Correlation of oocyte number with serum anti-Müllerian hormone levels measured by either Access or Elecsys in fresh in vitro fertilization cycles

  • Objective: The aim of this study was to assess the correlation of oocyte number with serum anti-Müllerian hormone (AMH) levels measured by two automated methods (Access or Elecsys) in fresh stimulated in vitro fertilization (IVF) cycles. Methods: In this retrospective study at a university hospital, data were collected from 243 fresh stimulated IVF cycles performed from August 2016 to December 2020. The serum AMH level was measured by Access in 120 cycles and by Elecsys in 123 cycles. The cut-off of serum AMH for prediction of poor responders (three or fewer oocytes) or high responders (15 or more oocytes) was calculated by the receiver operating characteristic curve analysis. Results: For the two automated methods, the following equations were derived: total oocyte number=2.378+1.418×(Access-AMH) (r=0.645, p<0.001) and total oocyte number=2.417+2.163×(Elecsys-AMH) (r=0.686, p<0.001). The following combined equation could be derived: (Access-AMH)=0.028+1.525×(Elecsys-AMH). To predict poor responders, the cut-off of Access-AMH was 1.215 ng/mL (area under the curve [AUC], 0.807; 95% confidence interval [CI], 0.730-0.884; p<0.001), and the cut-off of Elecsys-AMH was 1.095 ng/mL (AUC, 0.848; 95% CI, 0.773-0.923; p<0.001). To predict high responders, the cut-off of Access-AMH was 3.450 ng/mL (AUC, 0.922; 95% CI, 0.862-0.981; p<0.001), and the cut-off of Elecsys-AMH was 2.500 ng/mL (AUC, 0.884; 95% CI, 0.778-0.991; p<0.001). Conclusion: Both automated methods for serum AMH measurement showed a good correlation with oocyte number and good performance for predicting poor and high responders in fresh stimulated IVF cycles. The Access method usually yielded higher measured serum AMH levels than the Elecsys method.

  • 목 적: 체외수정을 위한 과배란유도 시 난소 반응성의 예측 인자로서 항뮬러리안 호르몬 (AMH)의 효용성을 FSH와 비교하여 알아보고, 체외수정 결과와의 관계도 알아보고자 하였다. 연구방법: 2007년 1월부터 2007년 8월까지 단기 요법이나 GnRH antagonist 요법을 이용하여 체외수정을 시행하는 111명을 대상으로, 생리주기 3일째 혈중 AMH 및 FSH를 전향적으로 측정하였다. 과배란유도 후 채취된 난자 개수와 AMH 또는 FSH의 상관관계를 분석하였고, 채취된 난자 수 및 AMH 수치에 따라 과배란유도 결과와 체외수정 결과를 비교하였다. 저반응군 및 과반응군의 예측을 위한 AMH의 임계치를 알아보기 위해 ROC curve 분석을 시행하였다. 결 과: AMH는 채취된 성숙 난자 수와 밀접한 양의 상관관계를 나타냈으며 (r=0.792, p<0.001), FSH (r=-0.477, p<0.001) 보다 더 높은 상관관계를 보였다. 난소 반응성에 따라 저반응군 (성숙난자수 ${\leq}2$), 정상반응군 ($3{\sim}16$), 과반응군 (${\geq}17$)으로 나누어 AMH와 FSH 수치는 유의한 차이를 보였으며, 저반응군 (채취된 성숙 난자 수${\leq}2$) 및 과반응군 (채취된 성숙 난자 수${\geq}17$)을 예측하는데 ROC curve 상 AMH의 임계치는 각각 <0.50 ng/ml (sensitivity 88.9%, specificity 89.5%), 및 ${\geq}2.60\;ng/ml$ ((sensitivity 85.7%, specificity 87.0%)였다. AMH level에 따라 low AMH group (${\leq}0.60\;ng/ml$), normal AMH group ($0.60{\sim}2.60\;ng/ml$), high AMH group으로 구분하여, 각 군간 채취된 성숙 난자 수와 ($2.7{\pm}2.2$, $8.1{\pm}4.8$, $16.5{\pm}5.7$, p<0.001), 투여된 성선자극호르몬제의 용량에 ($3530.5{\pm}1251.0$, $2957.1{\pm}1057.6$, $2219.2{\pm}751.9\;IU$, p<0.001) 유의한 차이를 보였다. 성숙 난자의 비율과 수정율에 유의한 차이는 없었으며 임신율도 23.8%, 34.0%, 37.5%로 각 군간 유의한 차이는 없었다. 결 론: 혈중 기저 AMH는 과배란유도 시 채취되는 성숙 난자 수와 높은 상관관계가 있어 난소 반응성의 예측 인자로서 유용할 것으로 생각된다. AMH 수치에 따라 체외수정 결과에는 차이가 없었으나, 저반응군 및 과반응군의 예측에 유용하여 체외수정 시 주기의 취소나, 환자별 적정 용량의 결정 및 난소과자극 증후군의 위험을 감소시키는데 도움이 될 것으로 사료된다.

