혈관 비틀림이 백서 천층하복부 동맥의 초미세문합에 미치는 효과 (Twisting Effect on Supermicroanastomosis of the Superficial Inferior Epigastric Artery in a Rat Model)

  • Purpose: The advent of microsurgical technique and instruments, particularly in the field of perforator flap and supermicrosurgery, which have expanded the scope of microsurgery. However, supermicroanastomosis without any compression, tension, or distortions must be achieved to reach successful outcomes. Small-caliber vessels, such as those with an internal diameter less than 0.2 mm, are susceptible to inadvertent twisting of the anastomosis. In this study, using the superficial inferior epigastric artery (SIEA)-based flap model in Sprague-Dawley (SD) rats, we evaluated the acceptable limits of twisting effects on supermicroanastomotic sites. Methods: A total of 20 supermicroanastomoses were performed using the SIEA-based flap model in 10 male SD rats, 10-weeks-of-age, weighing 300~350 g. Rats were divided into five groups of two with four flaps as follows: 1) sham, 2) control group with end to end SIEA arterial supermicroanastomosis, 3) experimental I (EA1) with $90^{\circ}$ twisting, 4) experimental II (EA2) with $180^{\circ}$ twisting, and 5) experimental III (EA3) with $270^{\circ}$ twisting of the supermicroanastomosis. Each SIEA was anastomosed using six 11-0 $Ethilon^{(R)}$ (Ethicon Inc. Co., NJ, USA) stitches except in the sham group where the SIEA was only clamped with Supermicro vascular $clamps^{(R)}$ (S&T, Neuhausen, Switzerland) for 20 minutes. Results: The anastomosed arterial patency showed no remarkable changes according to doppler waveforms measured with a Smardop 45 Doppler System (Hadeco Inc., Kawasaki, Japan). The pulsatility index (PI) was increased at postoperative day 10 in the EA2 and EA3 groups, and the resistance index (RI) showed no statistically significant difference between preoperative and postoperative values at 10 days. Histologic specimens from the EA3 group showed increased tunica media necrosis, convolution of the internal elastic lamina, densely packed platelets, fibrin, and erythrocytes. Flap viability and anastomosed vessel patency were not significantly affected by the degree of arterial twisting in this study, other than in the EA3 group where minor effects on arterial patency of the microanastomoses were encountered. Conclusion: It appears that minor twisting on small caliber arteries, used in supermicroanastomoses, can be tolerated. However, twisting should be avoided as much as possible, and more than $180^{\circ}$ twisting must be prevented in clinical practice.

락툴로스 함유 듀올리고 섭취에 의한 피부개선 효과 (Skin Health Effect of DuOligo Intake Containing Lactulose)

  • 본 연구에서는 락툴로스(lactulose) 51.67%와 갈락토올리고당(galactooligo saccharides) 15.8%로 이루어진 듀올리고(DuOligo)의 섭취를 통하여 피부개선 효능을 조사하였다. 40~60 대의 건강한 여성 37명을 대상으로 대조군(덱스트린)과 실험군(듀올리고군)으로 나누어 각각 8주 동안 섭취 후, 수분보유량, 경표피수 분손실량, 멜라닌지수, 홍반지수와 주름지수를 측정하였다. 그 결과, 듀올리고 섭취 8주 후의 수분 보유량은 대조군에 비하여 수분 보유량이 38.22% 유의적으로 증가하였다(p < 0.01). 대조군의 경피수분손실량은 8주후 $3.39g/h/m^2$로 감소한데 비하여 듀올리고군의 경피수분손실량은 8주후 $5.32g/h/m^2$ 감소하였다. 대조군의 멜라닌 지수는 시간에 따라 그 값이 유의적으로 증가하였으나, 듀올리고군의 멜라닌 지수는 시간에 따른 유의적 차이가 나타나지 않았다. 또한, 듀올리고군은 대조군에 비하여 주름의 총 넓이, 총 주름의 길이, 주름의 수 및 주름의 깊이가 확연하게 감소하였다(p < 0.05). 듀올리고의 피부개선 기능성 소재로서의 응용 가능성을 확인하였다.

