Clinical Outcomes Associated with Degree of Hypernatremia in Neurocritically Ill Patients

  • Objective : Hypernatremia is a common complication encountered during the treatment of neurocritically ill patients. However, it is unclear whether clinical outcomes correlate with the severity of hypernatremia in such patients. Therefore, we investigated the impact of hypernatremia on mortality of these patients, depending on the degree of hypernatremia. Methods : Among neurosurgical patients admitted to the intensive care unit (ICU) in a tertiary hospital from January 2013 to December 2019, patients who were hospitalized in the ICU for more than 5 days and whose serum sodium levels were obtained during ICU admission were included. Hypernatremia was defined as the highest serum sodium level exceeding 150 mEq/L observed. We classified the patients into four subgroups according to the severity of hypernatremia and performed propensity score matching analysis. Results : Among 1146 patients, 353 patients (30.8%) showed hypernatremia. Based on propensity score matching, 290 pairs were included in the analysis. The hypernatremia group had higher rates of in-hospital mortality and 28-day mortality in both overall and matched population (both p<0.001 and p=0.001, respectively). In multivariable analysis of propensity score-matched population, moderate and severe hypernatremia were significantly associated with in-hospital mortality (adjusted odds ratio [OR], 4.58; 95% confidence interval [CI], 2.15-9.75 and adjusted OR, 6.93; 95% CI, 3.46-13.90, respectively) and 28-day mortality (adjusted OR, 3.51; 95% CI, 1.54-7.98 and adjusted OR, 10.60; 95% CI, 5.10-21.90, respectively) compared with the absence of hypernatremia. However, clinical outcomes, including in-hospital mortality and 28-day mortality, were not significantly different between the group without hypernatremia and the group with mild hypernatremia (p=0.720 and p=0.690, respectively). The mortality rates of patients with moderate and severe hypernatremia were significantly higher in both overall and matched population. Interestingly, the mild hypernatremia group of matched population showed the best survival rate. Conclusion : Moderate and severe hypernatremia were associated with poor clinical outcomes in neurocritically ill patients. However, the prognosis of patients with mild hypernatremia was similar with that of patients without hypernatremia. Therefore, mild hypernatremia may be allowed during treatment of intracranial hypertension using hyperosmolar therapy.

  • Background: Mussels are stubborn organisms attached to solid substrata by means of byssus threads. The abundance of marine mussel Mytilus edulis in marine facilities like power stations was reason to select among fouling animals. Methods: Mortality patterns as well as physiological behavior (oxygen consumption, foot activity, and byssus thread production) of two different size groups (14- and 25-mm shell length) of M. edulis were studied at different hydrogen peroxide concentrations ($1-4mg\;l^{-1}$). Results: Studied mussels showed progressive reduction in physiological activities as the hydrogen peroxide concentration increased. Mussel mortality was tested in 30 days exposure, and 14 mm mussels reached the highest percentage of 90% while 25 mm mussels reached 81%. Produced data was echoed by Chick-Watson model extracted equation. Conclusions: This study points that, while it could affect the mussel mortality moderately in its low concentrations, hydrogen peroxide has a strong influence on mussels' physiological activities related to colonization. Therefore, hydrogen peroxide can be an alternative for preventing mussel colonization on facilities of marine environment.

Administration of ketoprofen in postpartum sows to control the incidence of post-parturient disorders and improve piglet survival rate

  • Objective: Inflammation and pain management in postpartum hyperprolific sows is currently an important animal welfare issue in the swine industry. The present study investigates effects of ketoprofen treatment on the incidence of post-parturient disorders, feed intake, colostrum yield, piglet colostrum intake, colostrum immunoglobulin G (IgG) and piglet mortality rate during the first 3 days of postnatal life. Methods: In total, 61 Danish Landrace×Yorkshire crossbred sows and their offspring (n = 833) were included in the experiment. The sows were randomly distributed into two groups: i) control (n = 31), sows were treated with tolfenamic acid 2 mg per kg for 2 days postpartum; ii) ketoprofen (n = 30), sows were treated with ketoprofen 3 mg per kg for 2 days postpartum. The farrowing process of the sows was monitored for 24 h daily, and data associated with farrowing were collected. Piglet colostrum intake, sow colostrum yield and colostrum IgG were determined. Results: During the first 3 days postpartum, the incidence of sows that had fever did not differ between control and ketoprofen groups (51.6% and 56.7%, respectively, p = 0.692). Piglet colostrum intake did not differ between control and ketoprofen groups (p = 0.736). However, the proportions of piglets that had inadequate colostrum intake were 71.3%, 22.6%, and 5.4% in those with birth weights of <1.0 kg, 1.0 to 1.29 kg, and ≥1.30 kg, respectively (p<0.001). The piglet mortality rate did not differ between control and ketoprofen groups (p = 0.808). Conclusion: Administration of ketoprofen in postpartum sows for 2 days can control the evidence of post-parturient disorders in sows as effectively as the use of tolfenamic acid. No deleterious effect of ketoprofen was detected on sow colostrum yield, piglet colostrum intake and piglet mortality. Therefore, ketoprofen can be recommended as an alternative anti-inflammatory drug used in postpartum sows.

