가정방문 물리치료의 필요성 및 요구하는 서비스의 특성 (Necessity and Features of Service Required for Home Visiting Physical Therapy)

  • This study was performed to investigate the degree of demand and general features of services required of home visiting physical therapy for chronic ill patients. The study subjects were ambulatory and admitted patients treated with physical therapy at six general and one oriental hospitals, one welfare center, four health centers located in Taejon from March 2, 1999 to March 16. Authors developed structured questionnaire, and distributed it to each physical therapist of study organizations. Total number of distributed questionnaire was 500, and 405 questionnaire were collected and analysed finally. 1. $82.4\%$ of patients and $90.0\%$ of caregivers are showed that home visiting physical therapy was needed(p<0.05) 2. The rate of necessity for home visiting physical therapy by kinds of disease was $949\%$ in cerebral palsy, $95.0\%$ in upper spinal cord injury, $83.3\%$ in lower spinal cord injury, $84.5\%$ in cerebral vascular accident, $89.6\%$ in traumatic brain injury, $83.5\%$ in other diseases. 3. In the general features of required service for home visiting physical therapy, $33.7\%$ of patients and $34.4\%$ of caregivers want special isolated physical therapy center, $33.1\%$ of patients and $43.3\%$ of caregivers want 3 times per week in frequency, $46.7\%$ of patients and $45.0\%$ of caregivers want 30-60minutes in treatment duration, and $48.0\%$ of patients and $46.7\%$ of caregivers want more intensive care than general hospitals. 4. In the working place of home visiting physical therapy, $36.1\%$ of patients and $36.2\%$ of caregivers wants physical therapist worked in general hospital. Also, $53.3\%$ of patients and $52.2\%$ of caregivers answered no interested in physical therapist's gender. The most preferential age of home visiting physical therapist is thirties in $43.2\%$ of patients and $63.4\%$ in caregivers

Poisson분포를 이용한 도시가스 화재 폭발사고의 발생 예측프로그램 및 사회적 위험기준에 관한 연구 (Study on Predictable Program of Fire.Explosion Accident Using Poisson Distribution Function & Societal Risk Criteria in City Gas)

  • 가스로 인한 화재 및 폭발사고의 예측 가능한 프로그램을 구체화하고, 적절한 사회적 위험기준을 제시하기 위하여 최근 11년간의 가스사고 데이터베이스를 분석하였다. 먼저 동일유형의 가스사고 발생가능 성을 판단할 수 있는 Poisson 분석 방법을 적용하기 위해 3개의 사고유형, 즉 누출, 폭발, 화재로 구분하여 총 16개 항목으로 나누어서 적용하였다. 그 결과, 시공 작업 부주의-폭발-배관의 항목의 사고발생반도가 가장 작았으며, 배관연결부이완부식-누출-배관의 경우는 가장 높은 빈도를 나타내었다. 따라서 이에 대한 적절한 가스사고 대응책이 마련되어야 할 것이다. 또한 치명적인 가스사고의 추세와 합리적인 위험정책에 대한 지침을 결정하기위해 D. O. Hagon 방정식과 회기직선식을 이용하여 F.N 곡선의 누적사망자의 최소 및 최대점에 대응시켜 허용가능영역을 설정하였다. 향후 가스사고에 대한 신뢰성 있는 분석을 위해서는 가스로 인한 화재 폭발사고에 대한 데이터베이스를 지속적으로 확충보완을 시켜야 되며, 이를 위한 표준 코드화 작업이 요구된다.

뇌졸중 환자 대상 흡인 음성의 음향변수 변동 (Acoustic parameter delta of an aspirated voice in stroke patients)

  • The present study aimed to investigate the changes of acoustic parameters of the aspirated voice in stroke patients. The eighty-eight subjects diagnosed with cerebro-vascular accident were divided into 32 penetration/aspiration (P/A) and 56 Non-P/A groups according to the videofluroscopic swallowing study (VFSS) results, and 26 control subjects participated. All subjects preformed VFSS and vowel /a/ was recorded three times pre- and post VFSS. Since the variation in the acoustic parameters within a single phonation has been observed, we proposed a delta formula for the acoustic parameters which can reflect the temporal changes of the each parameter in an utterance. We measured from the voice data eight acoustic parameters: fundamental frequency (F0), standard deviation of F0 (F0_SD), Jitter, relative average perturbation (RAP), Shimmer, amplitude perturbation quotient (APQ), harmonic to noise ration (HNR), noise to harmonic ratio (NHR). Then we found parameters which show the meaningful biggest temporal change in an utterance using the suggested delta parameter. Among them, the deltas of shimmer and APQ were significantly different pre- and post VFSS. These deltas of the P/A and the control group were increased after VFSS, while those of the Non-P/A group was descended. The variation patterns of the P/A and the control group were similar but the change width of the P/A group was larger. The large variations in an aspirated phonation of the P/A group are thought to be caused by irregular changes in air resistance due to residual food on the vocal cords.

