Clinical Characteristics of Patients with Tinnitus accompanied Vertigo (현훈을 동반한 이명 환자들의 임상적 특성)

  • Objectives : This study is to find out clinical characteristics of patients with Tinnitus and Vertigo. Methods : 28 patients were recruited in this study who were ill with tinnitus accompanied vertigo. They told me the aspects of tinnitus and wrote out the Tinnitus Handicap Inventory(THI). Also, they were divided into central and peripheral vertigo. Through symptoms of tinnitus and vertigo, the discrimination of Heo-sil(虛實) was based on the specific aspect of tinnitus along with Zhuang fu bian zheng(臟腑辨證). Results : In tinnitus, high-narrow tone group is more than low-wide tone group and the rate of Heo symptom(虛證) group is higher than Sil symptom(實證) group. In vertigo, the rate of peripheral group is higher than central group. Conclusions : It is expected that this survey will help patients with both tinnitus and vertigo in their treatment progress and prognostic judgment.

  • Tetanus is a life-threatening infection that is rare in the developed country. Because of the rarity: of the disease, the clinician may be unfamiliar with the clinical presentation and unsuspecting of the diagnosis. However, tetanus can rapidly progress into lethal muscle spasms accompanied by respiratory insufficiency, and it has a mortality of 15 to 30%. The most common presenting symptom was trismus, followed by neck pain, dysphagia, generalized pain and facial muscle contractions. Dysphagia is a common symptom of tetanus, but not common as an initial symptom, the correct diagnosis and adequate therapy are likely to be delayed. Treatment involves administration of penicillin, tetanus immune-globulin, debridement of wounds, aggressive supportive care, and initiation of active immunization. We report an elderly woman presenting with dysphagia as an initial symptom of tetanus with review of literature.

Efficacy of Daehuanghuanglianxiexin-tang in stabilization of hypertension patient's blood pressure (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 대황황련사심탕(大黃黃連瀉心湯) 투여 후 혈압이 안정된 고혈압 증례 보고)

  • Objective : The purpose of this study is to review a case of hypertension. The patient was 41 years, a male. We stabilized blood pressure after administration of Daehuanghuanglianxiexin-tang. In this paper, we review the interpretation of Shanghanlun by analyzing the patient's progress, diagnosis, treatment. Methods : Blood pressure changes have been monitored during the period. Additionally, We have observed the patients' compliance and accompanied symptom by the timeline analysis. Results : According to Shanghanlun disease pattern identification diagnostic system, Wediagnosed a Greater yang disease. The patient was treated with Daehuanghuanglianxiexin-tang for 2 months. Daehuanghuanglianxiexin-tang was able to control blood pressure to under 120mmhg / 80mmhg. During the treatment period, the accompanied symptom Stiffness and pain in posterior neck has improved, patient's compliance was good, and symptoms improved without significant complication Conclusions : Taking of the Daehuanghuanglianxiexin-tang makes patient's blood pressure controlled to safe and stable range and eases the discomfort of posterior neck pain.

A Report of the Symptoms Accompanied with Acne and Heart Rate Variability Analysis of Acne Patients (여드름 환자의 동반증상 연구 및 환자 특성별 HRV 분석)

  • Objectives: The purpose of this study is to investigate the symptoms accompanied with acne and HRV pattern of the acne patients. Methods: We reviewed 60 acne patients(age 15-38) who visited ${\bigcirc}{\bigcirc}$ hospital from January 2010 to October 2011. We investigated the symptoms accompanied with acne of 60 patients and evaluated HRV of 41 acne patients. The data were analyzed by Mann-Whitney test using SPSS/PC ver 13.0 program. Results: 1. Prevalence of female post-adolescent acne was 66.7% in this study. The symptoms accompanied with acne were dischezia(35%), gastric discomfort(28.3%) in all patients and dysmenorrhea(40%), leukorrhea(38%), irregular menstrual period (24%) in female patients. 2. SDNN, RMSSD, VLF and HF of female were significantly lower than male. 3. SDNN, TP and VLF of un-dysmenorrhea group were significantly lower than dysmenorrhea group. 4. PSI of under 6 months group was significantly higer than over 6 months group. 5. In postadolescent acne patients SDNN, RMSSD, TP, LF and HF were significantly lower and PSI was significantly higher than adolescent acne patients. Conclusions: There are many female post-adolescent acne pateints and the assessment of the symptoms accompanied with acne and stress degree is helpful in treating acne. The further research and development about the diagnostic and therapeutic methods of acne are needed in oriental obstetrics and gynecology field.