  • Anti-$M{\ddot{u}}llerian$ hormone (AMH), a peptide growth factor of the transforming growth $factor-{\beta}$ family, is a reliable marker of ovarian reserve. Regarding assisted reproductive technology, AMH has been efficiently used as a marker to predict ovarian response to stimulation. The clinical use of AMH has recently been extended and emphasized. The uses of AMH as a predictive marker of menopause onset, diagnostic tool for polycystic ovary syndrome, and assessment of ovarian function before and after gynecologic surgeries or gonadotoxic agents such as chemotherapy have been investigated. Serum AMH levels can also be affected by environmental and genetic factors; thus, the effects of factors that may alter AMH test results should be considered. This review summarizes the findings of recent studies focusing on the clinical application of AMH and factors that influence the AMH level and opinions on the use of the AMH level to assess the probability of conception before reproductive life planning as a "fertility test."

Clinical application of anti-M$\ddot{u}$llerian hormone as a predictor of controlled ovarian hyperstimulation outcome

  • Objective: In 2009 anti-M$\ddot{u}$llerian hormone (AMH) assay was approved for clinical use in Korea. This study was performed to determine the reference values of AMH for predicting ovarian response to controlled ovarian hyperstimulation (COH) using the clinical assay data. Methods: One hundred sixty-two women who underwent COH cycles were included in this study. We collected data on age, basal AMH and FSH levels, total dose of gonadotropins, stimulation duration, and numbers of oocytes retrieved and fertilized. Blood samples were obtained on cycle day 3 before gonadotropin administration started. Serum AMH levels were measured at a centralized clinical laboratory center. The correlation between the AMH level and COH outcomes and cut-off values for poor and high response after COH was analyzed. Results: Concentration of AMH was significantly correlated with the number of oocytes retrieved (OPU; r=0.700, p<0.001). The mean${\pm}$SE serum AMH levels for poor ($OPU{\leq}3$), normal ($4{\leq}OPU{\leq}19$), and high ($OPU{\geq}20$) response were $0.94{\pm}0.15$ ng/mL, $2.79{\pm}0.21$ ng/mL, and $6.94{\pm}0.90$ ng/mL, respectively. The cut-off level, sensitivity and specificity for poor and high response were 1.08 ng/mL, 85.8%, and 78.6%; and 3.57 ng/mL, 94.4%, and 83.3%, respectively. Conclusion: Our data present clinical reference values of the serum AMH level for ovarian response in Korean women. The serum AMH level could be a clinically useful predictor of ovarian response to COH.

The likelihood of achieving pregnancy through timed coitus in young infertile women with decreased ovarian reserve

