Inhibitory Effects of Transforming Growth Factor and Drynariae Rhizoma on Leukocytosis Associated with the Chronic Phase of Arthritis in Mice

  • Drynariae Rhizoma (DR), an herbal medicine known to clean blood and improve its circulation, frequently appears as the main ingredient in the prescriptions for bone injuries. Currently, it is unclear how contributes pharmacologically to the reformation of bone. Therefore, we have done this study. Systematic administrations of TGFβ1 and water extract of DR diminished the polyarthritis development in rats. TGFβ administration (0.1-2 ㎍/animal) and DR (10-100 ㎍/animal), initiated 1 day before an arthritogenic dose of streptococcal cell wall fragments, virtually eliminated the joint swelling and distortion observed during the acute phase and the chronic phase of the disease. The TGFβ and DR synergistically suppressed the arthritis when the administration was begun after the acute phase of arthritis. Also, the synergistic activity between TGFβ and DR was confirmed in their suppression of arthritis in rats. Consistent with the inhibition of inflammatory cell recruitment into the synovium, TGFβ1 and DR reversed the leukocytosis associated with the chronic phase of the arthritis.

A Survey on Pain and Self-Care Behavior of Patients with Chronic Arthritis (만성 관절염 환자의 통증과 자기간호행위 관련요인)

  • Purpose: This study was designed to explore pain and self-care behaviors and identify related variables in patients with chronic arthritis. Method: One hundred fifty patients with arthritis were recruited from two university based arthritis centers according to selection criteria. Collected data were analyzed using the SAS program to analyze the responses to the structured questionnaires of the study. Result: Most of the participants expressed pain and the intensity of the pain was moderate. There were significant differences according to age, educational level, diagnosis, duration of illness, number of affected joint, and use of complementary therapy. Self-care behavior scores were moderately high. The highest practice was for 'regular visits to the hospital', and the lowest for 'applying physical therapy at home'. The mean self-care behavior scores showed significant differences according to economic status and educational level. Pain scores showed no correlation with self-care behavior. Conclusion: Developing self-management programs for patients with chronic arthritis should focus on self-care skills which are applicable in the relief of pain and enhancement of knowledge. The skills are recommended not only for better health practices but also for enhancing the level of well-being and life satisfaction.

A Study on Depression and Activity of Daily Living in Patients with Rheumatoid Arthritis by Gender (성별에 따른 류마티스 관절염 환자의 우울과 일상활동정도)

  • Purpose: The purpose of this study was to investigate depression and activity of daily living of patients with rheumatoid arthritis by their gender. Methods: The subjects for this study were 184 patients registered in S University Hospital. and the period of data collection was from May 1, 2012 to August 20, 2012. The data were analyzed with t-test, ANOVA, Mann-Whitney U test, Kruskal-Wallis H test, and Pearson's correlation. Results: For depression, there were significant differences by marriage and religion in male patients and by education in female patients. For activity of daily living, there were significant differences by marriage, age and education in female patients. Depression was negatively correlated with activity of daily living in patients with rheumatoid arthritis. Conclusion: Managing depression and activity of daily living were important for both male and female patients with rheumatoid arthritis. To promote the quality of life of patients with rheumatoid arthritis, nursing intervention strategies must be developed for decreasing depression or improving their activity of daily living.

Recombinant Adeno-Associated Virus Expressing Truncated IK Cytokine Diminishes the Symptoms of Inflammatory Arthritis

  • IK can downregulate interferon-gamma-induced major histocompatibility complex (MHC) class II expression through the MHC class II transactivator, which suggests that IK can inhibit the interactions between immune cells. We delivered adeno-associated virus serotype 2 (AAV2) encoding the genes for truncated IK (tIK) or green fluorescent protein (GFP) to DBA1/J mice via intravenous injection. Seven weeks after injection, collagen-induced arthritis was induced in the AAV2-treated mice. AAV2-tIK injection reduced the severity of arthritis and the percentage of pathogenic Th17 cells compared with AAV2-GFP injection. These results suggest a novel gene therapy strategy for treatment of inflammatory arthritis.

The Effect of Swimming and Low Power Laser on the Healing of the Freund's Complete Adjuvant Induced Arthritis in Rat (수영과 저출력 레이저 치료가 Freund's Complete Adjuvant 유발 흰쥐의 관절염 치유에 미치는 영향)

  • Purpose: This study was to find that what mechanism take effects that was Adjuvant Induced Arthritis in Sprague-Dawley rat and then treated the swimming and low power laser. Methods: Adjuvant Induced Arthritis was induced 24 Sprague-Dawley rat by the subcutaneous injection of a 0.2ml Freund's Complete Adjuvant into the right hind paw and right knee joint. Second injection used of 0.05ml Freund's Complete Adjuvant by same method. Arthritic rat were divided 3 groups; arthritic swimming group, arthritic laser group and case control group. The author performed several experimental tests which were the hind paw thickness, step length, knee joint space, activity of enzyme. Results: Hind paw thickness decreased in swimming and laser group. Left step length and knee joint space increased in swimming and laser. Conclusion: Swimming and low power laser therapy on the Adjuvant Induced Arthritis in rats does effective for the rheumatic arthritis therapy by decrease of hind paw thickness, increase of opposite side step length, increase of activity of albumin and IgG and increase of knee joint space.

