구급차동승실습 시 프리셉터 제도의 도입에 관한 인식과 수용도 (Recognition and acceptance concerning the introduction of preceptorship to the ambulance ride practice)

  • Purpose : The purpose of the study is to provide the basic data for preceptorship education program development by analyzing the recognition and acceptance concerning the introduction of preceptorship to ambulance ride practice of 119 EMT-paramedics in Korea. Methods : A self-reported questionnaire was filled out by 157 paramedics in the fire fighters from December 21, 2013 to February 12, 2014. The questionnaire consisted of 31 questions and the data were analyzed by SPSS version 21.0. Results : Most of the subjects answered the positive choice and were very interested in the introduction of preceptorship in the prehospital settings. Mean of preceptorship acceptance level was 3.64 points in 5 points Likert scale and 75.4% of the subjects were able to explain the preceptorship. 57.4% of the subjects considered that preceptorship was an effective training method. Conclusion : Most of the subjects agreed that the preceptorship is very important to the education of paramedic students. The preceptorship education program development will be the basis of ambulance ride practice.

국내 구급차의 안락성 및 편의성 조사와 개선에 관한 연구 (Investigation and Improvement of the Comfort and Convenience of Domestic Ambulances)

  • 본 연구에서는 국내에서 사용되고 있는 구급차의 안락성과 편의성의 조사를 위하여 소음과 진동 및 사이즈의 타당성 등에 관하여 파악하였으며, 대상차량으로는 현재 사용되고 있는 화물형, 승합형 및 벤츠 구급차등을 이용하여 측정 및 조사하였다. 현재 국내의 구급차 제작기준을 살펴보면, 응급환자를 이송하고 구급차 내에서 응급처치를 시행해야 하는 특수목적을 가진 구급차에 대해 별도의 규정 없이 일반자동차와 동일한 기준을 적용하고 구급차량에 대한 안전도, 응급처치에 대한 공간규정, 안락성 및 편의성 등에 관한 규정이 없는 실정이다. 따라서 구급차 환자실의 소음과 진동 측정 실험을 통하여 안락성과 편의성에 대한 비교 실험을 측정하였고, 구급차 환자실의 사이즈 및 내부의 재배치에 대하여 연구하여 안락성과 편의성에 개선이 될 수 있는 부분에 대하여 조사하였다. 소음 측정 결과 승합형, 벤츠타입 및 화물형 타입의 구급차 순으로 소음이 큰 것으로 측정이 되었고, 40 km/h 이상의 속도에서는 60~70 dB 이상의 소음측정 결과를 보여주었다. 진동의 경우는 승합형과 벤츠타입의 구급차량이 양호하고 역시 화물형 타입이 진동 또한 가장 안 좋은 결과를 보여주었으며, 이에 대한 개선사항이 필요하다고 측정되었다. 마지막으로 국내의 구급차량 환자실의 내부를 살펴보면, 구조적으로 내부의 크기를 늘릴 수는 없는 구조이나, 침상의 재배치와 응급처치요원의 위치, 환자보호용 장의자의 배치 및 수납장 등의 재배치를 통하여 편의성을 개선시키는 것이 구급차의 사용을 보다 안락하고 편의성 있게 사용할 수 있을 것이라는 결과를 얻을 수 있었다.

구급차 내부 디자인 개선에 관한 연구 (A study on improving the interior design of ambulance)

  • Purpose: This research project addressed the need to designing more safe and efficient interior of the future ambulance in Korea. Methods: The study sample contained 760 paramedics in 4 districts. Data was collected by using a revised and complemented questionnaire based on literature review. Results: In relation to the efficacy and safety of work, answers related to storage closet showed to be the highest, and the most difficult part of paramedic work in an ambulance was lurching. CPR is the most frequently used emergency care inside an ambulance, but 66% of the paramedics responded that accurate CPR is not possible during vehicle transfer. Safety belts are not worn for 82.8% of the time, because of discomfort (51.3%). 13.8% of the paramedics responded that stretchers are unstable, 29.5% had an experience of having patients fall off the stretcher inside an ambulance. There were comments on installing equipments to prevent noise, and assist communication. Conclusion: The suggested practical layout contains five main modifications 1. Developing specially designed belt is needed for paramedic safety & efficient work. 2. The seats are molded to be ergonomically friendly. 3. Equipments to secure the body and safety devices for CPR are needed. 4. System improvement for communication between the driver seat and paramedics is needed. 5. The stretchers are molded to be maximize efficiency and minimize injury.

