심장 종양의 수술적인 치료의 임상적 고찰 (Clinical Experience of the Surgical Treatment of Cardiac Tumor)

  • 배경: 심장의 원발성 종양은 아주 드문 질환이다. 이중 대부분의 경우는 양성인 점액종이고 이는 조기의 수술적인 치료로 거의 완치되는 반면 육종과 같은 악성종양은 수술적 제거가 어렵고 예후도 안 좋은 것으로 알려져 있다. 본원에서는 심장 종양으로 수술적인 치료를 했던 환자를 모아서 분석해 보았다. 대상 및 방법: 1993년 8월부터 2008년 12월까지 심장 종양으로 수술적인 치료를 하였던 28명의 환자를 대상으로 의무 기록 검토를 통한 후향적 분석을 하였다. 결과: 환자의 연령은 20세에서 76세 사이로 평균 $54.2{\pm}15.6$세였다. 남자가 11명(39%), 여자가 17명(61%)이었다. 15예(54%)에서 심부전의 증상 호전을 위해 응급 수술을 시행하였다. 술 전 주 증상은 호흡곤란이 15예(54%)로 제일 많았다. 전 환자에서 술 전 심장초음파로 진단이 되었다. 수술 시 종양의 크기는 장축이 2∼40 cm의 범위로 평균 $7.0{\pm}6.9$ cm였으며 종양의 부착부위는 18예(64%)에서 심방중격에, 9예(32%)에서 좌심방에, 2예(7%)에서 승모판막윤에, 2예(7%)에서 좌심실에 위치하고 있었다. 수술은 전 환자에서 양 심방절개를 통해 접근하였고 25예(89%)에서 완전절제가 가능하였다. 조직검사에서 육종이 4예(14%), 지방종이 1예(4%), 점액종이 23예(82%)였으며 완전절제를 못했던 3예는 모두 육종이었다. 술 후 사망은 없었다. 외래 추적은 24예(86%)에서 가능했으며 평균 추적 기간은 $46.8{\pm}42.7$개월이었다. 추적 환자 중 만기 사망은 조직검사에서 육종이었던 3명이 있었다. 육종으로 수술했던 환자로 재발 혹은 타 조직으로 전이하여 1예에서 2차례 재수술, 1예에서 전이 부위 절제술, 1예에서는 항암치료만을 했던 환자였다. 평균 재발 및 전이기간은 각각 $12.7{\pm}10.8$개월, $20.5{\pm}16.8$개월이었다. 결론: 심장 종양의 대부분인 점액종은 색전 등의 위험을 야기할 수 있으므로 조기에 수술함이 원칙이고 수술적 제거로 근본적인 치료가 가능하다. 악성종양인 육종은 발견 시 이미 상당히 진행되어 있는 경우가 많고 주위 조직으로의 침윤이 심해 수술적인 제거가 어려운 경우가 많다. 그러나 심부전 증상 등의 증상완화를 위해서는 가능한 부위의 절제를 함으로써 환자의 향후 삶의 질을 높일 수 있는 방편으로 보인다.

