의료기관들의 인증평가 준비와 비용지출에 대한 실태분석 (An Analysis of Accreditation Preparation Process and Costs in Hospitals)

  • While the influence of healthcare accreditation system to the quality improvement of hospitals has more increased, regarding the preparation costs for healthcare accreditation, it has never been empirically studied about the costs that are actually invested by hospitals. This study is going to determine the difficulties in the preparation process of accreditation and details of accreditation preparation costs for hospitals that participated in the healthcare accreditation system and acquired accreditation. The survey was performed in a self-reported form from February 28 to March 21 2014 for 189 acute hospitals accredited as a hospital from 2011 to February 2014. Of all questionaries of survey participants, 98 were recovered; the response rate was 51.9%. A total of 40 questionnaires were used except for 58 containing insincere answers. Main findings are followings: Firstly, findings showed that advanced general hospitals spent the most statistically significantly highest in terms of equipments and total costs among cost items for accreditation preparation. When accreditation preparation costs items were classified according to classification of hospitals, advanced general hospitals spent the most statistically significantly highest in the equipments and total costs. Also in terms of regional, Gyeonggi, Incheon regions were found to spend statistically significantly higher costs in the equipments costs. Secondly, as a result of the survey in the distribution of the total accreditation preparation costs, advanced general hospitals have disbursed the most out of all. However, the result in hospitals does not show significant difference to the expense of advanced general hospitals and that especially other regional hospitals spent higher costs. As such, all hospitals are under a heavy burden of higher costs on accreditation preparation, especially hospitals. The build-up of infrastructures by hospitals through an accreditation system consequently led to a higher initial investment; if the accreditation system is effective in improving the quality of health care and patient safety, appropriate responses are needed. In other words, financial support for investment costs needs to be given to allow hospitals to actively participate in the accreditation system.

간호사의 국제의료기관평가에 대한 인식과 간호업무성과, 평가 관련 직무 스트레스 및 소진과의 관계 (Relationships of Nurses' Perception, Nursing Performance, Job Stress, and Burnout in Relation to the Joint Commission International Hospital Accreditation)

  • Purpose: The purpose of this study was to ascertain current status of nurses' perception, nursing performance, job stress, and burnout in relation to the Joint Commission International (JCI) hospital accreditation and to verify the relationships among these variables. Methods: This cross-sectional study was performed using questionnaires. Data were collected from 220 nurses who worked at one hospital from April 5 to May 4, 2013. Results: The scores for perception, nursing performance, job stress and burnout in relation to the JCI hospital accreditation were(on a five-point Likert scale) 3.23, 4.01, 3.56 and 3.40 respectively. A positive correlation was observed between perception and nursing performance. Burnout was negatively correlated with perception and nursing performance, and positively correlated with job stress. Conclusion: These results indicate that developing positive perception and reduced burnout in relation to the JCI hospital accreditation can produce good nursing performance. These findings can be utilized to develop strategies for reducing job stress and burnout in relation to the JCI hospital accreditation.

국내 의료기관 인증제 전망 (Korean Healthcare Accreditation Perspectives)

  • Two years have passed since hospital accreditation was introduced in Korea by the Korean Institute of Healthcare Accreditation (KOIHA), which takes, as its mission, continuously improving healthcare quality and patient safety for the public. KOIHA also has a plan to impose on outcomes in management of patient care. With a view to enhance the practice of accreditation in terms of quality as well as quantity, and to help improve healthcare quality and patient safety in Korea, this paper reviews some issues on healthcare policies which are expected to be adopted in 2013 by the forthcoming government. A few suggestions are added.

  • 본 연구는 의료기관 인증평가 후 요양병원 직원들의 조직문화, 직무만족, 경영성과 및 환자안전에 대한 인식 및 태도의 변화를 알아보고 의료기관 인증평가의 효과를 확인하고 이 제도를 통한 요양병원의 질 향상을 도모하고자 시행하였다. 이를 위해 요양병원의 의료기관 인증평가를 받은 부산의 8곳의 요양병원 직원을 대상으로 2014년 4월 1일에서 2014년 7월 20일까지 설문조사를 하여 283부의 설문지를 연구에 사용하였다. 연구결과는 첫째, 인증평가 후 요양병원 직원들의 조직문화, 직무만족, 경영성과 및 환자안전에 대한 인식과 태도에 변화가 있었고, 둘째, 조직문화, 직업만족 및 경영성과 및 환자안전 등 변수들 간의 정(+)의 상관관계가 있었으며, 셋째, 조직문화, 직업만족 및 경영성과는 환자안전에 정(+)의 영향을 미쳤다. 따라서 요양병원의 의료기관 인증평가 과정에서 직원들의 직무만족을 더 높이고 완전한 환자안전을 보장하는 양질의 의료를 구현할 수 있도록 요양병원 인증제도와 같은 외부평가와 병원의 자발적인 내부평가를 유기적으로 함께 발전시켜야 하겠다.