People Participation Towards Opisthorchis viverrini Prevention and Control in Chaiyaphum Province, Northeastern Thailand

  • This cross-sectional descriptive and qualitative study was aimed to study the people participation and their approaches toward the human carcinogenic liver fluke, Opisthorchis viverrini, prevention and control in Ban Chaun sub-district administrative organization (BCSAO) and Bamnet Narong sub-district municipality (BNSM), Bamnet Narong district, Chaiyaphum Province, Thailand between June 2013 and February 2014. Participants were purposive selected, included chiefs of sub-district administrative organizations, sub-district municipalities, sub-district health promotion hospitals (SHPHs), heads of village, and a further sample was selected with a multi-stage random sampling for public health volunteers, and villagers. The pre-designed questionnaire contained items for individualized status and the participatory steps of sharing ideas, decision-making, and planning, procession, evaluation, and mutual benefit, for the project O. viverrini prevention and control (POPC). In-depth interviews were used for collection of need approaches to POPC. With 375 participants who completed the questionnaire, it was found that people had a high level regarding to participate in the POPC, particularly in the process stage ($\bar{X}=3.78$, S.D. = 0.56), but the lowest level was found in sharing ideas, decision making, and planning step ($\bar{X}=3.65$, S.D. = 0.63). By comparison, participant status and organization did not significantly differ with people participation. In each step, Ban Chaun sub-district had a high level of participation in the step of sharing ideas, decision making, and planning toward POPC, more than Bamnet Narong sub-district municipality (t=2.20, p=0.028). Approaches for POPC in Ban Chaun sub-district and Bamnet Narong sub-district municipality included requirements for budget support, annual campaigns for liver fluke prevention and control, campaign promotion, risk group observation, home visiting, community rules regarding reducing raw fish consumption in their communities, and a professional public health officer for working in their communities, BCSAO, BNSM, and SHPH, for O. viverrini prevention and control. This study indicates that people realize that eradication of the liver fluke needs a continuous people participation for O. viverrini prevention and control in their communities.

광조사 강도가 복합레진의 중합반응속도에 미치는 영향에 관한 실시간 체적측정법을 이용한 연구 (EFFECT OF LIGHT INTENSITY ON THE POLYMERIZATION RATE OF COMPOSITE RESIN USING REAL-TIME MEASUREMENT OF VOLUMETRIC CHANCE)

  • Objectives : The aim of this study is to evaluate the effect of light intensity variation on the polymerization rate of composite resin using IB system (the experimental equipment designed by Dr. IB Lee) by which real-time volumetric change of composite can be measured. Methods : Three commercial composite resins [Z100(Z1), AeliteFil(AF), SureFil(SF)] were photopolymerized with Variable Intensity Polymerizer unit (Bisco, U.S.A.) under the variable light intensity (75/150/225/300/375/450mW$^2$) during 20 sec. Polymerization shrinkage of samples was detected continuously by IB system during 110 sec and the rate of polymerization shrinkage was obtained by its shrinkage data. Peak time(P.T.) showing the maximum rate of polymerization shrinkage was used to compare the polymerization rate. Results : Peak time decreased with increasing light intensity(p<0.05). Maximum rate of polymerization shrinkage increased with increasing light intensity(p<0.05). Statistical analysis revealed a significant positive correlation between peak time and inverse square root of the light intensity (AF:R=0.965, Zl:R=0.974, SF:R=0.927). Statistical analysis revealed a significant negative correlation between the maximum rate of polymerization shrinkage and peak time(AF:R=-0.933, Zl:R=-0.892, SF:R=-0.883), and a significant positive correlation between the maximum rate of polymerization shrinkage and square root of the light intensity (AF:R=0.988, Zl:R=0.974, SF:R=0.946). Discussion and Conclusions : The polymerization rate of composite resins used in this study was proportional to the square root of light intensity Maximum rate of polymerization shrinkage as well as peak time can be used to compare the polymerization rate. Real-time volume method using IB system can be a simple alternative method to obtain the polymerization rate of composite resins.

에티오피아 남부 켄티차 페그마타이트의 분화양상과 희유원소 광화작용 (Fractionation and Rare-Element Mineralization of Kenticha Pegmatite, Southern Ethiopia)

