Purpose: To investigate the MRI and arthroscopic findings of osteochondral lesion of the talus which looked normal on plane radiography. Materials and methods: We investigated the MRI and arthroscopic findings of seven osteochondral lesions in which there were no abnormal finding on plane radiography and no cystic changes on MRI. Average age was 31 years(range, 19-43 years). Arthroscopic findings were classified according to the Ferkel's criteria. Results: History of injury was reported in all cases and the average duration from injury to presentation was 4 years and 4 months. Low signal change in T1WI was found in 6 of 7 lesions, no signal change in 1 case. Low signal change in T2WI was found in 4, no signal change in 3. 6 STIR images were obtained. High signal change was found in 3, no signal change in 2 and intermediate signal change was in 1. Arthroscopic grading was A in 1, C in 1, D in 2 , E in 1 and F in 2. We could not find any correlation between the findings on MRI and arthroscopic examination. Conclusion: We suggest arthroscopic examination is needed for accurate diagnosis of the osteochondral lesions of the talus which looked normal on plane radiography, because they have various MRl findings and high likelihood of existence of unstable cartilage lesions.
Purpose : The purpose of this study was to investigate the quantitative evaluation of the corticospinal tract (CST) at the multiple levels by using functional MRI (fMRI) co-registered to diffusion tensor tractography (DTT). Materials and Methods : Ten normal subjects without any history of neurological disorder participated in this study. fMRI was performed at 1.5 T MR scanner using hand grasp-release movement paradigm. DTT was performed by using DtiStudio on the basis of fiber assignment continuous tracking algorithm (FACT). The seed region of interest (ROI) was drawn in the area of maximum fMRI activation during the motor task of hand grasp-release movement on a 2-D fractional anisotropy (FA) color map, and the target ROI was drawn in the cortiocospinal portion of anterior lower pons. We have drawn five ROIs for the measurement of FA and apparent diffusion coefficient (ADC) along the corona radiata (CR) down to the medulla. Results : The contralateral primary sensorimotor cortex (SM1) was mainly found to be activated in all subjects. DTT showed that tracts originated from SM1 and ran to the medulla along the known pathway of the CST. In all subjects, FA values of the CST were higher at the level of the midbrain and posterior limb of internal capsule (PLIC) than the level of others. Conclusion : Our study showed that co-registered fMRI and DTT has elucidated the state of CST on 3-D and analyzed the quantitative values of FA and ADC at the multiple levels. We conclude that co-registered fMRI and DTT may be applied as a useful tool for clarifying and investigating the state of CST in the patients with brain injury.
Several photogrammetric and geographic information system applications such as surface matching, object recognition, city modeling, environmental monitoring, and change detection deal with multiple versions of the same surface that have been derived from different sources and/or at different times. Surface registration is a necessary procedure prior to the manipulation of these 3D datasets. This need is also applicable in the field of medical imaging, where imaging modalities such as magnetic resonance imaging (MRI) can provide temporal 3D imagery for monitoring disease progression. This paper will present a general automated surface registration procedure that can establish correspondences between conjugate surface elements. Experimental results using light detection and ranging (LIDAR) and MRI data will verify the feasibility, robustness, and accuracy of this approach.
Purpose : A new inhomogeneity correction method based on two-point Dixon sequence is proposed to obtain water and fat images at 0.35T, low field magnetic resonance imaging (MRI) system. Materials and Methods : Joint phase-magnitude density function (JPMF) is obtained from the in-phase and out-of-phase images by the two-point Dixon method. The range of the water signal is adjusted from the JPMF, and 3D inhomogeneity map is obtained from the phase of corresponding water volume. The 3D inhomogeneity map is used to correct the inhomogeneity field iteratively. Results : The proposed water-fat imaging method was successfully applied to various organs. The proposed 3D inhomogeneity correction algorithm provides good performances in overall multi-slice images. Conclusion : The proposed water-fat separation method using JPMF is robust to field inhomogeneity. Three dimensional inhomogeneity map and the iterative inhomogeneity correction algorithm improve water and fat imaging substantially.
