대구광역시 신천의 친수공간 평가 연구 - 유니버설디자인 개념을 중심으로 - (An Assessment on the Urban Riverfront in Shincheon, Daegu - Focused on a Universal Design Concept -)

  • 본 연구는 도시하천 친수공간에 대해 유니버설디자인 관점에서의 평가 및 분석을 실시하여 모든 도시민의 이용과 활동을 배려한 보다 바람직한 친수공간 조성방안을 마련하고자 하였다. 이를 위해 선행연구를 고찰하고, 유니버설디자인 개념을 접목한 평가틀을 도출하였으며, 연구 대상지로 선정한 대구광역시 신천에 대해 현장조사를 실시하여 유니버설디자인 관점에서의 문제점 도출 및 개선방향을 제시하였다. 주요 결과를 요약하면 다음과 같다. (1) '공평성' 측면에서는 안내판의 통합기능 설치 및 그림 기호 외국어 병용 표기, 화장실 출입구 옆 남녀 구분 안내 표시 등의 항목이 전 구간에 걸쳐 '모두 부적합' 판정을 받았으므로, 이에 대해 모든 사람들이 공평하게 이용 가능하도록 보완할 필요가 있다. (2) '기능성(이용성)' 측면에서는 접근로 자전거 도로 설치, 안내판 통합기능 및 야간 조명 설치 등의 항목이 모든 구간에 '모두 부적합' 하였으므로 이에 대해 시설물 각각이 제 기능을 다할 수 있도록 보완할 필요가 있다. (3) '편리성' 측면에서는 자전거 주차구역 설치, 휴식시설 연속 설치, 휴식시설에 등받이와 팔걸이 설치 등의 항목이 많은 구간에서 '모두 부적합' 판정을 받았으므로 이에 대해 시설물 이용에 불편함이 없도록 보완할 필요가 있다. (4) '정보성(식별성)' 측면에서는 접근로에 보행 유도존 설치, 안내판에 시설 종합 표기 및 그림 기호 외국어 병용 표기, 그리고 야간 조명 설치, 화장실 위치 안내 및 유도 표시 설치, 화장실 출입구 옆 남녀 구분 안내 표시 등의 항목이 전 구간에 걸쳐 '모두 부적합' 판정을 받았으므로 이에 대해 누구나 알기 쉽고 알아차리기 쉽도록 보완할 필요가 있다. (5) '안전성' 측면에서는 접근로 미끄럼 방지 및 충격 흡수 가능 재료 사용, 주차구역에 안전보행통로 설치, 화장실 바닥 미끄럼 방지 및 충격흡수 재료 사용, 자전거와 분리된 내부 보행로 연속 설치 등의 항목이 모든 구간에 '모두 부적합' 판정을 받았으므로 이에 대해 안전사고 예방 차원에서의 보완이 필요하다. (6) '쾌적성' 측면에서는 접근로, 주차구역, 위생시설, 휴식시설, 편의시설, 수변시설 등이 많은 구간들에서 '모두 부적합'하다고 판단되었으므로, 이에 대해 보다 집중적인 청결 관리가 이루어져야 할 것이다. (7) '접근성(이동성)'측면에서는 접근로와 차도 접점부분의 보도 높이 유지, 접근로에 보행 유도존 설치, 주차구역에 안전보행통로 설치, 수변시설 수평 접근 가능, 자전거와 분리된 내부 보행로 연속 설치 등의 항목이 모든 구간에 '모두 부적합' 판정을 받았으므로, 이에 대해 사람들이 쉽게 접근할 수 있는 방향으로 보완이 이루어져야 할 것이다. (8) '내구성' 측면에서는 접근로, 주차구역, 휴식시설, 편의시설, 운동시설, 수변시설 등이 많은 구간들에서 '모두 부적합' 하다고 판단되었으므로 각 구간별 시설물 점검을 통해 침하, 파손된 곳의 보수가 이루어져야 할 것이다.

Extrapulmonary Small Cell Carcinoma - a Case Series of Oropharyngeal and Esophageal Primary Sites Treated with Chemo-Radiotherapy

  • Background: The optimal sequence and extent of multimodality therapy remains to be defined for extrapulmonary small cell carcinoma because of its rarity. The purpose of our study was to assess the response to neoadjuvant chemotherapy followed by chemoradiation/radiation in patients with extrapulmonary small cell carcinoma. Materials and Methods: Four consecutively diagnosed patients were included in this study. The primary tumor site was oropharynx in three patients and esophagus in one. The patients with the limited disease were treated with chemotherapy followed by concurrent chemoradiation (n=2) or radiotherapy (n=1). The patient with the extensive disease with the primary site in vallecula was treated with chemotherapy and palliative radiotherapy to the metastatic site. Results: The median follow-up was 22.5 months (range, 8-24 months). Three patients with the limited disease (base of tongue, n=2; esophagus, n=1) were in complete remission. The patient with the extensive disease died of loco-regional tumor progression at 8 months from the time of diagnosis. Conclusions: The combination of chemotherapy and radiotherapy is the preferred therapeutic approach for patients with extrapulmonary small cell carcinoma. Induction chemotherapy followed by concurrent chemoradiation or radiation provides a good loco-regional control in patients with limited disease.

