Enterobacteriace, CPE)의 분리가 증가하고 있다. CPE 감염증은 치사율이 높고, 집단 발병의 가능성이 높아 지속적인 감시가 필수적이다. 저자들은 CPE 주요 유전자들을 한 번에 검출하기 위한 multiplex PCR을 개발하고 이를 평가하고자 하였다. 방법: 총 7종의 내성 유전자를 동시에 검출하기 위한 시발체를 새롭게 디자인하고 PCR 조건을 설정하였다. 주요 카바페넴분해효소인 KPC, IMP, VIM, NDM-1, GES, OXA-23 및 OXA-48 유전자를 대상으로 하였다. 각 유전자의 증폭 산물은 100 bp 이상의 차이가 나도록 고안하였다. 개발된 multiplex PCR은 총 69주의 CPE 양성 임상분리균주를 이용하여 평가하고, 비특이적 증폭에 의한 위양성 가능성의 배제를 위해 71주의 카바페넴 감수성 균주로 확인하였다. 결과: 본 연구에서 개발한 시발체 및 PCR 조건을 이용하여 실험한 결과 CPE 내성 유전자인 KPC 양성 14주, IMP 양성 13주, OXA-23 양성 12주, OXA-48 양성 11주, VIM 양성 9주, GES 및 NDM 양성 각 5주 등 총 69주 모두에서 CPE 유전자의 검출이 가능함을 확인하였다. 카바페넴 감수성 균주를 이용한 특이성 확인 시험에서 Enterococcus faecalis, Escherichia coli, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa 등을 포함한 총 71주의 다양한 그람양성 및 음성균주 모두에서 음성임을 확인하였다. 결론: 본 연구에서 개발된 multiplex PCR은 총 7종의 CPE 유전형을 한 번에 검출할 수 있어 CPE 확인 시험에 유용할 것으로 생각한다.
