For monitoring the status of industrial accidents, many statistical indexes have been developed and applied such as fatal rate, frequency rate, and severity rate. These accident indexes are measured by frequency and loss time according to the accidents in the individual industry level. However, it is less considered to use the index of identifying the industrial concentration of accidents in the holistic view. Thus, this study aims to suggest the accident concentration level among domestic industries through index analysis. The concentration level of industrial accidents is calculated by the accident composition of sub-industries. This concentration level shows whether an industry is comprised of a few sub-industries generating more accidents or an industry consists of sub-industries having the similar number of accidents. To this end, the concentration rate (CR) and concentration index (CI) are proposed to take a look at the industry composition of accidents by embracing the concept of market concentration indexes such as Hirschman-Herfindahl Index. As for the case study, four industries of mining, manufacturing, transportation, and other business (usually service) are analyzed in terms of indexes of accident rate, death(fatality) rate, and CR and CI of accident and death. Finally, we illustrate the positioning map that the accident concentration level is compared with the traditional accident frequency level among industries.

The concentration of the HNS Accident for each region was confirmed to prepare against an HNS Spill accident by using a Concentration Index which is used to assess industry concentration trend. This is to present the HNS Accident Concentration Index by combining HNS Accident Scale Concentration Index and an HNS Accident Frequency Concentration Index based on the data of marine spill accidents including the HNS accident. Based on the HNS Accident Concentration Index, Ulsan was identified as a top priority region for preparedness, Yeosu, Busan and Taean were identified as priority regions for preparedness, Gunsan, Mokpo, Wando, Incheon, Tongyeong, Pyeongtaek and Pohang were identified as necessary regions for preparedness, Donghae, Boryeong, Buan, Seogwipo, Sokcho, Jeju and Changwon, in which no marine spill accidents occurred, were identified as support regions for preparedness.
Background: From only frequency rate of industrial accidents, it is difficult to define the industry composition of accident statistics in a nation. This study aims to propose and develop a new index for measuring the degree of concentration of industrial accidents using the concept of the Herfindahl-Hirschman Index in the case of European countries. Methods: Using the concept of the Herfindahl-Hirschman Index, the concentration index of accidents in the country is developed, and the conditions of European countries are compared using indexes of frequency rate and concentration ratio. Results: The frequency rate and concentration ratio of fatal and nonfatal accidents in European countries are compared. According to the economic condition and geographical position, different patterns of accidents concentration are presented in terms of frequency rate and concentration ratio. Conclusion: We develop the concentration index of industrial and occupational accidents that identifies the industrial ratio of accident occurrence, and the differentiated strategy can be formulated such as approaches to reducing frequency and prioritizing target industries.
Recently, various engineering approaches have been widely used in the accident investigation field to identify the cause of the accident and to predict damage by accident. Computational analysis is the most commonly used method of accident investigation technique. This technique is mainly used to identify the mechanism of the accident generation and to determine the cause when it is difficult to reproduce the situation at the time of the accident or when it is impossible to perform a reproduction experiment. In this study, The computational fluid dynamics analysis for nitrogen asphyxiation accident generated by defect of building structural between diffusion outlet and cooling tower was performed to determine the inflow path of the suffocation gas, death possibility by concentration of suffocation gas and predicted the time of death due to the accident using 3D modeling and FLACS program. We can quantify diffusion concentration of asphyxiation gas and predict mechanism of death occurrence by accident and evaluate the consequence Analysis through this study. In the future, This method can be widely used in the field of gas safety by improving the reliability and validity of the analysis.

Recently, severe accidents in nuclear power plants (NPPs) have become a global concern. The aim of this paper is to predict the hydrogen buildup within containment resulting from severe accidents. The prediction was based on NPPs of an optimized power reactor 1,000. The increase in the hydrogen concentration in severe accidents is one of the major factors that threaten the integrity of the containment. A method using a fuzzy neural network (FNN) was applied to predict the hydrogen concentration in the containment. The FNN model was developed and verified based on simulation data acquired by simulating MAAP4 code for optimized power reactor 1,000. The FNN model is expected to assist operators to prevent a hydrogen explosion in severe accident situations and manage the accident properly because they are able to predict the changes in the trend of hydrogen concentration at the beginning of real accidents by using the developed FNN model.

