옹벽 및 교대 신기술 특허 (New Patent Technology for Retaining Wall and Bridge Abutment)

  • 본 연구는 최근 국내의 옹벽 및 교대의 신기술 특허 등록 동향을 조사하여 파악하며, 이들 신기술특허를 적절히 실제 옹벽 및 교대 공사에 적용하려는데 목적을 두고 있다. 본 연구에서 조사된 신기술옹벽으로는 PS강봉에 프리스트레스를 도입한 단면력 저감형 조립식 PC옹벽공법, Coupler-Tension 조립식 옹벽, 도로용 블록식 보강토옹벽, 균등침하를 유도할 수 있는 블록식 보강토옹벽이 있으며, 신기술 교대로는 시트파일을 이용한 교대의 시공공법, 일체식 복합교대 교량, 반 일체식 교대교량의 시공방법, 보강토 교대구조를 기술하였다.

2-unit cantilever 레진접착성 가공의치 (resin-bonded fixed partial denture) 임상의 현재 (CLINICAL PERSPECTIVES ON 2-UNIT CANTILEVERED RESIN-BONDED FIXED PARTIAL DENTURE)

  • Resin-bonded bridge has been an alternative to conventional bridge, since resin-bonded bridge has many attractive advantages such as minimal tooth preparation, short chair time and low cost over conventional bridge. Unfortunately, however, it was reported that resin-bonded bridge showed high failure rate from debonding of retainer in spite of consecutive advances in preparation and materials. And it was shown that multiple abutments were more likely to fail. The majority of debonding failure was considered due to the mobility of the abutment during function. In this view, recently, modification in resin-bonded bridge design was tried. Single retainer, single pontic. 2-unit cantilevered resin-bonded bridge was applied to clinical performance and was shown as retentive or more retentive than fixed-fixed type resin-bonded bridge. This was consistent with the results of studies in 2-unit cantilevered resin-bonded bridges made with all ceramic, In-ceram. The purpose of this article was to overview principles of design and to analyze clinical results of 2-unit cantilevered resin-bonded bridge in comparison with the reports of fixed-fixed resin-bonded bridge.

Precision of the milled full-arch framework fabricated using pre-sintered soft alloy: A pilot study

  • PURPOSE. This study aimed to evaluate the marginal discrepancy of full-arch frameworks in implant-supported prostheses fabricated using pre-sintered soft alloy (PSA). MATERIALS AND METHODS. Full-arch metal frameworks were fabricated on the edentulous implant model using casting alloy (CA), fully-sintered hard alloy (FHA), and PSA (n = 4 in each group). To evaluate the misfit of the framework to the abutments, the absolute marginal discrepancy (AMD) values of the frameworks were measured in cross-sectional images that had been drawn as part of the triple-scan protocol. The AMD values were compared among the tested alloy groups using the Kruskal-Wallis test, with a post hoc Mann-Whitney U test (${\alpha}=.05$). RESULTS. The FHA and PSA groups showed lower marginal discrepancies than the CA group (P<.001). However, the FHA group did not differ significantly from the PSA group. CONCLUSION. Soft alloy milling is comparable to hard alloy milling, and it is more precise than casting in terms of the marginal fit of implant-supported, full-arch prostheses.

PSC 박스 거더 철도교량의 해석 및 스트럿-타이 모델에 의한 격벽부 설계 (Analysis of PSC Box Girder Railway Bridge and Design of its Diaphragm using Sturt-and-Tie Model)

  • The functions of diaphragms at abutments and piers of PSC box girder railway bridge are to transfer forces from the superstructure onto bearings or columns and to stiffen the superstructure cross-section against in -plane deformation. Due to stress disturbance at diaphragm, the design for the diaphragm using conventional design method is relatively irrational than those for other structural members. And, due to contribution to boundary condition of deck slab by the diaphragm, the behavior of deck slab near the diaphragm is different from that of the deck slab obtained from two dimensional analysis of the bridge, which is basis for the design of deck slab. In this paper, three dimensional behavior of deck slab near the diaphragm of prestressed concrete (PSC) box girder railway bridge constructed by the precast span method are analyzed by using three dimensional finite element modeling and using the strut-and-tie model design of the diaphragm are presented. The modeling techniques used in this paper can be applied effectively to examine the causes of cracks at deck slab near diaphragm and to design diaphragm rationally.

고정성 분할 브릿지의 연결 어태치먼트 (Connect Attachment of Fixed Segmented Bridge)

  • There are some cases that dental prosthesis does not operate as properly as expected in oral mouth. The reasons are such as a distortion of the mandibular, a fault of impression taking system or an extrusion of remaining teeth. One of dental prostheses to consider in the situations is the attachment which connects segment bridge. Active discussions are managed on theoretical side of this field but few on clinical side of it, which must be considered first. Accordingly I'd like to suggest a theoretical background for connect attachment of fixed segmented bridge. 1. As a bridge gets longer, burden on dental ligament is increased and the hardness of a bridge is lessened. 2. The flexibility of a bridge increases in ratio to 3 multiplication of the length and decreases in ratio to 3 multiplication of the width of occlusal surface and base of pontic. 3. Precision rest is needed to cope with the shake of teeth and the difference of axis direction among abutments. 4. Female part of the precision rest should be on middle abutment distal and male one on mesial of pontic. 5. Segmented attachment can be efficiently used to cope with long span bridgework and also in case that one piece casting can't be done because of slant of abutment.