Outcome of Childhood Acute Lymphoblastic Leukemia Treated Using the Thai National Protocols

  • Background: In recent decades, the prognosis for childhood leukemia has improved, especially for acute lymphoblastic leukemia (ALL). In Thailand, though, the survival rate for ALL is unimpressive. In 2006, standard national protocols for childhood leukemia treatment were implemented. We herein report the outcome of the ALL national protocols and explanations behind discrepancies in outcomes between institutions. Materials and Methods: Between March 2006 and February 2008, 486 children with ALL from 12 institutions were enrolled in the Thai national protocols. There were 3 different protocols based on specific criteria: one each for standard risk, high risk and Burkitt's ALL. We classified participating centers into 4 groups of institutions, namely: medical schools in Bangkok, provincial medical schools, hospitals in Bangkok and provincial hospitals. We also evaluated supportive care, laboratory facilities in participating centers, socioeconomics, and patient compliance. Overall and event-free survival were determined for each group using the Kaplan Meier method. Statistical differences were determined using the log-rank test. Previous outcomes of Thai childhood ALL treatment between 2003 and 2005 served as the historic control. Results: Five-year overall survival of ALL treated using the Thai national protocol was 67.2%; an improvement from the 63.7% of the 12-institute historical control (p-value=0.06). There were discrepancies in event-free survival of ALL between centers in Bangkok and up-country provinces (69.9% vs 51.2%, p-value <0.01). Socioeconomics and patient compliance were key elements in determining the outcome (65.5% vs 47.5%, 59.4% vs 42.9%) (p-value < 0.02). Conclusions: Implementation of standard national protocols for childhood leukemia in Thailand did not significantly improve the outcome of ALL. Factors leading to better outcomes included (a) improvement of treatment compliance (b) prevention of treatment abandonment and (c) financial support to the family.

FRACTURE STRENGTH AND MARGINAL FIT OF IN-CERAM, COPY-MILLED IN-CERAM, AND IPS EMPRESS 2 ALL-CERAMIC BRIDGES

  • All-ceramic restorations have become an attractive alternative to porcelain-fused-to-metal crowns. In-Ceram, and more recently IPS Empress 2 were introduced as a new all-ceramic system for single crowns and 3-unit fixed partial dentures. But their strength and marginal fit are still an important issue. This study evaluated the fracture resistance and marginal fit of three systems of 3 unit all-ceramic bridge fabricated on prepared maxillary anterior resin teeth in vitro. The 3 all-ceramic bridge systems were: (1) a glass-infiltrated, sintered alumina system (In-Ceram) fabricated conventionally, (2) the same system with copy-milled alumina cores (copy-milled In-Ceram), (3) a heat pressed, lithium disilicate reinforced glass-ceramic system (IPS Empress 2). Ten bridges of each system with standardized design of framework were fabricated. All specimens of each system were compressed at $55^{\circ}$ at the palatal surface of pontic until catastrophic fracture occurred. Another seven bridges of each system were fabricated with standard method. All of the bridge-die complexes were embedded in epoxy resin and sectioned buccolingually and mesiodistally. The absolute marginal discrepancy was measured with stereomicroscope at ${\times}50$ power. The following results were obtained: 1. There was no significant difference in the fracture strength among the 3 systems studied. 2. The Weibull modulus of copy-milled In-Ceram was higher than that of In-Ceram and IPS Empress 2 bridges. 3. Copy-milled In-Ceram($112{\mu}m$) exhibited significantly greater marginal discrepancy than In Ceram ($97{\mu}m$), and IPS Empress 2 ($94{\mu}m$) at P=0.05. 4. The lingual surfaces of the ceramic crowns showed smaller marginal discrepancies than mesial and distal points. There was no significant difference between teeth (incisor, canine) at P=0.05. 5. All-ceramic bridges of three systems appeared to exhibit sufficient initial strength and accept able marginal fit values to allow clinical application.

