Antioxidant, Anti-inflammatory and Anti-allergenic Effects of Citrus Junos seed Oil and its Human Skin Protection (유자씨 오일의 항산화, 항염, 항알러지 효과 및 인체 피부보호 효과에 대한 연구)

  • In this study, in order to increase the utilization of Citrus junos seeds, which account for 13% of the weight ratio of Citrus junos ripened fruit, but are mostly discarded and not utilized, the efficacy of skin beauty of Citrus junos seed oil extracted by cold pressing was studied. Citrus junos seed oil was found to contain approximately 74% of unsaturated fatty acids consisting mainly of oleic acid and linoleic acid, and limonene, which is mainly contained in Citrus junos peel, contained a very low content of about 0.0187%. As a result of evaluating the DPPH radical scavenging activity of Citrus junos seed oil, 26% of DPPH radical scavenging ability was confirmed at 5% concentration of Citrus junos seed oil. To confirm the anti-inflammatory effect, as a result of testing RAW 264.7 cytotoxicity test and NO production for Citrus junos seed oil, NO production was suppressed by 53% at a concentration of 0.05% that does not show cytotoxicity. In addition, in the RBL-2H3 cytotoxicity and β-hexosaminidase release inhibitory efficacy test for anti-allergic efficacy confirmation, it was confirmed that β-hexosaminidas release was suppressed by 26% at a concentration of 0.05% that did not show cytotoxicity. Lastly, in the human skin application test result of O/W emulsion containing 5% of Citrus junos seed oil, it showed higher skin moisturizing effect than the control emulsion containing the same amount of caprylic/capric triglyceride. Therefore, it is thought that Citrus junos seed oil might be used as a excellent skin care material.

Parenchymal-sparing anatomical hepatectomy based on portal ramification of the right anterior section: A prospective multicenter experience with short-term outcomes

  • Backgrounds/Aims: Parenchymal-sparing anatomical hepatectomy (Ps-AH) based on portal ramification of the right anterior section (RAS) is a new technique to avoid unnecessarily transecting too much liver parenchyma, especially in cases of major anatomical hepatectomy. Methods: We prospectively assessed 26 patients with primary hepatic malignancies having undergone major Ps-AH based on portal ramification of the RAS from August 2018 to August 2022 (48 months). The perioperative indications, clinical data, intra-operative index, pathological postoperative specimens, postoperative complications, and follow-up results were retrospectively evaluated. Results: Among the 26 patients analyzed, there was just one case that had intrahepatic cholangiocarcinoma The preoperative level of α-Fetoprotein was 25.2 ng/mL. All cases (100%) had Child-Pugh A liver function preoperatively. The ventral/dorsal RAS was preserved in 19 and 7 patients, respectively. The mean surgical margin was 6.2 mm. The mean surgical time was 228.5 minutes, while the mean blood loss was 255 mL. In pathology, 5 cases (19.2%) had microvascular invasion, and in the group of HCC patients, 92% of all cases had moderate or poor tumor differentiation. Six cases (23.1%) of postoperative complications were graded over III according to the Clavien-Dindo system, including in three patients resistant ascites or intra-abdominal abscess that required intervention. Conclusions: Parenchymal-sparing anatomical hepatectomy based on portal ramification of the RAS to achieve R0-resection was safe and effective, with favorable short-term outcomes. This technique can be used widely in clinical practice.

Isolation and Identification of Bioactive Compounds from the Tuber of Brassica oleracea var. gongylodes

  • Brassica oleracea var. gongylodes (red kohlrabi) is a biennial herbaceous vegetable whose edible bulbotuber-like stem and leaves are consumed globally. Sliced red kohlrabi tubers were extracted using methanol and the concentrated extract was partitioned successively with dichloromethane (CH2Cl2), ethyl acetate (EtOAc), n-butanol (n-BuOH) and water (H2O). Repeated column chromatography of EtOAc fraction through silica, sephadex LH-20 and RP-18 gel led to isolation of eleven compounds of which compound 1 was a new glycosylated indole alkaloid derivative, 1-methoxyindole 3-carboxylic acid 6-O-β-D-glucopyranoside. Others were known compounds namely, β-sitosterol glucoside (4), 5-hydroxymethyl-2-furaldehyde (5), methyl-1-thio-β-D-glucopyranosyl disulfide (6), 5-hydroxy-2-pyridinemethanol (7), (3S,4R)-2-deoxyribonolactone (8), n-butyl-β-D-fructopyranoside (9), uridine (10) and three fructose derivatives, D-tagatose (11), β-D-fructofuranose (12) and β-D-fructopyranose (13). Similarly, isolation from CH2Cl2 fraction gave two known indole alkaloids, indole 3-acetonitrile (2) and N-methoxyindole 3-acetonitrile (3). The structure elucidation and identification of these compounds were conducted with the help of 13C and 1H NMR, HMBC, HMQC, EIMS, HR-ESIMS and IR spectroscopic data, and TLC plate spots visualization. Compounds 2, 3, 4, 5, 6, 7, 8 and 9 are noted to occur in kohlrabi for the first time. Different bioactivities of these isolated compounds have been reported in literature.