  • Objective: To evaluate the pregnancy rate and time to pregnancy after timed coitus with or without superovulation in infertile young women younger than 35 years old with low serum $anti-M{\ddot{u}}llerian$ hormone (AMH) levels ( < 25th percentile). Methods: A total of 202 patients younger than 35 years old were recruited retrospectively between 2010 and 2012. Ninety-eight women had normal serum AMH levels (25-75th percentile), 75 women had low serum AMH levels (5th ${\leq}$ & < 25th percentile) and 29 women had very low serum AMH levels ( < 5th percentile), according to reference values for their age group. Results: The clinical pregnancy rate was positively associated with AMH levels, but this trend did not reach statistical significance (43.9% vs. 41.3% vs. 27.6% in the normal, low, and very low AMH groups, respectively). The time to pregnancy was longer in the very low AMH group than in the normal AMH group ($13.1{\pm}10.9months$ vs. $6.9{\pm}6.1months$, p= 0.030). The cumulative live birth rate over 18 months was lower in the very low AMH group than in the normal AMH group, with marginal significance (20.0% vs. 55.9%, p= 0.051). The duration of infertility was negatively correlated with achieving pregnancy (odds ratio, 0.953; 95% confidence interval, 0.914-0.994; p= 0.026). Conclusion: Conservative management, such as timed coitus with or without superovulation, should be considered in young patients who have low ovarian reserve without any infertility factors. However, for women with a long duration of infertility or very low serum AMH levels, active infertility treatment should be considered.

혈중 항뮬러관호르몬 저하를 진단 받은 난임 환자의 한방치료 후 임신 4례 (A Case Report on Four Pregnancies of Subfertile Patients with Low Anti-Müllerian Hormone (AMH) Level after Korean Medical Treatments)

  • Objectives : AMH (Anti-Müllerian Hormone) is considered a sensitive marker of ovarian reserve, and it tends to be included among basic fertility tests. This paper is to report four pregnancies which were achieved by subfertile women with low AMH level after Korean medical treatments. Methods : Four cases of pregnancy by subfertile women, aged 34 to 37, with low AMH level (0.04 to 0.76 ng/mL), after Korean medical treatments between July 2012 and May 2015, were analysed. The patients were diagnosed as Kidney deficiency (腎虛), Blood extravasation (瘀血) and/or Liver Qi stagnation (肝氣 鬱結). Herbal medicine, acupuncture, pharmacopuncture and moxibustion treatments were applied. Results : The four subfertile women with low AMH level achieved pregnancy after Korean medical treatments. Conclusions : The case report suggests that Korean medical treatment can be an effective option for subfertile women with low AMH level before Assisted Reproductive Technology.

Anti-Müllerian hormone levels as a predictor of clinical pregnancy in in vitro fertilization/intracytoplasmic sperm injection-embryo transfer cycles in patients over 40 years of age

  • Objective: The aim of the current study was to determine the predictive value of anti-$M{\ddot{u}}llerian$ hormone (AMH) levels for pregnancy outcomes in patients over 40 years of age who underwent in vitro fertilization or intracytoplasmic sperm injection-embryo transfer (IVF/ICSI-ET) cycles. Methods: We retrospectively analyzed the medical records of 188 women aged 40 to 44 years who underwent IVF/ICSI-fresh ET cycles due to unexplained infertility in the fertility center of CHA Gangnam Medical Center. Patients were divided into group A, with AMH levels <1.0 ng/mL (n=97), and group B, with AMH levels ${\geq}1.0ng/mL$ (n=91). We compared the clinical pregnancy rate (CPR) in the two groups and performed logistic regression analysis to identify factors that had a significant effect on the CPR. Results: The CPR was significantly lower in group A than group B (7.2% vs. 24.2%, p<0.001). In multivariate logistic regression analysis, AMH levels were the only factor that had a significant impact on the CPR (odds ratio, 1.510; 95% confidence interval, 1.172-1.947). The area under the receiver operating characteristic curve for AMH levels as a predictor of the CPR was 0.721. When the cut-off level of AMH was set at 1.90 ng/ mL, the CPR was 6.731-fold higher in the group with AMH levels ${\geq}1.90ng/mL$ than in the group with AMH levels <1.90 ng/mL (p<0.001). Conclusion: Our study showed that AMH levels were predictive of clinical pregnancy in infertility patients over 40 years of age. Further prospective studies should be conducted to validate the predictive capability of AMH levels for the outcome of clinical pregnancy.