흉부(胸部)의 X선촬영(線撮影) 및 촬영장치(撮影裝置) 현황(現況)에 관(關)한 조사(調査) (A Study on Radiographic Equipments and Situation for the Chest)

  • From the study on the chest radiologic equipments and situations of university hospitals, general hospitals, and radiologic clinics located in Seoul area and Choong-cheong province, the author got conclusions as follows : 1. The rectfication methods of the equipments were employing mainly single or three-phase full wave, and 78% of the examined were using single phase full wave. 2. The focal sizes in case of small focus were less than 1.0mm in chest-only use, and more than 1.0mm in 81% of combined use. The focal sizes in case of large focus were more than 1.6mm in 78% of the total. 3. Maximum ratings of x-ray tubes were 47% at 125 kVp maximum tube voltage and 41% at 150kVp. The maximum tube currentes higher than 500mA were shown in 72% of all the examined. And the maximum exposure time was the range of 5-8sec. 4. Grid was not used in the 66% of all the hospitals. Equipment for chest-only was employing 10:1 grid ratio and the 2.0-2.9mmAl of total filtration formed 38% of all the examined. 5. In the processing time of exposed film, 68% of all was 90sec, and only 20% of all the examined used optimum developing temperature according to this condition. 6. In the radiographic conditions, more than 50% used low tube voltages lower than 70 kVp and only 3% of all the equipments used high tube voltages higher than 100 kVp. The 46.9% of all the equipments 300mA, and 53% of all used shorter exposure time less than 1/20(0.05) sec. 7. In the monthly average number of radiographs, 44% of all was x-ray the radiographs.

Molecular Genetic Studies on 167 Pediatric ALL Patients from Different Areas of Pakistan Confirm a Low Frequency of the Favorable Prognosis Fusion Oncogene TEL-AML1 (t 12; 21) in Underdeveloped Countries of the Region

  • TEL-AML1 fusion oncogene (t 12; 21) is the most common chromosomal abnormality in childhood acute lymphoblastic leukemia (ALL). This translocation is associated with a good prognosis and rarely shows chemotherapeutic resistance to 3-drug based remission induction phase of treatment as well as overall treatment. Thus, the higher the frequency of this fusion oncogene, the easier to manage childhood ALL in a given region with less intensive chemotherapy. Although global frequency of TEL-AML1 has been reported to be 20-30%, a very low frequency has been found in some geographical regions, including one study from Lahore, Punjab, Pakistan and others from India. The objective of present study was to investigate if this low frequency of TEL-AML1 in pediatric ALL is only in Lahore region or similar situation exists at other representative oncology centers of Pakistan. A total of 167 pediatric ALL patients were recruited from major pediatric oncology centers situated in Lahore, Faisalabad, Peshawar and Islamabad. Patients were tested for TEL-AML1 using nested reverse transcription polymerase chain reaction (RT-PCR). Only 17 out of 167 (10.2%) patients were found to be TEL-AML1 positive. TEL-AML1+ALL patients had favorable prognosis, most of them (82.4%, 14/17) showing early remission and good overall survival. Thus, our findings indicate an overall low frequency of TEL-AML1 in Pakistan pediatric ALL patients, in accordance with lower representation of this prognostically important genetic abnormality in other less developed countries, specifically in south Asia, thus associating it with poor living standards in these ethnic groups. It also indicates ethnic and geographical differences in the distribution of this prognostically important genetic abnormality among childhood ALL patients, which may have a significant bearing on ALL management strategies in different parts of the world.

Type-Specific Incidence and Persistence of HPV Infection among Young Women: A Prospective Study in North India

  • Background: Infections with human papillomavirus (HPV) are highly prevalent among sexually active young women in India. However, not much is known about the incidence of type-specific human papillomavirus (HPV) infections and their patterns of persistence, especially in the Indian context. Objective: The objective of this study was to evaluate the rate of acquisition and persistence of HPV types in young women. Methods: Women residing in an urban slum in Delhi (n=1300) were followed for 24 months at 6 monthly intervals. Exfoliated cervical cells collected at each visit were tested for the presence of HPV DNA. Genotyping was performed using the reverse line blot assay. Results: The incidence rate for any HPV type was calculated to be 5 per 1000 women-months. Among high risk HPV types, HPV16 had the highest incidence rate followed by HPV59, HPV52 and HPV18, i.e., 3.0, 0.58, 0.41 and 0.35 women per 1000 women-months respectively. The persistence rate was higher for high-risk than low-risk HPV types. Among low-risk types, HPV42, HPV62, HPV84 and HPV89 were found to persist. Whereas almost all high risk types showed persistence, the highest rate was found in women with HPV types 16, 45, 67, 31, 51 and 59. The persistence rate for HPV16 infection was 45 per 1000 women-months. Conclusion: Incident HPV infections and high risk HPV type-specific persistence were found to be high in our study population of young married women. Understanding the patterns of HPV infection may help plan appropriate strategies for prevention programs including vaccination and screening.