Recently, advance on various modalities of diagnosing, prostate volume estimation became possible not only by the existing two-dimension medical images data but also by the three-dimensional medical images data. In this study, magnetic resonance image (MRI), computer tomography (CT) and ultrasound (US) were employed to evaluate prostate phantom volume measurements for estimation, comparison and analysis. For the prostate phantoms aimed at estimating the volume, total of 17 models were developed by using devils-tongue jelly and changing each of the 5ml of capacity from 20ml to 100ml. For the volume estimation through 2D US, the calculation of the diameter with C9-5Mhz transducer was conducted by ellipsoid formula. For the volume estimation through 3D US, the Qlab software (Philips Medical) was used to calculate the volume data estimated by 3D9-3Mhz transducer. Moreover, the images by 16 channels CT and 1.5 Tesla MRI were added by the method of continuous cross-section addition and each of imaginary prostate model's volume was yielded. In the statistical analysis for comparing the availability of volume estimation, the correlation coefficient (r) was more than 0.9 for all indicating that there were highly correlated, and there were not statistically significant difference between each of the correlation coefficient (p=0.001). Therefore, the estimation of prostate phantom volume using three-dimensional modalities of diagnosing was quite closed to the actual estimation.

Objective: To develop and evaluate a deep learning model for automated segmentation and detection of bone metastasis on spinal MRI. Materials and Methods: We included whole spine MRI scans of adult patients with bone metastasis: 662 MRI series from 302 patients (63.5 ± 11.5 years; male:female, 151:151) from three study centers obtained between January 2015 and August 2021 for training and internal testing (random split into 536 and 126 series, respectively) and 49 MRI series from 20 patients (65.9 ± 11.5 years; male:female, 11:9) from another center obtained between January 2018 and August 2020 for external testing. Three sagittal MRI sequences, including non-contrast T1-weighted image (T1), contrast-enhanced T1-weighted Dixon fat-only image (FO), and contrast-enhanced fat-suppressed T1-weighted image (CE), were used. Seven models trained using the 2D and 3D U-Nets were developed with different combinations (T1, FO, CE, T1 + FO, T1 + CE, FO + CE, and T1 + FO + CE). The segmentation performance was evaluated using Dice coefficient, pixel-wise recall, and pixel-wise precision. The detection performance was analyzed using per-lesion sensitivity and a free-response receiver operating characteristic curve. The performance of the model was compared with that of five radiologists using the external test set. Results: The 2D U-Net T1 + CE model exhibited superior segmentation performance in the external test compared to the other models, with a Dice coefficient of 0.699 and pixel-wise recall of 0.653. The T1 + CE model achieved per-lesion sensitivities of 0.828 (497/600) and 0.857 (150/175) for metastases in the internal and external tests, respectively. The radiologists demonstrated a mean per-lesion sensitivity of 0.746 and a mean per-lesion positive predictive value of 0.701 in the external test. Conclusion: The deep learning models proposed for automated segmentation and detection of bone metastases on spinal MRI demonstrated high diagnostic performance.
Background: To evaluate the feasibility, inter-reader reliability, and intra-reader reliability for various morphological features reported to be related to iliotibial band friction syndrome (ITBFS) on knee magnetic resonance imaging (MRI). Methods: A total of 145 patients with a clinical diagnosis and knee MRI findings consistent with ITBFS were included in the "study group" and 232 patients without knee pathology on both physical examination and MRI were included in the "control group". Various morphologic features on knee MRI were assessed including the patella shape, patella height, lateral epicondyle anterior-posterior (AP) width, lateral epicondyle height, ITB diameter (ITB-d), and ITB area (ITB-a). Results: Patients in the study group had significantly higher lateral epicondyle height (13.9 mm vs. 12.92 mm, P = 0.003), ITB-d (2.9 mm vs. 2.0 mm, P = 0.022), and ITB-a (38.5 mm2 vs. 23.8 mm2, P < 0.001) than the control group. ITB-a showed higher area under the curve index (0.849 with 74.1% sensitivity and 72.4% specificity at a 30.3 mm2 cutoff) than ITB-d (0.710 with 70.8% sensitivity and 61.2% specificity at 2.4 mm cutoff) and lateral epicondyle height (0.776 with 72.4% sensitivity and 67.8% specificity at 13.4 mm cutoff). However, only the inter-reader agreement for ITB-a (intraclass correlation coefficient = 0.65) was moderate, while the agreements for other morphologic features were good or excellent. Conclusions: Lateral epicondyle height seems to be a reliable and feasible morphologic feature for diagnosis of ITBFS.