Serum luteinizing hormone level and luteinizing hormone/follicle-stimulating hormone ratio but not serum anti-$M\ddot{u}llerian$ hormone level is related to ovarian volume in Korean women with polycystic ovary syndrome

  • Objective: The aim of the present study was to investigate the relationship between ovarian follicle count and volume on ultrasonography and serum hormone levels including the levels of the anti-$M\ddot{u}llerian$ hormone (AMH) and gonadotropin in women with the polycystic ovary syndrome (PCOS). Methods: A total of 118 Korean women aged 18-35 years who were newly diagnosed with PCOS at a university hospital were included in this study. Serum LH, FSH, and AMH levels were measured in the early follicular phase, and the total antral follicle count (TFC) and the total ovarian volume (TOV) were assessed by ultrasonography. The correlations between serum hormonal parameters and ultrasonography characteristics in women with PCOS were evaluated using Pearson's correlation coefficients and a linear regression analysis. Results: Serum AMH levels were significantly correlated with serum LH levels and LH/FSH ratios, and TFC and TOV were significantly correlated with each other on ultrasonography. Serum AMH and LH levels and the LH/FSH ratio were significantly correlated with TFC. Statistically significant correlations between TOV and the LH level (r=0.208, p=0.024) and the LH/FSH ratio (r=0.237, p=0.010) were observed. However, the serum AMH level was not significantly correlated with the ovarian volume, and this result did not change after adjusting for age and body mass index. Conclusion: Serum AMH is not related to the ovarian volume in women with PCOS. My results suggest that serum LH level and the LH/FSH ratio may be more useful than the serum AMH level for representing the status of the ovarian volume in women with PCOS.

난소기능저하 미혼여성의 난자동결보존 시 한약복용을 병행한 증례보고 (A Case Report about Herbal Medicine Treatment's a Single Woman Patient with a Low Level of AMH in Progress of Oocyte Cryopreservation)

  • Objectives: The aim of this case is to report the effects of herbal medicine on a single woman patient with a low level of AMH (anti-$M{\ddot{u}}llerian$ Hormone) in progress of Oocyte Cryopreservation. Methods: A patient with a low level of AMH had symptom of secondary amenorrhea. For preparing oocyte cryopreservation after a long time of secondary amenorrhea, she was treated by twice a day herb medication for 10 months. And we observed the effects of treatments by improvement of symptoms and following up endometrium ultrasonography. After oocyte cryopreservation, for maintaining her menstruation, she was also treated by twice a day herb medication for two and a half months. Results: After treatments, symptom of amenorrhea was improved and the thickness of endometrium was increased as well as AMH in progress of oocyte cryopreservation. So 20 oocytes could be cryopreserved. Conclusions: This case shows that herbal medicine can be a concurrent method for a single woman patient with secondary amenorrhea in progress of oocyte cryopreservation.

Association between serum anti-M$\ddot{u}$llerian hormone level and ovarian response to mild stimulation in normoovulatory women and anovulatory women with polycystic ovary syndrome

  • Objective: To evaluate the correlation between serum levels of anti-M$\ddot{u}$llerian hormone (AMH) and ovarian response to mild stimulation in normoovulatory women and anovulatory women with polycystic ovary syndrome (PCOS). Methods: Seventy-four cycles of mild stimulation (clomiphene citrate+gonadotropin followed by timed intercourse or intrauterine insemination) performed in normoovulatory women (57 cycles) and anovulatory women with PCOS (17 cycles). Ovarian sensitivity was defined by the number of mature follicles (${\geq}14mm$) on triggering day per 100 IU of gonadotropin. A correlation between ovarian sensitivity and the baseline serum AMH level (absolute or multiples of the median [MoM] value for each corresponding age) was calculated. Correlation between ovarian response and serum AMH level was evaluated. Results: Ovarian sensitivity to mild stimulation was positively correlated with absolute serum AMH (r=0.535, p<0.001) or AMH-MoM value (r=0.390, p=0.003) in normoovulatory women, but this correlation was not observed in anovulatory women with PCOS (r=0.105, p>0.05, r=-0.265, p>0.05, respectively). Conclusion: Ovarian response to mild stimulation is possibly predicted by the serum AMH level in normoovulatory women, but not in anovulatory women with PCOS.
