말기 소아 암 환자의 최면치료 1예 (A Case of Hypnotherapy with Terminal Pediatric Cancer Patient)

  • The authors reported a case of terminal pediatric cancer patient. The patient was ten-year-old girl, and she was diagnosed as osteosarcoma with multiple metastasis to lung and bones. She was markedly depressed and had severe bone and chest pain. The patient was treated with hypnotherapy once or twice a week for two months. There was marked improvement in pain control and emotional reactions, and the hospice team could establish good rapport with her. Hypnotherapy would be one of the effective treatment modalities in assisting patients.

Disseminated Cytomegalovirus Infection and Protein Losing Enteropathy as Presenting Feature of Pediatric Patient with Crohn's Disease

  • We report a pediatric patient admitted with abdominal pain, diffuse lower extremity edema and watery diarrhea for two months. Laboratory findings including complete blood count, serum albumin, lipid and immunoglobulin levels were compatible with protein losing enteropathy. Colonoscopic examination revealed diffuse ulcers with smooth raised edge (like "punched out holes") in the colon and terminal ileum. Histopathological examination showed active colitis, ulcerations and inclusion bodies. Immunostaining for cytomegalovirus was positive. Despite supportive management, antiviral therapy, the clinical condition of the patient worsened and developed disseminated cytomegalovirus infection and the patient died. Protein losing enteropathy and disseminated cytomegalovirus infection a presenting of feature in steroid-naive patient with inflammatory bowel disease is very rare. Hypogammaglobulinemia associated with protein losing enteropathy in Crohn's disease may predispose the cytomegalovirus infection in previously healthy children.

  • A recent High Court's decision regarding the withdrawal of life supporting medical treatment (artificial ventilator) from an elderly female patient in the terminal stage has opened up a new era of the "euthanasia dispute" in Korea. With this decision, the legitimate withdrawal of life supporting treatment became possible under certain conditions and the Korean Medical Association is working toward the establishment of practical guidelines for the terminal-stage patients. However, there are still very few debates on the cases of pediatric patients in the terminal stage or suffering from fatal diseases. For pediatric patients, the core principle of autonomy and following procedure of "advance directives" are hardly kept due to the immaturity of the patients themselves. Decisions for their lives usually are in the hands of the parents, which may often bring out tragic disputes around "child abuse", especially in Korea where parents have exclusive control of the destiny of their children. Some developed countries such as the U.S.A., the U.K. and Canada have already established guidelines or a legal framework for ensuring the rights of the healthcare system regarding children suffering from severe illness, permitting the withdrawal of Life supporting medical treatment (LSMT) in very specific conditions when the quality of life of the children is severely threatened. For the protection of the welfare and interest of the children, we should discuss this issue and develop guidelines for the daily practice of pediatricians.

The Utility of Contrast Enhanced Ultrasound and Elastography in the Early Detection of Fibro-Stenotic Ileal Strictures in Children with Crohn's Disease

  • Purpose: Crohn's disease (CD) is a chronic, idiopathic bowel disorder that can progress to partial or complete bowel obstruction. At present, there are no reliable diagnostic tests that can readily distinguish between acute inflammatory, purely fibrotic and mixed inflammatory and fibrotic. Our aim is to study the utility of contrast enhanced ultrasound (CEUS) in combination with shear wave elastography (SWE) to differentiate fibrotic from inflammatory strictures in children with obstructive CD of the terminal ileum. Methods: Twenty-five (19 male) children between 2016-2021 with CD of the terminal ileum were recruited into the study. Among these patients, 22 had CEUS kinetic measurements of tissue perfusion, including wash-in slope (dB/sec), peak intensity (dB), time to peak intensity (sec), area under the curve (AUC) (dB sec), and SWE. In total, 11 patients required surgery due to bowel obstruction. Histopathologic analysis was performed by a pathologist who was blinded to the CEUS and SWE test results. Results: Patients that underwent surgical resection had significantly higher mean area under the curve on CEUS compared to patients responsive to medical therapy (p=0.03). The AUC also correlated with the degree of hypertrophy and the percent fibrosis of the muscularis propria, as determined by histopathologic grading (p<0.01). There was no difference in the mean elastography measurements between these two patient groups. Conclusion: CEUS is a useful radiological technique that can help identify pediatric patients with medically refractory obstructive fibrotic strictures of the terminal ileum that should be considered for early surgical resection.