Clinical Nursing Survey of the Patients in the Intensive Care Unit (중환자실(I.C.U.)환자에 관한 임상 간호학적 관찰 -중환자실 임상 간호 교육을 위한 기초조사-)

  • The objectives of this study have been conducted to establish effective clinical teaching program to I.C.U in terms of proper assignment of the clients for the students, proper rotation schedule, priorities in critical nursing problems and selection of the teaching and learning. We have analyzed statistically 1,850 patients who have been admitted during a period from January 1977 to October 31 1979 in Ewha Woman's University Hospital. The results are as follows: 1. The proportion to the total inpatients number was 6.5% and mortality rate was 16.3%. 2. The average hospitalized days were 5.8 days in I.C.U and the total death was occured from 1 st hospital day to 5th hospital day. So it shows a certain difficulties for clinical experiences of the senior students in I.C.U. 3. In the age of the death, 41.3% of the patients were in the 41-60 year age group. It shows highest mortality rate in socially active and productive age groups. 4. The mortality rates of the departments of the medicine was 18.7%, general surgery 18.5%, and neurosurgery 14.7%. 5. The number of patients admitted to the department of neurosystem was 30.6%, cardiovascular system 22.6%, respiratory system 11.1 % and urinary system 2.9%. 6. On utilizing instruments and machine for diagnosis and client's assessment in I.C.U, they have utilized everything a usual. But they never utilize angiogram and cardiac catheterization in cardiovascular system, and retroperitoneal pneumography in the urologic system. Further more we would recommend as follows 1. In consideration of the average hospital days and the date of death, the rotation program for clincal experience need to be adjusted as continuing practice program in apposite to current alternative practice program for comprehensive nursing care. 2. Socioeconomic needs for the patient's families and himself should be emphasized by the students in addition to physical needs. 3. Course content for critical care might be built up in considering of core disease centered nursing problems. 4. The diagnostic procedures and client's assessment items which could not experience in our university hospital by the students might be considered and refilled as filled trips to another hospital and visual aids.

Investigation of the Proper Density for Rearing Protaetia brevitarsis Larvae (흰점박이꽃무지(Protaetia brevitarsis)의 유충사육 시 적정밀도 규명)

  • This study aimed to provide information and data for the management of insect breeding farms by identifying the appropriate density when rearing Protaetia brevitarsis larvae. The breeding box of the insects was filled with 2 L of fermented sawdust on a 50 × 35 × 15 cm sheet of plastic, and the density of the treatments was 200, 300 and 400 P. brevitarsis in the first, second, and third larval stages. Each treatment was repeated five times, and the sawdust medium was replaced three times (10 intervals). The experiments were conducted for a total of 30 days from March 1, 2020. Overall, 200 P.brevitarsis in the first, second, and third larval stages reared in the breeding box had a higher average survival rate and lower average mortality rate, thereby contributing to efficient production. In conclusion, this result suggests a way to increase production efficiency through the environmental management of insect breeding farms.