  • 1. 목적 성조숙증은 2차성징의 발달이 여아에서는 8세 이전 남아에선 9세 이전에 조기 2차성징이 출현하는 증상으로 골단의 조기 폐쇄를 일으켜 결국 최종 성인 신장의 감소를 가져오는 질환이다. 본 연구는 성조숙증으로 내원한 여아들의 GnRH(Gonado-tropin-releasing Hormone) 자극검사 결과와 의무기록을 후향 분석하여 진단별 차이를 알아보고자 하였다. 2. 방법 2015년 2월부터 2015년 12월까지 분당서울대병원 소아청소년 내분비내과를 내원한 여아 118명을 대상으로 하였다. 의무기록을 후향 분석하여 대상 환아들은 진성 성조숙증(true precocious puberty) 57명, 조기사춘기(early puberty) 39명, 유방 조기발육군(premature thelarche) 22명을 진단별로 분류하였다. 진단 당시의 신장, 체중, Tanner stage, 골연령, 역연령, 부모의 키, GnRH 검사에서 LH, FSH와 E2 (Estradiol) Basal 값도 함께 조사하였다. 성선 자극호르몬 결과값 LH (Basal,30min,45min,60min) FSH(Basal,30min,60min)을 비교하여 각 그룹별 LH, FSH Peak치 분포도와 LH 최고치/LH 기저치 비율에 대한 평균${\pm}$표준편차와 LH 최고치/FSH 최고치 비율에 대한 평균${\pm}$표준편차를 비교하여 각 3그룹 간 유의확률(P-value) 값을 구하였다. 골연령의 측정은 Greulich-Pyle법(왼쪽 수근골 X선 사진을 촬영하여 골성숙도를 평가하는 방법- 6개월 단위의 표준손목사진을 보고 비교해서 골연령을 유추하는 방식)을 이용한 소아 청소년 내분비내과 교수가 직접 판독한 결과로 사용하였고 GnRH 검사는 합성 LHRH 100 ug을 정맥 주사하여 LH는 Basal, 30분, 45분,60분 후에 채혈된 결과값과 FSH는 Basal, 30분, 60분후에 채혈된 결과값을 조사하였다. 3. 결과 진성성조숙증그룹의 평균 키는$131{\pm}14.85$, 평균체중은 $8.80{\pm}4.93$, 평균역연령은 $7.1{\pm}0.81$, 평균 골연령은 $9.9{\pm}0.9$, 조기사춘기 그룹의 평균키는 $134{\pm}5.10$, 평균체중은 $28.50{\pm}4.43$, 평균역연령은 $8.05{\pm}0.03$, 평균 골연령은 $10.0{\pm}0.62$, 유방조기발육군의 평균 키는 $129{\pm}6,01$ 평균체중은 $28.65{\pm}5.98$, 평균역연령은 $7.02{\pm}0.58$ 평균 골연령은 $8.04{\pm}1.29$로 세 그룹간에 키와 체중 비교 시 유의한 차이가 없었고 역연령(P < 0.0001), 골연령( P< 0.0001), 골연령과 역연령과의 차이(P < 0.0002 )에서는 그룹간 유의한 차이가 있었다. 각 그룹별 LH, FSH Peak치 분포도 비교시 진성성조숙증 그룹은 LH 30분-82.5%, 45분-12.3%, 60분-5.3%, FSH 30분-8.8%, 60분-91.2%로 높은 Peak치를 보였고 조기사춘기 그룹은 LH 30분-79.5%, 45분-17.9%, 60분-2.6%, FSH 30분-7.7%, 60분 -92.32%로 높은 Peak치를 나타내었다. 유방조기 발육군 그룹에서는 LH 30분-30%, 45분-59%, 60분-9.09%, FSH 30분 -0%, 60분-100%로 Peak치를 나타내었다. LH Peak/LH basal ratio 비교시 진성성조숙증 그룹은 $19.09{\pm}17.15$, 조기사춘기 그룹은 $15.23{\pm}10.88$ 유방조기 발육군 그룹은 $4.93{\pm}4.36$으로 3 그룹간 유의한 차이를 보였다(P < 0.0001). LH Peak/FSH Peak ratio 비교시 진성성조숙증 그룹은 $1.222{\pm}0.77$, 조기사춘기 그룹은 $1.34{\pm}1.23$ 유방조기 발육군 그룹은 $0.3{\pm}0.09$로 3 그룹간 유의한 차이(P < 0.0001)를 보였다. 4. 결론 여아들의 진성성조숙증을 진단하기 위한 GnRH Test 검사는 자극 후 30분과 60분 사이에 LH 최고치가 기저치에 비해 2~3배 이상 증가 되어 있거나 또는 5~10 IU/L 이상 증가되었을 때 유용한 판정 자료로 이용 될 수 있는 핵의학 연속검사라고 사료되어진다.

고 에너지 방사선치료에서 환자의 피폭선량 분포와 생식선의 차폐 (Shielding for Critical Organs and Radiation Exposure Dose Distribution in Patients with High Energy Radiotherapy)