In the intracranial regions, an accurate delineation of the target volume has been difficult with only the CT data due to poor soft tissue contrast of CT images. Therefore, the magnetic resonance images (MRI) for the delineation of the target volumes were widely used. To calculate dose distributions with MRI-based RTP, the electron density (ED) mapping concept from the diagnostic CT images and the pseudo CT concept from the MRI were introduced. In this study, the look up table (LUT) from the fifteen patients' diagnostic brain MRI images was created to verify the feasibility of MRI-based RTP. The dose distributions from the MRI-based calculations were compared to the original CT-based calculation. One MRI set has ED information from LUT (lMRI). Another set was generated with voxel values assigned with a homogeneous density of water (wMRI). A simple plan with a single anterior 6MV one portal was applied to the CT, lMRI, and wMRI. Depending on the patient's target geometry for the 3D conformal plan, 6MV photon beams and from two to five gantry portals were used. The differences of the dose distribution and DVH between the lMRI based and CT-based plan were smaller than the wMRI-based plan. The dose difference of wMRI vs. lMRI was measured as 91 cGy vs. 57 cGy at maximum dose, 74 cGt vs. 42 cGy at mean dose, and 94 cGy vs. 53 at minimum dose. The differences of maximum dose, minimum dose, and mean dose of the wMRI-based plan were lower than the lMRI-based plan, because the air cavity was not calculated in the wMRI-based plan. These results prove the feasibility of the lMRI-based planning for brain tumor radiation therapy.

3D CAD technology has been extended to a medical area including dental clinic beyond industrial design. The 2D images obtained by CT(Computerized Tomography) and MRI(Magnetic Resonance Imaging) are not intuitive, and thus the volume rendering technique, which transforms 2D data into 3D anatomic information, has been in practical use. This paper has focused on a method and its implementation for forming 3D geometric surface model from laminated CT images of the pubis. The implemented system could support a dental clinic to observe and examine the status of a patient's pubis before implant surgery. The supplement of 3D implant model would help dental surgeons settle operation plans more safely and confidently. It also would be utilized with teaching materials for a practice and training.
Background : Spatial analysis of EEG is a phenomenal assessment and not so informative for phase space and dynamic aspect of EEG data. In contrast, nonlinear EEG analysis attempts to characterize the dynamics of neural networks in the brain. We have analyzed the features of EEG nonlinearly in subjects with white matter change on brain MRI and compared the results with cognitive function in each. Methods : Digital EEG data were taken for 30 seconds in 9 subjects with white matter degeneration and in 5 healthy normal controls without white matter change on MRI. Then we analyzed them nonlinearly to calculate the correlation dimension(D2) using the MATLAB software. The cognitive function was assessed by 3MS(modified mini-mental state examination). The severity of white matter change was assessed by Scheltens scale. Results : The mean D2 value of normal control was greater than that of white matter degeneration group. The D2s of some channels were correlative with 3MS and degree of white matter degeneration significantly. Conclusions : nonlinear analysis of EEG can be used as one of adjuvant functional studies for prediction of cognitive impairment in subjects with white matter degeneration and subcortical white matter change can be influential on cognitive function and correlation dimension of EEG.