  • 켄티차(Kenticha) 희유원소(Ta-Li-Nb-Be) 광화대는 에티오피아 남부의 오피올라이트 습곡-쓰러스트 복합체내에 위치하고 있으며, 1980년대 에티오피아-구소련간 수행된 공동탐사 프로그램을 통해서 처음 발견되었다. 켄티차 희유원소 광화대는 남쪽에서 북쪽으로 덜미다마(Dermidama), 킬킬리(Kilkele), 슈니힐(Shuni Hill), 켄티차, 부포(Bupo) 페그마타이트를 포함한다. 켄티차 페그마타이트는 남북방향의 사문암과 활석-녹니석 편암층을 평행하게 관입하고 있으며, 대략 연장 2 km, 폭 400-700 m로 노출되어 있다. 켄티차 페그마타이트는 내부적으로 대상(zoning)을 이루고 있으며, 광물조합에 따라 하부 석영-백운모-조장석 화강암, 중부 백운모-석영-조장석-미사장석 페그마타이트, 상부 스포듀민-석영-조장석 페그마타이트로 세분된다. 주성분, 미량성분(Rb, Li, Nb, Ta, Ga), 원소비(K/Rb, Nb/Ta, Mg/Li, Al/Ga)는 페그마타이트의 분별작용과 고화작용이 하부에서 상부 페그마타이트로 진행되었음을 시사한다. 반면에 일반적인 페그마타이트 멜트의 분화양상과 달리 켄티차 페그마타이트의 Mg/Li 비는 상부 페그마타이트로 가면서 증가하는 양상을 보인다. 이러한 양상은 후기 마그마성 열수변질 또는 상부 초염기성암과의 반응 결과로 해석된다. 켄티차 페그마타이트의 희유원소 광화작용은 상부 페그마타이트에 집중되며, 최대 $Li_2O$ 3.0%, Rb 3,780 pm, Cs 111 ppm, Ta 1,320 ppm, and Nb 332 ppm을 함유하고 있다. 켄티차 페그마타이트에서 광석광물은 주로 탄탈라이트, 스포듀민, 레피돌라이트가 산출되며, 탄탈라이트는 거정의 석영-스포듀민과 상대적으로 세립의 당상 조장석과 함께 산출된다. 탄탈라이트는 $Mn^*$$Ta^*$ 값에 근거할 때 Mn-탄탈라이트로 분류된다. 거정질과 세립의 탄탈라이트는 조성($Mn^*$, $Ta^*$, Nb/Ta) 차이가 명확히 나타나며, 거정질 탄탈라이트는 세립의 탄탈라이트에 비해 Ta이 강하게 부화되며 Nb은 결핍되어 있다. 결론적으로 켄티차 페그마타이트에서 희유원소 광화작용은 마그마 멜트의 분별작용 최후기에 형성되었다.

치과위생사의 근로생활의 질(QWL)에 관한 연구 (A study of quality of working life to dental hygienist's)

  • Objectives : Dental hygienist's work satisfaction and stress affect the overall quality of work life(QWL). Therefore, this research is intended to suggest fundamental data to improve QWL by finding out characteristics of each work satisfaction and stress element. To this end, a total of 327 dental hygienists working at general hospitals, university hospitals, dental hospitals and dental clinics across Seoul, Gyeonggi and Incheon were surveyed. Results of survey are as follows. Methods : The collected data were analyzed by using an SPSS 12.0 statistical program, obtaining the following results. The collected data conducted a questionnaire survey for 327 dental hygienists who work at the hospitals, university hospitals, dental hospitals, and dental clinics located at Seoul, Gyeonggi-do, and Incheon district from January until March, 2009, and drew the conclusions as follows. Result : 1. Demographic characteristics, income from 1.5 to 1.99 million were the whole lot, more than 2 million to less than 1.5 million was similar. Marital status Married Unmarried higher than the atheist religion, Christianity, Catholicism, Buddhism, and other, respectively. Classification by level of education in the college graduate, university graduate, graduate diploma, respectively. 2. Are working in a job-related characteristics of dentistry, dental hospital, general and university hospital, respectively. The making in position, Mount, contractor, responsible, senior, was an intern in the order. The five-day workweek whether working at night and is not going to care whether the conduct was similar. Classification of working hours and 8 hours, 8 hours, 8 hours or less orderly, and total of less than 1-3 years of clinical experience, 5 years, less than one year, less than 3-5 years, respectively. 3. There comes out a significant difference according to age, income, position, gross clinical experience, and whether to put night shift into practice in job stability in terms of the quality subsequent to general characteristics(p<.05). 4. There comes out a significant difference according to marital status, one's place of work, position, whether to put a five-day workweek into practice in work environment and benefits package in terms of the quality subsequent to general characteristics (p<.05). 5. There comes out a significant difference according to age, marital status, income, position, and gross clinical experience in education & training and benefits packages in terms of the quality subsequent to general characteristics(p<.05). 6. There comes out a significant difference according to whether to put night medical treatment into practice in social usefulness in terms of the quality subsequent to general characteristics(p<.05). 7. There comes out a significant difference according to marital status, income, one's place of work, gross clinical experience, work hours, and whether to put a five-day workweek into practice in leisure activity in terms of the quality subsequent to general characteristics(p<.05). 8. There comes out a significant difference according to income, one's place of work, and position in wage level in terms of the quality subsequent to general characteristics(p<.05). 9. There was no significant difference in all items related to human relations and free communication in terms of the quality subsequent to general characteristics(p>.05). Conclusions : It is necessary to analyze factors related to work satisfaction and stress in order to improve dental hygienist's quality of work life. Hospitals must support them systematically and institutionally and related organizations must conduct practical research.