급성약물중독에 관한 임상적 고찰 (Clinical Observation of Acute Drug Intoxications)

  • Clinical observations were made on 349 cases of acute drug intoxication who were visited to emergency room of Yeungnam University Hospital during recent 7 years from January 1984 to December 1990. The following results were obtained 1) Total number of cases of acute drug intoxication was 349 which was 0.39% of the total patients of the emergency room during the same period. 2) The ratio of male to female was 1.1 : 1. The age incidence was highest in the third decade(26.7%). The monthly incidence was hightest in May. Higher frequency was observed in summer season. 3) The most common drug of the intoxication was pesticides and herbicides(71.9%), the remainders were miscellaneous drugs(11.2%), sedatives(7.7%), rodenticides(6.3%) and unknown drug(3.2%) in orders. 4) The most common cause of drug intoxication was suicide(69.1%) and the others were accident, unknown cause, intention in orders. 5) Main clinical manifestations were the impairment of consciousness, nausea, vomiting and convulsion. Physical examination revealed increased pulses, increased blood pressure, miosis of the pupil and sweating. Above symptoms and signs were more prominent in pesticide intoxication. Leukocytosis, glycosuria and abnormal LFT were common findings in acute intoxications. 6) The complications were developed in 18.3% among 349 cases and the most common complication was respiratory failure, pneumonia, cardiovascular collapse and pulmonary edema in orders. 7) Overall mortality rate was 8.3% of total cases and mortality rate was highest in herbicide intoxication(22.2%).

관골 골절의 양상 및 정도에 관한 임상적 연구 (A CLINICAL STUDY ABOUT NATURE AND SEVERITY OF THE ZYGOMATIC BONE FRACTURES)

  • This is a retrospective study on the nature and severity of zygomatic bone fractures. This study was based on a series of 358 patients with zygomatic bone fractures who treated as in-patient at Soonchunhyang Chonan Hospital during the period of Jan. 1993 through Dec. 1996. The results obtained are as follows : 1. The ratio of men to women was 4.41:1. The age frequency was highest in the second, third decade. The yearly distribution was the highest in 1993. The monthly distribution was highest in Feb. and Jun. 2. The most frequent cause was the traffic accident.(66.2%) 3. The most frequent type of fractures was the class I(undisplaced fx.).(25%) 4. The most frequent associated oral and maxillofacial injuries was soft tissue.(46.9%) 5. The most frequent associated systemic injuries was upper, lower extremity.(38%) 6. In A.I.S of oral and maxillofacial area, the mean score of A.I.S was $2.5{\sim}3$. In I.S.S, the highest mean score was in the ped TA.(19) 7. In respect of treatment, the most common method was open reduction with rigid fixation on fronto-zygomatic suture area.(47.8%) 8. The incidence of complication was 12% and the most common complication was malunion.(6.1%)

손상감시체계를 통한 천안지역 초․중․고교생의 손상실태 분석 (Students injuries and Injury Surveillance System in Cheonan)

  • Purpose : The purpose of this study is to explore the students injuries by analyzing the data which has been inputted by the emergency center of the cooperated hospitals and the 119 rescue party through the injury surveillance system in Cheonan city. Method : Students were divided into the elementary, middle, high school students with the 776 cases of children and teenagers(7-19years old) of injury surveillance system in Cheonan area from january to june in 2009. Frequency analysis and $x^2$-test was done to recognize the features of students injuries among the groups. The program to be used for the statistical analysis is SPSS 17.0. Result : Out of the injury incidence rate, the elementary school students(52.1%) are first, the high school students (24.9%) are second, the middle school students appear to be 23.1%. Male is about two times higher than female by 66.6% in the injury incidence. In terms of the injury mechanism, the injury(22.2%) by hit is the first, the traffic accident(21.5%) is the second, the slippery(16.8%) is followed. The injuries were occurred most largely at 16:00-20:00(33.4%), and the 33.6% of injury by daily leisure activity occurs at 16:00-20:00 chiefly. Conclusion : Analysis using the data of the injury surveillance system has some advantages compared to the previous research such as reliability and specification. To prevent the students injuries, not the individual problem but the social dimension should be acknowledged so that we can secure and promote the safety from the risk. Therefore, we must organize the role assignment and the cooperative network in the school, home and community.