의료기관 인증이 브랜드 충성도에 미치는 영향 (The Effects of Accreditation Program for Healthcare Organizations on Brand Loyalty)

  • This study is to check up weather a brand of accreditation program for healthcare organizations recognized by the consumers as users of the healthcare organizations has effects for healthcare organizations' brand trust, attitude, and loyalty. This study suggests the hypothesis that a brand value of accreditation program for healthcare organizations gives the positive effect for the brand trust, attitude and loyalty, and that the brand trust and attitude of healthcare organizations which gain a brand of accreditation program for healthcare organizations cause the brand loyalty. To verify the hypothesis, this study made a questionnaire by using a measuring instrument for the brand value, trust, attitude and loyalty which come from the former study. This study carried out an analyzing method such as frequency analysis, reliability analysis, confirmed factor analysis and structural equation model to verify the hypothesis. From hypothesis verification, emotional and social brand values of accreditation program for healthcare organizations gave a positive effect to the brand trust and attitude on healthcare organizations. Its functional value did not give an effect to a brand attitude but gave a positive effect to healthcare organizations' brand loyalty. Emotional and social brand values of accreditation program for healthcare organizations did not give an effect to the brand loyalty. The brand trust and attitude of the healthcare organizations which gained accreditation program for healthcare organizations gave a positive effect to the brand loyalty on healthcare organizations.

임상시험 및 대상자보호프로그램의 운영과 현황에 대한 설문조사 연구(2019) (Survey of Operation and Status of the Human Research Protection Program (HRPP) in Korea (2019))

  • Purpose: The purpose of this study is to assess the operational status and level of understanding among IRB and HRPP staffs at a hospital or a research institute to the HRPP guideline set by the Ministry of Food and Drug Safety (MFDS) and to provide recommendations. Methods: Online survey was distributed among members of Korean Association of IRB (KAIRB) through each IRB office. The result was separated according to topic and descriptive statistics was used for analysis. Result: Survey notification was sent out to 176 institutions and 65 (37.1%) institutions answered the survey by online. Of 65 institutions that answered the survey; 83.1% was hospital, 12.3% was university, 3.1% was medical college, 1.5% was research institution. 23 institutions (25.4%) established independent HRPP offices and 39 institutions (60.0%) did not. 12 institutions (18.5%) had separate IRB and HRPP heads, 21 (32.3%) institutions separated business reporting procedure and person in charge, 12 institutions separated the responsibility of IRB and HRPP among staff, and 45 institutions (69.2%) had audit & non-compliance managers. When asked about the most important basic task for HRPP, 23% answered self-audit. And according to 43.52%, self-audit was also the most by both institutions that operated HRPP and institutions that did not. When basic task performance status was analyzed, on average, the institutions that operated HRPP was 14% higher than institutions that only operated IRB. 9 (13.8%) institutions were evaluated and obtained HRPP accreditation from MFDS and the most common reason for obtaining the accreditation was to be selected as Institution for the education of persons conducting clinical trial (6 institutions). The most common reason for not obtaining HRPP accreditation was because of insufficient staff and limited capacity of the institution (28%). Institutions with and without a plan to be HRPP accredited by MFDS were 20 (37.7%) each. 34 institutions (52.3%) answered HRPP evaluation method and accreditation by MFDS was appropriate while 31 institutions (47.7%) answered otherwise. 36 institutions answered that HRPP evaluation and accreditation by MFDS was credible while 29 institutions (44.5%) answered that HRPP evaluation method and accreditation by MFDS was not credible. Conclusion: 1. MFDS's HRPP accreditation program can facilitate the main objective of HRPP and MFDS's HRPP accreditation program should be encouraged to non-tertiary hospitals by taking small staff size into consideration and issuing accreditation by segregating accreditation. 2. While issuing Institution for the education of persons conducting clinical trial status as a benefit of MFDS's HRPP accreditation program, it can also hinder access to MFDS's HRPP accreditation program. It should also be considered that the non-contact culture during COVID-19 pandemic eliminated time and space limitation for education. 3. For clinical research conducted internally by an institution, internal audit is the most effective and sole method of protecting safety and right of the test subjects and integrity for research in Korea. For this reason, regardless of the size of the institution, an internal audit should be enforced. 4. It is necessary for KAIRB and MFDSto improve HRPP awareness by advocating and educating the concept and necessity of HRPP in clinical research. 5. A new HRPP accreditation system should be setup for all clinical research with human subjects, including Investigational New Drug (IND) application in near future.

