The Effects of Virtual Reality Program on Fall Efficacy and Activities of Daily Living for Patients with Stroke (가상현실 프로그램이 뇌졸중 환자의 낙상 효능감과 일상생활활동에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effect of virtual reality program on fall efficacy and activities of daily living(ADL) for patients with stroke. Method : Twenty stroke patients were participated voluntarily and were assigned randomly into the control and the experimental groups. Both control and experimental groups were given same basic rehabilitation treatment while only the experimental group was given virtual reality program. The fall efficacy of both groups was assessed using fall efficacy scale(FES) and ADL performance ability was assessed by functional independence measure(FIM). Result : Fall efficacy and ADL performance ability showed a statistically meaningful difference for both experimental and control groups(p< .05), the fall efficacy and ADL performance ability of the experimental group improved meaningfully in a statistical way than that of the control group(p<. 05). Conclusion : It was found that carrying out virtual reality program with occupational therapy has an effect on fall efficacy and ADL performance ability of patients with stroke. Therefore, virtual reality program can be a way of treatment for patients with stroke to improve their fall efficacy and ADL performance ability, and the program can also be applied in clinics as well as occupational therapy.

A study on Changes in ADL Functioning of Residents in Taejon City (가정간호환자의 일상생활작동수행능력 변화에 대한 연구)

  • This study was done to investigate the ADL differences between before and after home care. For this survey, the Barthel index, an ADU(activities of daily living) assessment, and general history questions were asked. Functional performance, i.e. ADL, was studied in a population a total of 56 men and women aged 65 and older from the city of Taejon. Among the independent subjects, women, 73 years of age and older, married status, 1-2 times taking home care per month, the case answering 'Quite' about satisfactory of home care, elderly dwelling with others and who have helper and spouse, elderly having a snack regularly, are statistically significant. Also ADL differences were found in grooming, getting in and out chair, getting on and off toilet, walking 500 meters on the level. Further studies should evaluate the activities of daily living to predict important disability-related outcomes.

The Impacts of Cognitive Function, Disease Severity, and Disability on Ability to Perform Activities of Daily Living after Stroke (뇌졸중 환자의 인지기능, 질병의 심각도 및 장애 정도가 일상생활수행능력에 미치는 영향)

  • Purpose: This study aimed to examine influence of cognitive function, disease severity and disability on ability to perform activity of daily living (ADL) after ischemic stroke. Methods: A total of 88 patients with ischemic stroke were recruited from January 1, 2008 to December 31, 2012 and assessed with the standardized cognitive test battery and self-reports about disease severity, disability, and ADL. To analyze the data, ANOVA, Pearson correlation coefficients and multiple regression were conducted using SPSS/WIN program. Results: Significant correlations were found between ADL and visuospatial function, visual memory, executive function, and disability (r=.29~.38). Executive function and disability explained 17.3% of total variability in ADL performance after ischemic stroke. Conclusion: Executive function may be a promising target for cognitive rehabilitation after ischemic stroke. Thus, effective therapeutic interventions such as cognitive training for stroke patients should be considered to improve their ability to perform ADL.

Improvement of Activities of Daily Living through Visiting Nursing Care under Long-Term Care Insurance: A Case Report using the OMAHA System (방문간호를 통한 일상생활동작 수행능력 개선에 대한 사례보고: 오마하시스템을 활용하여)

  • Purpose: This study was done to report nursing case for ADL improvement of elders who have CVA(Cerebrovascular Accident) sequelae. Methods: The client had registered in the C visiting nursing center after being decided a long-term care Grade 2. Data were collected through consultation logs for recipients, Activities of Daily Living (ADL) records, fall risk assessment (Huhn) sheets, decubitus ulcer risk assessment (Braden Scale) sheets, cognition assessment (K-MMSE) sheets, long-term care benefit provision records, and interviews with visiting nurse. Data were collected and analyzed according to the Omaha System problem classification. The intervention scheme and the problem rating scale for performance were applied to present the case for home-visit nursing. Results: The client registered in August, 2018, was provided home-visit nursing care once a week as of September 2020. ADL, cognitive levels and decubitus ulcer risks were found to have improved. Conclusion: This case report presents the value of classifying nursing problems and checking nursing intervention provided to patients with problems of ADL. The presentation of home-visit nursing cases applying a standardized nursing problem classification scheme for clients with various problems showed that a high quality level of care is guaranteed and evidence-based nursing can be provided by visiting nurses.