Objectives: Acute exposure to high concentrations of chemicals can occur when a chemical accident takes place. As such exposure can cause ongoing environmental pollution, such as in the soil and groundwater, there is a need for a tool that can assess health effects in the long term. The purpose of this study was assessing the health risks of residents living near a chemical accident site due to long-term exposure while considering the temporal concentration changes of the toxic chemicals leaked during the accident until their extinction in the environment using a multimedia environmental dynamics model. Methods: A health risk assessment was conducted on three cases of formaldehyde chemical accidents. In this study, health risk assessment was performed using a multimedia environmental dynamics model that considers the behavior of the atmosphere, soil, and water. In addition, the extinction period of formaldehyde in the environment was regarded as extinction in the environment when the concentration in the air and soil fell below the background concentration prior to the accident. The subjects of health risk assessment were classified into four groups according to age: 0-9 years old, 10-18 years old, 19-64 years old, and over 65 years old. Carcinogenic risk assessment by respiratory exposure and non-carcinogenic risk assessment by soil intake were conducted as well. Results: In the assessment of carcinogenic risk due to respiratory exposure, the excess carcinogenic risk did not exceed 1.0×10-6 in all three chemical accidents, so there was no health effect due to the formaldehyde chemical accident. As a result of the evaluation of non-carcinogenic risk due to soil intake, none of the three chemical accidents had a risk index of 1, so there was no health effect. For all three chemical accidents, the excess cancer risk and hazard index were the highest in the age group 0-9. Next, 10-18 years old, 65 years old or older, and 19-64 years old showed the highest risk. Conclusion: This study considers environmental changes after a chemical accident occurs and until the substance disappears from the environment. It also conducts a health risk assessment by reflecting the characteristics of the long-term persistence and concentration change over time. It is thought that it is of significance as a health risk assessment study reflecting the exposure characteristics of the accident substance for an actual chemical accident.
Background: A model to assess the activity concentration of agricultural products and the public ingestion dose as result of a nuclear accident is necessarily required to manage the contaminated agricultural systems by the accident, or to estimate the effects of chronic exposure due to food ingestion at a Level 3 PSA. Materials and Methods: A dynamic compartment model, which is composed of three sub-modules, namely, an agricultural plant contamination assessment model, an animal product contamination assessment model, and an ingestion dose assessment model has been developed based on Korean farming characteristics such as the growth characteristics of rice and stockbreeding. Results and Discussion: The application study showed that the present model can predict well the characteristics of the activity concentration for agricultural products and ingestion dose depending on the deposition date. Conclusion: The present model is very useful to predict the radioactivity concentration of agricultural foodstuffs and public ingestion dose as consequence of a nuclear accident. Consequently, it is expected to be used effectively as a module for the ingestion dose calculation of the Korean agricultural contamination management system as well as the Level 3 PSA code, which is currently being developed.
Backgrounds: The accident at Fukushima Daiichi Nuclear Power Plant (NPP), March 2011, caused serious radioactive contamination over wide area in east Japan. Therefore, it is important to know the effect of the accident and the status of NPP. Materials and Methods: This paper provides a review on the status of radiation dose and radioactive contamination caused by the accident on the basis of publicized information. Results and Discussion: Monitoring of radiation dose and exposure dose of residents has been conducted extensively by the governments and various organizations. The effective dose of general residents due to the accident proved to be less than a mSv both for external and internal dose. The equivalent committed dose of thyroid was evaluated to be a few mSv in mean value and less than 50 mSv even for children. Monitoring of radioactivity concentration has been carried out on food ingredients, milk and tap water, and actual meal. These studies indicated the percentage of foods above the regulation standard was over 10% in 2011 but decreasing steadily with time. The internal dose due to foods proved to be tens of ${\mu}Sv$ and much less than that due to natural $^{40}K$ even in the Fukushima area and decreasing steadily, although high level concentration is still observed in wild plants, wild mushrooms, animals and some kind of fishes. Conclusion: According to extensive studies, not only the effect of the accident but also the pathway and countermeasures against radioactive contamination have been revealed, and they are applied very effectively for restoration of environment and reconstruction of the area.

Silicone tetrachloride (SiCl4) leak accidents cause enormous human and environmental damage because it is highly toxic. Some handling facilities use water curtains to reduce the impact range of SiCl4. Although the water curtain is known as one of the most efficient technologies for post-release mitigation, its effect on reducing SiCl4 concentration needs to be investigated scientifically and quantitatively. In this study, three-dimensional computational fluid dynamics (CFD) was used to investigate the physical and chemical effects of water curtains as a release-mitigation system for SiCl4. SiCl4 is released and dispersed five seconds prior to the operation of the water curtain. Once the water curtain works, the SiCl4 reacts chemically with the water and its concentration decreases rapidly; it reaches an emergency response planning guidelines level 2 (ERPG-2) of 5 parts per million (ppm) at about 570 m. We observed, however, that the physical effect of water curtains on reducing SiCl4 concentration is insignificant when the chemical effect is eliminated. These results are crucial since they can be a scientific and quantitative basis for the 'technical guidelines for estimating the accident affected range'. In order to protect the public from chemical accidents, more toxic gas mitigation technologies need to be developed.
In the last five years, 45 people died and 104 were wounded because of carbon monoxide poisoning accident. CO poisoning accident is higher than any other gas accident in the rate of deaths/incidents. Most of these CO poisoning accidents were caused by defective exhaust tube in the old gas boiler and multi-use facility. In this study, the spread of CO gas released from leakage hole of exhaust tube was analyzed by computational flow modeling and concentration measuring test. CO gas leaked form exhaust tube in a building was highest concentrated near the ceiling and formed the circular currents along the walls. Through these experiments and simulation, the reasonable installation location of CO alarm was made certain and suggested.
