Patch Reconstruction with Radial Forearm Free Flap of Hypopharyngeal Cancer Using the Narrow Strip Pharynageal Wall (소폭의 잔존 하인두벽을 이용한 첩포형 전완유리 피판 인두 재건술)

  • Purpose: Various attempts of reconstruction for pharyngoesophageal defects after ablative surgery have been made to restore the function of the pharyngoesophagus. A fabricated tubed radial forearm free flap or free jejunal free flap was used when the width of remnant pharyngeal wall was less than 50% of the normal width. However there are many disadvantages such as stricture, saliva leakage and fistula formation on tubed radial forearm free flap. The jejunal free flap has the problem such as short pedicle, poor tolerance of ischemic time, wet voice and delayed transit of swallowed food due to the uncoordinated contraction. The authors studied the utility of patch-type radial forearm free flap using the remnant posterior pharyngeal wall of the hypopharynx. Methods: Retrospective reviews in Severance Hospital were made on 25 patients who underwent reconstruction surgery with patched radial forearm free flap because of the hypopharyngeal cancer between 1996 and 2005. The patients of Group I had the narrow posterior pharyngeal wall and its width was less than 3centimeters after the tumor was resected. Those of Group II had the partial pharyngectomy and the width of the remnant pharynx was larger than 3 centimeters. Results: Seven patients belonged to the group I and the flap of this group had 100% survival rate. One case of fistula and no swallowing discomfort due to stricture was reported. The Group II including 18 patients also had the 100% flap survival rate. Neither fistula nor stricture was seen but the lower diet grade was checked. Conclusion: The patch type radial forearm free flap using the remnant pharyngeal wall have the advantage of the radial forearm free flap, and furthermore this flap is the safe reconstructive method even if the width of the remnant pharyngeal wall is less than 30% of that of normal pharynx.

Electron Microscopic Study on the Effect of Albendazole Against Paragonimus Westermani (Albendazole의 폐흡충에 미치는 영향에 관한 전자현미경적 연구)

  • Albendazole is a broad spectrum anthelmintics with high activity against intestinal nematodes as well as tremtodes and cestodes infections. But so far wormicidal effect of albendazole against Paragonimus westermani is uncertain. The present study undertaken to observe the morphological changes of Paragonimus westermani which was obtained from experimentally infected with metacercariae isolated from Cambaroides similis and 9 of them were given albendazole 2 doses of 25mg/kg daily for the 2 days and 4 days from the day of 20 weeks after infection. The Paragonimus werms were collected from the lung of the cats which were autopsied at 24 hours after treatment. The fine structures of the collected worms were studied by means of scanning and transmission electron microscopy. The findings of the observations were compared with those of untreated worms. The results are as follows : 1) In the scanning electron microscopic(SEM) observations, the worms obtained from 2 days treated group showed many blebs which were formed on the surface of damaged tegument in between two suckers. Protrusion of oral sucker typically observed and surrounding tegumental ridges were damaged. 2) The worms obtained from 4 days treated group showed marked contraction revealing half size of normal worms. 3) The transmission electronmicroscopic(TEM) observations indicated that large blebs of irregular shapes were formed by the destruction of tegumental ridge of tegumental syncytium and also showed degeneration of mitochondriae. 4) Destruction of tegumental syncytium formation of blebs were usually observed in the ventral side of the worms. 5) Differentiation of tegumental layer and basement layer muscular layer and parenchymal layer around two suckers of worms obtained at 4 days treated group was difficult. 6) Many host cells invaded and destroyed the degenerated tegument by the occurrence of blebs or the exposed muscular layer of the worm after exfoliation of the tegument.

