Development of a Gait Diagnosis Supporting System using Korean Normal Gait Data (한국 성인의 정상 보행데이터를 이용한 보행진단 지원 시스템의 개발)

  • A gait diagnosis supporting system is necessary to evaluate the characteristics of abnormal gait of a patient in a systematic and efficient manner. The present study developed a gait diagnosis supporting system which compares abnormal gait of a patient with a reference gait data and presents abnormal gait characteristics in an organized form. Three types of diagnosis modules were developed for the spatio-temporal, kinematic and kinetic gait parameters, and a gait data for Korean normal adults was used for the reference data of the system. The system was applied to evaluate the gait pattern of three arthritis patients and the abnormal gait characteristics of them could be easily identified with a systematic and graphical presentation.

Study on Correlation between Difference of Pelvic Height and Gait Balance of Patients with Abnormal Postures (자세이상을 호소하는 환자의 골반 높이 차이와 보행 밸런스 관계에 대한 연구)

  • Objectives This study was designed to investigate the correlation between the difference of pelvic height and difference of gait balance. Methods 62 cases of patients who received treatment from January 2011 to March 2014 for abnormal postures were analyzed. Their difference of pelvic height were estimated by whole spine X-ray analysis and gait balance were estimated by Treadmill Gait Analysis system. The data were analyzed to find out correlation between difference of pelvic height and difference of gait balance, and correlation between the position of pelvic tilt and gait balance higher side. Pearson correlation and Chi-square analysis were used. Results Pelvic height heigher side were more left than right side, and gait balance higher side were also more left than right side. Difference of pelvic height and difference of gait balance had a positive linear relationship, but there was no significant correlation. The position of pelvic tilt had significant correlation with gait balance higher side. Conclusions The position of pelvic tilt had significant correlation with gait balance higher side and difference of pelvic height had no significant correlation with difference of gait balance.

A Study on Particular Abnormal Gait Using Accelerometer and Gyro Sensor (가속도센서와 각속도센서를 이용한 특정 비정상보행에 관한 연구)

  • Recently, technologies to help the elderly or disabled people who have difficulty in walking are being developed. In order to develop these technologies, it is necessary to construct a system that gathers the gait data of people and analysis of these data is also important. In this research, we constructed the development of sensor system which consists of pressure sensor, three-axis accelerometer and two-axis gyro sensor. We used k-means clustering algorithm to classify the data for characterization, and then calculated the symmetry index with histogram which was produced from each cluster. We collected gait data from sensors attached on two subjects. The experiment was conducted for two kinds of gait status. One is walking with normal gait; the other is walking with abnormal gait (abnormal gait means that the subject walks by dragging the right leg intentionally). With the result from the analysis of acceleration component, we were able to confirm that the analysis technique of this data could be used to determine gait symmetry. In addition, by adding gyro components in the analysis, we could find that the symmetry index was appropriate to express symmetry better.

  • The foot plays an important role in supporting the body and keeping body balance. An abnormal walking habit breaks the balance of the human body as well as the function of the foot. The foot orthotics which is designed to consider biomechanics effectively distributes the load of the human body on the sole of the foot. In this paper, gait analysis was performed for three male subjects wearing the orthotics. In this study, three male subjects were selected. The experimental apparatus consists of a plantar pressure analysis system and digital EMG system. The gait characteristics are simulated by ADAMS/LifeMOD. The COP (Center of Pressure), EMG and ground reaction force were investigated. As a result of gait analysis, the path of COP was improved and muscle activities were decreased with orthotics on the abnormal walking subjects.

  • The present study was performed to investigate gait characteristics of 30-39 year-old normal adults and 9-10 year-old children. The results focused on joint motions in the sagittal plane and ground reaction forces. The results will play an important role as a valuable data to determine normal and abnormal gait patterns as well as gait characteristics of Korean people.

A Design of the Expert System for Diagnosis of Abnormal Gait by using Rule-Based Representation (규칙처리 표현방식을 이용한 이상 보행용 전문가 시스템의 설계)

  • This paper describes a design of the expert system for diagnosis of abnormal gait patients. This system makes the rule-based representation that can easily extend the knowledge-base and naturally represent the uncertainty, and the inference engine that uses forward chaining which covers the reasoning from the first condition to the goal. The results of inferring various maladies using this system are as follows: 1) In cases of progressive muscular dystrophy, cerebral vascular accident, peripheral neuropathic lesion and peroneal nerve injury, the result of inference is the same as that of medical specialists' with 100% accuracy. 2) In cases of Neuritis, Paralysis agitan and Brain tumor, the accuracy of inference is less than 50% compared to that of medical specialists. With above results, we decide that the rule-based representations of some maladies ard accurate relatively, but that the correction and the extention of some rules and some methods of problem solving are required in order to construct the complete expert system for diagnosis of abnormal gait patients.