의료기관 인증 조사위원의 만족도와 신뢰도 관련 요인 (Factors Associated with Satisfaction as Surveyor and Reliability of Surveyors in Hospital Accreditation Program)

  • Background: The hospital accreditation program in Korea has been conducted since 2011 in order to improve patient safety and healthcare service quality. This study was conducted to find factors associated with satisfaction as surveyor and reliability of surveyors in hospital accreditation program. Methods: This study was performed targeting 217 responded to the survey among 412 surveyors who had participated in the accreditation survey for acute care hospitals from December 2010 to February 2014. Results: The average number of survey per surveyor is 2.35. We divided surveyors into those who participated in the survey more than 3 times and less than 3 times in order to judge the professionalism of surveyors according to the number of survey participation. Those factors that have an influence on the satisfaction as surveyors include: activity period as surveyor, role in the survey team, experience of survey in other fields, experience as consultant and the useful education and proper composition of survey team (p<0.05). Those factors that have an influence on the reliability for fellow surveyors include: number of beds of hospitals they belong, experience of survey in other fields, useful education, proper composition of survey team and difficulty in leadership interview (p<0.05). Conclusion: It is important to provide useful education and proper composition of survey team to increase the satisfaction as surveyors and the reliability for fellow surveyors.

종합병원 간호사의 의료기관인증제에 대한 인식과 직무 스트레스가 이직의도에 미치는 영향 (Effects of Perception of the Healthcare Accreditation, and Job Stress on Turnover Intention in Nurses)

  • Purpose: The purpose of this study was to examine the perception of the healthcare accreditation and the level of job stress and also to identify the factors affecting turnover intention in general hospital nurses. Methods: This cross-sectional study was performed using questionnaires. Data were collected from 230 nurses who worked at one general hospital from February 20 to April 10, 2014. Data were analyzed with independent t test, ANOVA, Pearson correlation coefficient, and regression using SPSS/WIN v 21.0. Results: The scores of perception of the healthcare accreditation, job stress, and turnover intention were 3.05 out of 5, 4.30 out of 5, and 3.18 out of 5. A positive correlation was observed between turnover intention and job stress. Turnover intention was negatively correlated with perception. The factors affecting turnover intention were the perception of healthcare accreditation (35.3%), total clinical experience (12.3%) and job stress (7.4%). The total explanatory power was 55.3%. Conclusion: The perception of the Healthcare Accreditation was confirmed as a new factor affecting turnover intention in nurses. These findings can be utilized to the development of strategies for reducing job stress, and enhancing perception and resulting in both the quantitative and qualitative development of the healthcare system.

국내외 JCI 인증병원의 웹사이트 비교 분석 (Comparative analysis of hospital websites with JCI accreditation across four nations)

  • The purpose of this study is to compare the characteristics of hospital websites across four nations(e.g., South Korea, Singapore, Thailand and India). We selected English websites of 58 hospitals with JCI accreditation. For the analysis of websites, we developed assessment tool with four domains(e.g., contents, convenience, design, interactivity). The results showed that Korean hospitals provided more channels for appointment on the website compared to other countries. In contrast, foreign hospitals used actively SNS(e.g., Facebook, Twitter or Youtube) on the websites. They also provided more information on diverse services(e.g., airport pickup services, treatment costs, consultation services).

급성기병원 인증기준의 가중치 도출: 계층적 분석법을 활용하여 (Deriving Criteria Weights for Acute Care Hospital Accreditation in South Korea: Using Analytic Hierarchy Process)

  • Purpose:The acute hospital accreditation program launched in South Korea has shown positive effects on safety culture and quality of care. However, relative weights have not yet been investigated for accreditation criteria with a hierarchical structure. This study aimed to derive the relative weights of acute-care hospital accreditation criteria. Methods: We conducted an online survey using the analytic hierarchy process (AHP) technique to assess the validity, importance, and urgency of acute hospital accreditation criteria. The AHP online survey link was distributed in November 2022 after obtaining informed consent from 10 experts in hospital accreditation. Results: 'Basic value system' ranked highest, while 'patient care system' ranked second in terms of validity, importance, and urgency. 'Performance management system' had the lowest validity and urgency, while 'organizational management system' carried the lowest importance. Within the 'patient care system' domain, 'surgery and anesthesia sedation management' scored highest in validity and importance, and 'patient care' scored highest in urgency. 'Care delivery system and evaluation' received the lowest scores for all three aspects. In the 'organizational management system' domain, infection control ranked highest in terms of validity, importance, and urgency. The lowest validity was observed for 'management and organizational operation' and the lowest importance and urgency were noted for 'human resource management'. Conclusion: The weights for validity, importance, and urgency, as shown in each domain and chapter, and the number of measurable elements included, are largely inconsistent. This study will contribute to the development of the structure and scientific improvement of accreditation standards.
