The Roles of Gold Plate (140${\mu}{\textrm}{m}$) Loaded on TLD-100 Chips in the High Energy Radiation Beams (고에너지 광자선속에서 TLD-100 chip 위에 있는 금박막(140 ${\mu}{\textrm}{m}$) 역할)

  • Lithium Fluoride (LiF; TLD-100) crystal chips are normally used as thermolu minescence dosimeters (abbreviated as NC-100) for estimating the absorbed dose to the skin of a patient or in a solid water phantom undergoing radiotherapy with megavoltage photon (6 and 15MV) beams. In general, investigation has revealed a reduction in the sensitivity of NC-100 chips after many runs through heating cycles. A TLD-100 chip laminated with gold plate (140${\mu}{\textrm}{m}$) on the upper surface layer of its face toward the photon beam (abbreviated as GC-100) has properties different from that of a NC-100 chip activated by incident photons and contaminant electrons with various lower energies coming from the gantry head and air. Activation of the valence band electrons of GC-100 chips by incident photons, positrons and electrons-which come from the gold plate by mainly pair production process and partly from Compton scattering-results in more enhanced signal intensity, higher response per monitor unit, as well as a good linearity with monitor units and independence of dose rate. Since the electron beams (6 and 15 MeV) do not have the probability of pair production process with gold plate, there is only a small difference (about a 3.3% increase for 15 MeV) in the signal gaps in the TL readout for electron beams between GC- and NC-100 chips. The 3.3% increase is entirely due to the buildup caused by the 140 m gold plate. The sensitivity of GC-100 chips is much more susceptible to high energy photon beams than electron one because of pair production. The interaction of high energy photon with a material of high atomic number, such as the good plate in this case, results in a considerably significant probability of pair production. The gold plate on the NC-100 chips acts as not only an intensifier of their signals but also acts as a filter of contaminant electrons in therapeutic high energy X-ray beams.

  • The comparison of the KAPM Dosimetric Protocol (1990) with the TG-21 and $C_{\lambda}/C_E$(ICRU-21 and SCRAD protocol) method is studied. The therapetutic range of radiation (photon 4MV-l5MV and electron 6 MeV-20MeV) and three kinds of the chambers were used in the water phantom. The results from 7G-21 and KAPM protocol did not show much differences (less than 1$\%$) throughout the whole energy range; $N_D$ from KAPM protocol and Ngas from TG-21 showed 0.2$\%$ deviation mainly from W/e difference between two protocols. But the results from KAPM protocol (1990) and those from $C_{\lambda}/C_E$ Method showed $-1.9{\pm}0.6\%$ (KAPM protocol is higher) deviation for photom beam and $+3.3{\pm}1\%$ (KAPM protocol is lower) deviation for electron beams.

  • In radiation therapy, accurate Quality Assurance (QA) is required to irradiate tumor tissue while minimizing damage to normal tissue. Therefore, a dosimeter that can accurately measure radiation is needed. The purpose of this study is to develop a highly efficient radiation dosimeter with high sensitivity by applying a particle in binder method that can reduce manufacturing costs and simplify processes to perovskite materials that are cheaper and simpler to manufacture. By evaluating the response characteristics to high-energy photon, the applicability of QA dosimeter to radiation therapy was evaluated. As a result of reproducibility evaluation, RSD at 6 MV energy was presented as 1.178% and 15 MV energy was presented as 1.141%. As a result of linearity evaluation according to linear regression analysis, R2 values of 0.9999 were presented under each condition of 6 MV and 15 MV energy. It was found that the CsPbBr3 dosimeter manufactured based on the results of reproducibility and linearity evaluation is highly applicable as a QA dosimeter in the field of therapeutic radiation. The CsPbBr3 dosimeter manufactured in this study presented more than the standard performance in the evaluation of reproducibility and linearity, and it can be used as a radiotherapy QA dosimeter through improvement.

