A Study on the Risk Assessment and Improvement Methods Based on Hydrogen Explosion Accidents of a Power Plant and Water Electrolysis System (발전소 및 수전해 시스템의 수소 폭발 사고 사례 기반 위험성 평가 및 개선 방안 연구)

  • This study addresses the escalating issue of worldwide hydrogen gas accidents, which has seen a significant increase in occurrences. To comprehensively evaluate the risks associated with hydrogen, a two approach was employed in this study. Firstly, a qualitative risk assessment was conducted using the bow-tie method. Secondly, a quantitative consequence analysis was carried out utilizing the areal locations of hazardous atmospheres (ALOHA) model. The study applied this method to two incidents, the hydrogen explosion accident occurred at the Muskingum River power plant in Ohio, USA, 2007 and the hydrogen storage tank explosion accident occurred at the K Technopark water electrolysis system in Korea, 2019. The results of the risk assessments revealed critical issues such as deterioration of gas pipe, human errors in incident response and the omission of important gas cleaning facility. By analyzing the cause of accidents and assessing risks quantitatively, the effective accident response plans are proposed and the effectiveness is evaluated by comparing the effective distance obtained by ALOHA simulation. Notably, the implementation of these measures led to a significant 54.5% reduction in the risk degree of potential explosions compared to the existing risk levels.

  • A lot of hazardous materials have been used for product processing and utility plant. Many accidents including toxic release, fire and explosions occur in the ammonia related facility and plant. Various safety and environment management program including PSM, SMS, ORA etc. are being implemented for risk management and accident prevention in the production industry. Also much study and research have been carried about risk assessment of accident scenario in the academic and research area. In this paper, firstly risk level was assessed by using a typically used KORA program and LOPA PFD method for the selected ammonia unloading and storage facility. And then risk reduction measures for the risk assessed facility were studied in 3 aspects and some measures were proposed. Those Risk Reduction measures are including a leak detection and emergency isolation, water spray, dilution tank, dike and trench, scattering protection in hardware impovement aspect, and a applicable risk criteria, conditional modifier for existing LOPA PFD, alternative supporting modeling program in risk estimation methodology aspect, and last RBPS(Risk Based Process Safety) program, re-doing of process hazard analysis, management system compliance audit in managerial activity aspect.

Analysis of Injuries in the Ghanaian Mining Industry and Priority Areas for Research

  • Background: Despite improvements in safety performance, the number and severity of mining-related injuries remain high and unacceptable, indicating that further reduction can be achieved. This study examines occupational accident statistics of the Ghanaian mining industry and identifies priority areas, warranting intervention measures and further investigations. Methods: A total of 202 fatal and nonfatal injury reports over a 10-year period were obtained from five mines and the Inspectorate Division of the Minerals Commission of Ghana, and they were analyzed. Results: Results of the analyses show that the involvement of mining equipment, the task being performed, the injury type, and the mechanism of injury remain as priorities. For instance, mining equipment was associated with 85% of all injuries and 90% of all fatalities, with mobile equipment, component/part, and hand tools being the leading equipment types. In addition, mechanics/repairmen, truck operators, and laborers were the most affected ones, and the most dangerous activities included maintenance, operating mobile equipment, and clean up/clearing. Conclusion: Results of this analysis will enable authorities of mines to develop targeted interventions to improve their safety performance. To improve the safety of the mines, further research and prevention efforts are recommended.

Capacitor Failure Detection Technique for Microgrid Power Converter (마이크로그리드 전력변환장치용 커패시터 고장 검출 기법)

  • The DC part of the DC microgrid power conversion system uses capacitors for buffers of charge and discharge energy for smoothing voltage and plays important roles such as high frequency component absorption, power balancing, and voltage ripple reduction. The capacitor uses an aluminum electrolytic capacitor, which has advantages of capacity, low price, and relatively fast charging/discharging characteristics. Aluminum electrolytic capacitors(AEC) have previous advantages, but over time, the capacity of the capacitors decreases due to deterioration and an increase in internal temperature, resulting in a decrease in use efficiency or an accident such as steam extraction due to electrolyte evaporation. It is necessary to take measures to prevent accidents because the failure diagnosis and detection of such capacitors are a very important part of the long-term operation, safety of use, and reliability of the power conversion system because the failure of the capacitor leads to not only a single problem but also a short circuit accident of the power conversion system.