Analysis of Factors Affecting Satisfaction of 119 Emergency Service Users in Korea

  • This study attempted to identify the service satisfaction with the 119 emergency service and the factors that affect it based on the responses of subjects who used the service. Data collection was conducted from February 11 to March 11, 2021, using convenience sampling. A total of 1842 subjects who used emergency medical services using 119 ambulance in 2020 participated. For the collected data, frequency, percentage, mean, and standard deviation were calculated, and t-test, analysis of variance, and multiple regression analysis were performed using SPSS 23.0. Based on these results, to further enhance the satisfaction of users of the 119 emergency service and the quality of emergency medical services, it is important to improve the quality of paramedics through education and systems. In addition, to increase user satisfaction, efficient and systematic communication education is required. The quality of emergency medical services will increase only when communication skills required for explanations and promoting understanding are improved.

출동 응답 향상을 위한 적정 구급차 수량 및 배치 위치 결정 연구 (Optimal Decisions on the Quantity and Locations of Ambulances for the Timely Response to Emergency Requests)

  • 응급 상황 발생 시의 구급차 부족에 따른 출동 지연 감소를 위하여, 충분한 수의 구급차가 운영되어야 한다. 본 논문은 출동 지연을 최소화하기 위하여 안전센터별 적절한 119 구급차의 수량을 결정하는 효과적인 방법을 제시한다. 한 지방자치단체의 구급차 출동 요청을 분석하고, 출동 요청의 통계적 특성을 도출하였다. 119 안전센터 간의 상호 지원을 포함한 구급차 출동 시스템은 컴퓨터 시뮬레이션을 활용하여 모델링하였다. 구급차 요청의 통계적 특성과 시뮬레이션을 활용한 분석 결과, 대부분의 출동 요청에 성공적으로 응답할 수 있음을 확인하였고, 출동수행률 향상을 위한 구급차 수 증가가 효과적임도 확인하였다. 또한, 구급차를 추가로 배치할 안전센터를 찾아내는 효율적인 방법론도 제시하였다. 본 연구는, 적절한 구급차 수량 확보와 배치를 통하여, 응급환자 구조 서비스 개선에 활용될 수 있다.

119 구급대의 후송활동 및 개선방안 (Transport Activities and Improvement strategies of 119 Rescue)

  • This study was performed to investigate transport activities and strategize improvement of 119 rescue. We reviewed emergency care records of users who were transported by 119 rescue of six agencies in Chungnam from July 13, 1998 to August 8, 1998. The results were as follows ; 1. In sex distribution of users, the male was 65.0%. And the highest age group among users was above sixties(21.6%), then thirties in second order(19.3%). Accident was 50.8% as occupied first cause of transport, and then acute disease 22.8%. The highest requester for 119 rescue call was patient's families(47.1%) and average number of 119 rescue users per day was 20.9. 2. The nonurgent state of users was 58.9%. The frequency of users was 26.0 persons at sunday in most frequently, weekend and holiday was more common than ordinary day, and most frequent weather state was cloudy(23.8 persons). 3. Total running distance of 119 ambulance was 7.0km in average. Call time by users was 20-24 hours most commonly(21.9%). In then running time by each transport stage, 8 minutes were taken from 119 call receipt to scene arrival, 13 minutes from scene arrival to hospital. The kinds of pre-hospital care by 119 rescuer was vital sign check(81.2%), wound dressing or fracture fixation(41.2%), airway maintenance and O2 supply(30.4%).

119구급대원의 근골격계 증상과 환자운반 작업의 인간공학적 평가 (The Evaluation of Musculoskeletal Symptom and Patient Transport Work of 119 EMTs by Ergonomics Tools)

  • 본 연구는 119구급대원의 근골격계 증상 호소율을 파악하고 증상 유소견자들에 대한 작업성 근골격계질환(WMSDs)을 유발하는 환자운반 작업에 대한 인간공학적 평가를 통해 작업 위험도를 파악하고자 하였다. 이를 위해 근골격계 유해요인 조사지침(KOSHA Code H-30-2008)을 활용한 설문도구를 이용하여 근골격계 증상 호소율을 파악하였으며, 환자운반 작업의 위험도는 인간공학적 평가도구인 OWAS, RULA, REBA를 이용하여 평가하였다. 연구결과 근골격계 증상 유경험자는 60.9%였으며 허리 부위의 증상 자각율(36.1%)이 가장 높았고 WMSDs을 유발하는 가장 큰 원인은 환자운반 작업(48.4%)이었다. 환자운반 작업의 인간공학적 위험도 평가 결과 주 들것을 구급차에 싣고 내리기, 들 것 장비를 이용하여 환자를 들어올리는 작업에서 OWAS는 3단계, RULA와 REBA는 3~4단계 수준이었고, 응급용들것으로 계단 이송, 척추고정판으로 차량내 환자 이동, 업거나 안고 계단이나 경사로를 이동하는 작업에서 OWAS, RULA, REBA 모두 3~4단계로 작업자세의 개선이 필요한 것으로 평가되어 119구급대원의 WMSDs 예방을 위해서는 즉각적인 작업자세의 개선이 요구된다.