  • Transmesenteric hernia, a type of internal hernias, is a rare cause of intestinal obstruction. This intraperitoneal hernia has no sac and is formed by protrusion of a loop of bowel through an aperture in the mesentery. Incarceration leads to intestinal obstruction and subsequently, strangulation and gangrene of varing lengths of intestine. This is a case report of 4-year-old girl with transmesenteric herniation of the terminal ileum through a defect in its own mesentery. Strangulation of the affected bowel necessitates resection and primary anastomosis with repair of mesenteric defect. The postoperative course was uneventful. Acute intestinal obstruction in the absence of an external hernia and with no history of a previous surgical procedure suggests the possibility of an internal hernia, especially if the patient has a history of chronic intermittent abdominal distress.

  • 아동에게 있어서 말기 질환의 범위는 악성종양을 비롯하여 선천성 대사이상, 염색체 이상, 선천성 기형, 신경근육계 질환, 기타 다른 불치의 상태까지 포함한다. 병원환경에서 이러한 말기 질환 아동은 질병의 치료 과정과 관련하여 공포심, 분노, 좌절감을 경험할 수 있으며 통제된 병원 환경안에서 자율성을 제한받기 때문에 통제감을 상실하고 무기력해질 수 있다. 본 연구에서는 환경적 지지 모델(Contextual Support Model of Music Thterapy)을 중심으로 음악치료가 병원환경에 있는 말기질환자들의 정서 및 기능 변화에 긍정적 변화를 줄 수 있는지를 알아보았다. 환경적지지 모델 음악치료는 음악 환경의 구조적인 특성(structure), 자발성 지지(autonomy support) 그리고 참여(involvement)라는 세 가지 요소로 구성된다. 그리고 이러한 치료적 구성 요소로 이루어진 음악 환경은 아동의 활동을 지지하고 격려하도록 도와준다. 이 세 가지 치료적 요소를 중심으로 하여 본 연구는 말기질환 아동과의 음악치료사례를 분석하여 말기 질환 아동의 행동이 환경적 지지 역할을 하는 음악 안에서 어떠한 유형으로 나타나는지에 대해서 알아보았다. 본 연구의 대상자는 만 10살의 급성 림프성 백혈병 여아로 총 9회의 세션을 통한 음악 치료 활동 내용을 질적 연구방법으로 사용하여 내용분석을 하였다. 내용분석은 환경적 지지모델의 세 가지 치료적 요소 중심으로 본 연구자가 설정한 행동들을 기준으로 이루어졌다. 환아와의 주 음악 치료 활동은 악기 연주하며 노래부르기, CD플레이어로 노래 들으면서 노래 부르기, 노래 만들기, 노래 가사 탐색하기 활동으로 이루어졌다. 본 연구의 결과는 다음과 같다. 환경적인 구조적 특성(Structure) 측면에서는 음악치료는 노래를 부를 수 있는 환경과 악기를 연주할 수 있는 환경을 제공해주고 음악 만들기 활동을 할 수 있는 준비를 해줌으로써 아동이 성공적으로 음악 활동을 할 수 있도록 방향을 제시해주었다. 자발성 지지(Autonomy support) 측면에서 음악치료는 노래를 부르거나 악기를 연주할 때 여러 가지 선택의 기회를 제공함으로써 아동이 자발적으로 참여할 수 있도록 하였다. 참여(Involvement)의 측면에서는 치료사가 아동에게 긍정적인 강화를 제시함으로써 아동이 긍정적으로 경험하고 공유된 활동 안에서 치료사와 상호 협력적으로 음악활동을 진행하는 모습을 볼 수 있었다. 이와 같이 음악치료는 환경적으로 지지 역할을 하며 정서적으로 위축되어있는 아동에게 자발성과 독립성을 개발시켜주고 병원 환경이라는 제약된 환경 안에서 자율성을 촉진시켜줄 수 있음을 보여주었다. 이러한 음악 치료의 효과를 근거로 하여 입원 환경에 있는 아동들을 위한 더 다양한 접근 방법의 음악치료 연구가 이루어지기를 기대한다.

Massive traumatic abdominal wall hernia in pediatric multitrauma in Australia: a case report

  • Traumatic abdominal wall hernia is a rare presentation, most commonly reported in the context of motor vehicle accidents and associated with blunt abdominal injuries and handlebar injuries in the pediatric population. A 13-year-old boy presented with multiple traumatic injuries and hemodynamic instability after a high-speed motor vehicle accident. His injuries consisted of massive traumatic abdominal wall hernia (grade 4) with bowel injury and perforation, blunt aortic injury, a Chance fracture, hemopneumothorax, and a humeral shaft fracture. Initial surgical management included partial resection of the terminal ileum, sigmoid colon, and descending colon. Laparostomy was managed with negative pressure wound therapy. The patient underwent skin-only primary closure of the abdominal wall and required multiple returns to theatre for debridement, dressing changes, and repair of other injuries. Various surgical management options for abdominal wall closure were considered. In total, he underwent 36 procedures. The multiple injuries had competing management aims, which required close collaboration between specialist clinicians to form an individualized management plan. The severity and complexity of this injury was of a scale not previously experienced by many clinicians and benefited from intrahospital and interhospital specialist collaboration. The ideal aim of primary surgical repair was not possible in this case of a giant abdominal wall defect.