지체 장애인의 베체트병 약물치료 중 발생한 구강 건조증 치험 1례 (A Case of Xerostomia that Occurred during Behcet's Disease Drug Treatment of a Person with Physical Disability)

  • 2020년 7월부터 2020년 12월까지 일개 보건소 건강증진과에서 관할지역 거주 중증 장애인 중 뇌병변 장애와 지체 장애인을 대상으로 한의약 장애인 방문건강관리 표준프로그램을 진행하였고, 이 중 1명의 중증 지체 장애인이 구강 건조증을 앓고 있어, 한의 치료의 개입을 통한 통합적인 관리한 결과는 다음과 같다. 1. 팔물탕(八物湯) 연조엑스제를 포함한 한의 치료는 YDQ에 의해 음허(陰虛)로 변증된 지체 장애인의 약물 유발성 구강 건조를 완화시키고 삶의 질을 호전시켰다. 2. 건강 보험에 포함된 한의 치료를 제공하여 한·양방 통합적 개입으로 양약으로 발생한 구강 건조 증상을 관리할 수 있었다. 3. 6개월간 투약된 팔물탕(八物湯) 연조엑스로 인한 이상 반응은 없었다. 4. 팬데믹 상황에서 한의약 방문 진료와 전화 진료가 혼합된 형태의 공공보건기관의 한의약 건강증진사업으로 장애인의 SHCN를 충족하고 장애인의 만성 질환 관리가 가능한 부분을 확인할 수 있었다. 5. 본 연구는 한방안이비인후피부과 증상 및 질환도 보건소 한의약 건강증진사업으로 포함할 수 있는 가능성을 확인하였으며, 유관 학회의 협력 및 지원과 연구가 필요하다고 생각된다.

Anemia-inducing Murine Friend Virus 감염에 대한 2',3'-dideoxycytidine 및 $Interferon-{\alpha}-A$의 항retrovirus효과 (Antiretroviral Effects of 2',3'-Dideoxycytidine and Recombinant $Interferon-{\alpha}-A$ on the Infection of Anemia-inducing Murine Friend Virus)

  • Anemia-inducing strain of Friend virus (FVA)는 적혈구 progenitor cell의 증식을 촉진하는 생쥐 retrovirus의 일종이다. FVA에 감염된 생쥐는 생성이 촉진된 progenitor cell이 분화되지 못하고 비장내에 축적되므로서 비장비대를 초래한다. 이에 본 실험에서는 FVA에 감염된 생쥐의 비장비대를 지표로 사용하여 2',3'-dideoxycytidine (ddC) 및 $interferon-{\alpha}-A\;(rIFN-{\alpha}-A)$의 항retrovirus효과를 측정하였다. 매일 ddC (100 mg/kg) 및 $rIFN-{\alpha}-A$ (10 KU/mouse)를 각각 단독 또는 병용하여 18일간 복강내 투여시 비장의 비대가 각각 15.1%, 52.7%, 61.6% 억제되었다. 또 다른 실험군으로 ddC를 식수중에 용해하여 (0.1 mg/ml) 경구로 18일간 투여시, 그리고 ddC의 경구투여와 병용하여 $rIFN-{\alpha}-A$을 위와 마찬가지 용량으로 복강내 투여시, 비장비대를 각각 38.4% 및 83.2% 억제하였다. 이 결과는 ddC의 투여시 복강내 주사보다는 경구투여가 더 유효하며, ddC와 $rIFN-{\alpha}-A$는 병용투여시 상가적인 효과가 있음을 제시한다. ddC 투여시 progenitor cell의 특성상 변화를 검토하기 위해, $Ca^{++}$ uptake $[^3H]cyclohexyladenosine$ (CHA) binding 실험을 실시하였다. CHA bindng 실험결과 성숙된 적혈구에서는 저친화성의 결합부위 하나뿐인데 반해, progenitor cell에서는 고친화성과 저친화성의 두가지 결합부위를 나타내었다. $Ca^{++}$ uptake 측정결과 성숙된 적혈구에 비해 대조군의 정상적인progenitor cell은 약 20배 증가를 나타내었으며, ddC를 연속투여한 군에서도 유사한 결과를나타내었다. 이때 CHA에 의한 $Ca^{++}$ uptake의 억제효과를 측정한 바, ddC 100 mg/kg 투여군의 경우 76%로 대조군의 86% 보다 억제효과가 크게 나타났으며, 이들 모두는 adenosine 길항약인 theophylline의 전처치시 대조군과 유사하게 회복되었다.