Effect of Post-traumatic Stress and Ways of Stress Coping on Problem Drinking in Firefighters (남자소방공무원의 외상 후 스트레스와 대처방식이 문제음주에 미치는 영향)

  • Purpose: The study aimed to determine the impact of post-traumatic stress and ways of stress coping on problem drinking in firefighters. Methods: Data were collected from May 11 to June 11, 2015, in fire brigades across South Korea. Participants were 183 male firefighters who used self-report questionnaires containing questions from the Impact of Event Scale-Revised (IES-R), Ways of Coping Checklist (WCC), and Alcohol Use Disorders Identification Test (AUDIT). Data were analyzed using $x^2$ test, t-test, and logistic regression with the SPSS/WIN 21.0 program. Results: The prevalence of PTSD among firefighters was 36.7% and that of problem drinking was 39.3%. Firefighters with single (OR=0.23, 95% CI=0.038~1.321) and married status (OR=0.46, 95% CI=0.069~3.040) were less likely to have problem drinking than those who were divorced or bereavement. Those with invasion symptoms (OR=1.17, 95% CI=1.031~1.327) and hyperarousal symptoms (OR=0.85, 95% CI=0.722~0.989) were more likely to have problem drinking than those with avoidance symptoms. Conclusion: Post-traumatic stress symptom was a major risk factor that increased problem drinking, and married status reduced problem drinking. There is a need to develop post-traumatic stress symptom management program and early education content for symptom management.

The Effect of Sexual Abuse on Posttraumatic Psychiatric Symptoms in Children and Adolescents with Sexual Abuse (성학대를 경험한 소아, 청소년에서 성학대가 외상 후 정신 증상에 미치는 영향)

  • Objectives : The purpose of this study is to examine the effects of the characteristics of victim and sexual abuse on posttraumatic psychiatric symptoms in children and adolescents with a history of sexual abuse. Methods : A total of 137 children and adolescents were recruited from the Seoul Sunflower Children Center, a nation-funded sexual violence victim protection center, from January 2009 to December 2013. We collected the demographic data of the victims and the Trauma Symptom Checklist for Children (TSCC) from victims. We hypothesized victims' age, sex, and intelligence quotient, and the characteristics of sexual abuse as the affecting factors of posttraumatic psychiatric symptoms. Descriptive analysis and hierarchical regression analysis were performed for analysis of demographic data, TSCC scores, and psychiatric symptoms. Results : The victims' age and the characteristics of sexual abuse were significantly related to the traumatic distress of sexual abuse. R-square was 23% for anxiety, 39% for depression, 21% for posttraumatic stress, and 37% for dissociation on TSCC. Conclusion : This study suggests that victims' age, type, frequency and duration of exposure, and disclosure of sexual abuse are significant affecting factors on posttraumatic psychiatric symptoms in children and adolescents. Exploration of psychiatric symptoms other than posttraumatic symptoms, and relations between pretraumatic and posttraumatic psychiatric symptoms is needed through collection of larger samples.

Development and validation of an analytical method for the determination of lepimectin residues by HPLC-PDA (HPLC-PDA를 이용한 lepimectin 잔류량 분석법 개발 및 확인)

  • A new, rapid, and simple analytical method was developed and validated using high performance liquid chromatograph-photodiode array detector (HPLC-PAD) for the determination of lepimectin residues in agricultural commodities. The lepimectin residues in samples were extracted with methanol, partitioned with dichloromethane, and then purified with glass column filled with subsequently to aminopropyl ($NH_2$) solid phase extraction (SPE) cartridge. The purified samples were detected using HPLC-PAD. Correlation coefficient ($R^2$) of both lepimectin $A_3$ and $A_4$ solutions were 0.9999. The method was validated using cucumber spiked with lepimectin at 0.02 and 0.2 mg/kg and pepper, mandarin, hulled rice, potato, soybean at 0.02 and 0.5 mg/kg. Average recoveries were 76.0~114.8% with relative standard deviation less than 10%, and limit of detection (LOD) and limit of quantification (LOQ) were 0.005 and 0.01 mg/kg, respectively. The result of recoveries and overall coefficient of variation of a laboratory results in Gwangju regional KFDA and Daejeon regional KFDA was followed with Codex guideline (CAC/GL 40). Therefore, developed method in this study is accurate, rapid, and appropriate for lepimectin determination and will be used to keep safety of lepimectin residues in agricultural products.