Clinical Characteristics of Patients with Tinnitus Accompanied Hearing Loss (난청을 동반한 이명 환자의 임상적 특성)

  • Background : Tinnitus is common disorder with many possible causes and a symptom of many different diseases but has no effective treatment. Most of tinnitus patients experience the disorder to a degree that their quality of life and productivity are impaired. Especially tinnitus accompanied hearing loss causes severe discomfort in patients. Objectives : 1. Through symptoms of tinnitus, we tried to find out the relationship of Heo-sil(虛實) and Zuang fu bian zheng(臟腑辨證) among tinnitus patients accompanied Hearing Loss. 2. By testing THI, we tried to evaluate the quality of life of tinnitus patients group and tried to compare the quality of life between high-narrow tone and low-wide tone tinnitus patients group. 3. In the future, this study was planned to help monitor the prognosis and determine the progress of treatment for tinnitus patients who also have hearing loss. Methods : 43 patients were recruited in this study who was ill with tinnitus accompanied hearing loss. They wrote out the questionnaire about tinnitus and the Tinnitus Handicap Inventory(THI). Results : 1. In tinnitus patients, there were more men than women accompanied hearing loss. The rate of visits was higher as the age increased. 2. Most of the patients who visited the hospital had sensitive hearing loss. 3. In high-narrow tone tinnitus group, phlegm fire congesting-type are more uncomfortable than spleen-stomach weakness-type. 4. In low-wide tone tinnitus group, liver qi depression was the most uncomfortable. Conclusions : The present study suggests that tinnitus pattern may be helpful in differentiating patients with tinnitus, and the effectiveness of treatment can be predicted through differentiation. Zuang fu bian zheng(臟腑辨證) and Distinguishing between Heo(虛) and Sil(實) is expected to have positive effects on tinnitus accompanied hearing loss treatment.

The Analysis of usage and characteristic of Cancer-Related Anorexia Symptom Assessment Tool (암 환자 식욕부진 증상 평가 도구의 활용 및 특성에 대한 분석)

  • Objectives : Anorexia is the primary symptom impinging cancer patients' Quality of Life. It is usually accompanied by gastrointestinal symptoms(GI symptoms). Thus, to measure anorexia symptom precisely, assessing anorexia and GI symptoms together is recommended. This study was designed to analyze cancer-related anorexia assessment tools, extract GI symptoms included in these tools and investigate usefulness of instruments in clinical trials. Methods : Instruments were selected by searching PubMed, PROQOLID database. We analyzed instruments by number of items, assessment method, type of question, GI symptoms. Results : 9 instruments were selected to assess cancer-related anorexia symptom. Most tools adopt Likert scale as response scale and 'during past week' as recall period. Assessment method of all 9 instruments is the self-administration. Questions measuring anorexia are able to be sorted into 3 forms (frequency, severeness, distress of anorexia symptom). Among the GI symptoms, nausea is included in all 9 instruments. In clinical trials of cancer-related anorexia, Edmonton Symptom Assessment Scale(ESAS) and Functional Assessment of Anorexia/Cachexia Therapy Questionnaire(FAACT) were selected as endpoint measure. Conclusions : The result showed that FAACT is the only specialized tool to assess cancer-related anorexia. To measure cancer-related anorexia precisely, the need to develop new instrument exists.

Clinical study on one patient with oculomotor nerve palsy accompanied by Diabetes mellitus complication history (당뇨 합병증의 과거력이 있는 동안 신경마비 환자1례에 대한 임상적 고찰)

  • Objective : To carry out the oriental medicine treatment on a patient with oculomotor nerve palsy accompanied by Diabetes mellitus complication history Methods : We conducted one patient with oculomotor nerve palsy at Won-Kwang University Kwang-Ju Oriental Medical Hospital. We treated patient by herbal medicine, acupuncture, etc. Results : We had an effect that one patient's symptom had improved. Conclusion : We suggest that the oriental medical treatment and management have an effect with oculomotor nerve palsy accompanied by DM complication history.