The Inhibitory Effects of 5-Hydroxytryptamine on the Intestine (5-Hydroxytryptamine의 장억제작용(腸抑制作用))

  • The inhibitory effect of 5-hydroxytryptamine (5-HT) on the isolated intestinal strips of the tortoise (Amyda japonica), rat, rabbit and guinea pig was investigated. 1) The strips from the middle or lower part of the tortoise intestine responded with relaxation to 5-HT $(10^{-9}{\sim}10^{-5}g/ml)$, and the magnitude of the relaxation was proportional to the dose of 5-HT. The rectal part of the tortoise intestine, in contrast, showed contraction, the magnitude of which also was proportional to the dose of 5-HT. 2) Various blocking agents such as methysergide, morphine, tetracaine, nethalide, bretylium, hexamethonium, mecamylamine and chlorisondamine, showed no selective blocking activity on the relaxant effect of 5-HT on the tortoise intestine. The inhibitory effect of isoproterenol on the tortoise intestine, however, was selectively blocked by nethalide, and the stimulatory effect of 5-HT on the rectal part of the tortoise was blocked by methysergide. 3) In the presence of 5-HT, the stimulatory effect of DMPP on the tortoise intestine was remarkably attenuated, whereas that of acetylcholine and $BaCl_2$ was little affected. In the presence of isoproterenol, the stimulatory effect of acetylcholine and $BaCl_2$ were affected, but that of DMPP was little affected. 4) Large dose of 5-HT($10^{-4}$g/ml) produced inhibitory effect on the strips from the distal part of the isolated colon of the rat, rabbit and guinea pig, when the strips had been exposed to 5-HT($10^{-4}$g/ml), methysergide or phen`oxybenzamine. 5) Bretylium, as well as nethalide, abolished or remarkably reduced, in a few cases of the experiments, the inhibitory effect of the large dose of 5-HT on the distal part of the colon, whereas morphine did not affect it. 6) The ileal strips of the guinea pig also showed relaxation, as in the colonic strips, having been exposed to the large dose of 5-HT or phenoxybenzamine. This effect, however, was not obsered in the case of the rabbit ileum. 7) The property of the action-site of 5-HT in the tortoise intestine seemd to be different from the 5-HT receptors which have been revealed by several investigators. 8) Adrenergic component seemed to be participated in the inhibitory effect of 5-HT on the colon of the rat and rabbit.

Anatomical Difference Between Two Rice Cultivars Selected to Bensulfuron (Bensulfuron에 내성(耐性) 및 감수성(感受性)인 수도품종간(水稻品種間)의 해부학적(解剖學的) 비교(比較) 연구(硏究))

  • Bensulfuron concentrations of $10^{-7}$, $10^{-6}$, $10^{-5}$ and $10^{-4}M$ were applied to agar medium on susceptible (cv. KH 17854 and cv. IR 1846) and tolerant (cv. Chinsurah Boro II and IR 14252) rice cultivars were grown for microscopic inspection. Susceptible cultivars showed the decrease in shoot and root growth at the concentration of $10^{-5}M$ while ones showed no difference. Such a tendency was also observed from microscopic inspection in the elongation zone of shoot meristematic tissue. Seedlings grown in soil for 10 days were transfered to distilled water containing only bensulfuron solutions. There were significant differences between cultivars in terms of supression of shoot meristematic activity and swelling of cell volume. Observations of those cells made it clear that especially susceptible cultivars showed the irregular cell layering, vacuolation, cell swelling and partial damage in membrane of shoot tissue. The major response of root tips of susceptible cultivars showed the disorganization of cortex, rupture and contraction of membrane, inhibition of cell division, swelling and emergence of lateral root while tolerant ones showed no such responses.

Induction of Soft Tunic Syndrome by Water Temperature and Physiological and Histological Responses of the Sea Squirt, Halocynthia roretzi (수온에 의한 멍게(Halocynthia roretzi)의 물렁증 유도와 생리 및 조직학적 반응)

  • In this study, we investigated the changes in the physiological and histological traits of a sea squirt (Halocynthia roretzi) with the emergence of the soft tunic syndrome induced by the water temperature control (6, 9, 12, 15, 18, 21, 24 and $27^{\circ}C$). It was observed that the induction rate of the soft tunic syndrome was highest at $15^{\circ}C$, but lowest at $24^{\circ}C$. Based on the tunic color condition and contraction strength, the whole process were classified into 4 stages as S0, S1, S2 and S3. Interestingly, there were significant differences in oxygen consumption and filtration rate were observed during S0-S3. The most distinctive aspects were change of blood cell composition at stage S3, whereas multi-vacuole cell ratio was decreased by 1/2 and morula cell ratio expanded about 10 times during S0-S3. Further, change of organ structure started following the syndrome such as degeneration of epithelial cells, microfilaments, increment in hemocytes and damage in muscle fiber have been detected in tunic, siphon, branchial sac, body wall musculature and pyloric gland. Briefly, our study results indicated that the normal physiological functions of the sea squirt can be affected due to the soft tunic syndrome induced by water temperature.