Effect of the Space Dose Rate due to Change of X-ray Irradiation Energy and MU Value in Radiation Therapy Room (선형가속기의 엑스선 조사에너지와 MU값의 변화가 치료실 내 공간선량률 변화에 미치는 영향)

  • This study investigated the radiation protection of therapeutic radiologists. Based on the change in X-ray energy and MU value, the space dose rate in the treatment room after the irradiation was measured. 6MV, 10MV and 15MV photon beams were exposed to radiation inside the treatment room based on 300MU, 600MU and 1000MU using a linear accelerator. And repeated 10 times under the same conditions. As a result of the experiment, 0.1555 μSv/h for 6MV 300MU, 0.157 μSv /h for 300sec, 0.152 μSv/h, 0.156 μSv/h for 600MU, and 0.157 μSv/h 0.152 μSv/h for 1000MU. 300MU of 10MV was 0.49 μSv/h, 0.309 μSv/h, and 0.69 μSv/h, 0.416 μSv/h for 600MU, respectively, and 1000MU was 0.977 μSv/h and 0.478 μSv/h, respectively. The 300MU of 15MV was 3.02 μSv/h, 1.2 μSv/h, 5.459 μSv/h at 600MU, 7.34 μSv/h at 1.836 μSv/h 1000MU, and 2.709 μSv/h. The average spatial dose rate of 6MV was not significantly different from the natural spatial dose rate in the treatment room. High spatial dose rates were measured at 10 MV and 15 MV and were attenuated over time. Therefore, entering the treatment room after a certain period of time (more than 60 seconds) is considered to be effective to prevent the exposure dose of radiation workers.

  • There has been necessity of an air free ionization chamber using the gold-crystal-aluminium plates, henceforth called the crystal chamber. The crystal chamber formed of parallel plates is very small in size and has more response for absorbed dose of therapeutic radiation beams. The gold plate on the crystal facing the photon and electron beam acts as an intensifier of signals and crystal plate as an ionization medium respectively. Both the copper guard ring and the aluminum collecting electrode are connected to an electrometer. Using high energy photon (6, 15 MV) and electron (9, 12, 15, 18 MeV) beams, the responses of the crystal chamber are evaluated against a PTW Farmer-type chamber at a field size of 10${\times}$10cm$^2$ and 100 cm SSD. The responses of crystal chamber for therapeutic radiation electron and photon beams are greater in magnitude by several order than Farmer. The crystal chamber has good linearity without correction factor C$\_$t,p/ with respect to the signals, a reading reproduction with good accuracy and precision less than 0.5%, and has other useful functions in measuring radiation beams.

  • Currently small and accurate dosimeters are on the rise. In this study, the feasibility and energy dependency of the electret dosimeter that made of PET (polyethylene terephtalate) were observed by irradiating 4, 6, 15 MV photon beams from the clinical linear accelerator to develop a dosimeter for the clinical field. $10{\times}10cm$ field size of the photon beams were irradiated to the electret dosimeter where the 2.5 cm depth in the polystylene phantom from 100 cm SSD, while 300 DCV was applied to the electret dosimeter. The result showed that the absorbed dose was proportional to the charge linearly, and the volume of a dosimeter could be reduced and the signals were high enough. According to this study, it was found that the polymer electret detector could be produced as a large quantity with a small cost and showed the feasibility of a realtime measurement.

Surface Dose and Transmission Factor for Vacuum Cushion (Vacuum Cushion 사용시 표면선량과 투과율 평가)

  • The individual (customized) immobilization has been used to reproduce the patients' set-up on daily base. There are many various devices available commercially. To evaluate dosimetric characteristics of vacuum cushion, we analysed the surface dose and transmission factor for d$_{max}$ when patient is immobilized with vacuum cushion. Experiments were performed with 4 MV (Varian 4/100, USA), 6 MV, 15 MV (Varian CL2100C/D, USA) photon beams and five field sizes (5$\times$5, 10$\times$10, 20$\times$20, 30$\times$30, 40$\times$40 $\textrm{cm}^2$) on each occasion. Outputs were measured from surface of polysterene phantom to d$_{max}$ with four different thicknesses of cushion, which is 12, 32, 48 mm and only vinyl without styroforms. As results, the transmission factor for thicknesses of vacuum cushion was ranged from 0.9953 to 1.0043. The more the thickness of vacuum cushion is thick, the more surface dose delivered to patient is increased. The surface dose vary with the thickness of vacuum cushion for energy and field size. The skin reactions may result. But the variation is not serious in the clinic.

