A CLINICAL STUDY ON THE CONDYLRAR FRACTURES OF MANDIBLE (하악과두 골절에 대한 임상적 연구)

  • The author investigated the 162 patients with the condylar fractres of the mandible who were admitted in Dept. of Oral and Maxillofacial Surgery, Hospital of Chosun Dental School from January 1985 to September 1992 clinically, including fracture incidennce, age and sex of the patients, causes of injures, associated injures, treatments and complications and obtained the following results. 1. Of the 449 patients with mandibular fractures, 162patients suffered condylar fractures(36.1%). 2. Of the 162 patients, males with condylar fractures were 128 patients, by a ratio of 3.8 : 1. 3. The most frequently affected age group was the third decade(32.7%). 4. Falls were ranked as the predominant cause(50.6%), followed by traffic accident(27.8%) and violence (16.0%). 5. The incidence of single condylar fractures was 28% and symphysis fractures were the most commomest of the concomittant injuries(60%). 6. The subcondylar fractures occurred most frequently(41.6%) and anteromedial displacement of the condylar fragments occurred most frequently(45.4%) 7. Of the all condylar fracture patients, children under 15 of age comprized 31 patients(19.1%) and condylar head fractures occurred most frequently at those children. 8. Of the managetments in condylar fractures, open reduction was 52.5% and remainder were closed reduction(47.5%). 9. Complications ensured such as TMJ ankylosis, limitation of mouth opening and mandibular movements, TMJ dysfunction, and anterior open bite.

Management of Ipsilateral Fractures of Humerus and Forearm in Adults (성인에서 동측에 발생한 상완골과 전완골 골절의 치료)

  • Concomitant ipsilateral fractures of the humerus, radius and ulna are uncommon combined injury and are also called "floating elbow". It was found that this injury was usually a result of rather severe trauma and frequently associated injuries to other organ systems. It is controversial in the treatment of the "floating elbow", but the current treatment recommendations are open reduction and internal fixation of both the humerus and the forearm fracture with early initiation of range of motion exercises. The authors reviewed thirteen cases of ipsilateral fractures of the humerus, radius and ulna treated in our clinic from January 1992 to March 1997, and average follow-up period was over 18 months(range, 12 to 36 months). The results obtained were as follows; 1. The most common cause of injury was traffic accident and most common location of fractures was mid-third in both humerus and forearm. 2. The shape of fractures was transverse or comminuted in most cases. 3. The good clinical results were obtained by open reduction and internal fixation of both the humerus and the forearm fracture with early initiation of range of motion exercises. 4. The recovery was affected by the severity of the initial trauma and method of the treatment. 5. According to the Lange and Foster method, the functional result was good in 8 cases, fair in 4 cases and poor in 1 cases.

  • Workers in industrial fields are highly exposed to accidents or injuries caused by long working hours. An exoskeleton that is able to support the arm muscles of the worker and thereby reduce the probability of an accident and enhance working efficiency could be a solution to this problem. However, existing exoskeletons demand the use of high-priced sensors and motors, which makes them difficult to use in industrial fields. To solve this problem, we developed an arm assisting exoskeleton that consists only of mechanical components without any electronic sensors or motors. The exoskeleton follows the movement of the human arm by shoulder joint and ankle joint. In addition, counterbalance mechanisms are installed on the exoskeleton to support arm strength. The experimental validation of the exoskeleton was conducted using an EMG sensor, confirming the performance of the exoskeleton.