  • Since online betting occurs on online where anybody can access through internet, thus, the online gambling market has been expanding. Domestic illegal gambling market size is 4 times bigger than that of legal gambling market, and illegal online betting market is rapidly growing as well. Currently online gambling does exist domestically in form of Sports promotion voting rights, also known as, Toto and Proto and internet lottery. However, the need for other types of online gambling to be permitted, expanded and managed has been suggested. This research has analyzed online gambling users, online gambling addicts and general population's awareness on expansion and operation of online gambling, contraction measures of illegal online gambling and operation measures in case of permitted expansion of online gambling. The result of this research's analysis showed that each group has significantly different awareness level on positive and negative effects of permitting online gambling's expansion. Also, it showed that each group has significantly different awareness on permitting expansion of racing betting as a legal gambling item. Since it is the proven fact that illegal online gambling users could turn into legal users and that users could use the service more positively if online gambling is legalized through allowance and expansion process; one could expect the positive effects from permitting expansion of online gambling.

Pharmacological Characterization of Synthetic Tetrahydroisoquinoline Alkaloids, YS 51 and YS 55, on the Cardiovascular System

  • Tetrahydroisoquinoline (THI) alkaloids can be considered as cyclized derivatives of simple phenylethy-lamines, and many of them, especially with 6,7-disubstitution, demonstrate relatively high affinity for catecholamines. Two -OH groups at 6 and 7 positions are supposed to be essential to exert ?${\beta}-receptor$ activities. However, it is not clear whether -OH at 6,7 substitution of THIs also shows ?${\alpha}-adrenoceptor$ activities. In the present study, we investigated whether -OH or $-OCH_3$ substitutions of 6,7 position of THIs differently affect the ?1-adrenoceptor affinity. We synthesized two 1-naphthylmethyl THI alkaloids, $1-{\beta}-naphthylmethyl-6,7-dihydroxy-1,2,3,4-tetrahydroisoquinoline$ HBr (YS 51) and $1-{\beta}-naphthylmethyl-6, 7-dimethoxy-1,2,3,4-tetrahydroisoquinoline$ HCl (YS 55), and their pharmacological actions on ?${\alpha}_1-adrenoceptor$ were compared. YS 51 and YS 55, concentration-dependently relaxed endothelium-denuded rat thoracic aorta precontracted with phenylephrine (PE, 0.1 ${\mu}M$) in which $pEC_{50}$ were $5.89{\pm}0.21$ and $5.93{\pm}0.19$, respectively. Propranolol (30 nM) did not affect the relaxation-response curves to YS 51 and YS 55. Concentration-response curves to PE were shifted to right by the pretreatment with YS 51 or YS 55. The $pA_2$ values of YS 51 and YS 55 showed $6.05{\pm}0.24$ and $5.88{\pm}0.16$, respectively. Both probes relaxed KCl (65.4 mM)-contracted aorta and inhibited $CaCl_2-induced$ contraction of PE-stimulated endothelium- denuded rat thoracic aorta in $Ca^{2+}-free$ solutions. In isolated guinea pig papillary muscle, 1 and 10 ${\mu}M$ YS 51 increased contractile force about 4- and 8- fold over the control, respectively, along with the concentration-dependent increment of cytosolic $Ca^{2+}$ ions. While, 10 ${\mu}M$ YS 55 reduced the contractile force about 50 % over the control and lowered the cytosolic $Ca^{2+}$ level, in rat brain homogenates, YS 51 and YS 55 displaced $[^3H]prazosin$ binding competitively with Ki 0.15 and 0.12 ${\mu}M$, respectively. However, both probes were ineffective on $[^3H]nitrendipine$ binding. Therefore, it is concluded that two synthetic naphthylmethyl-THI alkaloids have considerable affinity to ?1-adrenenoceptors in rat aorta and brain.