Effects of Low-intensity Exercise on Functional Ability in Hospitalized Elderly (저강도 운동프로그램이 입원노인의 일상활동 기능회복에 미치는 영향)

  • The purpose of the study was to identify the effects of a 12-week low-intensity exercise program on muscle strength, flexibility, balance, and cognitive characteristics related to the performance of activity of daily living(ADL). A total of 16 patients who were admitted to the medical unit of a general hospital in ChoongChung province were recruited, eight for the exercise group and eight for the comparison group. Four levels of low-intensity exercise from 'ROM on bed' to 'exercise while walking' were then applied to the exercise group according to their physical condition. During hospitalization, patients in the exercise group performed each level of the prescribed exercise with the researchers until they felt comfortable doing it independently. The researchers also visited the patients' homes after discharge to make sure they could perform the exercise with Theraband in their living environment. The exercise group was contacted by phone once a week to assess the frequency and intensity in which they performed the exercise as well as their physical condition. The subjects in the comparison group participated in measurements for the study without performing the exercise and were contacted by phone after discharge, in a matched time frame with the exercise group, to assess physical condition. Muscle strength, flexibility, balance, cognitive characteristics, and performance of ADL for the two groups were compared at the pretest and the posttest after the low-intensity exercise program by utilizing SPSSWIN and the results are as follows : 1) At the postest, measurements of muscle strengths showed that the strength of the dorsal flexor in the exercise group was significantly higher than in the comparison group. 2) Objective balance for the exercise group was significantly better than for the comparison group as measured by 'standing on one foot' and Tinetti gait and balance control. 3) The exercise group showed significantly higher task self-efficacy than the comparison group. 4) Perceived exertion for ADL for the exercise group was significantly lower than for the comparison group. 5) Improvement of performance of ADL without assistance was significantly higher for the exercise group than the comparison group. The findings suggest that a low-intensity exercise program would be useful for the elderly who show decline in their physical functioning due to hospitalization by partly improving physical strength, task self-efficacy, and performance of ADL. Directions for further research on issues of motivating people to exercise as well as of standardizing various types of exercise were discussed.

The Effect of the Cognitive State of the Elderly on the Performance Level in the Activities of Daily Living(ADL) (노인의 인지상태가 일상생활활동 수행정도에 미치는 영향)

  • Background: The purpose of this study is to determine how the cognitive state of the elderly influences the activities of daily living(ADL) in order to increase their achievement capacity and their ability to exercise this capacity in daily life. The MMSE-K(Mini-Mental State Examination-Korean) was used to evaluate the daily activities and cognitive state of a group of subjects more than 65 years of age. The goal was to understand (1) How their achievement level in their daily activities changes according to the increase and decrease of their cognition points by analyzing and comparing their achievement points in the mental state examination, and (2) What variables influence that daily achievement level. Methods: The survey was conducted by a group of students from AnSan College who were trained for this method of data collection. The group surveyed 224 senior citizens older than 65 to determine their ADL performance. The subjects were selected from Silver Care Centers in the Gyounggi Province area including centers in Suwon, Anyang, Kwachun, and Sanbon City. The subjects were given enough information to fully understand the purpose of the study and the method by which it would be conducted. The survey method was a direct interview: which involved an interviewer reading the questionnaire to the subject and recording the answer. Result: The following results were calculated using SPSS WIN ver. 13.0: In the lower score of the MMSE-K regarding ADL performance (K-MBI) for activities such as individual hygiene, bathing activity, food activity, toilet management, going up stairs, getting dressed ad undressed, controlling stools and urination, walking, and chair/bed movement, most of the subjects responded that they could not perform these activities by themselves (p<.01). In the higher scores for the same activities, most response that they could perform the tasks by themselves (p<.01). In the results of the multiple regression analysis, there influence of the variables for ADL performance (K-MBI) and MMSE-K score was higher for females than males. Conclusion: This study evaluated the cognitive state of the elderly using the MMSE-K, which is the most convenient method, and examined how the cognitive state influences the achievement capacity of the daily lives with other variables. In conclusion, the higher the score on the MMSE-K, the more independent the elders are in the activities of daily living, especially among women more than men.