Development of Fast Side-impact Sensing Algorithm (고속 측면 충돌 감지 알고리즘의 개발)

  • Accident statistics shows that the portion of fatal occupant injuries due to side impacts is considerably high. The side impact usually leads to a severe intrusion of side structure into the passenger compartment. Furthermore, the safety zone for the side impact is relatively small compared to the front impact. Those kinds of physics for side impact frequently result in a fatal injury for the occupant. Therefore, NHTSA and EEVC are trying to intensify the regulation for the occupant protection against side impact. Both the regulation and recent market trends are asking for an installation of side airbag. There are several types of system configuration for side impact sensing. In this paper, we adopt the acceleration-based remote sensing method for the side airbag control system. We mainly focus on the development of hardware and crash discrimination algorithm of remote sensing unit. The crash discrimination algorithm needs fast decision of airbag firing especially for high-speed side impact such as FMVSS 214 and EEVC tests. It is also required to distinguish between low-speed fire and no-fire events. The algorithm should have a sufficient safety margin against any misuse situation such as hammer blow, door slam, etc. This paper introduces several firing criteria such as acceleration. velocity and energy criteria that use physical value proportional to crash severity. We have made a simulation program by using Matlab/Simulink to implement the proposed algorithm. We have conducted an algorithm calibration by using real crash data for 2,500cc vehicle. The crash performance obtained by the simulation was verified through a pulse injection method. It turned out that the results satisfied the system requirements well.

Clinical Analysis of Blunt Abdominal Trauma in Childhood (소아에서 발생한 복부 둔상의 임상적 고찰)

  • Traumatic injury is one of the leading causes of morbidity and mortality in children. This is a clinical review of pediatric blunt abdominal trauma. A retrospective analysis of the 112 children with blunt abdominal trauma aged 15 years or less treated at the Department of Pediatric Surgery, Chonbuk National University Hospital was performed. The analysis included age, sex, injury mechanism, number and site of the injured organ, management and outcomes. The average age of occurrence was 7.6 years, and the peak age was between 6 and 8 years. There was a male preponderance with a male to female ratio of 2.3:1. The most common cause of blunt abdominal trauma was traffic accidents (61.6 %), principally involving pedestrians (79.7 %). The accident prone times were between 8:00 AM and 8:00 PM, the weekends (40.2 %), and the winter respectively. Thirthy-five patients (31.2 %) had multiple intra-abdominal organ injuries and the most common injured organ was the liver. Seventy-four cases (66.1 %) were managed non-operatively and eleven cases (9.8 %) expired. Of the patients who were treated surgically or were to be operated on one patient died before surgery, the remainder died during or after surgery. Risk factors such as number of injured organ, systolic and diastolic blood pressure, and trauma scores by Glasgow coma scale (GCS), Pediatric trauma score (PTS), revised trauma score (RTS), injury severe score (ISS), TRISS were significantly correlated with mortality rate.

Blunt Abdominal Trauma in Children (소아 복부 외상에 관한 임상적 고찰)

  • Trauma is one of the leading causes of death in children. Abdominal trauma is about 10 % of all pediatric trauma. This study describes the sex and age distribution, injury mechanism, site of intraabdominal injury, management and mortality of children aged 16 years or less who suffered abdominal trauma. The hospital records of 63 patients treated for abdominal injury between March 1997 and February 2007 at the department of surgery, Inje University Pusan Paik Hospital, were analyzed retrospectively. The peak age of incidence was between 2 and 10 years (78%) and this report showed male predominance(2.7:1). The most common mechanism of blunt abdominal trauma was pedestrian traffic accident (49%). The most common injured organ was liver. More than Grade IV injury of liver and spleen comprised of 4(12%) and 5(24%), respectively. Fourteen cases (22%) had multiple organ injuries. Forty nine cases (78%) were managed nonoperatively. Three patients (4.8%) died, who had Grade IV liver injury, Grade IV spleen injury, and liver and spleen injury with combined inferior vena cava injury, respectively. All of the three mortality cases had operative management. In conclusion, the liver or spleen injury which was more than Grade 4 might lead to mortality in spite of operation, although many cases could be improved by nonoperative management.