Curing Behavior and Interfacial Properties of Electrodeposited Carbon Fiber/Epoxy Composites by Electrical Resistivity Measurement under Tensile/Compressive Tests (전기증착된 탄소섬유/에폭시 복합재료의 인장/압축 하중하에서의 전기저항 측정법을 이용한 경화 및 계면특성)

  • Curing behavior and interfacial properties were evaluated using electrical resistance measurement and tensile/compressive fragmentation test. Electrical resistivity difference (${\Delta}R$) during curing process was not observed in a bare carbon fiber. On the other hand, ${\Delta}R$ appeared due to the matrix contraction in single-carbon fiber/epoxy composite. Logarithmic electrical resistivity of the untreated single-carbon fiber composite increased suddenly to the infinity when the fiber fracture occurred under tensile loading, whereas that of the ED composite reached relatively broadly up to the infinity. Comparing to the untreated case, interfacial shear strength (IFSS) of the ED treated composite increased significantly in both tensile fragmentation and compressive Broutman test. Microfailure modes of the untreated and the ED treated fiber composite showed the debonding and the cone shapes in tensile test, respectively. For compressive test, fractures of diagonal slippage were observed in both untreated and the ED treated composite. Sharp-end shape fractures exhibited in the untreated composite, whereas relatively dull fractures showed in the ED Heated composite. It is proved that ED treatments affected differently on the interfacial adhesion and microfailure mechanism under tensile/compressive tests.

Influence of Crosslinking on Gelatinization Behavior and Morphological Change of Potato Starch (가교결합 감자 전분의 호화특성과 형태학적 변화)

  • Gelatinization behavior and morphology of epichlorohydrin-crosslinked potato starches (XPs) were investigated. Native potato starch showed a very steep single stage swlling pattern, but crosslinked starches showed various patterns with the degree of crosslinking. Swelling power, solubility and light transmittance were reduced drastically as the degree of crosslinking increased. Brabender initial pasting temperature and peak temperature of crosslinked starches increased because the crosslinking reinforces the intermolecular net work of the starches. Although the swelling of the potato starch granule was inhibited by crosslinking as compared to that of the native one, Brabender peak viscosities (6.5% w/v, db) were on the order of 2,500 units for the native potato starch, 3,700 for the XP with 2.300 anhydroglucose units per crosslinking (AGU/CL) and 3,400 for the XP with 2,100 AGU/CL, due to the decreased breakdown of the swollen granule resulting from the resistance to heat and shear. The XP with 1,900 AGU/CL, however, did not show the peak viscosity and the viscosity was on the order of 500 units because of the excessive unhibition of the swelling. Unlike the native potato starch, 6.5%(m/v, db) pastes of the crosslinked potato starches could form gels, which could be predicted from the Brabender setback and consistency index. When the degree of crosslinking is low, random contraction and radial swelling of the granule was possible. As the degree of crosslinking increased, morphological change became similar to the single dimensional tangential swelling observed from the lenticular wheat starch. These morphological change during heating in excess water explained the gelatinization behaviors of crosslinked starches tested.

A STUDY ON THE EVALUATION OF POLYMERIZATION SHRINKAGE OF COMPOSITE AND COMPOMER USING STRAIN GAUGE METHOD (스트레인 게이지법을 이용한 복합레진과 컴포머의 중합수축 평가에 관한 연구)

  • The purpose of this study was to compare the polymerization shrinkage and the compressive strength of composite and compomer cured with two different light sources ; conventional halogen-light curing unit and recently-developed plasma arc curing unit. The 'strain gauge method' was used for determination of polymerization shrinkage and the compressive strength was measured by universal testing machine. The results of the present study can be summarized as follows: 1. Filling materials in polyethylene molds showed the initial expansion in the early phase of polymerization. This was followed by the rapid contraction in volume during the first 60 seconds and gradually diminished as curing process continued. 2. The polymerization shrinkage in tooth samples was generally lower than in the mold samples. 3. The generally lower amount of linear polymerization shrinkage was observed in compomer and plasma arc curing unit group when compared to composite and conventional curing unit. 4. The higher compressive strength values was found in composite groups regardless curing methods. The results of this study strongly support the application of plasma arc system and fluoride-containing compomer in the field of clinical pediatric dentistry claiming its effectiveness in curing the esthetic dental materials and the anticariogenic capacity.