국내 구급차 내 수납공간의 문제점과 개선방안을 위한 디자인 연구 (Design Study for the Improvement of Storage Space in the Problem of Domestic Ambulances)

  • This research proposed a design direction for the ambulance interior storage space based on the placement and access to the equipment to facilitate an emergency medical practice taking into account the safety and efficiency by the emergency medical technician ergonomic factors. Therefore, through a field survey for the current derivation of the storage space problem 'placement', 'enable', 'management' like these three items resulting clarity, accessibility, simplicity and functionality of the four functional elements by targeting ambulances in operation in this country. First, there is a need to improve standardized of cabinet sizes fit to loaded meet emergency equipment standards. Currently, the equipment designed to use a simple storage cabinet has had problems in the usability. Second, it must be located in the continuous exposure in progress to the user's traffic line. Third, the problem was the insufficient function of the cabinet to ensure the safety of patients by paramedics from the emergency rescue equipment and due to the leaning of the ambulance driving. Besides, it was pointed out that in addition to the proposed alternatives, including the agenda, states that safety belt replacement equipment, emergency personnel and patients with the handle of a guardian for the efficient use storage space within the 119 ambulance. However, in the present study suggest that there is a state proceeds mainly limited usability in the field, having been installed.

구급차량 내 전자파 방출에 관한 조사 (Research on the emission of electromagnetic waves in ambulance)

  • Purpose: The study aims to provide basic data to enhance the health of paramedics responsible for patient transport and treatment by analyzing the exposure level of paramedics to electromagnetic waves generated by electric devices used in ambulances. Methods: The study measured electromagnetic waves in ambulances in N region from July to December 2018. ME3030B produced by German Gigahertz Solutions was used to measure these waves and the maximum value was selected by moving it slowly in various directions. Each measurement part was selected and the mean value was calculated by repeatedly measuring at 10-minute intervals three times in total: $1^{st}$, $2^{nd}$, and $3^{rd}$ phase. Results: Among the electrical devices installed in the patient room of the ambulance measured at distances of 1 cm or 30 cm, results showed a high level of electric waves at the inverter ($26.25{\pm}39V/m$) and high level of electromagnetic waves ($564.00{\pm}31.75nT$) at the ozone sterilizer. According to measurements toward the front near the driver's seat, results indicated high levels of electric waves ($3.67{\pm}1.15V/m$) and electromagnetic waves ($450.00{\pm}19.52nT$) at the black box hard drive. Conclusion: Electromagnetic waves within the ambulance were stable and not beyond the range that might impact human health. However, in the case of the black box hard drive ($3.67{\pm}1.15V/m$, $450.00{\pm}19.52nT$) located under the passenger seat, it may have a direct effect on the human body and, thus it is necessary to move it to a storage area further away from the paramedics to minimized the impact.

119 구급대원의 곤충교상환자에 대한 응급처치 현황 및 개선방안 (Prehospital care and improvement of 119 emergency medical technician for the insect bite patients)

  • Purpose : This study aimed to improve quality of prehospital emergency care for the insect bite patients by figuring out its current situations and problems. Methods : This study was conducted to 219 insect bite patients who were transferred to the hospital by 119 ambulance in D-City from July 1, 2009 to June 30, 2011. This study was a descriptive study by 119 run sheets. A total of 171 emergency medical technicians(EMTs) working in D-city and C-province completed the questionnaire. All of data were analyzed by SPSS 18.0. Results : Among the 219 patients, bee sting accounted for 62.6% and wasp sting accounted for 20.1%. Some patients complained of dizziness, headache, dyspnea, and disturbance of consciousness. The 119 EMTs initially treated the patients with oxygen supply to 57 patients (31.1%). Knowledge toward insect bite was statistically significant (p=.000) between advanced EMTs and basic EMTs. The obstacle to the prehospital care, many EMTs answered that the obstacle was due to the lack of 119 EMTs. Conclusion : It is necessary to differentiate the patients' symptoms and provide the better prehospital emergency care to verify the exact symptoms and signs.