  • 의료용 선형가속기에서 발생되는 고 에너지 광자선은 콜리메이터에 의하여 누출되며 치료두부(head), 콜리메이터, 환자를 포함한 치료실내의 모든 벽과 구성 물질들에 의하여 많은 산란선이 발생된다. 방사선치료는 종양에 따라서 최소한 40 Gy에서 80 Gy까지 조사되기 때문에 주위건강조직 특히 생식가능한 사람에 대한 생식선의 피폭선량을 평가하여야하며 종양치료에 영향을 주지 않은 범위에서 가능한 방법을 동원하여 피폭선량을 줄여야한다. 방사선 안전관리등의 기술기준에 관한 규칙(과학기술부령 제17호) 제3절 의료분야의 특별기준, 제44조(진료환자의 방사선 피폭)에 의하면 진료를 위한 환자 피폭선량을 합리적으로 달성 가능한 최소의 수준으로 유지하기 위한 절차를 구비하여야 하며 과학기술부 장관은 이에 준하는 의료시설 및 장비취급의 기술기준을 정하고 고시하여야한다고 명시 되어있다. 고 에너지방사선은 악성종양환자들의 치료성과를 향상시키는 동시에 치료후 방사선에 의한 만성효과가 발생 될 수 있기 때문에 주선속의 다양한 산란선과 누출선의 선질변화와 선량을 측정하고 생식선과 같은 주요장기를 산란선으로부터 차폐할 수 있는 기구를 제작 사용함으로서 방사선 피폭선량을 최대한으로 감소시킬 수 있었다. 고 에너지 방사선은 의료용 선형가속기(CLINAC 2100C/D. 2100C. 600C)에서 발생시킨 4, 6, 10 MV x-ray와 코발트원격치료장치(ALCYON II)의 코발트선원에서 방출되는 1.25 MV의 감마선을 이용하였다. 선량측정은 폴리스틸렌과 인체팬텀(Rando)사용하였으며 측정기는 이온함, TLD 및 필름을 사용하였다. 고 에너지 방사선에 의한 산란선은 장치의 콜리메이터 뿐만 아니라 치료실 벽 인체내부등 모든 방향에서 방사됨으로 납 벽돌에 의한 차폐율측정은 많은 변수를 가졌으며 고환인 경우에는 3면이 모두 차폐되도록 항아리모양으로 제작하였다. 태아인 경우 태아가 위치하고 있는 골반위에 육교모양의 선반을 만들고 그 위에 납 벽돌을 장치하도록 고안하였다. Co-60 감마선, 4 MV x-선, 10 MV x-선에서 발생되는 누출선량과 산란선량에 의한 평균 피폭선량은 조사면 중심으로부터 10, 30, 60cm 거리에서 조사면내 최대선량에 대하여 각각 $10^{-2},\;10^{-3},\;10^{-4}$의 비율로 측정되었으며 거리에 따라 지수함수로 줄어들었다. 흉부에 국한된 종양을 10 MV x-ray, $12{\times}12 cm^2$ 조사면으로 치료하였을 때 자궁에 받는 피폭선량은 0.9 mGy/Gy이며 고환이 받는 피폭선량은 0.6 mGy/Gy 이었으며 체장과 신장은 각각 4.8 mGy/Gy 와 2.5 mGy/Gy이다 10 MV x-선, $14{\times}14cm^2$ 조사면 경계로부터 10 cm 밖에서 납벽돌의 반가층 두께는 약 9.0 mm 이였고 20cm 밖에서는 반가층 두께가 약 6.5 mm로 측정되었다. 복부에 위치한 악성종양을 60 Gy 조사하였을 경우 태아가 위치하고 있는 자궁의 피폭선량은 약 370 mGy이고 이곳을 10 mGy이하가 되도록 차폐하려면 약 6.2 cm두께의 납 벽돌을 자궁위에 장착하여야 하며 골반치료시 고환에 10 mGy이하가 되도록 차폐하려면 약 5 cm 두께의 납 항아리가 요구된다. 고 에너지 고 준위 방사선치료시 고환은 3면을 항아리모양으로 차폐할 수 있어 피폭선량을 상당히 줄일 수 있으며 자궁인 경우 체내에서 산란된 선량의 차폐는 불가능하였다.

High Resolution HC$_3$N Observations toward the Central Region of Sagittarius B2

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500,550,570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500,550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100,300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. 스마트폰 다종 데이터를 활용한 딥러닝 기반의 사용자 동행 상태 인식 (A Deep Learning Based Approach to Recognizing Accompanying Status of Smartphone Users Using Multimodal Data)

  • 스마트폰이 널리 보급되고 현대인들의 생활 속에 깊이 자리 잡으면서, 스마트폰에서 수집된 다종 데이터를 바탕으로 사용자 개인의 행동을 인식하고자 하는 연구가 활발히 진행되고 있다. 그러나 타인과의 상호작용 행동 인식에 대한 연구는 아직까지 상대적으로 미진하였다. 기존 상호작용 행동 인식 연구에서는 오디오, 블루투스, 와이파이 등의 데이터를 사용하였으나, 이들은 사용자 사생활 침해 가능성이 높으며 단시간 내에 충분한 양의 데이터를 수집하기 어렵다는 한계가 있다. 반면 가속도, 자기장, 자이로스코프 등의 물리 센서의 경우 사생활 침해 가능성이 낮으며 단시간 내에 충분한 양의 데이터를 수집할 수 있다. 본 연구에서는 이러한 점에 주목하여, 스마트폰 상의 다종 물리 센서 데이터만을 활용, 딥러닝 모델에 기반을 둔 사용자의 동행 상태 인식 방법론을 제안한다. 사용자의 동행 여부 및 대화 여부를 분류하는 동행 상태 분류 모델은 컨볼루션 신경망과 장단기 기억 순환 신경망이 혼합된 구조를 지닌다. 먼저 스마트폰의 다종 물리 센서에서 수집한 데이터에 존재하는 타임 스태프의 차이를 상쇄하고, 정규화를 수행하여 시간에 따른 시퀀스 데이터 형태로 변환함으로써 동행 상태분류 모델의 입력 데이터를 생성한다. 이는 컨볼루션 신경망에 입력되며, 데이터의 시간적 국부 의존성이 반영된 요인 지도를 출력한다. 장단기 기억 순환 신경망은 요인 지도를 입력받아 시간에 따른 순차적 연관 관계를 학습하며, 동행 상태 분류를 위한 요인을 추출하고 소프트맥스 분류기에서 이에 기반한 최종적인 분류를 수행한다. 자체 제작한 스마트폰 애플리케이션을 배포하여 실험 데이터를 수집하였으며, 이를 활용하여 제안한 방법론을 평가하였다. 최적의 파라미터를 설정하여 동행 상태 분류 모델을 학습하고 평가한 결과, 동행 여부와 대화 여부를 각각 98.74%, 98.83%의 높은 정확도로 분류하였다.