태아 소장 부분염전에 의한 회장 무공증 (Ileal Atresia Secondary to Intrauterine Segmental Volvulus)

  • A 2-day-old male (Premie, Large for gestational age(LGA), Intrauterine period(IUP) 33 weeks, birth weight 2,955 gram) was transferred with marked abdominal distention, bilious return via the orogastric tube, respiratory difficulty, and generalized edema (hydrops fetalis). He was born by cesarean section to a 36 year-old mother. Antenatal ultrasonogram at IUP 31 weeks demonstrated multiple dilated bowel loops suggestive of intestinal obstruction. There was no family history of cystic fibrosis. Simple abdominal films disclosed diffuse haziness and suspicious fine calcifications in the right lower quadrant. Barium enema demonstrated a microcolon. Sweat chloride test was not available in our institution. At laparotomy, there noted 1) a segmental volvulus of the small bowel with gangrenous change, associated with meconium peritonitis, 2) an atresia of the ileum at the base of the volvulus, and 3) the terminal ileum distal to the volvulus was narrow and impacted with rabbit pellets-like thick meconium. These findings appeared to be very similar to those of a complicated meconium ileus. In summary, the ileal atresia and meconium peritonitis seemed to be caused by antenatal segmental volvulus of the small intestine in a patient with probable meconeum ileus.

Exploring Communication Difficulties in Pediatric Hematology: Oncology Nurses

  • Background: Communication plays an important role for the well being of patients, families and also health care professionals in cancer care. Conversely, ineffective communication may cause depression, increased anxiety, hopelessness and decreased of quality life for patients, families and also nurses. Objective: This study aimed to explore communication difficulties of pediatric hematology/oncology nurses with patients and their families, as well as their suggestions about communication difficulties. Materials and Methods: It was conducted in a pediatric hematology/oncology hospital in Ankara, Turkey. Qualitative data were collected by focus groups, with 21 pediatric hematology/oncology nursing staff from three groups. Content analysis was used for data analysis. Results: Findings were grouped in three main categories. The first category concerned communication difficulties, assessing problems in responding to questions, ineffective communication and conflicts with the patient's families. The second was about the effects of communication difficulties on nurses and the last main category involved suggestions for empowering nurses with communication difficulties, the theme being related to institutional issues. Conclusions: Nurses experience communication difficulties with children and their families during long hospital stays. Communication difficulties particularly increase during crisis periods, like at the time of first diagnosis, relapse, the terminal stage or on days with special meaning such as holidays. The results obtained indicate that pediatric nurses and the child/family need to be supported, especially during crisis periods. Feeling of empowerment in communication will improve the quality of care by reducing the feelings of exhaustion and incompetence in nurses.

Inflammation and hyponatremia: an underrecognized condition?

  • Timely diagnosis of hyponatremia is important for preventing potential morbidity and mortality as it is often an indicator of underlying disease. The most common cause of eurvolemic hyponatremia is the syndrome of inappropriate antidiuretic hormone (SIADH) secretion. Recent studies have demonstrated that proinflammatory cytokines such as interleukin (IL) $1{\beta}$ and IL-6 are involved in the development of hyponatremia, a condition that is associated with severe inflammation and is related to antidiuretic hormone (ADH) secretion. Serum sodium levels in hyponatremia are inversely correlated with the percentage of neutrophils, C-reactive protein, and N-terminal-pro brain type natriuretic peptide. Additionally, elevated levels of serum IL-6 and IL-$1{\beta}$ are found in inflammatory diseases, and their levels are higher in patients with hyponatremia. Because it is significantly correlated with the degree of inflammation in children, hyponatremia could be used as a diagnostic marker of pediatric inflammatory diseases. Based on available evidence, we hypothesize that hyponatremia may be associated with inflammatory diseases in general. Understanding the mechanisms responsible for augmented ADH secretion during inflammation, monitoring patient sodium levels, and selecting the appropriate intravenous fluid treatment are important components that may lower the morbidity and mortality of patients in a critical condition.