  • This study was intended to evaluate the surface dose and depth dose of according to the distance of the treatment room wall. High energy photon beams from linear accelerators produce large scattered radiation by various components of the treatment head, collimator and walls or objects in the treatment room including the patient. The scattered radiation measured by thermoluminescence dosimeter(TLD). Linear accelerators rotation center of the four walls(X) distance was measured to be 236, 272, 303, and 337 cm. The result of 100 cGy and 200 cGy of 6 MV photon irradiation, surface dose was 0.49, 0.83 mSv at 236 cm of the shortest distance to the wall, In 272 cm 0.41, 0.53 mSv, 303 cm in the 0.28, 0.57 mSv, and 337 cm distance from the wall in the 0.33, 0.76 mSv surface dose respectively. There was remarkable difference in the surface dose among the treatment room wall distance. The results of useful data in relation to stochastic effect for radiation therapy patients.

Efficient Verification of X-ray Target Replacement for the C-series High Energy Linear Accelerator

  • The manufacturer of a linear accelerator (LINAC) has reported that the target melting phenomenon could be caused by a non-recommended output setting and the excessive use of monitor unit (MU) with intensity-modulated radiation therapy (IMRT). Due to these reasons, we observed an unexpected beam interruption during the treatment of a patient in our institution. The target status was inspected and a replacement of the target was determined. After the target replacement, the beam profile was adjusted to the machine commissioning beam data, and the absolute doses-to-water for 6 MV and 10 MV photon beams were calibrated according to American Association of Physicists in Medicine (AAPM) Task Group (TG)-51 protocol. To verify the beam data after target replacement, the beam flatness, symmetry, output factor, and percent depth dose (PDD) were measured and compared with the commissioning data. The difference between the referenced and measured data for flatness and symmetry exhibited a coincidence within 0.3% for both 6 MV and 10 MV, and the difference of the PDD at 10 cm depth ($PDD_{10}$) was also within 0.3% for both photon energies. Also, patient-specific quality assurances (QAs) were performed with gamma analysis using a 2-D diode and ion chamber array detector for eight patients. The average gamma passing rates for all patients for the relative dose distribution was $99.1%{\pm}1.0%$, and those for absolute dose distribution was $97.2%{\pm}2.7%$, which means the gamma analysis results were all clinically acceptable. In this study, we recommend that the beam characteristics, such as beam profile, depth dose, and output factors, should be examined. Further, patient-specific QAs should be performed to verify the changes in the overall beam delivery system when a target replacement is inevitable; although it is more important to check the beam output in a daily routine.

A Study on Dobe Distribution outside Co-60 $\gamma$ Ray ana 10MV X Ray Fields ($^{60}Co\;\gamma$선과 10MV X선의 조사면 밖의 선량분포에 관한 연구)

  • The peripheral dose, defined as the dose outside therapeutic photon fields, which is responsible for the functional damage of the critical organs, fetus, and radiation. induced carcinogenesis, has been investigated for $^{60}Co\;\gamma$ ray and 10 MV Xray. It was measured by silicon diode controlled by semiautomated water phantom without any shielding or with lead plate of HVL thickness put horizontally or vertically to shield stray radiations. Authors could obtain following results. 1. The peripheral dose was larger than $0.7\%$ of central axis maximum dose even at 20cm distance from field margin. That is clinically significant, so it should be reduced. 2. Even for square fields of 10 MV Xray, radial peripheral dose distribution did not coincide with transverse distribution, because of the position of collimator jaws. 3. Between surface and $d_m$, the peripheral dose distributions show a pattern of the dose distribution of electron beams and the maximum doss was approximately proportional to the length of a side of square field. 4. The peripheral doses depended on radiation quality, field size, distance from field margin and depth in water. Distance from field margin was the most important factor. 5. Except for near surface, the peripheral dose from phantom was approximately equal to that from therapy unit. 6. To reduce the surface dose outside fields, therapist should shield stray radiations from therapy unit by lead plate of at least one HVL for 10 MV X-ray and by bolus equivalent to tissue of 0.5cm thickness for $^{60}Co$. 7. To reduce the dose at depth deeper than $d_m$, it is desirable to shield stray radiations from therapy unit by lead.