병렬 U형 현장타설 에너지파일의 열교환 효율 및 적용성 평가 (Evaluation of Heat Exchange Efficiency and Applicability for Parallel U-type Cast-in-place Energy Pile)

  • 에너지파일은 기존의 수직밀폐형 지중열교환기를 경제적으로 대체할 수 있는 새로운 형태의 지중열교환기이다. 즉, 에너지파일은 건물의 기초 구조물과 지중열교환기의 역할을 동시에 수행하는 에너지 구조체로서, 말뚝 기초 내부에 열교환 파이프를 삽입하고 파이프를 통해 유체를 순환시켜 지반과의 열교환을 유도한다. 본 연구에서는 병렬 U형(5쌍, 8쌍, 10쌍)의 열교환 파이프를 대구경 현장타설 에너지파일에 삽입하여, 3본의 에너지파일을 실규모로 시공하였다. 또한 현장 열응답 시험(In-situ thermal response tests, TRTs)을 수행하고 시공된 현장타설 에너지파일과의 열교환 효율을 비교하기 위하여 30m 깊이의 수직밀폐형 지중열교환기를 별도로 시공하였다. 병렬 U형 현장타설 에너지파일에 대해서는 냉난방 부하를 인공적으로 주입하는 열교환 성능 평가시험을 수행하여 열교환 성능(heat exchange rate)을 평가하였다. 마지막으로 현장타설 에너지파일의 적용성 평가를 위해 산정된 상대 열교환 효율(relative heat exchange efficiency) 및 열교환 성능을 선행 연구 결과와 비교하였으며, 본 연구에서 시공된 현장타설 에너지파일은 안정적이고 효율적인 열성능을 보이는 것으로 나타났다.

신개발 키토산 제재의 지혈 효과에 대한 비교 (Experimental Assessment of Hemostatic Agents: Comparison with New Developed Chitosan-Based Material)

  • Purpose: Many hemostatic agents and dressings have been tested with variable degree of success. Chitosan has a positive charge, it attracts red blood cells, which have a negative charge. Our goal is to test the efficacy of new developed chitosan-based hemostatic materials in providing durable hemostasis in a high-flow arterial wound model. Methods: We compared each group with SD rats motality tests and in vitro blood compatibility test by blood clotting index (BCI). We devided the SD rats into 6 groups (N =15) by type of hemostatic agents. A: 100% nonwoven chitosan (degree of the deacetylation: 90%). B: 50% N-acetylation on nonwoven of chitosan gel (degree of the deacetylation: 50%). C: 60% N-acetylation on nonwoven of chitosan ge (degree of the deacetylation: 40%)l. D: Cutanplast$^{(R)}$. E: HemCon$^{(R)}$ F: Gauze. In vivo test, a proximal arterial injury was created in unilateral femoral arteries of 90 anesthetized SD rats. Each materials was made same size and thickness then applied to the injury site for 3 minutes. In vitro test, we compared each group with BCI in human blood. Results: In vivo test, group A showed lower motality rate of 46% than any other groups, Group B and C showed lower motality rate of 60% than group D and E's motality rate of 66%. In vitro test, BCI of group A ($30.6{\pm}1.2$) and B ($29.3{\pm}1.0$) were showed nearly about group D ($29.1{\pm}1.8$) and E ($27.4{\pm}1.6$). Group C ($37.1{\pm}2.0$) showed higher BCI than group A and B, it means group C decreased blood clotting. Conclusion: In conclusion, this study suggests a newly developed chitosan-based hemostatic materials induced durable hemostasis and increased blood clotting, and are considered as effective biologic hemostatic agents.