A Study of Parental Knowledge and Confidence in Newborn Care (신생아 부모의 양육지식과 양육자신감에 관한 연구)

  • Purpose: To explore the level of and relationship between parental knowledge and confidence in newborn care. Method: The sample of this study was consisted of 113 newborn's parents. Data was collected by researcher from Oct. 28th to Dec. 18th, 2002. Data were analyzed SPSS WIN(10.0) Program. Result: Total score of parents knowledge: Father's mean was 13.59(SD=5.32) and Mother's mean was 17.55(SD=3.72). There was meaningful difference in knowledge about newborn care between father and mother. The mother's knowledge was significant higher than the father's(t=6.45, p=.000). Total score of parents of newborn confidence: Father's mean was 40.66(SD=7.34) and Mother's mean was 41.88(SD=7.23). There was no difference in confidence in newborn care between father and mother(t=1.26, p=.209). There was significant correlation between knowledge and confidence about newborn care of fathers(r=.331, p<.001) and mothers(r=.317, p<.01). Conclusion: There is a need to educate fathers to enhance their knowledge about newborn care. More research is needed to study the relationship between parental knowledge and accuracy of behavior regarding newborn care.

Cytochrome P450 2C8 and CYP3A4/5 are Involved in Chloroquine Metabolism in Human Liver Microsomes

  • Chloroquine has been used for many decades in the prophylaxis and treatment of malaria. It is metabolized in humans through the N-dealkylation pathway, to desethylchloroquine (DCQ) and bisdesethylchloroquine (BDCQ), by cytochrome P450 (CYP). However, until recently, no data are available on the metabolic pathway of chloroquine. Therefore, the metabolic pathway of chloroquine was evaluated using human liver microsomes and cDNA-expressed CYPs. Chloroquine is mainly metabolized to DCQ, and its Eadie-Hofstee plots were biphasic, indicating the involvement of multiple enzymes, with apparent $K_m and V_{max}$ values of 0.21 mM and 1.02 nmol/min/mg protein 3.43 mM and 10.47 nmol/min/mg protein for high and low affinity components, respectively. Of the cDNA-expressing CYPs examined, CYP1A2, 2C8, 2C19, 2D6 and 3A4/5 exhibited significant DCQ formation. A study using chemical inhibitors showed only quercetin (a CYP2C8 inhibitor) and ketoconazole (a CYP3A4/5 inhibitor) inhibited the DCQ formation. In addition, the DCQ formation significantly correlated with the CYP3A4/5-catalyzed midazolam 1-hydroxylation (r=0.868) and CYP2C8-catalyzed paclitaxel 6$\alpha$-hydroxylation (r = 0.900). In conclusion, the results of the present study demonstrated that CYP2C8 and CYP3A4/5 are the major enzymes responsible for the chloroquine N-deethylation to DCQ in human liver microsomes.

Effects of Nursing Work Environment and Self-Efficacy of General Hospital Nurses on Medical Safety Competence in Korea (종합병원 간호사의 근무환경과 자기효능감이 투약안전역량에 미치는 영향)

  • Purpose: This study aimed to examine the effects of the nursing work environment and self-efficacy on medical safety competence of nurses in general hospital. Methods: Participants included 183 nurses who were recruited through convenience sampling in a general hospital in South Korea. Questionnaires were collected from August 25 to September 22, 2023. The nursing work environment, self-efficacy and medical safety competence scales were used to assess the study variables. The t-test, one-way ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression tests were performed using IBM SPSS ver. 26.0. Results: Statistically significant correlations were found between nurses' medical safety competence and nursing work environment (r=.34, p<.001), and self-efficacy (r=.48, p<.001). Self-efficacy (β=.38, p<.001), nursing work environment (β=.25, p<.001), and clinical experience (β=.14, p=.043) had the greatest effect on nurses' medical safety competence (F=21.21, p<.001), with an explanatory power of 30.0%. Conclusion: This study confirmed that the nursing work environment and nurses' self-efficacy have a significant impact on nurses' medical safety competence. In order to implement patient safety in hospitals, it is necessary to develop and apply strategies to improve the nursing work environment and increase nurses' self-efficacy to enhance their medical safety capabilities.

Respiratory air flow transducer calibration technique for forced vital capacity test (노력성 폐활량검사시 호흡기류센서의 보정기법)

  • Peak expiratory flow rate(PEF) is a very important diagnostic parameter obtained from the forced vital capacity(FVC) test. The expiratory flow rate increases during the short initial time period and may cause measurement error in PEF particularly due to non-ideal dynamic characteristic of the transducer. The present study evaluated the initial rise slope($S_r$) on the flow rate signal to compensate the transducer output data. The 26 standard signals recommended by the American Thoracic Society(ATS) were generated and flown through the velocity-type respiratory air flow transducer with simultaneously acquiring the transducer output signal. Most PEF and the corresponding output($N_{PEF}$) were well fitted into a quadratic equation with a high enough correlation coefficient of 0.9997. But only two(ATS#2 and 26) signals resulted significant deviation of $N_{PEF}$ with relative errors>10%. The relationship between the relative error in $N_{PEF}$ and $S_r$ was found to be linear, based on which $N_{PEF}$ data were compensated. As a result, the 99% confidence interval of PEF error was turned out to be approximately 2.5%, which was less than a quarter of the upper limit of 10% recommended by ATS. Therefore, the present compensation technique was proved to be very accurate, complying the international standards of ATS, which would be useful to calibrate respiratory air flow transducers.