Subxiphoid Pericardial Drainage of Pericardial Effusions (심낭삼출액에 대한 검상하 심낭배액술)

  • To assess the effectiveness of subxipoid pericardial drainage for the treatment of pericardial effusion, we reviewed 80 cases of subxiphoid pericardial drainage between January 1986 and December 1995. There were 39 males and 41 females with ages ranging from 20 to 80 years. The diagnosis of pericardial effusion was made by echocardiography. The procedure was carried out under general anesthesia in 50(62.5%) and under local anesthesia in 30 patients(37.5%). Among the 33 p tients with malignant pericardial effusion, cytology was positive .in 14 of 31(45%), and pericardial biopsy showed malignancy in 7 of 29 patients(24%). Among the 27 patients with tuberculous pericardial effusion, the diagnosis was confirmed by histology of pericardial biopsy in 12 patient or bacteriologic culture in 1 patient. The operative mortality was 17.5% (14180 patients) and all the mortality occurred in the malignant group. There were no operation-related mortality Sixty six patients were followed from 9 days to 5 years; mean follow-up was 452 days. Recurrent pericardial effusions, necessitating further surgical intervention, occurred in 6 (7.5%) patients. Constrictive pericarditis developed later in 4 patients(5%) and two of them had undergone complete pericardiectomy. In summary, subxiphoid pericardial drainage allowed safe and efficient drainage of pericardial effusions with sampling for cytology and pericardial biopsy, and had an acceptable morbidity and mortality.

Outcomes of an outpatient home-based prehabilitation program before pancreaticoduodenectomy: A retrospective cohort study

  • Backgrounds/Aims: Prehabilitation aims for preoperative optimisation to reduce postoperative complications. However, there is a paucity of data on its use in patients undergoing pancreaticoduodenectomy (PD). Thus, this study aims to evaluate the outcomes of a home-based outpatient prehabilitation program (PP) versus no-PP in patients undergoing PD. Methods: This retrospective cohort study compared patients who underwent PP versus no-PP before elective PD from January 2016 to December 2020. Inclusion criteria for PP were < 65 years or 65-74 years with FRAIL score < 3. No-PP included dietician, case manager and anesthesia review. PP included additional physiotherapy sessions, caregiver training and interim phone consultation. Univariate and multivariate analysis were used to evaluate length of stay (LOS), morbidity, 30-day readmission, and 90-day mortality. Results: Seventy-one patients (PP: n = 50 [70.4%]; no-PP: n = 21 [29.6%]) were included in this study. Median age was 65 years (interquartile range [IQR]: 58-72 years). Majority (n = 58 [81.7%]) of patients underwent open surgery. Ductal adenocarcinoma was the most common histology (49.3%). Patient demographics were comparable between both groups. Overall median LOS was 11.0 days (IQR: 8.0-17.0 days). Compared to no-PP, PP was not independently associated with reduced intra-abdominal collections (odds ratio [OR]: 0.43; 95% confidence interval [CI]: 0.03-6.11, p = 0.532), major morbidity (OR: 1.31; 95% CI: 0.09-19.47; p = 0.845) or 30-day readmission (OR: 3.16; 95% CI: 0.26-38.27; p = 0.365). There was one (1.4%) 30-day mortality. Conclusions: Our outpatient PP with unsupervised exercise regimes did not improve postoperative outcomes following elective PD.

  • In order to judge appropriately the suitability as the second intermediate host of Clonorchis sinensis, the present experiments were proposed to survey the metacercaria of C. sinensis in the fish host, Cuztriculus eigenmanni, in the 6eld, and to observe, by experimental infection of the fish with Clonorchis cercaria, the penetrating ability of the cercaria, maturity, the process of degeneration and extinction, and infectivity of the metacercaria. The following is a brie summary of the leading facts gained through the experiments. 1. All the ten fish (mean body weight $13.4{\pm}3.439$) which were caught in Ok-ku irrigation reservoir were in acted with the metacercariae and the average number of the metacercariae found in the fish body/gram was 4.55. 2. The Bish was easily subject to invasion of the cercaria, and the examination of the cercariae after having the fish infected disclosed that 5:l hours after the infection, all of the cercariae formed their cysts in muscle and the metacercariae kept growing, that 7 days later the metacercariae were found folding their bodies twice, and that in 15 days the characteristic structure of the metacercariae was complete and made a vigorous rotary movement intermittently. 3. In the case of control fish, Pseudorasbora parka, both penetrating ability of the cercariae and the infective degree of the metacercariae to control host were stronger as compared with the 5sh host because that the control fish had higher suceptibility to C. sinensis. 4. Afterwards, the metacercariae in the fish host came to a state of maturity and beyond this stage some metacercariae started the Process of degeneration and extinction in as early as 30 days, and as more days elapsed, mortality of the metacercariae was not uniformly increased but difference of mortality appeared to be according to the individual of Ssh host. That is, in 390, 540 and 586 days, all of the metacercariae in the fish hosts became dead, meanwhile almost all of those found in the fish host of 596 days after the infection survived (mortality; 8, 51%) , Percentage of orally administered metacercariae developing to maturity in rabbit is 10.465 in the last case. 5. As the days were progressed the shapes of the metacercariae in the fish host tended to change from ellipsoid to almost round shapes, the length became shorter while the width was wider. The results shown above revealed that C. eigenmanni was, though it could not be proper host than p. larva. suitable as the second intermediate host in addition to our previous observation results.