A Study on the wearing practices and Preference for the Aged Female Arthritis Patients' Pants (노년여성 관절염질환자의 바지 착용실태 및 선호도)

  • The purpose of this study consist in the investigation into the pants wearing practices and preference with arthritis patient of aged female and the gathering of basic data to help to their clothing lifestyle. For this study, personal interviews were carried out with 124 arthritis patients of aged female over 65 year old and data analysis were done with SPSS. The results of the study are that on pants wearing practices with the subjects of aged female arthritis patient showed that many persons experience inconvenience for both knees and the wearing of knee protectors which use a purpose of a reducing pain such as they feel inconvenience of knee showed a high rate of 79.8% during winter. They purchased the pants with much space in the knee part or span trousers in consideration of knee protector wearing. When they purchase the pants, they attached great importance to the convenience or activities during their wearing period, prices, and the pants which were wide at the thigh, narrow at the pants hole, hanging down to the anklebones, and setting over the waistline as for the pants' waist position. The arthritis patients who felt uncomfortable at knees paid attention to the excellent materials at keeping warmth and with good flexibility.

Anti-inflammatory Effect of Gyulpidaehwangbakcho-tang (Jupidahuangpoxiao-tang) in the Collagen-induced Arthritis Mouse Model

  • Objectives: To investigate anti-inflammatory and anti-arthritic effects of Gyulpidaehwangbakcho-tang (GDBT) extract in a murine model of rheumatoid arthritis. Methods: The mice received $100{\mu}g$ of bovine type II collagen in Freund's complete adjuvant by intradermal injection at the base of the tail on day 0 and a booster injection on day 21. The mice were orally administered with GDBT (200 or 50mg/kg dissolved in distilled water) daily from day 1 to day 21 after arthritis incidence, and monitored for disease incidence and the severity of arthritis up to day 21. In order to evaluate the effect of GDBT on disease progression, we examined pro-inflammatory cytokines including IL-$1{\beta}$, IL-6, TNF-${\alpha}$, COX-2 and NOS-II. Results: GDBT produced a significant and dose dependent inhibition of arthritis and inflammation during the entire duration of the study. This action was characterized by the decreased production of IL-$1{\beta}$, IL-6, TNF-${\alpha}$, COX-2, and NOS-II in vivo. Conclusion: We believe that the anti-arthritic activity of GDBT is due to its modulatory effect on the expression of pro-inflammatory cytokine in the synovium. Our results contribute towards validation of the traditional use of GDBT in the treatment of RA and other inflammatory joint disorders.

Suppressive Effect of Water Extract from Leaf of Cirsium japonicum var. ussuriense on Collagen-induced Arthritis (엉겅퀴 잎 수용성 추출물의 콜라겐 유도 관절염 억제효과)

  • Cirsium japonicum var. ussuriense is often used in treatment of human disease such as hemorrhage, blood congestion and inflammation. The present study has been undertaken to investigate the effect of the leaf extract from C. japonicum var. ussuriense (CLE) on the development of collagen-induced arthritis (CIA) in DBA1/J mice. CLE administration suppressed markedly the arthritis incidence and arthritis score in CIA mice. Also, CLE significantly suppressed the release of $PGE_2$, TNF-${\alpha}$, IL-4 and IL-6 in CIA mice. However, CLE significantly increased the production of IL-10, but not IL-4. These results suggest that CLE suppress inflammatory mediators and regulates Th1 and Th2 cytokines. These properties may contribute to the anti-arthritis action of CLE.

A Case Report of Oligoarticular Juvenile Idiopathic Arthritis with Oriental Medication and DMARDs & NSAIDs Tapering (소수관절형 소아기 류마티스관절염의 항류마티스제 감량과 한약투여 병행을 통한 치료사례)

  • Objectives This study is to report a case that has an effective result to oligoarticular type juvenile idiopathic arthritis patient with the oriental medicine treatment. Methods We treated the patient with an oriental medicine and tapered down with the DMARDs and NSAIDs. We followed up the laboratory blood tests every four or five months and throughout the experiment. Results The symptoms of oligoarticular type juvenile idiopathic arthritis were vanished and the patient maintained her good health condition with oriental medicine treatment after discontinued all Disease-Modifying Antirheumatic Drugs (DMARDs) and NSAIDs (Non-Steroidal Anti-Inflammatory Drugs). Her ESR, CRP levels were stable in normal and other blood test results were back to normal range. Her joint mobility and condition was back to normal. Now she is in clinical remission status. Conclusions According to the result, the oriental medicine treatment is considered to be effective on the oligoarticular juvenile idiopathic arthritis and further studies will be needed with more cases.