  • This paper deals with the analysis of leg motion using RGB image. We used RGB image as gait analysis element by using BMC(Background Model Challenge) method and by using combining object recognition segmentation algorithm and attitude detection algorithm. It is considered that gait analysis incorporating image can be used as a parameter for classification of gait pattern recognition and abnormal gait.

Study on Correlation between the Gait Analysis Indices and Lumbar X-ray Indicators (보행과 기립위 요추 방사선 지표와의 상관성 연구)

  • Objectives This study was designed to investigate the correlation between the gait analysis indices and Lumbar X-ray indicators. Methods 21 cases of patients who received treatment from October 2013 to February 2017 for abnormal posture were analyzed. Three indicators were measured in the lumbar spine X-ray. These indicators include Ferguson's angle, Lumbar lordotic angle, L4-5 IVD angle. Gait analysis indices were estimated by Treadmill Gait Analysis system. The data were analyzed to find out correlation between the gait analysis indices and Lumbar X-ray indicators. Spearman correlation was used. Results Ferguson's angle and Difference of gait balance of front and rear had a negative linear relationship, but there was no statistical significance. Lumbar lordotic angle and Difference of gait balance of front and rear had a negative linear relationship, but there was no statistical significance. L4-5 IVD angle and Difference of gait balance of right and left had a negative linear relationship, but there was no statistical significance. Conclusions Difference of gait balance of front and rear had strong level of a negative linear relationship with Ferguson's angle and Lumbar lordotic angle.

Effects of Treadmill Gait Training Combined with Muscle Tone Control Technique on Gait Ability in Patient with Chronic Stroke (근긴장도 조절기법을 병행한 트레드밀 보행훈련이 만성 뇌졸중 환자의 보행능력에 미치는 영향)

  • Purpose : Stroke patients exhibit considerable variations in gait patterns. Stroke patients generally show abnormal muscle tone and gait. This study was performed to evaluate the effects of treadmill gait training combined with muscle tone control technique (TM) on gait ability in patient with chronic stroke. Methods : A single-blind, randomized controlled trial was conducted with 36 patient with chronic stroke. They were randomly allocated 2 groups; treadmill gait training combined with muscle tone control technique group (TM group; n=18) and conservative treatment group (CG group; n=18). The TM group received 15 minutes muscle tone control technique and 15 minute treadmill gait training. In the conservative treatment group received 30 minutes conservative physical therapy. Each group performed 30 minutes a day 3 times a week for 8 weeks. The primary outcome gait ability were measured by gait measurement system (Optogait, Microgate, Italy) and 10 m walking test (10MWT). An independent t-test was used to statistically analyze the pre-test and pos-ttest gait ability results. Results : Both groups demonstrated significant improvement of outcome in gait ability during intervention period. TM group showed significant differences in gait ability as compared to the CG groups (p<.05). TM group showed significant differences in 10MWT as compared to the CG groups (p<.05). Our results showed that TM was more effective on gait ability in patients with chronic stroke. Conclusion : Our findings of this study confirmed that the treadmill gait training combined with muscle tone control technique provided significant improvements on gait ability in patient with chronic stroke. Therefore treadmill gait training combined with muscle tone control technique may positive influenced gait ability. This study will be able to be used as an intervention data for recovering gait ability in patients with chronic stroke.

  • When we see normal gait, gait cycle is seperated as stance phase and swing phase. It needs 6 determinant of gait of pelvic rotation, pelvic tilt, knee joint of stance phase, ankle and foot motion, ankle and knee motion, and pelvic movement to be accomplished. In addition, a joint and muscle action is accomplished biomechanically at the same time with its gait cycle. In oriental medicine, the relationships between chang-fu physiology and meridian physiology are summaried as follows ; ${\bullet}$ chang-fu physiology : Spleen manages the extremities. Liver manages soft tissues. Liver stores blood. Kidney stores essences. Kidney manages bones. ${\bullet}$ meridian physiology : The Leg Greater Yang Meridian and meridian soft tissues The Leg Yang-Myeong Meridian and meridian soft tissues The Leg Lesser Yang Meridian and meridian soft tissues The Leg Greater Yin Meridian and meridian soft tissues The Leg Lesser Yin Meridian and meridian soft tissues The Leg Absolute Yin Meridian and meridian soft tissues Especially, we can find out relations between in a "blood supplied feet can walk well" that explains "blood regulations and by liver nourishing effects"that is the closest concept of muscle. Abnormal gaits are due to three causes as following; first, physical defect secoud, pain third, nervous system or instability of muscle. In oriental medicine, we can know relationship in "atrophy, numbness, stroke, convulsion, muscular dystrophy of knee, rheumatoid arthritis, five causes of infantile growing defects, five causes of softening, sprain". Especially, atrophy is the most important symptom. Gait evaluation should be emphasized where a point can walk 8 feet to 10 feet considering stride width, stride length, the body weight center, stride number, flexion, extension, rotation of a joint as a standard factor. The point is we should find out something strange in a patient's side, front and back view. After that we should find out its cause as an index that we can observe abnormal findings in a joint and muscle.