A Comparison of Income Level and Work-Related Fatalities for Finding Causes and Measures for Construction Sector (소득수준 대비 산재사망지수 비교를 통한 건설분야 산업재해 분석 및 저감대책)

  • As the income level of a country gets improved, people's social, economic, and cultural expectation will grow at the same time. Although the income level of Korea has been remarkably improved, her work-related fatality rate is the highest among OECD countries. Moreover, the rate hasn't been reduced for more than 10 years. This study aims to provide appropriate measures for work-related fatality reduction by comparing the income levels and the work-related fatality rates of countries and by analysing the problems of our worker-oriented accident prevention policy. For the comparison of income level and work-related fatalities, this study suggests income-level incorporated fatality rate, and it turned out that the rate of Korea is the highest among the countries. In order to find out the causes of that result, this study has found that our law enforcement is much weaker than that of UK, which implicates more strict law enforcement policy is needed to improve our work-related safety.

Identification of Hazards for Offshore Drilling through Accident Statistics and JSA-based Risk Reduction (사고 통계 분석을 통한 해양 시추작업 위험요소 제시 및 JSA 기반 위험저감 방안)

  • Offshore drilling units have a very dangerous working conditions due to the harsh working environment of the ocean and the high possibility of fire or explosion. This study would identify the hazards that emerge from the marine environment in the operation and maintenance phase of offshore drilling units and show how these hazards can be reduced through risk assessment/management. Various risk reduction and management measures were first reviewed, and Job Safety Analysis (JSA) was selected as the risk assessment technique of this study. In order to understand the characteristics of offshore drilling operations, accident statistics of onshore and offshore drilling were analyzed and compared with each other, and major risk factors for offshore drilling were derived. The jobs in which offshore drilling accidents occur more frequently than onshore drilling was analyzed as the job of fastening, transporting and moving pipes and various materials. This result is due to the limited space of the ocean and the work environment that is prone to being shaken by wind, waves and ocean currents. Based on these statistical results, the job of picking and making up drill pipes was selected as a high-risk job, and JSA was performed as an example. A detailed safety check procedure is proposed so that workers can fully recognize the danger and perform work in a safe state that has been confirmed.

Research on Improvement Measures for Chemical Risk Assessment for Small and Medium-Sized Businesses (중소규모 사업장을 위한 화학물질 위험성평가 개선방안 연구)

  • The Ministry of Employment and Labor announced a roadmap for reducing serious accidents to create a "safe and healthy workplace, happy Korea" and ordered a risk assessment for all work processes in all workplaces on November 30, 2022. Accordingly, even workplaces that handle new chemicals must conduct risk assessment when introducing new chemicals, but the risk assessment techniques currently proposed by the government focus on preventing conventional accidents such as falls, getting caught, and bumping. Currently, there is a Chemical Hazard Risk Management (CHARM) technique that is most commonly used for chemical risk assessment, but in small and medium-sized businesses that lack professional manpower related to chemical substances, there are many technical terms in the technique and it is difficult to interpret, so its utilization is very low. Therefore, the purpose of this study is to present a method that can make risk assessment easier and simpler before handling new chemicals so that it can be used well in small and medium-sized businesses that lack professional manpower related to chemical substances. Through actual cases, the method of using CHARM was confirmed step by step, and based on this, risk reduction establishment and execution methods were derived. Based on the results of this study, it is expected that it will be used as the best tool for preventing accidents such as acute poisoning when handling chemicals in small and medium-sized workplaces that lack professional manpower related to chemical substances and make every effort to protect workers' health.