The Correlation Between Relationship Ability and Activities of Daily Living(ADL) Performance Ability of Patients With Traumatic Brain Injury(TBI) (외상성 뇌손상 환자의 대인관계 능력과 일상생활활동 수행능력과의 상관관계)

  • Objective : To investigate the correlation between relationship ability and ADL performance ability of patients with TBI. Methods : This study was carried out from March 2012 to April 2012 with 20 patients with TBI. Relationship ability assessed using Relationship Change Scale(RCS), ADL performance ability assessed using Functional Independence Measure(FIM). Results : The RCS showed a high level of correlation with marital status and duration of illness. The RCS showed a high level of correlation with the FIM total score and the FIM cognition area. However the RCS did not show a correlation with the FIM motor area. Conclusions : Functional disability of social cognition is a factor that disturbs rehabilitation after TBI and it has a negative influence on the relationship in ADL. If accurate evaluation and treatment on relationship ability after TBI are carried out side by side from the initial stages of onset, we could expect more patients to improve their social cognition and ADL performance ability.

A Comparative Study of the Cognition, ADL and Quality of Life According to General and Pathological Characteristics of the Elderly with Dementia (치매노인의 일반적 및 병리학적 특성에 따른 인지기능, 일상생활동작, 삶의 질의 비교연구)

  • This study was conducted to compare the effects of general and pathological characteristics of elderly with dementia on their cognitive ability, performance of activities of daily living (ADL), and quality of life. Data were collected between July 15 and August 30, 2016 from 136 elderly with dementia who used day care centers. The Korean version of Mini-Mental Status Examination, Korean version of Modified Barthel Index, and Korean version of the WHO Quality of Life Scale Abbreviated Version were used for data collection and values obtained were analyzed accordingly. Significant correlations were found between cognitive ability and performance of ADL, between quality of life and cognitive ability, and between cognitive ability and performance of ADL (p<.001). Moreover, quality of life of elderly with dementia was greatly affected by cognitive ability and performance of ADL (p<.01). Therefore, it is believed that more active therapeutic interventions and studies are needed from sociophysical aspect of elderly with dementia.   • This study was to evaluate the effects how to have an influence on activities daily living and quality of life in day hospital program for the stroke patient. 41 experimental groups and 41 control groups in National rehabilitation hospital selected and examined K-ADL, K-IADL and quality of life. The result of this study indicated that experimental group was higher than control group in activity daily living performance and instrumental activity daily living performance. also experimental group was higher than control group all of total items in quality of life. age and economic state of general characteristic had an effect on K-ADL, K-IADL and quality of life and economic state, scholarship, marriage, job, helper had effect on quality of life. pain, social function, mental health, physical role in item of quality of life had effect on K-ADL and limit of physical role had best effect on K-IADL. Consequently we found that it needs to continuous rehabilitation to maintain body function, to prevent secondary disability, to improve social integration and quality of life for stroke patient actually.

  • The growth in the number of single-member households is a critical issue worldwide, especially among the elderly. For those living alone, who may be unaware of their health status or routines that could improve their health, a continuous healthcare monitoring system could provide valuable feedback. Assessing the performance adequacy of activities of daily living (ADL) can serve as a measure of an individual's health status; previous research has focused on determining a person's daily activities and extracting the most frequently performed behavioral patterns using camera recordings or wearable sensing techniques. However, existing methods used to extract common patterns of an occupant's activities in the home fail to address the spatio-temporal dimensions of human activities simultaneously. Though multiple sequence alignment (MSA) offers some advantages - such as inherent containment of the spatio-temporal data in sequence format, and rapid identification of hidden patterns - MSA has rarely been used to extract in-home ADL routines. This research proposes a method to extract a household occupant's ADL routines from a cumulative spatio-temporal data log of occupancy collected using a non-intrusive method (i.e., a tomographic motion detection system). The findings from an occupant's 28-day spatio-temporal activity log demonstrate the capacity of the proposed approach to identify routine patterns of an occupant's daily activities and to reveal the order, duration, and frequency of routine activities. Routine ADL patterns identified from the proposed approach are expected to provide a basis for detecting/evaluating abrupt or gradual changes of an occupant's ADL patterns that result from a physical or mental disorder, and can offer valuable information for home automation applications by enabling the prediction of ADL patterns.