  • This study conducted a questionnaire survey on the real condition of clothing with focus on related to general motorcycle wear and motorcycle jacket targeting a quick service carrier affiliated with a quick service business in Seoul. In addition, this study is aimed at providing basic data on developing the motorcycle jacket, whose motional flexibility, safety and functionality are excellent, exclusively for a quick service carrier by grasping inconveniences and problems and deducting improvements on the basis of the questionnaire survey. This study, on the basis of the questionnaire survey results, grasped the general part related to quick service and motorcycle wear, such as their general matters, whether they were having on the motorcycle wear in the middle of doing business, whether it's necessary to wear the motorcycle wear, where they had a driving accident, and kinds of external injuries, etc. From the gathered results of analysis of the collected questionnaires, the item which got the lowest satisfaction was the inconvenience from the chafed front neck when driving. Besides, the results showed carriers' complaints like the elbow part felt tight, discomfort in the horizontal movement of the shoulders or back, and wind admission in between zippers. In addition, the respondents showed complaints in the item about hygroscopic property and air permeability at the armpits and back part, and 5 items about material flexibility, wind shielding property, living water repellency, weighty sensation, and night visibility were found to be low in respondents' satisfaction.

  • Forensic engineering is the area of expertise of people qualified to serve as engineering experts in courts of law or during arbitration proceedings. An aerial-lifter can lift and carry load, including people, using power. Recently, failure of aerial-lifter internal parts while working and sweeping causing injuries and damage to property almost always generates conflict between the automaker and customer. Hence, the investigation of such events generally involves an engineering analysis. One of the possible reasons for accidents, such as a vehicle catching fire is the failure of oil pressure machine and the supporting pin. The results of formal inspections and engineering tests can reveal the cause for the failure of the mechanical parts. Therefore, the failure mechanism is analyzed by adopting fractography methods and by applying an instrumented indentation technique to compare the material properties of the reference part with those of the malfunctioning part.

A Case of Post-Traumatic Pulmonary Pseudocyst Mimicking Pulmonary Cavitary Tuberculosis (결핵성 공동으로 오인된 외상 후 발생한 가성 폐낭종 1예)

  • A traumatic pulmonary pseudocyst is a rare complication of blunt thoracic trauma. The clinical symptoms and signs are similar to other respiratory diseases, such as pulmonary tuberculosis. Therefore, a trauma history with the resulting radiologic and clinical findings is important for making a diagnosis. A 26-year-old male was admitted to our hospital due to cough for 3 days. The chest x-ray revealed diffuse infiltrations and a cavitary lesion at the left lung. His left chest had hit a tree as a result of motorcycle accident one day before admission. Initially, it was assumed that his symptoms and chest X-ray might be due to a tuberculosis infection. However, bronchoscopy revealed old blood clots at both lungs, particularly in the left lower lobe bronchus. A transbronchial lung biopsy showed alveolar hemorrhage. A traumatic pulmonary pseudocyst was diagnosed from his trauma history and these findings. Computed tomography of the chest performed 4 months later showed regression of the cavitary lesion.

Heat Transfer Characteristics of Bulkhead Penetration Piece for A60 Class Compartment I: Transient Thermal (A60급 구획 적용 격벽 관통용 관의 열전달 특성 I: 관의 설계에 따른 과도 열해석)

  • In order to protect lives and prevent large-scale injuries in the event of a fire on a ship or an offshore plant, most classification societies are strengthening their fire resistance designs of relevant cargo holds and accommodation compartments to keep flames from being transferred from a fire point to other compartments. Particularly in critical compartments, where flames should not propagate for a certain period of time, such as the A60 class division, both the airtightness and fire-resistant design of a piece passing through a bulkhead are subject to the Safety of Life at Sea Convention (SOLAS) issued by the International Maritime Organization (IMO). In order to verify the suitability of a fire-resistant design for such a penetrating piece, the fire test procedure prescribed by the Maritime Safety Committee (MSC) must be carried out. However, a numerical simulation should first be conducted to minimize the time and cost of the fire resistance test. In this study, transient thermal analyses based on the finite element method were applied to investigate the heat transfer characteristics of a bulkhead penetration piece for the A60 class compartment. In order to determine a rational bulkhead penetration piece design, the transient heat transfer characteristics according to the variation of design parameters such as the diameter, length, and material were reviewed. The verification of the design specification based on a numerical analysis of the transient heat transfer performed in this study will be discussed in the following research paper for the actual fire protection test of the A60 class bulkhead penetration piece.