A Study on the Death Accident Analysis of Ladder and Prevention Measures for Fall Accidents (산업현장 사다리 관련 사망재해 분석 및 추락재해 예방대책에 관한 연구)

  • Among the industrial disasters caused by drop, ladder related drop disasters are occurred the mostly. The victims are occurred continuously. This study analyzed current situation of industrial disasters for recent 10 years(2005~2014) and compared analyzed statistics of death disasters of ladder by workplace scale, age, occupation, employment type, working content, scarred area, etc. in detail. This study suggested direction of safety standard modification for ladder that is an original cause material of many drops, direction of safety training strengthening of small-scaled workplace, and safety model based on disaster statistics and should contribute to reduction of disaster rate for ladder working.

A Study on the Occurrences and Policy Development for Accident Prevention (한국의 사고발생 실태와 사고예방을 위한 정책 연구)

  • There is no national system to manage, evaluate, and analyse the information about accidents, even though it is necessary for accident prevention policies and health promotion of the general public. The existing studies are, also limited as they focus only on particular group(for example : inpatients, emergency room patients) or on particular geographic areas. Thus the results of the studies cannot be ap-plied to the general public. In order to overcome these limitations, this research focuses on data collection and analysis from accident information for the general population. By providing the analysis on types and causes af accidents, this research aims to produce the basic data necessary for accident prevention policy development. The specific aims of this research are to : 1. Analyse the actual occurrences and characteristics of accidents. 2. Suggest for the accident prevention policies and safety education. Accident report form three major newspaper printed in Korea between January 1, 1989 and December 31, 1993 were collected, and the cause, place, time, and personal injury related to the accidents were classified and then analyzed by de-scriptive statistics. The results of this research conclude : 1. The number of accidents reported by the three newspapers were 2155. 2. The highest proportion of accidents were as follows ; occurred during the June-August(31.2%), Sunday(24.8%), and 5 p.m. (7.6%) of the day. 3. The highest proportion of the accident occurred in Seoul(33. l%), Kyunggi province(14.5%), Kangwon province (7.8%), and Kyungnam prov-ince(6.7%) were next highest. 4. The main causes include car accidents(32.4%), drowning(10.9%), falls(8.8%), explosion(7.1%), and poisoning(5.8%). 5. Slightly more than half of injuries(50.5%) and about two fifths of deaths(40.3%) were caused by car accidents. Therefore, the most serious type of accidents were car accidents. 6. The number of males in accidents were almost three times higher than that of females(males 72.3%, females : 27.7%). 7. The age group from 10 to 19 years old represents the highest proportion(21%) of accidents. 8. The number of the accidents in Korea, based on the information collected from the newspapers, is estimated to be 14, 367 per year. The number of in-juries is estimated to be 88, 480 persons, and the number of deaths 29, 007 respectively. It is said that ninety percent of accidents can be prevented. Several accidents prevention policies are suggested here. 1. Safety education should be done more actively throughtout life, with special emphasis on safety education for children. 2. Safety measures for children(halmets for cycling, children car seats, seats belts, and so on) should be emphasized. 3. An injury surveillance system should be initiated. The initiation of injury reporting system in each factory and school could contribute considerably to the reduction of accidents.

Development of Standard Risk Indicators for the Prevention of Serious Accidents in Mobile Crane Operations (Focused on Construction Industry) (이동식크레인 작업의 중대재해예방을 위한 표준 리스크 평가 지수 개발 (건설업을 중심으로))

  • Purpose: Mobile cranes are machines that contribute to high mortality, and the High Risk Factor (SIF) information, which analyzed 2,574 accidental deaths in the construction industry in the past 6 years (2016~2021), resulted in a total of 61 mobile crane accidents. Despite safety measures in the field, it is not used properly. In this study, we present standard risk assessment indicators that contribute to accident prevention. Method: Through expert interviews, fatal accident case analysis, field analysis, and literature research, we present the standard risk assessment index method of the 4M risk assessment method. Result: As a result of analyzing the risk assessment of eight sites, it was concluded that it cannot make a significant contribution to disaster prevention and should be applied as an improvement measure of the Standard Risk Assessment Index Law. Conclusion: Switching to the standard risk assessment index method at construction sites has been proposed to make it easier for health and safety personnel and workers to use, contributing to the reduction of accidents.