Clinical outcome of double crown-retained implant overdentures with zirconia primary crowns

  • PURPOSE. This retrospective study aims at the evaluation of implant-supported overdentures (IODs) supported by ceramo-galvanic double crowns (CGDCs: zirconia primary crowns + galvano-formed secondary crown). MATERIALS AND METHODS. In a private practice, 14 patients were restored with 18 IODs (mandible: 11, maxilla: 7) retained by CGDCs on 4 - 8 implants and annually evaluated for technical and/or biological failures/complications. RESULTS. One of the 86 inserted implants failed during the healing period (cumulative survival rate (CSR) implants: 98.8%). During the prosthetic functional period (mean: $5.9{\pm}2.2years$), 1 implant demonstrated an abutment fracture (CSR-abutments: 98.2%), and one case of peri-implantitis was detected. All IODs remained in function (CSR-denture: 100%). A total of 15 technical complications required interventions to maintain function (technical complication rate: 0.178 treatments/patients/year). CONCLUSION. Considering the small sample size, the use of CGDCs for the attachment of IODs is possible without an increased risk of technical complications. However, for a final evaluation, results from a larger cohort are required.

Effect of cyclic loading and retightening on reverse torque value in external and internal implants

  • PURPOSE. The aim of this study was to evaluate the effect of cyclic loading and screw retightening on reverse torque value (RTV) in external and internal type implants. MATERIALS AND METHODS. Cement-retained abutments were connected with 30 Ncm torque to external and internal type implants. Experimental groups were classified according to implant connection type and retightening/loading protocol. In groups with no retightening, RTV was evaluated after cyclic loading for 100,000 cycles. In groups with retightening, RTV was measured after 3, 10, 100 cycles as well as every 20,000 cycles until 100,000 cycles of loading. RESULTS. Every group showed decreased RTV after cyclic loading. Before and after cyclic loading, external type implants had significantly higher RTVs than internal type implants. In external type implants, retightening did not affect the decrease in RTV. In contrast, retightening 5 times and retightening after 10 cycles of dynamic loading was effective for maintaining RTV in internal type implants. CONCLUSION. Retightening of screws is more effective in internal type implants than external type implants. Retightening of screws is recommended in the early stage of functional loading.

A root submergence technique for pontic site development in fixed dental prostheses in the maxillary anterior esthetic zone

  • Purpose: This case report discusses the effect of a root submergence technique on preserving the periodontal tissue at the pontic site of fixed dental prostheses in the maxillary anterior aesthetic zone. Methods: Teeth with less than ideal structural support for fixed retainer abutments were decoronated at the crestal bone level. After soft tissue closure, the final fixed dental prostheses were placed with the pontics over the submerged root area. Radiographic and clinical observations at the pontic sites were documented. Results: The submerged roots at the pontic sites preserved the surrounding periodontium without any periapical pathology. The gingival contour at the pontic site was maintained in harmony with those of the adjacent teeth, as well as the overall form of the arch. Conclusions: The results of this clinical report indicate that a root submergence technique can be successfully applied in pontic site development with fixed dental prostheses, especially in the maxillary anterior esthetic zone.

Seismic analysis of a masonry cross vault through shaking table tests: the case study of the Dey Mosque in Algiers

  • This paper presents the results of a monodirectional shaking table test on a full-scale unreinforced masonry cross vault characterized by asymmetric boundary conditions. The specimen represents a vault of the mosque of Dey in Algiers (Algeria), reproducing in detail the mechanical characteristics of masonry, and the constructive details including the presence of some peculiar wooden logs placed within the vault's abutments. The vault was tested with and without the presence of two steel bars which connect two opposite sides of the vault. The dynamic behaviour of both the vault's configurations were studied by using an incremental dynamic analysis up to the collapse of the vault without the steel bars. The use of an innovative high-resolution 3D optical system allowed measure displacement data of the cross vault during the shake table tests. The experimental results were analysed in terms of evolution of damage mechanisms, and in-plane and out-of-plane deformations. Moreover, the dynamic properties of the structure were investigated by means of an experimental modal analysis.

임상가를 위한 특집 2 - 심미 수복 - 같은 결과, 다른 접근 세라믹을 이용한 전통적인 접근법 (Traditional approach with ceramic)

  • The requirements for the successful treatment of all-ceramic restorations are not so different from the ones of conventional restorations. "The provisional restoration followed by an adequate tooth reduction and the accurately fitting prostheses with corresponding to final impression" can be the examples of them. Nevertheless, the one which all-ceramic restorations are distinguished from conventional restorations is the additional procedure of so called "bonding". In addition to the application of resin cement between "inner surface of restoration and outer surface of abutment", bonding technology can be also applied to the treatment process of "Post and Core" in particular if the abutments are non-vital teeth. Core build-up for all-ceramic crown is conducted with fiber post and tooth colored composite by considering the properties of the restorations transmitting light. We know well that a vital abutment is easier than a non-vital one to get the targeted goals for clinical success in connection with esthetics and structure. The creation of "Post and Core" with bonding technique is a decisive factor for a long-term success if the abutment is non-vital tooth with dentinal collapse. I would like to share my clinical experience about "post & core build-up and all-ceramic restoration bonding" out of several success strategies of all-ceramic crown with this presentation.