Insecticidal efficiency of orange peel (Citrus sinensis) essential oil nanoemulsions against Rhyzopertha dominica (Fabricius, 1792) and Tribolium castaneum (Herbst, 1797)

  • The insecticidal efficiency of orange (Citrus sinensis) peel essential oil (OP-EO) is limited because of its low stability under environmental conditions. Nanoemulsion formulations show promise in overcoming this limitation. Therefore, this study aimed to formulate and characterize the OP-EO nanoemulsion form (OP-EON) and investigate its insecticidal properties against two significant storage pests, Rhyzopertha dominica (Fabricius, 1792), and Tribolium castaneum (Herbst, 1797). The OP-EON (4:3:3:90 w/w, EO: Tween 80: Ethanol: water) was successfully created using an ultrasonic homogenizer. The major chemical components of the OP-EO were determined to be D-limonene (87.93%), myrcene (3.62%), and α-pinene (1.34%) through GC-MS analysis. The OP-EON was characterized using TEM (50-100 nm), Zeta sizer (the mean droplet particle size of 58.60 nm, the ζ-potential value of -12.6 mV, and the polydispersity index of 0.486), and FT-IR analysis. After 7 days, exposure to 500 ppm of the OP-EON resulted in 50% and 30% mortality rates in R. dominica and T. castaneum, respectively. Exposure to 1000 ppm of OP-EON resulted in 90% and 55% mortality in R. dominica and T. castaneum, respectively, after 7 days. Overall, these results clearly showed the potential to exceed the limits of the insecticidal activity of the OP-EO with its nanoemulsion form.

Effects of Sublethal Doses of Imidacloprid on the Brown Planthopper, Nilaparvata lugens $St{\aa}l$ (Homoptera: Delphacidae) (Imidacloprid의 아치사량이 벼멸구(Nilaparvata lugens $St{\aa}l$)의 생물적 특성에 미치는 영향)

  • Effects of sublethal dose of imidacloprid on biological characteristics, such as longevity, fecundity and excretions from brown planthopper (BPH) and its repellency to imidacloprid were examined. Topical application of imidacloprid to BPH at the dose of $LD_{10}$ and $LD_{30}$ (These values were determined at 48 hours after topical application of imidacloprid) was remarkably increased the mortality from 3 days after application and showed over 90% mortality on 4 days after application. Topical application of imidacloprid at dose of $LD_{10}$ (16 days) showed no difference compared to the control (16.9 days) in the longevity, while application of $LD_{30}$ was reduced the longevity of BPH to 13.5 days. Application of sublethal dose of imidacloprid to BPH adults reduced the number of eggs laid per day and for adult stage, and the hatchability of their eggs. By root zone application at the dose of $LC_{30}$, 50% longevity and 40% fecundity was reduced respectively, but showed no effect on number of eggs laid per day. The day on which BPH laid 50 % of eggs in number were 7.6 days at the $LC_{10}$ and 5.9 days the of $LC_{30}$ respectively. Hatchabilities of the groups treated with imidacloprid were lower than that of the control. Topical application to BPH at the dose of $LD_{10}$ did not reduce the amount of excretes, but application at the dose of $LD_{30}$ was reduced 26.5% of excretion compared to the control. Leaf dipping into the solution of concentration of $LC_{10}$ and $LC_{30}$ was reduced 96.4% and 98.0% of excretion, respectively. And root zone treatment at the concentration of $LC_{10}$ and $LC_{30}$ reduced 79.5% and 96.6% of excretion, respectively. Repellency rates of BPH to imidacloprid were 20% regardless of the dose of application.