국가지휘 무선통신망 구축 운영방안에 관한 연구 (A Study on the National Command Wireless Communication Network Construction and Operation)

  • When the national disaster accident is occurred, it is difficult to maintain the mutual cooperation systems. In order to solve the problems, the construction of the national unified command wireless network is necessary. In this paper, the specified state of the characteristic frequency of the digital TRS wireless network constructed recently is investigated and analyzed. Through the analysis, the problems of the construction of the national unified command wireless network are grasped. To solve the problem, it is proposed that the digital TRS wireless network is connected with the satellite communication network, and connected with the existing wireless network, LMR. In the concretely it is proposed that the natural unified wireless network should be proceeded step by step. At first, for 2 years the existing networks of the Fire Fighting Agency, the Police, the Forest Service and so on must be utilized and prepared to link with TRS. The second, for 2 years it is carried forward a scheme to maintain the properties of the agencies concerned. Further, it must be prepared to connect with satellite network. At third, for 2 years all agencies concerned with the fire fighting and the disaster prevention must be unified, and the systems have to be promoted for the p1an of linkage of TRS network and the existing network. Next the agencies concerned have to be unified and the authority has to be intensified. When a disaster is occurred, the National Emergency Management Agency has to play a central role. In a local area it has to be given the Fire Fighting Agency an authority and a duty to get ready for each emergency situation.

도난방지 및 구조신호 전송기능이 있는 저가형 농용트랙터 블랙박스 개발 (Development of an Inexpensive Black Box with Transmission of SOS and Theft Signal for an Agricultural Tractor)

  • Purpose: The inexpensive black box system was developed to acquire and save driving information, to give the slope information, and to transmit SOS and theft signal. Method: The device consists of a main micro controller to acquire and save data, a GPS sensor module, a CDMA module, a touch LCD module, a RF (Radio Frequency) ID module, a SD (Secure Digital) card module, an emergency electric power source, a theftproof circuit, and a sensing device. The sensing device consists of a 8 bit micro controller, a accelerometer to detect impulse, two slope sensors to detect roll and pitch angle and a circuit to detect operation of 6 lighting devices. Results: Test results are as follows: 1) a tractor can be start up only with an electronic key (password or RFID card), 2) theft signal was transmitted when a tractor moved without an electronic key, 3) SOS was transmitted at conditions that rollover or crash happened. 4) 5 more than per 1s data are recorded at 5 minute intervals as new file name in SD card. Conclusions: This system can be used to save travelling record, reduce accident, prevent theft and rescue life in the accidents.

도시 교통문제에 대한 연구 -천안시를 중심으로- (An Analysis on the Traffic Problems of the Urban City)

  • 본 연구는 천안시를 대상으로 도시교통문제를 다루었다. 본 연구는 문헌조사와 실증조사를 병행하였다. 회수된 설문지 394부를 SPSS 15.0버전으로 빈도분석을 실시하였다. 천안시의 도시교통문제는 출 퇴근시의 교통체증 현상, 대중교통의 불편, 주차문제, 자동차로 인한 대기오염, 교통사고로 인한 불안 등을 들 수 있다. 교통문제의 개선방안으로는, 첫째, 합리적 교통수요의 관리에 따른 교통체계의 개선방안이 마련되어야 한다. 둘째, 천안시는 대중교통의 이용대상인 대학생이 많으므로 대학생을 대상으로 할인을 적용 대중교통 이용률을 높여야 한다. 셋째, 대중교통중심체제의 강화 및 개선을 통한 교통체증의 해결이 필요하다. 넷째, 도로 등 교통시설의 공급확대, 가로망체계 정비 및 신호운영체계의 개선을 적극적으로 추진해 나가야 한다. 특히 천안에는 오래된 도로의 정비가 필요하고 도로망이 복잡하게 얽혀있는데 이를 간단하게 개선함으로써 보다 접근성이 좋아질 것이다. 다섯째, 주차문제의 해결과 이용자들의 교통질서 의식함양 등 선진 교통문화의 정착이 필요하다.

도·소매업종의 작업위험요인에 관한 연구 (A Survey for Ergonomic Risk Factors in Wholesale/Retail Establishments)

  • The purpose of this survey was to identify ergonomic risk factors of work-related musculoskeletal disorders (MSD) and understand the current MSD risk status in the wholesale/retail industry. The field survey was conducted at 120 wholesale/retail establishments, and the cause of MSD occurred from the national industrial accident statistics by the Ministry of Labor in 2006 was analyzed in terms of company size, a term of work, job title, and cause of occurrence. The numbers of disasters on MSD were 602 which were included 432 disasters who met with low back pain by incident. A checklist was developed by KOSHA to use for the field survey. A total of 419 tasks were identified at the 120 establishments surveyed. The highest frequency was Task 2(45%) which was mostly observed in repetitive work (e.g. displaying, scanning, cutting or wrapping of goods). The second and third were Task 9(34%) and Task 8(14%) which were identified in manual materials handling (e.g. lifting, lowering, carrying or stocking of goods). The results suggest that the three Tasks should be first considered for the control in wholesale/retail industry and that exposure to MSD risk factors in those Tasks should be primarily addressed in order to reduce MSD risk in that industry.