Clinical Study of 8 Diabetic Patients with Paresthesia (감각이상을 주소로 내원한 당뇨병 환자 8례에 대한 임상보고)

  • Objective : The purpose of this study was to report the effect of Bogijetong-tang on diabetic peripheral neuropathy. Methods : Nine patients who were diagnosed or suspected with diabetic peripheral neuropathy enrolled in this study. They took Bogijetong-tang 2 or 3 times a day and were given acupuncture therapy. To evaluate the therapeutic effect, Visual Analog Scale (VAS) or Total Symptom Score (TSS) were examined at intervals of about 7 days. Result : TSS score decreased an average of 2.74 points and VAS scores decreased an average of 3.67 points. As the pain decreased, accompanied gait disturbance also improved in some patients. Conclusion : Bogijetong-tang is effective in treating diabetic peripheral neuropathy, but study of the concrete mechanism and efficacy is needed.

  • Objectives This research was performed to establish the clinical practice guideline(CPG) for Lesser Yin Symptomatology of Soeumin disease. Methods Dongeui suse bowon(sinchuk edition), textbook for Sasang constitutional medicine, Clinical guidebook for Sasang constitutional medicine, and standardization reports on Sasang constitutional medicine and papers concerning symptomatology of Soeumin Disease, especially Lesser Yin Symptomatology was collected and classified. Additionally experts' conference was held to make agreement on the conflicting issues on a regular basis. Results & Conclusions There was no concerning paper on Lesser Yin Symptomatology. Experts' agreement was needed to establish the CPG. Lesser Yin pattern can be classified into 2 groups; Lesser Yin severe pattern and Lesser Yin critical pattern. There are Lesser Yin pattern accompanied abdominal pain and bowel irritability pattern and Lesser Yin pattern accompanied green tinged watery diarrhea pattern in Lesser Yin severe pattern. There are Visceral syncope pattern and Exuberant yin repelling yang pattern in Lesser Yin critical pattern. Lesser Yin symptomatology has several symptoms like abdominal pain and diarrhea, thirst, oral discomfort, chest discomfort, whole body pain, articular pain and coldness of hands and feet. Additionally there are abdominal pain and diarrhea in Lesser Yin symptomatology accompanied abdominal pain and bowel irritability pattern, there is green tinged watery diarrhea in Lesser Yin pattern accompanied green tinged watery diarrhea pattern and if this symptoms exacerbate, delirious speech and constipation can occur. There are restlessness and coldness on hands and feet in Visceral syncope pattern and severe restlessness and coldness on hands and feet and symptom which the patient cannot drink water in Exuberant yin repelling yang.

Clinical Study of Congenital Esophageal Stenosis: Comparison according to Association of Esophageal Atresia and Tracheoesophageal Fistula

  • Purpose: Congenital esophageal atresia (CES) is a rare congenital disease. The severity of symptoms is variable; thus, diagnosis is difficult and tends to be delayed. CES is frequently accompanied by esophageal atresia (EA) with/without tracheoesophageal fistula (TEF). We investigated the characteristics of CES by reviewing our experience with CES patients and researched the differences between CES with EA-TEF and isolated CES. Methods: A total of 31 patients underwent operations for CES were reviewed retrospectively. The patients were divided into two groups according to the association with EA-TEF, and compared the differences. Results: Sixteen boys and 15 girls were included. The mean age at symptom onset was 8 months old, and the mean age at diagnosis was 21 months old. Nine patients with EA-TEF were included group A, whereas the other 22 patients were assigned to group B. There were no differences in sex, gestational age, associated anomalies and pathologic results between the groups. In group A, the age at diagnosis and age at surgery were younger than in group B despite the age at symptom occurrence being similar. Postoperative complications occurred only in group A. Conclusion: In this study, symptoms occurred during the weaning period, and vomiting was the most frequent symptom. CES patients with EA-TEF tended to be diagnosed and treated earlier despite the age at symptom occurrence being similar. CES patients with EA-TEF had more postoperative complications; therefore, greater attention should be paid during the postoperative period.
